Abstract

Canalicular lacerations resulting from eyelid or periorbital area injuries are considered ophthalmologic emergencies. This condition transcends age groups and primarily affects the lower canaliculi, potentially leading to canaliculus stenosis and lacrimal dysfunction characterized by epiphora if not promptly addressed. In this medical journal abstract, we present a case study involving an elderly male patient who sustained eyelid and lacrimal canaliculus injuries. The management approach consisted of inserting a silicone tube and using a 6-0 nylon suture for eyelid reconstruction. Notably, the procedure was successful without any complications, resulting in a favorable outcome for the patient. This case underscores the importance of timely intervention and the efficacy of the described surgical technique in addressing canalicular lacerations.

Abstract

Keratitis adalah suatu inflamasi pada kornea, yang dapat terjadi akibat infeksi oleh mikroorganisme maupun akibat non-infeksi karena proses autoimun.Peradangan ini ditandai dengan edema kornea, infiltrasi sel inflamasi dan kongesti/kemacetan siliaris. Keratitis klasifikasikan superfisial dan profunda. Keratitis superfisial tidak akan meninggalkan parut ketika masa penyembuhan, sedangkan keratitis profunda dapat meninggalkan parut yang mengganggu penglihatan ketika masa penyembuhan. Pasien laki-laki berusia 38 tahun datang ke poliklinik mata RSUD Soewondo Pati dengan keluhan pandangan mata kabur, mata kanan merah, nyeri, mengganjal, Sebelumnya pasien sedang mencangkul di sawah mata kanan pasien kecipratan lumpur. setelah itu mata kanannya menjadi merah, nyeri, terasa mengganjal, silau dan berair bila terkena sinar matahari dan penglihatan sebelah kanan semakin kabur. Pada pemeriksaan tajam penglihatan didapatkan VOD 6/15 dan VOS 6/12. Kornea injeksi siliar (+), mata kanan terdapat adanya defek kornea. Pada pemeriksaan penunjang Tes Flourescein+ Slit lamp didapakan OD Flourescein (+), luas, infiltrat (+). Pada terapi yang diberikan dengan pemberian antibiotik topical, pemberian asam askorbat, dan mengedukasi pasien agar selalu memakai pelindung mata.

Abstract

Eyelid injuries, particularly palpebral lacerations, are common in pediatric patients and require meticulous management to prevent long-term functional and cosmetic issues. These injuries, often resulting from trauma, pose significant challenges, as they involve both the skin and deeper structures of the eyelid. This case report discusses the management of a 7-year-11-month-old child who sustained a full-thickness eyelid laceration after falling and hitting a stone. The patient presented with sharp pain, difficulty in opening the eyelid, and bleeding. A physical examination revealed a full-thickness laceration on the superior margin of the left eyelid, with accompanying edema and subconjunctival hemorrhage. Imaging tests were not required as no orbital injury was suspected. The patient underwent eyelid reconstruction within 72 hours, and postoperative care included antibiotic treatment and wound management. The surgical outcome was successful, with optimal cosmetic and functional results. This case underscores the importance of timely and precise surgical intervention to minimize complications and achieve satisfactory results in pediatric palpebra injuries. Further research is needed to establish comprehensive guidelines for managing eyelid trauma in children, particularly regarding surgical techniques and postoperative care to ensure the best outcomes for young patients.

Abstract

Preseptal cellulitis is a skin infection around the eye that can originate from various sources such as sinusitis or trauma. This study explores the case of a 54-year-old patient with uncontrolled type 2 diabetes who presented with preseptal cellulitis. The patient complained of redness and pain around the left eye, with a history of fever and swelling for three days prior. Physical examination revealed hyperemic edema on the upper eyelid of the left eye with positive tenderness. Additionally, the patient had a history of uncontrolled type 2 diabetes. CT-scan imaging of the orbit and paranasal sinuses revealed hyperpigmentation on the left extraocular bulb, opacity in the right maxillary sinus with a polyp impression, and a hyperdense lesion in the right maxillary sinus with evidence of hyperostosis of the right maxillary bone. Laboratory results showed hyperglycemia, leukocytosis, uremia, and increased creatinine. The established diagnosis was preseptal cellulitis with potential complications. The discussion involves clinical aspects and management of preseptal cellulitis, including orbital anatomy, infection spread, diagnosis, and treatment. Patient management included the administration of antibiotics, anti-inflammatory drugs, and glycemic control. This study emphasizes the importance of research into the relationship between type 2 diabetes and preseptal cellulitis for a better understanding of risk factors and optimal management. In conclusion, this research provides in-depth insights into the complexity and risks of preseptal cellulitis in patients with uncontrolled type 2 diabetes and underscores the need for proper management to reduce complications in this condition.
