Abstract

Introduction: Chronic disease is one of the health problems in Indonesia which continues to increase and has not been successfully reduced. Coronary heart disease (CHD) is a chronic disease which has been a major cause of death and global burden of disease. The prevalence of CHD in North Sulawesi is 1.8%, exceeding the national prevalence, making North Sulawesi the fourth highest province in Indonesia. The management of CHD aims to prevent recurrent coronary events and complications by modifying risk factors. Holistic approach is needed to identify the risk factors that cause CHD. Family medicine is a medical specialty that provides health care with a holistic approach at first-level health facilities. Program Rujuk Balik (PRB), as one of the leading programs, is optimized to improve the quality of health services for the participants of BPJS Kesehatan and to facilitate access to health services for participants who have chronic diseases. The catastrophic costs can be minimized through holistic management of CHD by family physicians maximizing the function of PRB. Objective: To determine the overview of the management of PRB participants with CHD by family physicians in Manado City. Methods: This research is a descriptive study with a cross-sectional design. Results: It was found that all respondents were in the good category at managing the participants of PRB with CHD (100%). Conclusion: The management of the participants of PRB with CHD by family physicians in Manado City has been well. Keywords: program rujuk balik, coronary heart disease, family physicians.

Abstract

Background: Diabetes Mellitus (DM) is one of the non-communicable diseases included in the top 10 causes of death worldwide, with 6.7 million reported cases. The prevalence of DM in North Sulawesi Province ranks 4th, with a prevalence of 2.3%, exceeding the national average. Comprehensive treatment, which does not solely focus on the patient's disease aspect, is needed to address the increasing cases of DM. The Indonesian government, through the Social Health Insurance Administration Body (BPJS), operates a primary care system that utilizes family physicians as the frontline in serving the community. However, several conducted studies reveal a lack of frequency in the knowledge, attitude, and practice of family physicians towards DM management.Aim: To determine the behaviour (knowledge, attitude, and action) of family physicians management of type 2 DM in Manado City.Methods: This study is a descriptive study with a cross-sectional research design.Results: The level of knowledge, attitude, and practice of family physicians in type 2 DM management was categorized as good at 98.2% and sufficient at 1.8%.Conclusion: The behaviour of family physicians in Type 2 DM management in Manado City is considered good.Keywords: behaviour, type 2 diabetes mellitus, family physicians

Abstract

Background: Asthma is a heterogeneous disease typically characterized by chronic inflammation of the airways, manifested by respiratory symptoms such as wheezing, shortness of breath, and coughing. Data from Social Security Administrative Body for Health (BPJS Kesehatan) in 2019 showed significance increase in this province reached 1974 cases. Lifestyle in big cities like Manado City can increase the risk factors for asthma. The management of asthma requires a holistic approach, starting at the Primary Health Care Facilities (FKTP—Fasilitas Kesehatan Tingkat Pertama), such as community health centres and private practices doctors. However, studies have found that family physicians’ behaviour have limited knowledge concerning their knowledge, attitudes, and practices in asthma management.Aim: To assess the overview of behaviour (knowledge, attitudes, and practice) of family physicians regarding management of asthma in Manado City. Methods: This study was a descriptive study with a cross-sectional research design.Results: The level of knowledge was in the good category for 82% of family doctors in Manado City and in the sufficient category for 18% of family doctors. Attitude was categorized as good for 39% of family doctors and fair for 61% of them. The proportion of family doctors who had actions in the good category was 75%, 23% in the fair category, and 2% in the poor category.Conclusion: The behaviour of family doctors in the aspect of asthma knowledge was considered good, attitudes were at a sufficient level, and actions were concluded to be good.Keywords:behaviour; family physicians; asthma

Abstract

Background: Globally, the prevalence of gouty arthritis has doubled from 1990 to 2020, with the total of number cases 55,8 million in 2020. There is no Indonesian national data, yet, studies in some regions noted the cases, in North Sulawesi, in Depok, and in Bali. Prevention of gouty arthritis involves a healthy diet and lifestyle. Knowing the knowledge, attitudes, and practice about diet in people with gouty arthritis can help health institutions plan more appropriate interventions to prevent an increase in cases. To assess this, a valid and reliable instrument to measurement is needed. Aim: To test validity and reliability of a questionnaire of knowledge, attitudes, and practice towards to diet in people with gouty arthritis. Methods: This study is a descriptive study with a quantitative approach involving 13 respondents, patients in Manado City Health Center selected using purposive sampling method. Validity was assessed using r-count obtained through bivariate Pearson which was compared with r-table. Reliability was assessed through Cronbach's alpha coefficient. Results: The validity test using bivariate Pearson was valid for 9 statement in knowledge and attitudes section, and 8 statement in practice section of the questionnaire, with r-count>r-table, and the reliability test using Cronbach's alpha with values on knowledge, attitudes, and actions of 0.906, 0.924, and 0.864, ≥0.7, respectively. Conclusion: 26 out of 30 valid and reliable questions can be used to measure the level of knowledge, attitudes, and practice in people with gouty arthritis. Abstract

Elementary school-age children are still easily persuaded by friends, the environment, advertisements, and the seduction of hawker sellers. Therefore, we make elementary school students as an educational target, so that they understand the importance of precision in buying snacks. The purpose of this activity is to find out the influence of counseling on the knowledge of SD Kartika XXI-1 students about healthy snacks at school. The method of implementation in this PkM activity is the counseling method for students of SD XXI-1 Kartika Jaya, Wanea District, Manado City. Counseling activities are a two-way communication process between communicators (extension workers) and communicators in an interaction. This activity will be carried out in May 2024, starting at 09.00 WITA. This health education activity is evaluated through knowledge change through direct questions and answers with students where the results are obtained that the students already understand about choosing healthy snacks, especially at school. The conclusion of this activity is that counseling activities can increase the knowledge of SD Kartika XXI-1 students about healthy snacks at school.

Abstract

Background: Non-Communicable Diseases (NCDs) can be prevented by promoting the CERDIK behavior in daily life. The CERDIK behavior consists of several key actions: regular health check-ups, avoiding cigarette smoke, engaging in physical activity, maintaining a balanced diet, getting adequate rest, and managing stress. The aim of this activity was to provide health education focused on the prevention of NCDs, targeting students of SMAN 6 Kota Manado. Adolescents were chosen as the primary audience to raise early awareness and encourage the implementation of CERDIK behavior from a young age. Methods: This activity was conducted on Friday, September 6, 2024, starting at 08:00 AM until completion. Health education was delivered by Dr. Ronald I. Ottay, M.Kes, and a team of master's students from the public health sciences program. The session lasted for two hours and concluded with a friendly gathering. A total of 60 students and teachers participated in the event. Pre- and post-educational assessments were conducted to measure changes in knowledge levels among the participants. Results: The results of this activity showed that the total knowledge score increased from 625 before the session to 670 afterward, indicating a gain of 45 points. The average score increased from 35.2 to 38.7, showing a 3.5-point improvement. This indicates a noticeable difference in knowledge scores before and after the health education session. Conclusion: This community service activity demonstrated that the educational intervention had a positive impact on students' knowledge regarding the prevention of non-communicable diseases through the implementation of CERDIK behavior at SMAN 6 Manado.
