Abstract

Anterior cruciate ligament (ACL) injury is one of the most common musculoskeletal injuries. Evaluating the clinical outcomes of the ACL reconstruction (ACLR) procedure is essential to provide information regarding the benefits of this procedure. This study aimed to assess the clinical outcomes of patients who underwent ACLR at Prof. Dr. R. D. Kandou Hospital Manado. This was a retrospective study. Secondary data were collected from all athlete patients who underwent ACLR. The ACLR procedure was performed by a single operator following the standard arthroscopic procedure. All grafts used Hamstring grafts (gracilis and semitendinosus). Suspense fixation was done using a button, while tibial fixation was done with interference fixation (bioabsorbable screw). Patients were evaluated over a short period, specifically at three months, six months, and nine months. Outcome parameters were assessed using the Visual Analog Scale (VAS), Range of Motion (ROM), Lachman test, Anterior Drawer test, and Knee Functional Score (KFS). Any complications that arose were documented. The results showed that among 21 patients at Prof. Dr. R. D. Kandou Hospital Manado, it was found that ACL injuries were more common in physically active men, especially in the age group of 26-30 years. Most patients who underwent ACLR had serious injuries that required immediate treatment. Hamstring graft was the most common choice (85.7%). Knee stiffness was the most common complication and required special attention during the rehabilitation program. Based on KFS, most patients successfully achieved good knee function recovery. There was a decrease in the VAS score reflecting the effectiveness of the procedure in reducing postoperative pain. There was an increase in the KSS reflecting the success of ACLR in restoring knee stability and function to almost normal levels. In conclusion, anterior cruciate ligament reconstruction in sport participants showed significant improvements in knee mobility and pain reduction, with low complications. Abstract

Abstract:  Femoral neck fracture is a serious injury to the upper femur that is very common in the elderly population due to falls or trauma. To restore patient mobility, prompt and timely treatment is an important factor in the management of this condition. One of the methods used to measure the functional outcome of these patient is the Harris hip score (HHS). This study aimed to compare the results of HHS on the functional outcome of the neglected femoral neck fracture patients with those who received early treatment at Prof. Dr. R. D. Kandou Hospital Manado. This was a retrospective, observational, and analytical study with a cross-sectional design using medical record data. Data were analyzed using the Fisher’s exact test through the SPSS Statistical Version program. There was no significant difference between the HHS of the neglected femoral neck fracture group and the group that received early treatment (p=0.14). In conclusion, there is no significant difference in the functional outcome of the patients in the early treatment and neglected groups. Keywords: neck femoral fracture; Harris hip score; neglected fracture; early treatment   Abstrak: Fraktur leher femur merupakan cedera serius pada tulang paha bagian atas yang sangat umum terjadi pada populasi lanjut usia akibat jatuh ataupun trauma. Dalam upaya untuk mengembalikan mobilitas pasien, penanganan yang diberikan dengan cepat dan tepat waktu menjadi faktor penting dalam manajemen kondisi ini. Salah satu metode yang digunakan untuk mengukur luaran fungsi panggul ialah Harris hip score (HHS). Penelitian ini bertujuan untuk membandingkan hasil HHS pada luaran fungsional pasien fraktur leher femur yang neglected  dengan yang mendapatkan early treatment di RSUP Prof Dr. R. D. Kandou Manado. Jenis penelitian ialah  analitik observasional retrospektif dengan desain potong lintang menggunakan data rekam medik.. Analisis data dilakukan dengan Fisher’s exact test menggunakan program SPSS Statistic Version. Hasil penelitian mendapatkan bahwa tidak terdapat perbedaan bermakna antara hasil HHS kelompok neglected dengan kelompok yang mendapatkan early treatment (p=0,14). Simpulan penelitian ini ialah tidak terdapat perbedaan bermakna pada luaran fungsional pasien kelompok early treatment dan neglected. Abstract

Total hip replacement is one of the most common and effective orthopedic surgical procedures to reduce pain and improve hip function in patients with severe conditions. This procedure has undergone many advances in the past few decades. However, there is debate among surgeons about which surgical approach is the most optimal to apply. This study aims to systematically assess and compare the outcomes of different surgical approaches in total hip replacement (THR) procedures. The method used in this study was a systematic literature review, in which literature was searched through academic databases such as PubMed, Scopus, and Google Scholar using relevant keywords. The collected data were then analyzed through three stages, namely data reduction, data presentation, and conclusion drawing. The results of this study show that each surgical method for total hip replacement has different advantages and disadvantages. The choice of surgical approach should be tailored to each patient's specific condition. Based on a review of the available literature, the anterior approach is often considered superior to the other approaches. The advantages include less surgical trauma, better postoperative pain reduction and a faster rehabilitation process. The anterior approach is considered safe, reliable and effective, and results in lower pain levels compared to other methods

Abstract

Osteoarthritis of the medial compartment of the knee is one of the common degenerative diseases among the adult population, causing chronic pain, stiffness, and limited joint function. High Tibial Osteotomy (HTO) has become an important treatment option to address deformity and reduce symptoms in patients with varus osteoarthritis. Objectives: This study aimed to conduct a systematic review of the existing literature to evaluate the efficacy of High Tibial Osteotomy in the treatment of osteoarthritis of the medial compartment of the knee. A literature search was conducted through PubMed, Google Scholar, and Cochrane Library databases for studies that met the inclusion criteria. Clinical outcomes, including improvement in pain, joint function, and radiological changes, were extracted and synthesized to provide a comprehensive picture of the efficacy of HTO. The results of the systematic review showed that High Tibial Osteotomy significantly reduced pain, improved joint function, and slowed disease progression in most patients with osteoarthritis of the medial compartment of the knee. Despite variations in surgical technique and reported clinical outcomes, HTO is generally considered an effective and sustainable procedure in the management of varus osteoarthritis. High Tibial Osteotomy shows strong promise as an important treatment option for patients with medial compartment osteoarthritis of the knee. However, it is important to take into account individual patient characteristics and consider additional therapies to maximize long-term outcomes. Further research is needed to deepen our understanding of this technique and to optimize its benefits in the treatment of osteoarthritis.

Abstract

Burnout is a condition of emotional exhaustion, depersonalization, and reduced work performance caused by chronic stress. Burnout can occur in various people, including medical students. The purpose of this study was to review the literature on burnout in medical students. This study used a literature research method. Research data were collected using literature search techniques. Research data were obtained from various sources, namely journal articles, books, and research reports. The research data were analyzed using qualitative analysis techniques. The results showed that burnout in medical students is a serious problem. Burnout can occur in medical students at various levels of education, ranging from early semester students to final semester students. Factors that can cause burnout in medical students include high academic loads, demands to be perfect doctors, and stressful working conditions. The negative impacts of burnout on medical students include impaired physical and mental health, decreased academic performance and lower chances of getting a job.
