Abstract

Nursing handover is the process of transferring responsibility and accountability among nurses in the provision of patient care in hospitals. Effective handover can reduce incident rates and improve patient safety. This study aimed to optimize nursing handover practices in inpatient wards. The method involved three stages: preparation (identifying issues and planning solutions), implementation (socializing handover procedures during shift changes, guiding the use of digital handover observation forms), and evaluation (assessing handover effectiveness). After the intervention, 88% of nursing handovers in the inpatient ward were carried out properly. It is concluded that regular socialization, strengthened supervision by head nurse or team leaders, reinforced team commitment, and balanced staffing considering education, work experience, and career level (Clinical Nurse I, II, and III) are necessary to sustain effective nursing handovers.

Abstract

The use of physical restraints on patients in intensive care units (ICUs) is a common intervention aimed at ensuring patient safety, particularly in preventing the dislodgement of vital invasive devices. However, this practice raises ethical and clinical dilemmas due to the complex considerations involved. The aim of this study was to explore the ethical and clinical considerations in the use of restraints for patient safety in the intensive care setting. This research employed a qualitative phenomenological design, with data collected through in-depth interviews involving eight participants in the intensive care unit. Data were analyzed using the Colaizzi method. The study identified three major themes: considering the patient's needs, preventing the dislodgement of invasive devices, and experiencing contradictory feelings. The findings highlight the need for clearer, standardized policies on the use of restraints in intensive care units that incorporate both ethical and clinical perspectives.

Abstract

Background: Patient safety culture is a critical component of healthcare quality and plays an essential role in reducing medical errors. Understanding healthcare professionals’ perceptions of patient safety is important for identifying strengths and areas requiring improvement within hospital systems. Objective: This study aimed to assess patient safety culture among hospital staff in a single private hospital to support quality improvement initiatives. Methods: A quantitative cross-sectional study was conducted among 217 hospital staff working in both clinical and non-clinical units in a single private hospital. Data were collected using the Hospital Survey on Patient Safety Culture. Data were analysed using descriptive statistics to calculate frequencies, percentages, and percent positive responses for each dimension. Results: The findings showed moderate variation across patient safety culture dimensions. The highest positive response was observed in response to error (73.5%), followed by handoffs and information exchange (68.7%). Other dimensions, including communication about error (64%), staffing and work pace (63.5%), communication openness (63%), teamwork (62%), and overall patient safety rating (62%), also reflected moderate perceptions. However, reporting patient safety events had the lowest positive response (43%), indicating a potential gap in incident reporting practices. Conclusion: Overall, patient safety culture in this setting demonstrates moderate positive perceptions across several dimensions. However, the relatively low score in incident reporting suggests that this area may require further attention. These findings highlight the need for continued evaluation of reporting practices to better understand underlying barriers and support ongoing quality improvement efforts

Abstract

Background: The application of simulation method is one of the keys to improve students' practical skills. However, many lecturers face challenges in integrating simulation effectively, mainly due to limited facilities, lack of training, and difficulty in measuring learning impact.Objectives: This study aims to explore how lecturers' experiences in implementing simulation methods, especially related to the strengths, challenges, student responses and the role of institutions related to simulation methods.Methods: This design is a qualitative study with a descriptive phenomenological approach involving seven nursing educators with in-depth interviews at the Akademi Keperawatan Ibnu Sina Kota Sabang. Data analysis used the Miles and Huberman Model which refers to data collection, data reduction, data presentation, and drawing conclusions.Results: There are four main themes in this study, namely the strengths of the simulation method, challenges of the simulation method, student responses and the role of educational institutions.Conclusion: The simulation method makes students more interactive, makes it easier to elaborate on the topics being studied, make decisions, gain direct experience, think critically and work together in a team.
