Abstract

Diabetes Melitus Tipe 2 (DM Tipe 2) merupakan faktor resiko terjadinya berbagai penyakit vaskular pada laki-laki dan perempuan. Komplikasi DM ini berhubungan dengan disfungsi makrovaskular dan mikrovaskular. Diagnostik dini sangat penting untuk menilai penyakit vaskular perifer, pengukuran nilai ankle brachial index (ABI) merupakan salah satu tindakan non invasif untuk untuk menilai resiko penyakit vaskular perifer dalam perawatan primer. Tujuan penelitian ini yaitu mengetahui gambaran nilai ABI pada pasien DM tipe 2. Metode yang digunakan dalam penelitian ini yaitu descriptif quantitatif. Total sampel  dalam penelitian ini adalah 35 pasien DM Tipe 2. Pengambilan sampel dengan menggunakan teknik consecutive sampling. Hasil penelitian menunjukkan bahwa mayoritas  nilai ABI pada pasien DM Tipe 2 tergolong kedalam katagori gangguan sirkulasi ringan yaitu pada ekstremitas kiri, responden yang mengalami gangguan sirkulasi ringan sebanyak 26 orang (74.30 %), pada ekstremitas kanan, responden yang mengalami gangguan sirkulasi ringan sebanyak 28 orang (80 %). Berdasarkan hasil penelitian dapat disimpulkan bahwa Banyak faktor yang dapat mempengaruhi nilai ABI antara lain lama menderita diabetes, riwayat hipertensi, peningkatan HbA1c, serum kreatinin dan riwayat retinopati. Penelitian ini dapat menjadi masukan bagi paisien DM Tipe 2 agar melakukan pemeriksaan ABI secara rutin untuk mendeteksi dini gangguan pembuluh darah pada ekstremitas serta sebagai pencegahan terhadap resiko kaki diabetik.Kata kunci: pergerakan sendi ekstremitas bawah, ankle brachial index(ABI), DM Tipe 2.

Abstract

Berbagai intervensi dilakukan untuk mencegah komplikasi diabetes, salah satunya adalah latihan pergerakan sendi ekstremitas bawah. Diagnostik dini sangat penting untuk menilai penyakit vaskular perifer, pengukuran nilai ankle brachial index (ABI) merupakan salah satu tindakan non invasif untuk untuk menilai resiko penyakit vaskular perifer dalam perawatan primer. Tujuan penelitian ini yaitu mengidentifikasi pengaruh latihan pergerakan sendi ekstremitas bawah terhadap nilai ABI pada pasien DM tipe 2. Metode yang digunakan dalam penelitian ini yaitu desain quasi-eksperiment jenis pre-test and post-test group design. Total sampel  dalam penelitian ini adalah 35 pasien DM Tipe 2. Pengambilan sampel dengan menggunakan teknik consecutive sampling. Intervensi yang diberikan yaitu latihan pergerakan sendi ekstremitas bawah yang dilakukan  secara aktif sebanyak satu kali setiap hari, dengan masing-masing gerakan sebanyak 10 kali pengulangan dan selama 4 minggu pengamatan. Uji statistik Wilcoxon Signed Ranks Test menunjukkan ada perbedaan antara sebelum dan sesudah dilakukan intervensi pergerakan sendi ekstremitas bawah terhadap nilai ABI, pada ekstremitas kiri (p=0.00) dan pada ekstremitas kanan (p=0.00). Berdasarkan hasil penelitian dapat disimpulkan bahwa latihan pergerakan sendi ekstremitas bawah dapat meningkatkan nilai ABI jika dilakukan secara teratur dan kontinyu. Latihan pergerakan sendi ekstremitas bawah dapat meningkatkan aliran darah ke arteri dan berefek positif pada metabolisme glukosa, dimana terjadinya penurunan  glukosa dan HbA1c. Penelitian ini dapat menjadi masukan bagi pelayanan keperawatan untuk menjadikan latihan fisik seperti latihan pergerakan sendi ekstremitas bawah sebagai pencegahan terhadap resiko kaki diabetik serta perlu dilakukannya pemeriksaan ABI untuk mendeteksi dini gangguan pembuluh darah pada ekstremitas pasien DM Tipe 2. Kata kunci: pergerakan sendi ekstremitas bawah, ankle brachial index(ABI), DM Tipe 2 ABSTRACTDiabetes mellitus Type 2 (DMN Type 2) is the risk factor for the incidence of vascular. Intervention was given by conducting lower extremity joint movement exercise once a day with 10 repetitions and 4 week observations. Wilcoxon Signed Ranks statistic test showed that there was the difference in pre and post intervention of lower extremity joint movement from ABI value, in left extremity (p=0.00) and in right extremity (p=0.00). The conclusion was that lower extremity joint movement exercise could increase ABI value if it was done regularly and continuously. It could also increase blood flow to artery and had positive effect on glucose metabolism in which glucose and HbA1c decreased. This research could bean input for nursing care to make physical exercises such as lower extremity joint movement as prevention from diabetic foot risk and ABI examination is required to early detect blood vessels disorder in DM Type 2 patients’ extremity.Keywords: lower extremity joint movement, ankle brachial index (ABI), DM Type 2.

