Abstract

Post Dural Puncture Headache (PDPH) merupakan kondisi yang umum terjadi setelah tindakan medis yang melibatkan tusukan dura, seperti prosedur epidural atau tulang belakang, kondisi ini dapat terjadi ketika tusukan tersebut menyebabkan kebocoran cairan serebrospinal. Kasus PDPH jarang terjadi komplikasi, namun PDPH yang berkepanjangan dapat mempengaruhi kualitas hidup bahkan kematian. Penelitian ini bertujuan mengetahui gambaran angka kejadian PDPH pada pasien sectionio cesarea pasca anestesi tulang belakang. Metode penelitian ini adalah kuantitatif dengan metode observasional rancangan cross-sectional . Sampel penelitian ini adalah pasien yang menjalani operasi sectionio caesarea dengan anestesi tulang belakang sejumlah 32 responden. Teknik pengambilan sampel penelitian ini adalah aksidental sampling . Hasil penelitian menunjukkan sebagian besar responden pada rentang usia 30 - 39 tahun yang mengalami PDPH sejumlah 5 responden (15,6%), responden yang memiliki riwayat sakit kepala sebelumnya yang mengalami PDPH sejumlah 5 responden (15,6%), sebagian besar responden yang menggunakan ukuran jarum 25G yang mengalami PDPH sejumlah 9 responden (28,1%). Responden yang mengalami PDPH dengan kategori nyeri ringan sejumlah 2 responden (6,3%), dan nyeri sedang sejumlah 7 responden (21,8%), sehingga dapat disimpulkan faktor usia, riwayat sakit kepala sebelumnya, dan penggunaan ukuran jarum yang berhubungan dengan kejadian PDPH . Penelitian tentang faktor-faktor yang berhubungan dengan kejadian PDPH masih perlu dikembangkan untuk mendapatkan informasi yang lebih luas

Abstract

Background: Patients who undergo cesarean sections typically experience postoperative pain, which can cause stress, hinder movement, and delay recovery. Pain can be managed through pharmacological and non-pharmacological therapies, one of which is the combination of Benson Relaxation and natural sounds. Purpose: To evaluate the effect of combining Benson relaxation and natural sounds on pain levels in patients after cesarean section at Bunda Arif Purwokerto Mother and Child Hospital. Methods: A single-group pre-test design was used, with 32 participants selected randomly. Pain was measured using a Numerical Rating Scale (NRS) before and after the intervention. Pain was reassessed after a 15-minute intervention. Results: Before the intervention, the majority of participants reported moderate pain (31 participants, 96.9%). After the intervention, the majority reported mild pain (30 participants, 93.8%). The Wilcoxon signed-rank test yielded a p-value of 0.001 (<0.05), suggesting the combination of Benson relaxation and natural sounds significantly reduces pain. Conclusion: This combination technique is effective as a complementary therapy for pain management following cesarean section. Relevance to clinical practice: This simple 15-minute intervention can be integrated into routine post-cesarean nursing care to reduce pain and promote comfort. It is low-cost, non-invasive, and easy to deliver, supporting early mobilization and recovery while potentially reducing reliance on analgesics and related side effects.

Abstract

Hypotension and bradycardia are common complications following spinal anesthesia due to extensive sympathetic blockade, which can lead to hemodynamic instability and decreased perfusion of vital organs. A simple non-pharmacological preventive measure is maintaining a sitting position immediately after induction to limit the upward spread of hyperbaric anesthetic agents toward the thoracic region. This study aimed to determine the effect of maintaining a sitting position for three minutes after spinal anesthesia induction on the incidence of hypotension and bradycardia at PKU Muhammadiyah Gombong Hospital. The research employed a quasi-experimental design with a pre–post test with control group approach involving 69 respondents selected through consecutive sampling. The intervention group (n = 34) maintained a sitting position for three minutes, while the control group (n = 35) was immediately placed in a supine position. Blood pressure and heart rate were observed at 5, 10, and 15 minutes after induction, and the data were analyzed using Chi-Square and binary logistic regression tests. The results showed that the incidence of hypotension and bradycardia was significantly lower in the intervention group (p < 0.05), indicating that the sitting position acted as a protective factor. In conclusion, maintaining a sitting position for three minutes after spinal anesthesia induction effectively reduces the incidence of hypotension and bradycardia and is recommended as a simple strategy to enhance patient hemodynamic stability.
