Abstract

Preeclampsia is a condition of increased blood pressure accompanied by proteinuria during pregnancy. Preeclampsia is one of the causes of maternal and fetal death. Efforts to reduce the incidence of preeclampsia are one of the efforts to reduce maternal mortality, one of which is by knowing the characteristics of preeclampsia patients to plan to overcome the risk of preeclampsia in the future. This study aims to determine the characteristics of preeclampsia patients at Wonogiri Regional Hospital dr. Soediran Mangun Soemarso, Wonogiri, Central Java. This research was conducted in a retrospective cross-sectional manner on the medical records of preeclampsia patients in Wonogiri Regional Hospital dr. Soediran Mangun Soemarso, Wonogiri, Central Java during 2019-2022. Sampling was carried out using a purposive sampling method among pregnant women who gave birth in the hospital from November 2021 – Januari 2022. A total of 106 medical record data from pregnant women were obtained in this study. The average age of pregnant women in this study was 31.37 ± 6.72 years with the majority aged 20 – 35 years with a history of giving birth more than twice. The majority of preeclampsia patients at Wonogiri Regional Hospital were severe preeclampsia patients (53.8%). A total of 59 pregnant women gave birth using the vaginal delivery method and 47 pregnant women gave birth using the cesarean method.   Maternal age and pregnancy history are factors related to the risk of preeclampsia which can influence the mother's delivery method. Further research is needed on risk factors for preeclampsia in a wider population so as to improve the health of mothers and babies during pregnancy

Abstract

Objective: This study aimed to find correlation between a history of hypertension with severity and onset of preeclampsia.Methods: A retrospective cohort study with retrospective analysis of medical record data in patients with preeclampsia and severe preeclampsia from November 2021 – February 2022 was conducted at dr. Soediran Mangun Sumarso Regional General Public Hospital, Wonogiri, Central Java. Analysis was performed between the history of hypertension with severity and onset (<34 weeks and age ? 34 weeks gestational age). Data analysis was performed using SPSS software with univariate Chi-Square analysis.Results: A total of 44 medical record data were obtained in this study. A total of 20 patients (45.5%) had a history of hypertension and 24 (55.5%) patients had no history of hypertension. A total of 23 patients (52.3%) were diagnosed with severe preeclampsia and 21 patients (47.7%) were diagnosed with preeclampsia. Based on statistical analysis, it was found that there was no significant correlation between the history of hypertension and the severity of preeclampsia (p=0.741; OR (CI 95%): 0,818 (0,249 – 2,690)) and onset (p=0.88; OR (CI 95%): 1,133 (0,222 – 5,788)).Conclusion: There is no relationship between history of hypertension with the severity and onset of preeclampsia. Further studies with larger populations are needed to analyze the relationship between history of hypertension and preeclampsia.Keywords: blood pressure, cardiovascular disease, hypertension, onset, preeclampsia.

Abstract

Latar Belakang: Pengendalian Hipertensi diantaranya dengan mengendalikan faktor risikonya. Pos Binaan Terpadu Penyakit Tidak Menular (Posbindu PTM) bertujuan untuk monitoring dan evaluasi faktor risiko untuk mecegah morbiditas dan mortalitas. Tujuan: Mengetahui determinan hipertensi pada peserta Posbindu PTM di Pusat Kesehatan Masyarakat  (Puskesmas) Sawangan II. Metode: Penelitian cross sectional yang melibatkan 272 peserta yang hadir dalam pelaksanaan posbindu PTM pada periode Juni-Juli 2022 di Puskesmas Sawangan II, Magelang, Jawa Tengah. Setiap peserta Posbindu PTM diukur data demografis, riwayat keluarga hipertensi, status merokok, indeks massa tubuh dan tekanan darah. Data dianalisis dengan chi square dan regresi logistik. Hasil: Terdapat 145 (53,31%) peserta posbindu yang mengalami hipertensi. Usia (p=0,002; OR 3,309), tingkat pendidikan (p=0,015; OR 1,895), dan riwayat hipertensi keluarga (p=0,001; OR 2,739) berhubungan dengan kejadian hipertensi. Dari Analisis multivariat dengan mengontrol variabel jenis kelamin, IMT, tingkat pendidikan dan status merokok, didapatkan subjek berusia 45 tahun ke atas (aOR 3,579, 95% IK 1,469-8,721) dan memiliki riwayat hipertensi keluarga (aOR 3,444, 95% IK 1,839-6,450) lebih berisiko terkena hipertensi. Simpulan: Usia dan riwayat hipertensi keluarga yang merupakan non modifiable risk factor menjadi determinan hipertensi pada peserta Posbindu PTM Puskemas Sawangan II. Diperlukan identifikasi lebih lanjut faktor risiko yang dapat dimodifikasi seperti diet dan aktivitas fisik serta upaya promotif untuk monitoring faktor risiko tersebut. Abstract

