Abstract

Introduction: Alzheimer's disease (AD) is a neurodegenerative disorder characterized by cognitive decline and memory impairment. While amyloid plaques have been a central focus of AD research, increasing evidence suggests that tau oligomers play a crucial role in disease progression. This study aimed to investigate the relationship between tau oligomers, cognitive function, and disease severity in AD patients in Medan, Indonesia. Methods: An observasional case series study was conducted involving 50 AD patients diagnosed according to the National Institute on Aging-Alzheimer's Association (NIA-AA) criteria. Cerebrospinal fluid (CSF) samples were collected and analyzed for tau oligomers using an enzyme-linked immunosorbent assay (ELISA). Cognitive function was assessed using the Mini-Mental State Examination (MMSE) and the Clinical Dementia Rating (CDR) scale. Correlation analyses were performed to examine the relationship between tau oligomer levels, cognitive performance, and disease severity. Results: The mean tau oligomer level in AD patients was 120.5 ± 35.2 pg/mL. A significant negative correlation was observed between tau oligomer levels and MMSE scores (r = -0.65, p < 0.001), indicating that higher tau oligomer levels were associated with poorer cognitive performance. Furthermore, tau oligomer levels were positively correlated with CDR scores (r = 0.58, p < 0.001), suggesting a link between tau oligomers and disease severity. Conclusion: This study provides evidence for the involvement of tau oligomers in AD progression in the Indonesian population. Elevated CSF tau oligomer levels are associated with cognitive decline and disease severity in AD patients. These findings highlight the potential of tau oligomers as a therapeutic target and emphasize the need for further research to develop effective interventions.

Abstract

Introduction: Late-life depression (LLD) is often associated with cognitive impairment and structural brain changes, particularly white-matter hyperintensities (WMH). This longitudinal study investigated the relationship between WMH burden, cognitive decline, and depressive symptoms in a cohort of older adults with LLD in Medan, Indonesia. Methods: A prospective, longitudinal study was conducted with 120 participants aged 60 years and older. Participants underwent baseline and 3-year follow-up assessments, including structural MRI, neuropsychological testing, and depression severity. Statistical analyses included mixed-effects models to examine longitudinal changes and correlations. Results: At baseline, the LLD group exhibited significantly higher WMH volume compared to controls (p < 0.001). Over the 3-year follow-up, the LLD group showed a significantly greater increase in WMH volume (average increase of 0.4 Fazekas points) compared to controls (average increase of 0.1 Fazekas points, p < 0.001). Greater WMH burden at baseline was associated with worse performance on all cognitive domains in both groups (p < 0.05). In the LLD group, the increase in WMH volume was significantly correlated with a decline in global cognition (r = -0.45, p < 0.001), executive function (r = -0.38, p = 0.003), and processing speed (r = -0.41, p = 0.001). Changes in depression severity were also correlated with WMH progression (r = 0.32, p = 0.012). Conclusion: This study provides evidence that WMH burden is significantly increased in LLD and that WMH progression contributes to cognitive decline and may exacerbate depressive symptoms over time. These findings highlight the importance of assessing and potentially targeting WMH in the management of LLD.

Abstract

Introduction: Detecting malingered psychiatric symptoms presents a significant challenge in Indonesian forensic evaluations, potentially impacting justice and resource allocation. Current methods rely heavily on clinical judgment and psychometric testing, lacking objective biomarkers. This study explored the potential of combining functional magnetic resonance imaging (fMRI) with machine learning (ML) to identify neural patterns differentiating malingered from genuine psychiatric symptoms in an Indonesian forensic context. Methods: This case-control study included 90 Indonesian male participants referred for forensic psychiatric evaluation (visum et repertum psychiatricum): 30 diagnosed genuine psychiatric patients (schizophrenia/psychotic depression), 30 individuals identified as malingerers, and 30 healthy controls. Participants underwent clinical assessment, psychometric testing (including symptom validity tests - SVTs), and an fMRI scan using a symptom-endorsement paradigm designed to probe cognitive control and deception-related neural activity. Preprocessed fMRI data were analyzed using group-level GLM and machine learning (Support Vector Machine - SVM; Random Forest - RF) classifiers trained on extracted features (ROI activation, functional connectivity) to distinguish malingerers. Performance was evaluated using k-fold cross-validation. Results: fMRI results indicated significantly greater activation in the malingering group compared to genuine patients and controls in prefrontal (dlPFC, vlPFC) and anterior cingulate cortex (ACC) regions during feigned symptom endorsement (p<0.001, FWE-corrected). An SVM classifier using combined ROI activation and functional connectivity features achieved the highest accuracy (83%), sensitivity (80%), specificity (86%), and AUC (0.88) in distinguishing malingerers from genuine patients. Conclusion: These findings suggest that integrating fMRI and ML techniques holds promise as an objective, supplementary tool for detecting malingered psychiatric symptoms within Indonesian forensic evaluations. While promising, the moderate accuracy highlights the need for further validation, consideration of ethical implications, and adaptation to the Indonesian context before any potential clinical application.

