Abstract

Chronic kidney failure is a non-communicable disease caused by a decrease in the ability of the kidneys to carry out their functions. Hemodialysis therapy is a therapy given to sufferers of chronic kidney failure which functions to replace the work of the kidneys using a dialysis machine. Hemodialysis procedures performed on kidney failure patients are likely to cause depression because sufferers undergoing hemodialysis may experience feelings of discomfort or depression due to weakness in their physical condition and dependence on dialysis machines throughout their lives. The aim of this study was to determine the effectiveness of reminiscence therapy in reducing depression in chronic kidney failure patients undergoing hemodialysis at Imelda Pekerja Indonesia Hospital. This type of research is quantitative research, with the Quasy Experiment method, and with a pre-test and post-test group research design. This research was conducted in April – July 2024 in the Hemodialysis Room at Imelda Pekerja Indonesia Hospital, Medan. This research uses a non-probability sampling technique using a purposive sampling method, namely using inclusion criteria in determining the sample, so that the total research sample is 20 respondents. The instrument used in this research is a questionnaire that refers to the Beck Depression Inventory (BDI-II) to determine depression status. Data analysis used univariate and bivariate analysis with the Wilcoxon test in the SPSS application. The results of the Wilcoxon statistical test showed a significant (p) value of 0.000 (p < 0.05), meaning that there was an effect of giving reminiscence therapy on depression status or in other words reminiscence therapy was effective in reducing depression in chronic kidney failure patients undergoing hemodialysis at Imelda Pekerja Indonesia Hospital, Medan.

Abstract

Carcinoma mammae is a malignant condition of breast tissue characterized by the uncontrolled proliferation of abnormal cells that form tumors, which may metastasize to other organs and become life-threatening if not properly treated. Cancer cells generally originate from the milk-producing lobules or the breast ducts. Mastectomy, as a primary therapeutic intervention, often leads to significant physical changes that can affect patients’ body image and self-confidence. This study aimed to analyze the relationship between body image and self-confidence among post-mastectomy carcinoma mammae patients. This research employed a quantitative approach with a survey design using questionnaires. The sample consisted of 51 respondents selected through a total sampling technique at Grandmed Hospital Lubuk Pakam. The frequency distribution results indicated that the majority of respondents had a positive body image perception, with 48 individuals (94.1%) agreeing and 3 individuals (5.9%) disagreeing. Meanwhile, the level of self-confidence among respondents was also relatively high, with 44 individuals (86.3%) agreeing and 7 individuals (13.7%) disagreeing. Statistical analysis using the chi-square test revealed a significant relationship between body image and self-confidence (p-value = 0.84; α < 0.05), indicating that the alternative hypothesis was accepted. These findings highlight the importance of effective body image management in enhancing patients’ self-confidence. Therefore, psychological interventions such as counseling, psychoeducation, and peer support are essential to facilitate patients’ adaptation and self-acceptance following mastectomy.

Abstract

Background: Buloh honey possesses antibacterial and anti-inflammatory properties, as well as the ability to accelerate tissue regeneration. The innovative combination of Moist Wound Healing (MWH) and modern dressing based on Buloh bee honey provides an effective, safe, and affordable solution for managing diabetic foot ulcers. This approach can improve patients’ quality of life and reduce both the economic and public health burdens caused by poorly managed diabetic complications. Objective: To analyze the acceleration of diabetic ulcer wound healing before and after the intervention using the combination of MWH and modern dressing based on Buloh bee honey. Method: This study employed a quasi-experimental research design with a control time series comparison group. Results: Wound healing was assessed using the Bates-Jensen Wound Assessment Tool, which evaluates parameters such as wound size, depth, edge condition, tunneling, necrotic tissue type and area, exudate type and amount, surrounding skin condition, peripheral edema, granulation tissue size, peripheral induration, and epithelialization area. The results showed that the combination of Moist Wound Healing (MWH) and Buloh bee honey ointment accelerated the healing of diabetic ulcers compared to conventional treatment. In the first week, the intervention group began to show healthy granulation tissue formation and reduced exudate, while the control group demonstrated slower improvement. By the fourth week, 73.1% of participants in the intervention group reached near-complete epithelialization, compared to 41.8% in the control group. Conclusion: The combination of MWH and Buloh bee honey ointment proved to be more effective in accelerating diabetic ulcer healing than conventional care. This intervention promotes faster new tissue formation, reduces exudate, and enhances wound epithelialization. The approach is safe, natural, and recommended as an alternative therapy for chronic wounds in diabetic patients.

Abstract

Breast cancer remains one of the most prevalent health problems among women and may cause long-term physical, psychological, social, and emotional consequences. Mastectomy, which is commonly performed as part of breast cancer treatment, can improve survival outcomes; however, postoperative changes in body appearance often affect body image, self-confidence, and the overall quality of life of survivors. In addition, weight fluctuations during the recovery period may influence Body Mass Index (BMI) and psychological well-being. This community service program aimed to improve breast cancer survivors’ knowledge and ability to adopt healthy lifestyle practices for maintaining an optimal BMI, increase awareness of physical and psychological health, and strengthen self-confidence following cancer treatment. The program was implemented using an educational-participatory approach through health education sessions, interactive discussions, nutritional status consultations, light physical activity demonstrations, and experience-sharing sessions. Evaluation was conducted using pretest and posttest assessments involving 20 breast cancer survivors aged 35–60 years at Grandmed Hospital, Lubuk Pakam. The results demonstrated that the proportion of participants with a good level of knowledge increased from 20.0% to 80.0%, while those with poor knowledge decreased from 45.0% to 0%. The mean knowledge score improved from 58.5 to 85.5, representing an increase of 27.0 points (46.2%). Furthermore, 90.0% of participants understood how to maintain an ideal BMI, 100.0% recognized the importance of regular physical activity, and 85.0% reported increased self-confidence after participating in the program. Participant satisfaction ranged from 75.0% to 85.0%. In conclusion, healthy lifestyle promotion was effective in improving knowledge, understanding, and self-confidence among breast cancer survivors, thereby supporting the adoption of sustainable healthy lifestyle behaviors.
