Abstract

Anestesi spinal dapat melebarkan pembuluh darah dan menurunkan curah jantung. Hal ini disebabkan oleh blok simpatis yang menurunkan tekanan darah. Situasi ini terjadi dalam beberapa menit awal setelah pemberian. Oleh karena itu, tekanan darah harus dipantau untuk mencegah masalah yang lebih parah. Penelitian ini meneliti hipotensi pada pasien anestesi spinal. Penelitian kuantitatif cross-sectional ini melibatkan 65 partisipan. Hasil analisis uji deskriptif menunjukkan bahwa responden didominasi oleh lansia akhir (41,5%), laki-laki (78,5%), dengan indeks massa tubuh rata-rata (63,1%), dan dalam kondisi ASA II (43,1%). Tergantung pada kondisi pasien selama intra anestesi, anestesi spinal dapat menyebabkan hipotensi pada menit ke-5. Temuan penelitian ini dapat membantu mencegah hipotensi dengan mengungkapkan penyebabnya. Penelitian di masa depan harus meneliti dosis obat, tinggi blok tulang belakang, dan faktor lingkungan yang dapat menyebabkan hipotensi.

Abstract

Background: Every patient has different views and reactions to surgery,one of which is experiencing anxiety. Anxiety can be influenced by several characteristics, including age, gender, education, work situation, and marital status. The purpose of this study was to determine whether there is a relationship between patient age and anxiety level in elective surgical patients at Goeteng Taroenadibrata Hospital. Methods: This study used an analytical correlation research method with a quantitative approach. The sampling technique used Consecutive Sampling and 79 pre-anesthesia respondents were sampled for this study. The testused in this study is Kendall Tau Result: The results of the correlation test obtained a significance value for the level of anxiety with a P-value of 0.002 <0.05, and the majority of respondents experienced a moderate level of anxiety with a total of 42 respondents. Implications: From the results of the study, it can be seen that there is a correlation between age and anxiety levels of adult surgical patients at Goeteng Taroenadibrata Purbalingga Hospital. The findings of this study, it is expected to improve pre-operative care for patients in emergencies such as providing preoperative education to reduce anxiety in surgery patients. Abstract

Fenomena persalinan secara section caesarea (SC) cukup tinggi terjadi di Indonesia, hasil Riskesdas tahun 2019 menunjukkan kelahiran dengan section caesarea (SC) sebesarr 17,6% dengan proporsi tertinggi di sumatera utara 23,9% dan terendah di papua 6,7%. .Kecemasan pada ibu hamil yang akan dilakukan section caesarea (SC) bermacam-macam alasan diantaranya cemas menghadapi ruang operasi dan peralatan operasi, terjadinya perubahan fisik, takut di bius, operasi gagal, masalah biaya yang membengkak dan cemas kalau bayinya meninggal. Salah satu cara mengatasi kecemasan adalah dengan pemberian komunikasi terapeutik metode helping relationship dan self efficacy. Tujuan : perbedaan efektivitas komuniksi terapeutik metode helping relationship dan self efficacy terhadap penurunan kecemasan pre-operasi pada pasien section caesarea (SC). Metode: penelitian ini kuantitatif, quasy experiment dengan menggunakan desain Pretest-Posttest Control Group design, dengan jumlah responden 80. Penelitian dilakukan pada 23 juli 2024 - 7 Agustus 2024 di RSUD Dr Soedirman Kebumen. Instrumen menggunakan Amsterdam Preoperative Anxiety and Information Scale (APAIS) dengan analisis statistik menggunakan uji Wilcoxon dan uji Mann Whitney-U. Hasil: untuk hasil helping relationship dan self efficacy keduanya tidak ada perbedaan, namun hasil mean rank lebih rendah helping relationship.

Abstract

Background: Over the past decade, the prevalence of diabetic foot ulcers (DFUs) has notably increased, posing a significant risk of foot amputation in individuals with diabetes. Purpose: This systematic review and meta-analysis aimed to determine the rates of amputation associated with DFUs in patients with diabetes mellitus. Methods: A comprehensive literature search was conducted across multiple databases, including Scopus, PubMed, Cochrane, Web of Science, DOAJ, and Google Scholar. Full-text articles related to diabetic foot ulcer amputation were reviewed and extracted for analysis. Publication Bias: Assessed using a funnel plot in Jamovi (2.0.0), indicating a likelihood of publication bias. Meta-analysis Summary: Conducted using the Joanna Briggs Institute methodology, yielding a pooled meta-analysis result of 0.027. Odds Ratio (OR): Calculated for amputation risk in DFU patients, with a 95% Confidence Interval (CI). Random Effects Model: Employed to synthesize relevant outcomes. Heterogeneity: Evaluated using the I² statistic. Forest Plots: Generated to visualize study results. Results: Eighteen studies were included in the meta-analysis, encompassing a total of 7115 DFU patients in the Asian region. Among these patients, 2730 underwent foot amputations. Amputation Rate: The incidence of foot amputation was significantly higher among DFU patients. Pooled Odds Ratio: The OR for foot amputation in DFU patients was 0.34 (95% CI: 0.23-0.46). Publication Bias: The asymmetrical funnel plot suggested a high likelihood of publication bias. Conclusion: The presence of DFUs significantly elevates the risk of foot amputation in diabetic patients. Continuous monitoring and preventive strategies are essential to mitigate this risk, particularly in the Asian population.

