Abstract

Abstract

Diabetes mellitus results in both microvascular and macrovascular complications due to inadequately managed glucose levels over an extended period. A notable complication is peripheral neuropathy, which arises from compromised peripheral circulation. Implementing Buerger Allen Exercises and Foot Exercises can enhance circulation and mitigate these complications, with effectiveness assessed using the ankle-brachial index. This study aims to evaluate the differential efficacy of Buerger Allen Exercises compared to Foot Exercises in improving peripheral perfusion among patients diagnosed with type 2 diabetes mellitus affiliated with the Patrang Community Health Center. The research employs a quasi-experimental design featuring a pretest-posttest two-group format. The study sample consists of 30 respondents diagnosed with type 2 diabetes mellitus. The intervention, which included both Buerger Allen Exercises and Foot Exercises, was conducted once and lasted 20 minutes. The results indicate that the Buerger Allen Exercise group exhibited an average ankle-brachial index value of 0.992, while the Foot Exercise group yielded an average value of 0.900. The obtained p-value (0.004) was less than or equal to the alpha level of 0.005. Therefore, a statistically significant difference exists between the Buerger Allen Exercise intervention group and the Foot Exercise group. In conclusion, the Buerger Allen Exercise intervention is more effective than the Foot Exercise regimen in enhancing peripheral perfusion, as evidenced by the measured ankle-brachial index values.

Abstract

Hypertension can have significant impacts if not addressed promptly, and treatment is often initiated too late. Effective management necessitates integrated care that considers both internal and external factors. Patients actively engaged in their treatment should possess essential qualities, including knowledge and motivation. This study aimed to investigate the factors associated with medication adherence in hypertensive patients residing in the rural area of Jember City. Using a cross-sectional design, this study focused on hypertensive patients in rural Jember. A simple random sampling technique was employed, including 94 respondents. Data collection was conducted using a questionnaire, with analysis performed using the Chi-Square test. The statistical results indicated a p-value of 0.015 (p < α = 0.05), suggesting that the level of knowledge is associated with medication adherence. Additionally, motivation yielded a result of p = 0.02 (p < α = 0.05), indicating that respondents' motivation correlates with adherence levels. The analysis of healthcare support showed a p-value of 0.048 (p < α = 0.05), demonstrating its association with adherence. Furthermore, family support revealed a p-value of 0.000 (p < α = 0.05), indicating significant influence on adherence levels. The variable concerning complementary therapy also showed a strong relationship, with a p-value of 0.000. In contrast, the type of medication indicated a p-value of 0.485 (p > α = 0.05), suggesting no correlation with adherence. All respondents reported that healthcare facilities were easily accessible.

Abstract

Chemotherapy is a primary treatment modality for cancer, but it often results in adverse effects such as anemia due to decreased hemoglobin levels. Factors including age, chemotherapy frequency, and nutritional status may influence hemoglobin levels, particularly in rural areas with limited healthcare resources. To examine the relationship between age, chemotherapy frequency, and nutritional status and hemoglobin levels among cancer patients undergoing chemotherapy in a rural setting. This cross-sectional, correlational study involved 154 cancer patients undergoing chemotherapy at Baladhika Husada Hospital, Jember, from July to September 2024. Data were collected through patient interviews, the Mini Nutritional Assessment (MNA), and hemoglobin test results. Bivariate analysis using Spearman’s rank correlation was conducted to assess associations among variables. The majority of participants were aged 46–55 years (42.8%), had received 1–3 chemotherapy cycles (57.1%), and were at risk of malnutrition (45.5%). A significant association was found between age (p = 0.009, r = 0.191) and nutritional status (p = 0.014, r = 0.178) and hemoglobin levels. No significant association was found between chemotherapy frequency and hemoglobin levels (p = 0.60). Age and nutritional status are significantly associated with hemoglobin levels in cancer patients undergoing chemotherapy, while chemotherapy frequency is not. These findings highlight the need for individualized care and nutritional interventions, particularly for elderly patients in rural areas, to mitigate the risk of Chemotherapy-Induced Anemia

Abstract

Chronic Kidney Disease (CKD) affects over 10% of the global population (>800 million people) and increases the risk of cardiovascular diseases such as coronary artery disease, heart failure, and stroke. One of the primary treatments for CKD is hemodialysis, with arteriovenous (AV) shunt access used in approximately 92% of procedures. AV shunt installation is a surgical procedure often associated with increased patient anxiety. In addition to pharmacological treatment, non-pharmacological approaches such as Progressive Muscle Relaxation (PMR) can be effective in reducing anxiety. This study aimed to determine the effect of PMR on the anxiety levels of patients undergoing pre-operative AV shunt installation. A quasi-experimental pretest-posttest design with a control group was used, involving 70 respondents at Jember Klinik Hospital, divided equally into intervention and control groups (n=35 each). Sampling was conducted using sequential sampling. The State-Trait Anxiety Inventory (STAI) was used to measure anxiety levels, and data were analyzed using the Wilcoxon Signed Rank Test. Results showed a significant decrease in anxiety in the intervention group after receiving PMR. The median anxiety score in the experimental group decreased from 42.66 (pre-test) to 29.57 (post-test), moving from moderate to mild anxiety. In contrast, the control group showed no significant change (pre-test median = 38.06; post-test = 40.03). The Wilcoxon test yielded a p-value of 0.000 in the intervention group, indicating a significant difference before and after the intervention. These findings support the use of PMR as an effective complementary therapy to reduce anxiety in pre-operative CKD patients undergoing AV shunt surgery.