AbstractIntroduction: An intracranial hemorrhage (ICH) is a rare condition but potentially life-threatening event, especially in pregnancy, and could happen in patients with eclampsia. At Hasan Sadikin General Hospital (RSHS), several eclampsia cases are admitted annually, some with worsening neurological conditions. Neuroimaging using CT-Scan is indicated in patients with seizures, decreased consciousness, or visual disturbances to detect intracranial pathology.  Case: We reported three cases of maternal eclampsia, two patients' chief complaint was seizures, but all of our patients were unconscious. All of our patients had a previous history of pre-eclampsia in the last pregnancy. All of our patients performed a CT scan performed in postpartum patients as a neurologic examination. All of our patients have worse outcomes. The Medical and Health Research Ethics Committee of the Faculty of Medicine, Padjajaran University with Ref no: DP.04.03/D/XIV.6.5/71/2025.Conclusion: Neuroimaging in patients with seizure-induced worsening of the condition has an important role in identifying other abnormalities, specifically intracranial hemorrhage. In this study, we conclude that patients with seizures in pregnancy should be screened with neuroimaging (computed tomography scan) to rule out central nervous system pathology, thereby improving the patient's condition and reducing maternal mortality.Keywords: CT scan, Convulsion, Eclampsia, Pregnancy Peran Kritis Pencitraan CT dalam Mendeteksi Komplikasi Neurologis Akut pada Eklampsia: Seri KasusAbstrakPendahuluan: Perdarahan intrakranial (PIK) merupakan kondisi langka namun berpotensi mengancam jiwa, terutama pada kehamilan, dan dapat terjadi pada pasien eklampsia. Neuroimaging menggunakan CT-Scan berperan dalam deteksi dini perburukan kondisi pasien dengan penurunan kesadaran.Kasus: Penelitian ini melaporkan tiga kasus eklampsia maternal, keluhan utama dua pasien adalah kejang, namun semua pasien pada penelitian ini tidak sadar. Semua pasien memiliki riwayat preeklampsia sebelumnya pada kehamilan terakhir. Semua pasien melakukan CT scan sebagai pemeriksaan neurologis. Semua pasien memiliki hasil yang lebih buruk. Komite Etik Penelitian Kedokteran dan Kesehatan Fakultas Kedokteran Universitas Padjadjaran dengan Ref no: DP.04.03/D/XIV.6.5/71/2025.Kesimpulan: Neuroimaging pada pasien dengan kondisi yang memburuk akibat kejang memiliki peran penting dalam mengidentifikasi kelainan lain, khususnya perdarahan intrakranial. Dalam penelitian ini, peneliti menyimpulkan bahwa pasien dengan kejang selama kehamilan harus diskrining dengan neuroimaging (pemindaian tomografi terkomputasi) untuk menyingkirkan patologi sistem saraf pusat sehingga memperbaiki kondisi pasien dan mengurangi angka kematian ibu.Kata kunci: CT scan, Eklampsia, Kehamilan, Kejang
