Abstract

Buah belimbing wuluh (Averrhoa bilimbi L.) adalah tanaman yang berkhasiat sebagai antioksidan, karena mengandung vitamin C yang berfungsi untuk menyumbangkan elektron kepada radikal bebas. Penelitian ini bertujuan untuk mengetahui nilai IC50 pada sari buah dan formula sediaan serum sari buah belimbing wuluh (Averrhoa bilimbi L.). Pembuatan sari buah dilakukan dengan buah dijuicer lalu diambil sari buahnya. Pengujian aktivitas antioksidan sari buah belimbing wuluh (Averrhoa bilimbi L.) menggunakan spektrofotometri UV-Vis dengan metode DPPH (1,1-Diphenyl-2-Picrylhyrdazyl), hasil pengujian menunjukkan bahwa sari buah belimbing wuluh (Averrhoa bilimbi L.) memiliki nilai IC50 sebesar 72,49 ppm dengan kategori kuat. Serum sari buah dibuat menjadi tiga formula yaitu FI, FII, dan FIII dengan konsentrasi 3%, 6%, dan 9% yang diuji evaluasi fisiknya antara lain uji organoleptik, uji homogenitas, uji daya sebar, uji daya lekat, uji pH, uji viskositas, uji bebas logam, uji iritasi, dan uji hedonik. Ketiga konsentrasi serum 3%,6%, dan 9% menunjukkan nilai aktivitas antioksidan dengan nilai IC50 berturut turut yaitu 87,64 ppm (kuat), 84,61 ppm (kuat), 78,87 ppm (kuat). Aktivitas antioksidan sediaan serum wajah tertinggi pada konsentrasi 9% di formula III dengan nilai IC50 78,87 ppm (kuat) dengan nilai P value (<0,05) untuk mengetahui perbedaan signifikan.

Abstract

Effervescent merupakan sediaan yang menghasilkan gelembung gas sebagai hasil dari reaksi kimia di dalam larutan. Tujuan penelitian ini adalah mengetahui pengaruh variasi asam sitrat dan asam tartrat terhadap mutu fisik dan aktivitas antioksidan dari sediaan granul effervescent ekstrak kulit buah rambutan (Nephelium lappaceum), mengetahui konsentrasi asam sitrat dan asam tartrat yang paling optimal dalam sediaan granul effervescent dari ekstrak kulit buah rambutan (Nephelium lappaceum) dengan metode Simplex Lattice Design. Optimasi dilakukan dengan metode simplex lattice design dengan parameter waktu alir, pH, dan waktu larut dan pengujian aktivitas antioksidan menggunakan metode ABTS. Analisis terhadap data hasil dilakukan dengan dua cara, yaitu menggunakan bantuan software Design Expert® version 13 dan SPSS normalitas dan one sample T-Test. Hasil analisis mendapatkan model yang signifikan (p < 0,05) untuk waktu alir dan waktu larut mengikuti model linear, sedangkan pH mengikuti model cubic. Hasil formula optimal diperoleh asam sitrat sebanyak 1,924 gram dan asam tartrat sebanyak 23,575 gram. Hasil aktivitas antioksidan masuk kategori sangat kuat pada kontrol positif  kuersetin nilai IC50 sebesar 6,020 µg/mL, ekstrak kulit buah rambutan nilai IC50 sebesar 34,674 µg/mL, sediaan granul effervescent nilai IC50 sebesar 39,350 µg/Ml. maka dapat disimpulkan bahwa asam sitrat dan asam tartrat berpengaruh terhadap evaluasi mutu fisik sediaan, konsentrasi asam sitrat dan asam tartrat yang paling optimal sebesar asam sitrat 1,924 gram dan asam tartrat 23,575 gram.