Abstract

Pembedahan adalah tindakan pengobatan menggunakan cara invasif dengan membuat sayatan (insisi) pada permukaan kulit, biasanya diperlukan anestesi untuk memfasilitasi tindakan pembedahan. Tindakan anestesi mempunyai efek samping salah satunya adalah Postoperative Nausea and Vomiting (PONV). Postoperative Nausea and Vomiting (PONV) dapat diobati dengan pengobatan farmakologis dan nonfarmakologis. secara non farmakologi diantaranya aromaterapi, acupressure, distraksi, relaksasi dan mobilisasi. Penelitian ini bertujuan Mengetahui Efektivitas Pemberian Aromaterapi Cajuput oil terhadap Post Operative Nausea and Vomiting (PONV) pada Pasien Elektif dengan General Anestesi di RSUD dr. Soekardjo Kota Tasikmalaya. Penelitian dilaksanakan di RSUD dr. Soekardjo Kota Tasikmalaya. Jenis penelitian ini adalah Quasi Experimental atau eksperimen semu Desain Quasi Exsperimental yang digunakan dalam penelitian ini adalah Pretest Posttest Nonequivalent Control Group Design. Pengambilan sampel menggunakan teknik Total sampling sebanyak 56 responden, yang dibagi menjadi 2 kelompok yaitu kelompok eksperimen dan kelompok kontrol dengan masing-masing kelompok berjumlah 28 responden. Instrumen penelitian menggunakan lembar observasi. Hasil penelitian didapatkan jenis kelamin kelompok intervensi perempuan lebih banyak, kelompok kontrol sebaliknya dimana responden laki-laki lebih banyak. Status fisik ASA II paling banyak baik pada kelompok intervensi maupun kelompok kontrol. Tipe pembedahan, yang mengalami PONV paling banyak dengan operasi kecil pada kedua kelompok. Hasil pre post test kedua kelompok didapatkan perbedaan antara nilai pre-test dan post-test. Terdapat efektifitas Pemberian Aromaterapi Cajaput Oil terhadap Post Operative Nausea and Vomiting (PONV) Pada Pasien Elektif Dengan General Anestesi dengan p-value 0,000 (p<0,05).

Abstract

Anxiety in patients who are about to undergo surgery is usually related to all kinds of foreign procedures that the patient must undergo and also threats to life safety due to surgical procedures and anesthesia. One of the non-pharmacological therapies is relaxation therapy by administering a Butterfly Hug, which is an intervention to reduce anxiety levels in pre-operative patients. This study aims to determine the effect of administering the Butterfly Hug on reducing anxiety in preoperative patients. This type of research is quasi-experimental with a pretest-posttest control group design. The sample in this research was a purposive sampling of 66 respondents. Data was taken using the HARS anxiety measurement. From this study, the Wilcoxon test results were obtained with a mean difference intervention value of -10.7 (p) 0.001. And the control group with a mean difference value of -3.85 with (p) 0.001. Mann Whitney U test with a mean difference value of -6.00 (-8.93, -3.13) with (p) 0.001. The results of the study showed that there was a difference before and after administering the Butterfly Hug on preoperative patient anxiety at the Jatiwinangun Special Surgery Hospital, Purwokerto. Giving a Butterfly Hug can be a method that can be used for patients who experience anxiety before surgery.

