Abstract

Introduction: HELLP (hemolysis, elevated liver enzymes, and low platelets) syndrome is a multisystemic disorder. HELLP syndrome is a life-threatening condition with high maternal and infant mortality rates. This study aimed to present an unexpected case of HELLP syndrome complicated by pulmonary edema. Case presentation: A 40-year-old woman, 80 kg, who was 26-27 weeks into her pregnancy, came to the obstetric department due to fever and nausea in the past three days. She was then referred to the internal medicine department and diagnosed with dengue fever by the symptom of fever and thrombocytopenia. After two days of in-patient treatment, the obstetrician diagnosed her with HELLP syndrome, followed by elevated liver enzymes. She was then posted for an urgent caesarean section, and we managed this case under general anaesthesia. Postoperatively, we aimed for -500 to -1,000 mL cumulative fluid balance to avoid further hypovolemia. Conclusion: The patient showed improvement, as evidenced by chest X-ray and oxygen saturation. Management of fluid overload in this patient with HELLP syndrome was challenging. On the second day in ICU, the antibiotic was changed from meropenem combined with levofloxacin to meropenem combined with amikacin because the leucocyte level increased and procalcitonin level increased in 48-72 hours.

Abstract

The relationship between trust and quality of patient service has an impact on patient satisfaction, and this has implications for their loyalty in using and recommending the Hospital. This will be proven by this study. Using a sample of 325 outpatients, an inferential test was conducted quantitatively with path analysis. The results found that; 1) there is a direct and indirect influence of patient trust on satisfaction, through the service quality variable, of 25.13%. 2) there is a direct and indirect influence of service quality on patient satisfaction, through the patient trust variable, of 23.59%. 3) Simultaneously, patient trust and service quality affect patient satisfaction. 4) there is a direct and indirect influence of patient trust on patient loyalty, through the service quality variable, of 37.78%. 5) there is a direct and indirect influence of service quality on patient loyalty, through the patient trust variable of 25.4%. 6) The total influence of patient trust and service quality on patient loyalty, through the patient satisfaction variable, is 17.61%.

Abstract

The low scores of students in the Islamic Religious Education (IRE) subject, which have not yet met the Minimum Completeness Criteria (KKM), indicate challenges in the learning process. This study aims to examine the effectiveness of a blended learning model based on the Quizizz application on student learning outcomes at SMA Negeri 1 Katibung using a quasi-experimental method with a post-test only control group design and simple random sampling technique. The sample consisted of 63 eleventh-grade students divided into an experimental class using the blended learning model and a control class using a traditional learning model. The instrument used was a valid and reliable multiple-choice test administered after the treatment. Data analysis included normality test, homogeneity test, independent sample t-test, and Mann-Whitney U test. The results showed that the Sig. (2-tailed) value in Levene’s Test for Equality of Variances was 0.228 (>0.05), thus the null hypothesis Ho was accepted and the alternative hypothesis Ha was rejected, meaning that the Quizizz-based blended learning model was not effective in improving student learning outcomes. Therefore, teachers are advised to evaluate this learning model and consider alternative methods. The school is also recommended to provide training on learning technology for teachers. Researchers are encouraged to investigate other factors affecting learning outcomes and expand the scope of research. Furthermore, relevant parties are expected to support the use of technology through appropriate policies and adequate facilities.

Abstract

Acess to Justice atau yang dalam Bahasa Indonesia lebih dikenal dengan akses terhadap keadilan adalah salah satu bentuk pengejawantahan prinsip negara hukum dan pengakuan hak asasi manusia (HAM) sebagaimana dijamin oleh Pasal 28 D ayat (1) Undang-undang Dasar Negara Republik Tahun 1945. Indonssia sebagai Negara Hukum, maka konsekuensi logis dari penetapan ini dalah berkewajiban  memberikan perlindungan  dan pengakuan Hak Azazi Manusia  dari setiap individu  atau warga negara dan seseorang berhak untuk diperlakukan sama dihadapan hukum, termasuk bagi rakyat miskin yang sedang bermasalah dengan hukum. Penyelenggaraan pemberian bantuan hukum kepada warga negara merupakan Upaya untuk memenuhi dan sekaligus  sebagai implementasi negara hukum  yang mengakui dan melindungi serta menjamin hak asasi warga negara akan kebutuhan askses terhadap keadilan (acess to justice) dan kesamaan di hadapan hukum (equlity befote the law).  Jaminan atas hak konstitusionil  tersebut  dengan dibentuknya  Pemberian Bantuan Hukum yang sudah diatur oleh pemerintah secara  jelas dalam sebuah peraturan perundang-undangan. yaitu Undang-undang Nomor 16 Tahun 2011 Tentang Bantuan Hukum dan Peraturan Pemerintah Nomor 42 Tahun 2013 Tentang Syarat dan Tata cara Pemberian Bnntuan Hukum dan Penyaluran Dana Bantuan Hukum. Dengan demikian dengan dibentuknya Undang-undangtersebut menjadi bukti bagi negara untuk menjamin warga negara khususnya bagi orang atau kelompok orang miskin untuk mendapatkan akses keadilan dan kesamaaan di hadapan hukum. Bantuan hukum yang diberikan kepada seseorang yang bermasalah atau berhadapan dengan hukum tersalurkan melalui pemberian bantuan secara cuma-cuma atau gratis atau pro bono yang di khusus bagi amsyarakat miskin atau tidak mampu, oleh karena itu pemberian bantuan hukum ini harus terus ditingkatkan dan kita harus bersama-sama mengingatkan dan membangun kesadaran hukum penerima bantuan  hukum secara Cuma-Cuma atau gratis bagi masyarakat miskin atau tidak mampu tersebut.
