Abstract

Tuberculosis (TB) remains a public health problem with a high risk of transmission, especially among close contacts of patients' families. Low knowledge and social stigma are persistent barriers to prevention. This community service activity aimed to improve family knowledge and preventive behavior through direct home visit-based education. The method used was structured counseling with the Counseling Event Unit (SAP) approach, conducted for families of TB patients in the working area of a Community Health Center (Puskesmas). Sessions included a pre-test, education on transmission routes, prevention measures, cough etiquette, and medication adherence, followed by a post-test for evaluation. Statistical analysis yielded a p-value of 0.000 (< α 0.05), confirming a significant increase in family knowledge and participation in preventive practices — including mask use, ventilation improvement, and medication supervision. Home visit-based mentoring proved effective in raising family awareness and reinforcing their role as medication supervisors (PMO). This activity produced measurable reductions in household transmission risk and strengthened family involvement in community-based TB control

Abstract

Background: Healthcare workers in referral hospitals are frequently exposed to chronic occupational stress due to high workloads and service complexity. Objective: This study aims to develop a stress management model for healthcare workers through a Transactional Theory approach, considering psychosocial epidemiology and social determinants at RSUD Provinsi NTB. Methods: This research employed a Systematic Literature Review (SLR) method, searching literature across Scopus and Google Scholar databases within the 2014–2024 period. Inclusion criteria focused on studies integrating coping mechanisms, social determinants, and referral services. Results: Key findings indicate that occupational stress in referral services is triggered by an imbalance between job demands and job control. From a Transactional Theory perspective, the effectiveness of stress management significantly depends on the staff's secondary appraisal of organizational support. The proposed model emphasizes three intervention levels: modifying social determinants (structural), strengthening cognitive capacity (individual), and providing social support systems (interpersonal). Conclusion: Integrating psychosocial epidemiology into the management policies of RSUD Provinsi NTB is crucial for creating a supportive work environment and sustainably improving the quality of patient care.

Abstract

Background: Health worker retention, particularly among nurses, is a critical challenge for service stability at RSUD Provinsi NTB. As a referral hospital, the nurse turnover phenomenon is influenced by the interaction between environmental determinants and individual professional capacity. Objective: This study aims to analyze the social determinants of the work environment and the epidemiology of health worker retention by exploring the mediating role of organizational commitment on nursing competence. Methods: This study employed a Systematic Literature Review (SLR) method. Literature searches were conducted on Scopus and Google Scholar databases spanning 2016-2026. Primary articles were selected based on strict inclusion criteria using the PRISMA protocol. Results: Findings indicate that positive social determinants of the work environment significantly suppress resignation rates epidemiologically. Nursing competence is proven not to stand alone in influencing retention; organizational commitment mediation (especially affective commitment) is required to transform individual expertise into institutional loyalty. Organizational commitment functions as a bridge that strengthens the relationship between a supportive environment and the nurses' intention to stay. Conclusion: Retention strategies at RSUD Provinsi NTB must focus on strengthening the social work climate and competence development, accompanied by increasing employees' emotional attachment to the organization.

Abstract

Background: Perioperative safety is a crucial aspect of anesthesiology, yet risk assessments often focus solely on conventional clinical indicators. At RSUD Provinsi NTB, the diverse social profiles of patients necessitate a more comprehensive assessment model. Objective: This study aims to identify the interaction between clinical epidemiology and social determinants of health in influencing perioperative safety and to formulate a basis for developing an Initial Assessment model to prevent anesthesia complications. Methods: This research uses a Systematic Literature Review (SLR) design. Literature searches were conducted through Scopus and Google Scholar databases with a publication range of 2014-2024. Inclusion criteria focused on studies linking clinical and social variables with surgical/anesthetic outcomes. Results: The review findings show that social determinants such as health literacy levels, economic status, and social support have an influence equivalent to clinical comorbidities (such as ASA scores) in predicting perioperative complications. Failure to identify social variables during the initial assessment stage contributes significantly to increased readmission rates and anesthetic adverse events. Conclusion: The development of an Initial Assessment model at RSUD Provinsi NTB must integrate clinical and social parameters to enhance risk detection sensitivity. The implementation of this model is expected to transform anesthesia practice into a more preventive and personalized approach to ensure holistic patient safety.

Abstract

Background: Nurse commitment is a fundamental pillar in maintaining the quality of healthcare services at RSUD Provinsi NTB. However, workplace dynamics and managerial challenges often hinder the retention of medical staff loyalty. Objective: This study aims to theoretically analyze the influence of a structural model of servant leadership and work culture on nurse commitment, examining the role of job satisfaction as a mediating variable and institutional trust as a moderating variable. Method: This study employs a Systematic Literature Review (SLR) method. Literature searches were conducted across Scopus, Google Scholar, and ScienceDirect databases spanning 2014-2024. Article selection followed strict inclusion and exclusion criteria focusing on organizational behavior studies in the healthcare sector. Results: Findings indicate that servant leadership and a positive work culture significantly enhance nurse commitment. Job satisfaction is proven to mediate this relationship, confirming that fulfilling nurses' emotional and professional needs is a prerequisite for commitment. Furthermore, institutional trust is identified as a key moderator that strengthens the impact of satisfaction on commitment; a crisis of trust in management can undermine the effectiveness of organizational policies. Conclusion: The integration of servant leadership styles and institutional transparency is essential for building sustainable nurse commitment at RSUD Provinsi NTB.
