Abstract

Oral health impact profile-14 (OHIP-14) is a method used to measure oral health quality of life (OHRQoL) after radiotherapy. This case report aims to evaluate OHIP-14 before and after head and neck cancer treatment. In case-1, a 51-year-old man complained of an unpleasant sensa-tion in the mouth, loss of taste, and dry lips after undergoing the 17th cycle of radiotherapy. Case-2, a 24-year-old man complained of dry mouth and unpleasant sensation inside the mouth especially when eating and drinking after undergoing 3rd cycle radiotherapy. Both were diagnosed with stage III tongue cancer. The patients were referred to the Department of Oral Medicine. The patients were diagnosed with oral mucositis, acute pseudomembranous candidiasis, exfoliative cheilitis and xerostomia. Patients were treated with mechanical cleaning using gauze soaked in 0.9% NaCl, gargling with 0.025% hyaluronic acid three times daily for oral mucositis, nystatin oral suspension for oral candidiasis, vaseline album and GC dry Mouth® topical commercial gel for dry lips and xerostomia. The OHIP-14 scores of both patients decreased. It was concluded that a decrease in OHIP-14 score indicates successful treatment of head and neck cancer patients with oral complications after radiotherapy.

Abstract

This article evaluates the DASS-21 and SXI-ID questionnaires to assess the level of anxiety and xerostomia in the management of SAR. A 23-year-old male with ulcers surrounded by areas of erythema, accompanied by pain in the right buccal mucosa and left lateral tongue that had not been treated; also accompanied by discomfort and dryness in the oral cavity. Quantitative assess-sment in the form of the depression, anxiety, and stress scale (DASS-21) questionnaire was conducted four times during one month, determine the patient's state of dealing with heavy work. The diagnosis of xerostomia was established with the summated xerostomia inventory-Indonesian version (SXI-ID) questionnaire. Pharmacological therapy in the form of 0.2% hyaluronic acid mouthwash for 3 times 10 mL, as well as a behaviour modification approach. It was concluded that periodic evaluation using DASS-21 can identify behavioural changes related to the healing process of SAR. SXI-ID can be a psychometric instrument in patients with xerostomia. Behavioural management and symptomatic management play a role in SAR healing.

Abstract

Introduction: Traumatic Ulcerative Granuloma with Stromal Eosinophilia (TUGSE) is a type of chronic traumatic ulcer. There are several factors that contribute to the development of TUGSE, but trauma is the primary one. If smoking or other risk factors are present, the condition may be exacerbated, as this may impede the healing of wounds. This case report will show the influence of smoking cessation on the recovery from TUGSE in patients and the resulting improvement in quality of life. Case report: A 64-year-old male patient presented to the dental hospital with a primary complaint of a painful sore on the tip of his tongue at the back for the past month. The patient had previously consulted a general practitioner and an ear, nose, and throat (ENT) specialist and was prescribed triamcinolone acetonide and povidone iodine mouthwash but no improvement was observed. An intraoral examination revealed an ulcer on the lateral side of the tongue. The patient had a cavity with sharp edges near the lesion, but it was ground down 2 days ago. The patient had a history of smoking clove cigarettes, consuming 24 sticks per day for more than five years. The patient was given chlorhexidine gluconate 0.2% mouthwash, to be used at a dose of 10ml, twice a day, and was instructed to apply a thin layer of triamcinolone acetonide 0.1% in orabase three times a day on the lesion. The patient recovered after seven days of treatment and reported their symptoms have decreased. The assessment using the Oral Health Impact Profile-14 (OHIP-14) indicated an improvement in the patient quality of life. Conclusion: Smoking cessation can improve the healing process of ulcers, thereby improving an individual's quality of life.

Abstract

Introduction: Oral frailty is a geriatric syndrome characterised by a decline in oral function associated with a decline in cognitive and physical function. Virtual reality hypnosis (VRH) is a current method that combines the benefits of virtual reality (VR) technology and hypnosis. Aim: This case report aims to describe the role of VRH in the management of elderly patients with oral frailty. Case and Management: A 65-year-old male patient came to the Oral Medicine Department with a chief complaint of dry mouth for the past three months. The patient often felt stressed because of his health problems. Intraoral examination showed frothy saliva, white plaque on the buccal and lateral mucosa of the tongue, missing teeth, and poor oral hygiene. Some additional examinations performed included the Indonesian Version of Summated Xerostomia Inventory (SXI-ID), Clinical Oral Dryness Score (CODS), sialometry, Depression Anxiety Stress Scale (DASS-21), and D-E-N-T-A-L oral frailty questionnaire. The test results showed that the patient had oral frailty. Case Management: Pharmacological therapy consisted of chlorine dioxide mouthwash, sucralfate syrup, and probiotic caplets. Non-pharmacological therapy consisted of instructions to maintain good oral hygiene, adopt a healthy lifestyle, and VRH. The relaxing effect of VRH was evidenced by the recording of increased alpha waves on quantitative electroencephalography (qEEG) during the procedure, and the results of SXI-ID, CODS, and DASS-21 at the last visit were normal. Oral frailty resolved after two weeks of treatment. The patient was also referred for oral care from the Departments of Periodontia, Prosthodontia, and Oral Surgery. Conclusion: VRH has an important role especially in the management of stress-induced xerostomia and pain in elderly patients with oral frailty.
