Abstract

The availability of pharmaceutical supplies in hospitals is a basic requirement must be maintained, because medicine and pharmaceutical supplies are one of the factors that influence health care in hospitals. Pharmaceutical product management is a complex process. Inefficient management has a negative impact on hospital operational costs. Business Process Reengineering (BPR) is an effort to improve or refinement strategies by utilizing technology in the form of SIM- RS to increase productivity, effectiveness and efficiency in managing pharmaceutical supplies. This study aims to determine the implementation of BPR in the management of pharmaceutical supplies in pharmaceutical installations in hospitals. The method used is descriptive qualitative by conducting natural observations and interviews. The interview results were then reduced, analyzed and the data were presented in narrative form accompanied by tables and graphs. The size of the population consists of eight informants included director of the hospital, pharmaceutical procurement staff, finance managers, IT staff, pharmacist and nursing service staff, pharmaceutical procurement staff of external hospital as an informer triangulation. The results showed that the implementation of BPR can reduce the costs of managing pharmaceutical supplies by up to 70%, linear purchase and sales graphs, and researchers can find out the success factors in implementing BPR. It is important for organizations to have specific and detailed standard operating procedures so that BPR can implemented according to vision and goals.

Abstract

The demand for healthcare personnel continues to increase. Hospitals must sustain the continuity of healthcare workers who recognize their own existence as essential actors in the hospital health service system to prevent a decline in performance and the decision to leave the hospital. The intention to stay refers to nurses' desire to continue working at their current hospital. The intention to stay is crucial for the continuity of a hospital organization and is influenced by several factors. This research aims to analyze the effects of resilience and workload on nurses' intention to stay at the hospital. The results indicate that resilience has a significantly positive direct impact, while workload has a significantly negative direct impact on nurses' intention to stay. Resilience and workload together have a significant effect on nurses' intention to stay. Emphasizing that, in predicting this intention, it is necessary to consider not only the workload's heaviness but also the nurses' resilience, or mental endurance, in facing their tasks.

Abstract

Strategic planning plays a crucial role for hospitals in maintaining sustainability and competitiveness amid the dynamic healthcare environment and the National Health Insurance (JKN) financing system. The implementation of the prospective payment system based on Indonesian Case Based Groups (INA-CBGs) requires performance indicators that adequately reflect service volume, case complexity, and hospitals’ financial implications. INA-CBGs–based indicators, namely Casemix (CM), Casemix Index (CMI), and Hospital Base Rate (HBR), have the potential to support performance assessment and strategic planning; however, their utilization in private hospitals remains limited. This study aims to analyze the performance achievement of INA-CBGs indicators and examine their relevance for strategic performance planning at Karya Dharma Husada Hospital (RS KDH) Singaraja. This study employed a mixed methods approach with a sequential explanatory design. Quantitative analysis was conducted longitudinally using a trend study approach on inpatient INA-CBGs claim data from 2022 to January–June 2024, followed by qualitative analysis through in-depth interviews to interpret and contextualize the quantitative findings. The results showed a total of 7,361 inpatient JKN cases, predominantly involving class III patients and low-severity cases. Casemix values fluctuated in line with changes in service volume and case mix, with obstetric cases being the dominant contributor. CMI values remained within the typical range for private class C hospitals, reflecting low to moderate service complexity. HBR performance demonstrated periods of surplus and deficit, indicating discrepancies between actual service costs and national INA-CBGs tariffs. Overall, the study indicates that INA-CBGs indicators have not yet been systematically integrated into performance evaluation and strategic planning; however, hospital management has recognized them as valuable information with potential to support future managerial decision-making.

Abstract

This study aims to evaluate the level of physician compliance in the implementation of Clinical Pathways (CP) at Moh Ridwan Meuraksa Class II Hospital, Jakarta. Clinical Pathways function as a clinical care management tool designed to ensure consistency, efficiency, and quality of patient services based on scientific evidence and standardized procedures. The research applied a quantitative approach with an observational cross-sectional design through direct observation and the distribution of questionnaires to physicians. The sample consisted of 40 physicians involved in the implementation of CP for five major diagnoses, namely Acute Myocardial Infarction (AMI), Severe Preeclampsia, Hypertrophic Tonsils, Femur Fracture, and Cervical Cancer. The research instruments included observation sheets and questionnaires that had been tested for validity (r > 0.361) and reliability (Cronbach’s Alpha = 0.857). The findings indicate that overall physician compliance with CP implementation falls into the moderate category (65.8%). The highest compliance occurred in emergency clinical examinations and initial therapy administration, while the lowest compliance appeared in variance documentation and multidisciplinary consultation. The study recommends strengthening training, improving information systems, and integrating CP into Electronic Medical Records to enhance compliance and service quality.