Abstract

Diabetes mellitus Type 2 (DM Type 2) is the risk factor for the incidence of vascular.  Diabetic neuropathy and/or peripheral arterial disease, are common and represent major causes of morbidity and  mortality in people with diabetes.  Physical  exercises is have been done to prevent from diabetes complication, and one of them is by foot massage and lower extremity joint movement. Measuring the value of ankle brachial index (ABI) is one of non-invasive actions to evaluate the risk for peripheral vascular in primary treatment. The objective of this research was to determine the influence of foot massage and  joint movement exercise on ABI value in DM Type 2 patients. The research used quasi experiment method with pretest-posttest group design and two intervention groups. The samples were 40 DM Type 2 patients, taken by using consecutive sampling technique. Intervention was given by conducting  foot massage and  lower extremity joint movement exercise. Wilcoxon Signed Ranks statistic test showed that there was the difference in pre and post intervention of foot massage and lower extremity joint movement from ABI value, for (p=0.00). The conclusion was that foot massage and lower extremity joint movement exercise could increase ABI value if it was done continuously. It could also increase blood flow to artery. Abstract

ABSTRAK Virus corona (COVID-19) dapat menyebar melalui percakapan dan pernapasan normal dengan orang yang terpapar. Salah satu cara untuk mencegah terjadinya penularan COVID-19 adalah melalui pemakaian masker. Hal ini sejalan dengan imbauan Pemerintah untuk menerapkan protokol kesehatan pencegahan COVID-19, salah satunya melalui pemakaian masker kain untuk masyarakat umum. Tujuan pengabdian masyarakat ini adalah meningkatkan kesadaran masyarakat Pidie untuk menggunakan masker kain ketika melakukan aktivitas di luar rumah sebagai pencegahan penyebaran COVID-19 serta memberikan penjelasan mengenai cara menggunakan masker kain yang benar di saat melakukan aktivitas di luar rumah selama masa pandemi COVID-19. Kegiatan ini berlangsung pada tanggal 22 April 2020 di Simpang 4 Kota Sigli dengan cara membagikan masker kain kepada masyarakat Pidie yang beraktivitas di luar rumah disertai dengan sosialisasi tentang cara pemakaian masker kain yang benar. Antusias masyarakat Pidie dalam menerima edukasi dan pembagian masker kain terlihat selama kegiatan ini. Diharapkan kesadaran masyarakat Pidie untuk selalu menggunakan masker kain ketika beraktivitas di luar rumah semakin meningkat sehingga dapat menurunnya resiko penyebaran COVID-19 di lingkungan masyarakat Pidie yang harus tetap beraktivitas di luar rumah. Kata Kunci : Pencegahan, COVID-19, Pemakaian Masker, Masyarakat Pidie    ABSTRACT The coronavirus (COVID-19) can be spread through interacting and normal breathing with exposed people. One of the ways to prevent the transmission of COVID-19 is through wearing masks. This is in line with the government's appeal to implement health protocols for the prevention of COVID-19, one of which is through wearing cloth masks for the general public. The aim of this community service is to increase the awareness of the Pidie community to use cloth masks when carrying out activities outside the home as a prevention of the spread of COVID-19 and to provide an explanation of how to use the correct cloth masks when doing activities outside the home during the COVID-19 pandemic. This activity took place on April 22, 2020, at Simpang 4, Sigli City by distributing cloth masks to Pidie people who had activities outside the home accompanied by socialization on how to use cloth masks correctly. The enthusiasm of the Pidie community in receiving education and distributing cloth masks was seen during this activity. It is hoped that the awareness of the Pidie community to always use cloth masks when doing activities outside the home will increase so that it can reduce the risk of spreading COVID-19 in the Pidie community who have to continue their activities outside the home. Keywords: Prevention, COVID-19, Use of Masks, Pidie Society

