Abstract

Introduction: Maintaining teeth in their corrected positions following orthodontic treatment can be extremely challenging. Coffee is one substance whose effects on relapse have been extensively studied. The aim of study is to analyze the effect of coffee administration on relapse after orthodontic retention by evaluating alkaline phosphatase levels. Methods: This experimental laboratory study enrolled SpraqueDawley rats, with brackets applied using Edgewise slot 0.022” system and a nickel-titanium open coil spring fixed between the lower central incisors for 14 days. Based on Federer’s formula, 36 SpraqueDawley rats were randomly divided by simple random sampling into four groups: Group 1 (no retention), Group 2(retention for 3 days), Group 3(retention for 7 days), and Group 4(retention for 2 weeks). The Spraque-Dawley rats in the four experimental groups were given aquadest, 50 mg, and 100 mg/kg coffee intake ad libitum. Orthodontic relapse was assessed by measuring changes in alkaline phosphatase levels between the lower central incisors (the relapse side). Changes in the distance between the central incisors during relapse were observed visually. Alkaline phosphatase levels, which serve as a predictor of recurrence, were determined using an enzyme-linked immunosorbent assay (ELISA). Data analysis was conducted with the Two-way analysis of variance (ANOVA) and a post hoc Tukey test. Results: Overall, we found that a longer retention period was associated with a slower rate of relapse and a shorter overall amount of relapse. In addition, inhibiting osteoclast formation using coffee also reduced orthodontic relapse. Alkaline phosphatase (ALP) and/or its biomarkers could serve as potential therapeutic targets in the prevention and treatment of orthodontic relapse, with changes observed at a value of 0.001. This demonstrates that coffee administration affects alkaline phosphatase levels during relapse after orthodontic retention. Conclusion: Coffee administration increased alkaline phosphatase levels during relapse after orthodontic retention in rats.

Abstract

Background: Irreversible pulpitis is an inflammatory process in the tooth that involves the pulp. Teeth that experience inflammation in the pulp require root canal treatment to eliminate bacteria. Post root canal treatment teeth with minimal loss of healthy tissue can be restored with direct composite. The purpose of this case report is to convey the success of multiple root canal treatment in cases of irreversible pulpitis with direct restoration. Case: A 47-year-old female patient came to RSGM Unimus, complaining of frequent toothache when eating cold food and in the morning. Intraoral examination showed blackness on the cervical of tooth 14 with positive sondation. Radiographic examination showed radiolucency on the cervical buccal of tooth 14 with pulp depth. Conclusion: The treatment performed showed good results, with root canal treatment in cases of irreversible pulpitis and direct restoration, so that it can maintain teeth, prevent further infection, provide good aesthetics and provide satisfaction to patients.

Abstract

Background: Sodium hypochlorite produces free oxygen and free radicals that cause damage to dentin collagen. Combination with Sodium thiosulfate with the aim of improving root canal hygiene and restoring the oxidizing effect on the dentin surface. The apical third of the root canal is the most difficult part, due to its narrower anatomy, curvature and frequent branching of the root canal. Method: The study used 30 specimens which were divided into 3 groups, group I was irrigated with 2.5% NaOCl, 5% sodium thiosulfate and saline, group II was irrigated with 2.5% NaOCl, 10% sodium thiosulfate and saline and group III was irrigated with 2.5% NaOCl and saline. The tooth length 14 mm measured from apical to crown. The crown down technique was prepared using the M3 Pro gold file. Observation of the level of cleanliness of the root canal was carried out using a Scanning Electron Microscope (SEM) with 5000x magnification at apical third . The results of micrographic photographs are then given a score of 1-4.Result: Irrigated with 5% and 10% sodium thiosulfate showed no significant difference. Conclusion: The differences is not significant, 2,5% irrigation with sodium thiosulfate resulted in a higher cleanliness apical third of the root canalthan 2,5% NaOCl irigation with saline
