Abstract

Chronic Renal Failure (CRF) is defined as abnormality of renal function indicated by proteinuria and renal dysfunction at least in periode of 3 months or more progressively toward Terminal Renal Failure. The most common cause was diabetic and hypertension condition. Angiotensin Converting Enzyme Inhibitors (ACE Inhibitors) are drug of choice for hypertension in CRF patients with benefit of decresing proteinuria, used as single medication or in combination with other antihypertension drugs.  The objective of the present study was to evaluate the use of ACE Inhibitors in CRF patient in Dr. Sardjito Hospital in 2005, based on NKF-K/DOQI 2002 and other related literatures. This study was non-experimental research with evaluative-descriptive analysis, and data was retrospectively collected. Result revealed that there were 40 cases (7,83%) of CRF patient using ACE Inhibitors out of total 511 cases diagnosed with CRF.  Evaluation of ACE Inhibitors usage showed that 100% of right indication, 95% right patient, 100% right medication, 95% right dosing, while only 32,26% of right dosing in haemodialytic patients. The effectiveness of ACE Inhibitors in lowering blood pressure was only 7,5% meet the target.

Abstract

Schizophrenia is a severe mental disorder with increasing prevalence. Treatment for schizophrenia is chronic and often leads to Drug-Related Problems (DRPs), especially in hospitalized schizophrenia patients. This systematic review aims to map the prevalence and most common types of DRPs and evaluate the impact of clinical pharmacist involvement in addressing DRPs. A literature search was conducted across six electronic databases (Scopus, PubMed, ScienceDirect, Web of Science, ProQuest, and Springer Nature) for publications published between January 2009 and December 2024. Eleven studies met the inclusion criteria, consisting of nine cross-sectional studies and two quasi-experimental studies, with a total of over 61,000 hospitalized schizophrenia patients. The prevalence of DRPs has been reported to range from 52.8% to 100%, depending on the study design and the DRP identification method. Studies using structured medication reviews reported a higher DRP burden, at 0.75–2.5 DRPs per patient. The most frequently identified types of DRPs include polypharmacy, drug interactions, inappropriate medication selection, and dosing problems (overdose and underdose). QTc prolongation was reported in 2.5–8.26% of patients. Clinical pharmacist involvement was reported in three studies, with physician acceptance of recommendations ranging from 49 to 50%. DRPs are a common problem in hospitalized patients with schizophrenia, and clinical pharmacist involvement has the potential to improve the rationality and safety of drug therapy.

Abstract

ABSTRACT Schizophrenia is a chronic psychiatric disorder characterized by psychotic symptoms that require prompt and effective management. Antipsychotic medications are the mainstay of treatment; however, under certain clinical conditions, they are combined with lorazepam to improve the management of acute agitation. This narrative review aimed to synthesize the available scientific evidence regarding the effectiveness of antipsychotic–lorazepam combination therapy in patients with schizophrenia. A literature search was conducted using Scopus, PubMed, ScienceDirect, and Google Scholar. English-language articles published between 2015 and 2025 were selected based on predefined inclusion and exclusion criteria and were analyzed narratively. Of the 2,092 articles identified, twelve studies met the eligibility criteria. The findings indicated that antipsychotic–lorazepam combination therapy effectively reduced acute agitation and psychotic symptoms, as assessed using the Positive and Negative Syndrome Scale (PANSS), Positive and Negative Syndrome Scale Excited Component (PANSS-EC), Brief Psychiatric Rating Scale (BPRS), Clinical Global Impression (CGI), and Agitation-Calmness Evaluation Scale (ACES). Risperidone–lorazepam demonstrated efficacy comparable to conventional intramuscular therapy with a lower risk of extrapyramidal symptoms, while haloperidol–lorazepam showed efficacy comparable to intramuscular olanzapine. Overall, the available evidence suggests that antipsychotic–lorazepam combination therapy may represent an effective treatment option with an acceptable safety profile for managing psychosis and acute agitation in patients with schizophrenia.

Abstract

Type 2 Diabetes Mellitus (T2DM) remains a major public health concern in Indonesia due to its high prevalence and potential to cause various long-term complications when glycemic control is not adequately maintained. Medication adherence and barriers to treatment are important factors that may influence therapeutic outcomes. This study investigated medication adherence, insulin-related treatment barriers, and their association with glycemic outcomes among patients with T2DM at RSPAU dr. Suhardi Hardjolukito Hospital, Yogyakarta. Employing a cross-sectional approach, this research gathered data between September and October 2024. Criteria for eligibility included adult individuals with T2DM who were treated with oral antidiabetic agents, insulin, or both and provided informed consent. Medication-taking behavior was evaluated by utilizing the Medication Adherence Report Scale (MARS-5) framework, while barriers to insulin therapy were evaluated using a specific questionnaire. Associations between adherence status and glycemic parameters were examined through Fisher's Exact Test. A total of 73 patients participated, most of whom were receiving oral therapy (65.8%), a combination of metformin and glimepiride (19.2%), or metformin and acarbose (11.0%). Among insulin users, the majority had low barriers to insulin use (92.0%). Statistical analysis yielded p-values of 0.038 and 0.000 for the associations between medication adherence and fasting blood glucose as well as HbA1c levels, and for the relationship between adherence status and random blood glucose measurements, a p-values = 0.542 was obtained. The findings demonstrated that therapeutic compliance is significantly correlated with both HbA1c and fasting blood sugar levels among individuals suffering from type 2 diabetes mellitus at RSPAU dr. Suhardi Hardjolukito Hospital, Yogyakarta. These results highlight the contribution of adherence behavior to successful glycemic management. Therefore, healthcare professionals are advised to implement strategies that encourage medication adherence to optimize glycemic control.
