Abstract

Increased access to health services after universal health coverage has not been matched by the fulfillment of the ratio of doctors will disrupt the resilience of national health insurance. The role of doctors in first-level health facilities as health service providers and referral screeners (gate keepers). This gap in the ratio of doctors can lead to high rates of non-specialistic referrals. This study aims to analyze the relationship between the ratio of doctors to Non-Specialistic Referrals (RNS) at BPJS Kesehatan Pamekasan Branch Office. The research method used was quantitative with an observational analytic approach using cross sectional and secondary data derived from all Status in Social Security Agency on Health, Pamekasan and Sumenep districts, and analyzed with the chi-square test (α=0.05). The results of this study indicate the existence of a doctor's ratio on the status of non-specialistic referrals, namely there is a relationship with a p-value of 0.001<0.05; OR=2.381 CI= 1.099-3.574. The conclusion of this study shows that there is a relationship between the ratio of doctors to the status of non-specialistic referrals. Local governments are expected to develop strategies to fulfill the ratio of doctors, especially in first-level facilities to achieve quality and standardized health services

Abstract

Tuberculosis is a contagious infectious disease with the highest risk of transmission occurring in households or household contacts of sufferers. Puskesmas Arjasa Sumenep District is located in the Kangen Islands with the highest number of TB cases in a row in the last 3 years since 2021-2023. The purpose of this study was to analyze the determinants of household TB incidence in the Arjasa Puskesmas work area, Sumenep District. This type of research is observational analytic using case control research design. The population of this study was TB patients who were treated and registered at SITB totaling 134 TB patients while the sample was taken by cluster random sampling with a ratio of 1: 1 in cases and controls. The research sample of 92 patients consisted of a case group of 46 TB patients who came from household contacts of TB patients (cases) and a control group of 46 TB patients who were not from household contacts of TB patients. The results showed that knowledge (p value = 0.000; OR = 2,165), infection prevention behavior (p value = 0.000; OR= 3.233), perception (p value = 0.001; OR = 0,137), occupancy density (p value = 0.039; OR = 2.521), and air chang per hour (p value = 0.037; OR = 3,790) had a relationship with the incidence of positive household TB at the Arjasa Health Center, Kangean Islands, Sumenep District. Meanwhile, the wall and floor components of the room did not have a relationship with the incidence of positive household TB (p value > 0.05). Puskesmas Arjasa is expected to increase the strengthening of education to patients and families related to the importance of conducting infection transmission prevention behavior and providing better assistance to TB patients

Abstract

Family support plays a very important and significant role in the success of Tuberculosis (TB) treatment. This support contributes greatly to improving treatment status by providing supervision and motivation to Tuberculosis patients. In this context, four types of family support were identified, namely emotional support, instrumental support, informational support, and appreciation support. The purpose of this study was to determine the relationship between family support and the success of drug-sensitive Tuberculosis (TB) treatment in the working area of the Arjasa Health Center, Kangean Islands, Sumenep Regency. This research method uses quantitative study research, namely analytical observational methods and adopts a cross-sectional design. The population in this study were 90 patients. The sampling technique used a total population sample technique and non-probability sampling, this study used questionnaires and interviews. The family support data scale is divided into 2, namely good and less, while treatment success is divided into 2, namely successful and unsuccessful, analyzing the data in this study using the Chi-Square Statistical Test. The results showed that most of the family support was good support (77.77%), most of the treatment success was successful (98.88%). The results of the chi square test obtained a P Value of 0.550 (P>0.05) or CI (lower = 0.952 and Upper = 1.164) means that there is no relationship between family support and the success of drug-sensitive tuberculosis (TB) treatment in the work area of the Arjasa Health Center, Kangean Islands, Sumenep Regency.