Diabetes mellitus is a chronic disease characterized by increased blood sugar levels due to interference with insulin production or decreased sensitivity of insulin receptors in the body. Diabetes itself cannot be cured so it requires intensive treatment, especially proper self-care to support the patient's life. The aim of this study was to determine the factors that influence the self-caring of type 2 diabetes mellitus patients undergoing outpatient care at Dr. RSUD. Moewardi Surakarta. This research was carried out observationally with a cross-sectional design. Self-care measurement uses the Diabetes Self-care Management Questionnaire (DSMQ) instrument. The results of this study show that of the 167 responses involved, 131 respondents (78.4%) had good self-care and 36 respondents (21.6%) had poor self-care. Data analysis using chi-square showed that age, gender, education, marital status, employment and duration of DM had no effect on the patient's self-care (p>0.05).

Abstract

Type 2 diabetes Mellitus is a chronic disease that requires lifelong treatment. In the course of the disease, if blood sugar levels are not controlled, it can lead to complications of other diseases. The variety of antidiabetic drugs used in therapy can lead to differences in the cost effectiveness of therapy. The aims of this study is to analyze the cost-effectiveness of antidiabetic therapy in outpatients at Dr. Moewardi Hospital. Data collection in this study was carried out retrospectively using medical records of outpatients with type 2 diabetes mellitus and billing data from the hospital The data obtained were then presented descriptively and the ACER value was calculated for the 2 most widely used drug groups. The results of this study showed that 30 patients received a combination of Insulin Analog (Basal) + Insulin Analog (Prandial/Premixed) and and 11 patients received Insulin Analog (Basal/ Prandial/ Premixed). The percentage effectiveness of combination therapy Insulin Analog (Basal) + Insulin Analog (Prandial/ Premixed) was higher than Insulin Analog (Basal/ Prandial/ Premixed) (83.33 vs 72.72). The ACER value of Insulin Analog (Basal) + Insulin Analog (Prandial/ Premixed) (IDR 13.802,42) is lower than Insulin Analog (Basal/ Prandial/ Premixed) (IDR 18.356). Based on the cost effectiveness table, the combination of Insulin Analog (Basal) + Insulin Analog (Prandial / Premixed) is more effective so that the combination of Insulin Analog (Basal) + Insulin Analog (Prandial / Premixed) is more cost effective than Insulin Analog (Basal / Prandial / Premixed).

Abstract

Diabetes Mellitus is a major endocrine disease and a global health problem characterized by increased blood glucose levels. Active compounds found in the essential oil of Eucalyptus camaldulensis Dehnh. have been reported to potentially act as in vitro inhibitors of the α-amylase and α-glucosidase enzymes, making them attractive candidates for antidiabetic agents. This study aimed to identify and predict the potential of derivative compounds through a computational approach using the Quantitative Structure-Activity Relationship method. The research methodology included structural modeling, geometry optimization, calculation of molecular descriptors, statistical analysis using multilinear regression with a backward approach in SPSS software, model validation, and new compound design. The best validated QSAR model equation was obtained with satisfactory statistical criteria (n=29; r=0.89; r²=0.79; SE=0.25; F-calculated/F-table=15.20). The selected descriptors with the most significant influence included LUMO energy, dipole moment, and estimated surface area, yielding the final equation: Log K = - 709,312 + 122,620·qC1 + 42,819·qC4 + 51,232·qC5 + 35,743·qC6  – 722,345·qO1 - 211,214·qO2. Based on this model, 3-(4-(ethylamino)butyl)-5-hydroxy-1H-pyrazole-4-carbonitrile, was designed and demonstrated significant potential as an inhibitor. This compound exhibited a predicted Log IC50 value of 2.75 (equivalent to an IC50 of 0.02 µM), which is substantially more potent than the IC50 value of the reference compound (30.58 µM). These results indicate that the novel compound has great potential as a candidate for an oral α-amylase and α-glucosidase inhibitor preparation for the management of DM. Further in vitro and in vivo validation studies are necessary to confirm these computationally predicted biological activities