Abstract

Livestream commerce (LSC) has redefined digital retail in Southeast Asia, with Indonesia as its most competitive market. The two dominant platforms, Shopee Live and TikTok Shop, leverage vastly different user interfaces and engagement philosophies—commerce-first versus content-first, respectively. However, the precise cognitive and affective mechanisms by which these platforms guide consumer attention and trigger impulse purchases remain empirically unexamined. This study employed a within-subjects laboratory experiment with 60 Indonesian consumers (aged 18-25). A multi-modal neuromarketing approach was used, synchronizing eye-tracking (ET) and electroencephalography (EEG) data. Participants viewed six 60-second LSC clips (three from Shopee Live, three from TikTok Shop) matched for product category. Key eye-tracking metrics (Total Fixation Duration, Time to First Fixation) were analyzed across predefined Areas of Interest (AOIs: Host Face, Product, Price, CTA Button, Chat). EEG data was processed to derive Frontal Alpha Asymmetry (FAA) for approach-avoidance motivation and Cognitive Load indices. Post-stimulus surveys measured Impulse Buying Urge (IBU). Significant differences emerged. Shopee Live generated longer Total Fixation Duration on the Host’s Face (M=12,500ms) and Price/Discount AOIs (M=8,800ms). Conversely, TikTok Shop elicited significantly faster Time to First Fixation on the Product (M=1,600ms) and CTA Button (M=2,800ms), and higher TFD on these AOIs. Neurologically, TikTok Shop produced significantly greater FAA (M=0.19 vs. 0.08), indicating higher approach motivation, and also induced a higher cognitive load. A multiple regression analysis revealed that the strongest predictors of IBU were TFD on the CTA Button, FAA, and TFD on the Host’s Face. TFD on the product itself was not a significant predictor. In conclusion, the findings demonstrate that platform architecture fundamentally shapes the "live" gaze. Shopee Live fosters a deliberative, host-centric, and price-evaluative attentional strategy. TikTok Shop promotes a rapid, immersive, and conversion-focused gaze, driving higher affective engagement (approach) and subsequent impulse buying. The study provides novel evidence that in LSC, impulse triggers are tied more to conversion-point (CTA) and para-social (Host) cues than to the product itself.

Abstract

Introduction: Rapid urbanization in Indonesia has necessitated a shift from horizontal settlements (kampung) to vertical public housing (Rusunawa). While providing shelter, these environments may exacerbate "crowded isolation”. This study investigates the synergistic impact of loneliness, depression, and vertical living constraints on all-cause mortality among the elderly. Methods: We conducted a multi-center, prospective cohort study (2018–2023) involving 1,450 elderly residents (> 60 years) in Jakarta and Surabaya. Loneliness was assessed annually using the UCLA Loneliness Scale, and depression via the GDS-15. The primary environmental exposure was "vertical constraint," defined as living above the 3rd floor (Floors 4–12). We utilized Kaplan-Meier survival curves and Multivariable Cox Proportional Hazards models with time-dependent covariates to estimate Hazard Ratios (HR), adjusting for sociodemographic factors and a composite Socioeconomic Status (SES) index. Results: Of 1,398 baseline participants, 218 deaths occurred over 6,890 person-years (15.6% mortality). Severe loneliness was an independent predictor of mortality (Adjusted HR: 1.89; 95% CI: 1.42–2.51). A significant interaction was observed between loneliness and vertical constraint (p=0.003). While residents on higher floors had lower baseline mortality due to selection bias (the "healthy resident effect"), loneliness in this subgroup amplified mortality risk substantially (Interaction HR: 1.76; 95% CI: 1.21–2.55), effectively negating their physical survival advantage. Conclusion: Loneliness is a potent predictor of mortality in Indonesian vertical slums, with lethality accelerated by the physical entrapment of high-floor living. Structural interventions are urgently required to mitigate this "vertical isolation."