Abstract

Elderly, chronic disease, depression, health perception, dependency level, which affect the quality of life of the elderly. Various interventions are needed by the elderly to overcome the decline in mobility related to age. The purpose of this study is to identify the effectiveness of stretching exercise on the quality of life and functional status of the elderly with type 2 diabetes mellitus. test group design with one intervention group. The total sample in this study was 30 elderly with type 2 diabetes. The sample was taken using a consecutive sampling technique. The intervention given was stretching exercise for 4 weeks. The Wilcoxon Signed Ranks Test statistical test showed that there was a difference between before and after the stretching exercise intervention on the functional status of the elderly, quality of life, general health dimensions, physical health, and social with a p-value (p=0.00), but there is no relationship on the  psychological health and environmental dimension. Based on the results of the study, it can be concluded that stretching exercises can improve physical fitness, muscle mass and joint flexibility which have an impact on functional status and quality of life of the elderly. It is hoped that stretching exercise can be used as one nursing independent treatment to improve the quality of life of and  increasing the independence of the elderly through the provision of education and regular physical exercise.

Abstract

Hypertension is still a dangerous health problem in the elderly. The number of people with hypertension is increasing every year from 25.8% in 2013 to 34.1% in 2018, even increasing at the age of 50-69 years. Many factors that cause hypertension, one of which is a lifestyle factor that is not good. The purpose of this study was to analyze the relationship between lifestyle and the incidence of hypertension in the elderly in the working area of Ujong Rimba Public Health Center. This research is quantitative with a cross sectional approach. The population in this study were 102 people. Samples were taken using the Slovin formula as many as 50 people, with purposive sampling technique. Data analysis to analyze the relationship of lifestyle with the incidence of hypertension by using the chi-square test. The results showed that there was no relationship between physical activity and the incidence of hypertension, with p value = 0.497, there was a relationship between diet and the incidence of hypertension, with p value = 0.002, there was a relationship between resting habits and the incidence of hypertension, with p value = 0.007. This study recommends the elderly to always follow the recommendations of healthy living, control blood pressure, exercise regularly. It is hoped that health workers at the Health Center will improve health education about a good lifestyle to prevent hypertension.

Abstract

The aging process (aging process) is a process characterized by a decrease in physical and cognitive function, or also called a degenerative process that involves all organs of the body. Progressive physical decline, decreased functional capacity, and the emergence of many diseases are the physiology of the aging process, the effects of aging on the neuromuscular system are related to degeneration of motor neurons and decreased muscle fibers, the number and size of which will have an impact on loss of muscle performance and will affect the independence of the elderly. Physical activity/exercise is considered as one of the main strategies to ward off physical disorders. The purpose of this service is to conduct counseling and apply elderly gymnastics, so that the elderly understand the importance of physical activity, one of which is elderly exercise which is useful for improving health and preventing the onset of ongoing diseases. The first stage of this activity is providing counseling, then measuring blood pressure before doing gymnastics, then lecturers and students together with the elderly do gymnastics, and after that blood pressure is measured again. The results of this dedication are expected to increase motivation for the elderly to do elderly exercise at least three times a week so as to improve the status of independence of the elderly and the quality of life of the elderly.

