Abstract

Background: Workload is, by definition, that which involves the existence of certain tasks imposed with certain deadlines; excess performance may sometimes lead to boredom. Life in an intensive care unit and operating room is quite labor-intensive. Interpersonal skill management is crucial for anesthetists in maintaining efficiency and avoiding burnout. Stress and tension are brought about by workloads, and indeed anesthetists experience some bodily and mental fatigue in performing their jobs. The administrators of anesthesia are health care personnel charged with overseeing the anesthetic care of a patient during anesthesia management, which includes before, during, and after anesthesia. Purpose: The aim of this study was to evaluate the workload of anesthetists working in the Banyumas area. Methods: The descriptive survey design in its combination of qualitative and quantitative approaches casts upon a cross-sectional design of this study. A total of 47 respondents were taken as samples; a complete sampling method was adopted. A specifically designed questionnaire was administered to respondents for data collection constituting 12 closed-ended questions. Results: It was found that three participants (6.4%) reported no workload, ten participants (21.3%) were considered to be under light workload, twenty-five participants (53.2%) worked under medium workload conditions, and nine participants (19.1%) were given heavy workload. Conclusion: In general, anesthetists working in the central surgical installation room at Banyumas District Hospital are assessed to be under a light workload.

Abstract

Anesthesia is required in surgery to overcome pain and induce loss of consciousness. As many as 80% of patients who undergo surgery use general anesthesia (Siregar et al., 2023). Restoration of consciousness after general anesthesia can cause physiological stress, so careful monitoring is essential (Olfah et al., 2019). A delay in consciousness within 30-60 minutes after general anesthesia is called delayed awakening, which can involve hypoactive consciousness and awakening with delirium (Cascella et al., 2020). One of the interventions to accelerate the recovery of consciousness is to implement a 30° head up position to accelerate recovery. The purpose of Community Service regarding the implementation of the 30° head up position can be carried out to accelerate recovery and see the characteristics of postoperative patients at the Jatiwinangun Purwokerto Surgical Special Hospital. The first monitoring carried out was data collection, patient preparation, implementation, monitoring and evaluation This method was carried out to 30 participants by positioning the patient's head 300 times higher than the bed. then given the position for 15 minutes and then measured the recovery assessment score using the Aldrete score. The general criteria assessed were Motor, respiration, circulation, consciousness, and saturation. The conclusion of the implementation of head up 300 is able to accelerate the recovery period of patients after general anesthesia at the Jatiwinangun Surgical Hospital Purwokerto in 2024. The output of this Community Service is in the form of a journal.

Abstract

Purpose: This study aimed to identify the incidence of hypotension after spinal anesthesia induction in SC patients during intra-anesthesia based on Body Mass Index (BMI), age, and baseline systolic blood pressure at Juanda Kuningan Hospital. Methods: This study used a quantitative descriptive method with a cross sectional approach. The sample in this study were all SC patients with spinal anesthesia techniques who met the criteria. The sampling technique used was purposive sampling with the slovin formula resulting in 90 respondents. And data analysis using univariate analysis. Findings: The results showed that the majority of respondents who experienced mild hypotension with excessive BMI status were 50 respondents (58.8%), the majority of respondents who experienced mild hypotension with early adulthood were 52 respondents (61.2%) and the majority of respondents who experienced mild hypotension with initial systolic blood pressure were at initial systolic blood pressure <120 mmHg and 120-130 mmHg as many as 31 respondents (36.5%). Conclusions: The results of this study illustrate that the majority of respondents will experience hypotension after spinal anesthesia induction. This is because spinal anesthesia can block sympathetic nerves, resulting in a decrease in circulating arterial tone, this condition causes peripheral arterial vasodilation. This vasodilating effect can result in a decrease in vascular resistance (hypotension). For further researchers to be able to develop this study to look for other variables that may be associated with the incidence of hypotension after induction of spinal anesthesia in SC patients so that the results of the study will show more variation.

Abstract

Purpose: Determine the effect of progressive muscle therapy on the anxiety level of pre-anesthesia patients at RSI Banjarnegara. Methods: This type of research is pre experiment pre test and post test with one group design. The sample technique in this study used purposive sampling of 37 pre-anesthesia patients. Data were taken using measurements of anxiety before and after progressive muscle therapy. This research instrument used questionnaires and SOP for progressive muscle therapy. Result: The anxiety level of respondents before being given progressive muscle therapy was the majority of 26 respondents (70.3%) experienced moderate anxiety. The anxiety level of respondents after being given progressive muscle therapy was the majority of 20 respondents (54.1%) experienced mild anxiety. The results of data analysis in this study with paired t test showed a value of (p) 0.000 ˂ 0.05. Conclusion: This shows that there is a significant influence on the provision of progressive muscle therapy at RSI Banjarnegara. Based on the results of the study, it is expected that the Banjarnegara RSI service will use progressive muscle therapy to reduce pre-anesthesia anxiety levels

Abstract

Background: Spinal anesthesia is said to be safer than general anesthesia. However, spinal anesthesia itself has several kinds of special obstacles in performing injection techniques, there are a number complex variables that can cause these obstacles, including difficulty in marking injections, failure in needle placement, age, Body Mass Index (BMI), spinal anatomical abnormalities, patient position at the time of needle placement, and the level of experience of spinal performers. Purpose: This study was to determine the description of spinal anesthesia injection techniques at RSI Fatimah Cilacap. Methods: This research method uses a quantitative approach with a descriptive type of research. Total sampling was used to select the sample for this study. Total sampling is a sampling method that uses the sample size of the entire population as the sample size. Results: this study indicate that the median injection technique is more dominant in using the median injection technique with the number of respondents as many as 23 (62.1%), while with the paramedian injection technique only 14 (37.8%) respondents based on the age of the respondent, while based on the BMI of the respondent, anesthesia stylists are more dominant in using the median injection technique as many as 23 (62.1%) respondents, on the contrary with the paramedian injection technique as many as 14 (37.8%) respondents. Conclusions: this study shows that anesthesiologists at RSI Fatimah Cilacap more often use spinal anesthesia injection techniques with median techniques compared to paramedian injection techniques at certain ages and Body Mass Indexes.

Abstract

Background: Hemodynamic observation during spinal anesthesia is necessary to assess the status of a patient's cardiovascular system. Hemodynamic decline is one of the most common acute complications that can occur in spinal anesthesia. Purpose: to determine the description of intra-anesthesia hemodynamic status in patients with spinal anesthesia at Morowali Hospital. Methods: This research design uses descriptive design. The sampling technique with total sampling obtained 85 patients who performed spinal anesthesia. The research instrument contains monitoring the patient's vital signs including systole blood pressure, diastole, pulse frequency, and MAP value. Results: The age of patients is most with early adulthood at 33 (38.8%) and the least with the elderly category at 2 (2.4%). The gender of the respondents was mostly female with 51 (60%). The type of surgery with the majority with major surgery amounted to 51 (60%) and the most ASA status with ASA 2 amounted to 74 (87.1%). Conclusion: hemodynamic status based on systole and diastole blood pressure in the normal category was 63 (74.1%), based on pulse frequency the majority with normal status was 58 (68.2%), based on MAP values the majority with normal was 53 (62.4%).