Abstract

Background: Postoperative pain is caused by wound stimulation that triggers the body to produce chemical mediators that cause pain. Nurses can detect it through patient complaints and physiological responses. Approximately 50% of elective postoperative patients experience pain, which is at risk of increasing chronic pain and reducing satisfaction with health services. One non-pharmacological approach to dealing with this pain is the Benson relaxation technique, which is a development of deep breathing and involves patient beliefs, helping to reduce pain by creating a calm environment and a relaxed body. Objective: The program for implementing postoperative pain management using the Benson relaxation method at RSI Banjarnegara aims to provide a demonstration to postoperative patients with 30 respondents to use the Benson relaxation method to reduce postoperative pain. In addition, this activity aims to increase the knowledge of postoperative patients if pain occurs, patients can overcome it with non-pharmacological therapy independently. Method: This PKM method applies the Benson relaxation technique as an approach to nursing care for postoperative patients who experience pain. Community service activities are carried out by implementing directly by applying the Benson Relaxation technique. The target of this PKM is patients who have undergone surgery at RSI Banjarnegara. Results: The results obtained from the community service at RSI Banjarnegara from 30 respondents who participated in this activity showed the level of pain before being given Benson relaxation therapy: mild pain (40%), moderate pain (50%), and severe pain (10%), with the majority in the moderate pain category (50%). Then after Benson relaxation therapy was given, mild pain increased to 80%, moderate pain decreased to 20%, and severe pain became 0%. Benson therapy significantly reduced the level of pain, from an average of moderate pain (50%) to mild pain (80%). And for the participants of this PKM activity, they also gained knowledge about how to reduce pain with non-pharmacological therapy, namely the Benson relaxation technique. Output: The output of this PKM activity is in the form of leaflets and publication articles that can be used to increase insight and learning at Harapan Bangsa University about how to reduce the intensity of post-operative pain by providing non-pharmacological Benson relaxation therapy.

Abstract

Sectio Caesarea is a procedure used to make an incision in the mother’s uterine wall. In this procedure , surgery and anesthesia are stressful because there is a risk to the integrity of a person’s body and soul because they do not know the actions to be taken. Purpose : Based on this PkM, it is to provide education on spinal anesthesia procedures with audiovisuals is section caesarea’s patients. Methode : The future community service method provides knowledge about education on spinal anesthesia procedures with audiovisuals in section caesarea patients. After being given education about spinal anesthesia, it is hoped that patients will be able to understand the spinal anesthesia procedure. The target of this PkM is pregnant women who will undergo childbirth through the section caesarea technique at RSUD dr. Soedirman Kebumen. Results : The results of this activity show the level of knowledge in patients in the pre-test in the sufficient category with a score of 4-5 and in the post-test it is categorized as good with a score of 6-7. Output : The output of the activity used in the form of audiovisual videos and publication articles which are expected to be able to add knowledge to patients and can be used as learning maerials at Harapan Bangsa University, Purwokerto. The results of this activity can be published in the Dikti Scientific Journal.

Abstract

Background: Cardiac arrest is one of the significant risks among those associated with surgery, interventional procedures, or anesthesia. Basic life support (BLS) is the immediate action taken in critical or emergency situations to save lives. The health care personnel must possess adequate knowledge and ability to handle such emergencies effectively. Anesthesiologists must necessarily be BLS proficient to give an early intervention during emergencies just like any other medical professional. Purpose: This study will evaluate the knowledge and preparedness of BLS among the anesthesiologists of the Central Surgical Installation in Banyumas Regency. Methods: The study used a cross-sectional design. Data were collected using surveys dispatched online to the 47 participants through Google Forms from June 19 to 28, 2024, in Banyumas Regency. Results: The results showed that 32 participants (72.3%) possessed excellent knowledge of BLS; however, 24 of 47 responders (51.1%) felt that they were adequately prepared for BLS action when needed. Conclusion: The study disclosed a positive correlation between BLS knowledge and preparedness to perform BLS among anesthesiologists in the Banyumas Regency, thus stressing continuing education and training to promote preparedness in case of emergencies

Abstract

Background: Hypotension is a frequent complication seen in spinal anesthesia. During spinal anesthesia, hypotension is more common with increasing block height, and an elevated body mass index (BMI) is considered a risk factor. Purpose: The aims of this research were to study the incidence of hypotension associated with spinal anesthesia for cesarean section and to relate block height to BMI. Methods: This is a cross-sectional correlational analytic study. A purposive sampling technique was used in picking 42 participants. The main instrument used for data collection was observation sheets. Results: The Spearman rank test for bivariate data showed that 35.7% of participants with a BMI greater than 25 experienced hypotension. In addition, 64.3% of participants with mild block elevation also developed hypotension. The Spearman rank test indicated a correlation value of -0.483 with a p-value of 0.001, signifying a strong and negative statistically significant relationship between hypotension and elevation of sympathetic block. Conclusion: There exists a significant relation between BMI and incidence of hypotension, with correlation values of -0.461 and a p-value of 0.002, further asserting that both BMI and block height are important characteristics that can be utilized in predicting the hypotension following spinal anesthesia in cesarean section patients