Abstract

Diabetes Mellitus dapat mengakibatkan berbagai komplikasi di masa mendatang apabila tidak ditangani sacara dini, komplikasi tersebut berdampak pada kualitas hidup penderita. Dukungan keluarga dan perawatan diri pada penderita diabetes mellitus merupakan faktor penting yang mempengaruhi serta mempertahankan kualitas hidup. Tujuan penelitian ini untuk mengidentifikasi hubungan dukungan keluarga dan selfcare management terhadap kualitas hidup penderita diabetes mellitus di wilayah kerja Puskesmas Bandar Baru Kabupaten Pidie Jaya. Desain penelitian ini yaitu analisis korelasi dengan pendekatan cross sectional. Populasi pada penelitian ini yaitu seluruh pasien diabetes mellitus di Wilayah Kerja Puskesmas Bandar Baru Kabupaten Pidie Jaya Sebanyak 42 pasien. Pengambilan sampel dalam penelitian ini sebanyak 42 responden dengan teknik total sampling. Penelitian ini dilakukan pada tanggal 18 s/d 24 Juli 2022. Hasil penelitian menunjukkan mayoritas kualitas hidup penderita diabetes mellitus berada pada kategori baik sebanyak 23 responden (54,8%). Terdapat hubungan dukungan keluarga pada kualitas hidup penderita diabetes mellitus (p = 0,015). Ada hubungan selfcare management terhadap kualitas hidup penderita diabetes mellitus (p = 0,004). Diharapkan kepada keluarga agar dapat meningkatkan pemberian dukugan serta meningkatkan motivasi penderita dalam menerapkan selfcare management yang baik sehingga dapat menumbuhkan rasa kepercayaan diri, terhindar dari stres  yang secara langsung juga akan meningkatkan kualitas hidup penderita.

Abstract

Diabetic ulcers are open sores on the skin layer to the dermis, which usually occur on the soles of the feet. Wounds that arise in DM patients due to complications of microangiopathy and macroangiopathy. Peripheral neuropathy will cause loss of sensation in the distal area of ​​the foot. The length of time a person suffers from DM will cause microangiopathic complications so diabetic neuropathy will cause ulcers on the feet. This study aims to determine the relationship between family support in the healing process of diabetic ulcers with patient compliance in undergoing treatment at the Asri Wound Care Center clinic in Medan using a descriptive correlation research design with a cross-sectional design. The sample in this study was 36 respondents using an accidental sampling technique. The results of the Spearman rank test (rho) with a p-value = 0.004 with a coefficient value of 0.467 then p-value <a (0.000 <0.05), meaning that there is a relationship between family support in the healing process of diabetic ulcers with patient compliance in undergoing treatment at the Asri Wound clinic. Care Center Medan. For education, it is expected that educational institutions can increase the knowledge of students at the institution regarding the importance of family support in the process of healing diabetic ulcers with patient compliance in undergoing treatment.

Abstract

Diabetic foot ulcers are the fastest growing chronic complications of diabetes mellitus, unmanaged diabetic complications such as diabetic foot ulcers will have a negative impact on the quality of life of individuals, health education, information and communication about DFUand foot care needs to be improved. The purpose of this study was to determine the impact of foot care education on knowledge of diabetic ulcer risk prevention in people with diabetes mellitus in the working area of the Mutiara Barat Health Center, Pidie Regency. This type of research is quasi-experimental research. The population in this study were all diabetes mellitus sufferers in the working area of the Mutiara Barat Health Center, Pidie Regency, in the last 1 month as many as 63 patients. Sampling was done purposive sampling technique as many as 25 people. The results showed that there was an effect of foot care education on knowledge of diabetic ulcer risk prevention in people with diabetes mellitus in the working area of the Mutiara Barat Health Center, Pidie Regency, which obtained a significance value of ρ = 0.001 where ρ <0.05. Health education provides moderate to moderate knowledge in helping diabetes mellitus patients to increase their knowledge about diabetic foot ulcer prevention. It is hoped that good knowledge will raise awareness, will and strength, so that it can change the behavior of diabetes mellitus patients into adaptive behavior that can prevent diabetic ulcers from occurring.
