Abstract

Penyakit kardiovaskular masih menjadi penyebab utama kematian di seluruh dunia, dengan gagal jantung menjadi salah satu manifestasi penyakit yang paling serius. Gagal jantung berdampak signifikan terhadap kualitas hidup pasien, sering kali menimbulkan gejala psikologis dan fisik seperti kecemasan, kualitas tidur buruk, dan nyeri. Penatalaksanaan gagal jantung mencakup pendekatan farmakologis dan nonfarmakologis. Di antara intervensi non-farmakologis, terapi musik telah diidentifikasi sebagai metode potensial untuk meringankan gejala fisik dan psikologis. Penelitian ini bertujuan untuk memetakan intervensi terapi musik yang ada yang bertujuan untuk mengurangi tingkat kecemasan pada pasien gangguan jantung. Penelitian ini menggunakan desain Scoping Review. Pencarian literatur yang komprehensif dilakukan dengan menggunakan database PubMed, CINAHL (via EBSCO), dan Scopus. Kriteria inklusi mencakup ketersediaan teks lengkap gratis, publikasi antara tahun 2014-2024, studi yang berfokus pada terapi musik untuk mengurangi kecemasan pada pasien dengan penyakit kardiovaskular, dan penelitian primer yang menggunakan uji Randomized Trial Control (RCT), uji klinis, atau desain kuasi-eksperimental. Pencarian awal menghasilkan 1.422 artikel, setelah dilakukan proses penyaringan, tersisa 7 artikel eligible untuk ditelaah menggunakan JBI aprasial tool. Temuan menunjukkan bahwa intervensi terapi musik berpengaruh dalam mengurangi kecemasan dan depresi, mengurangi rasa nyeri, meningkatkan kualitas tidur, dan meningkatkan kualitas hidup secara keseluruhan pada pasien dengan kondisi jantung. Terapi musik efektif mengurangi kecemasan, nyeri, gangguan tidur dan meningkatkan kualitas hidup pasien jantung. Temuan ini menunjukkan intervensi ini aman dan dapat mendukung perawatan holistik. Penelitian lanjutan dengan metode yang lebih kuat, protokol yang lebih seragam, serta evaluasi efek jangka panjang dan standar diperlukan, termasuk pemanfaatan teknologi inovatif untuk memperluas akses.

Abstract

Background: Valvular Heart Disease (VHD) occurs when one of the valves in the heart is damaged or dies. The highest prevalence of VHD is in mitral regurgitation. The management action that can be given to patients with mitral valve problems is Mitral Valve Replacement (MVR). Objective: To discuss the indications of Temporary Pacemaker (TPM) insertion in intra-operative MVR and the indications of collaborative drug administration in patients. Methods: Descriptive case-report of a 70-year-old woman with VHD after MVR surgery. Case Description: The patient had a history of Permanent Pacemaker (PPM 2023) ec. Bradycardia. Currently the patient was diagnosed with VHD and MVR surgery was performed. At the time of weaning Cardiopulmonary Bypass (CPB) to off, the patient's heart rhythm was not pacing and blood pressure dropped. So it was decided to go back on CPB and install a TPM. The patient received medical support therapy in postoperative care. Discussion: The indication for TPM insertion is to prevent postoperative heart rhythm disturbances and to temporarily replace the inactive PPM function. In postoperative management, the patient received medical therapy and therapeutic interventions. The results obtained are the patient's hemodynamic status is stable, pain is reduced, fluid balance is more negative, there are no signs of infection in the surgical wound, and minimal bleeding. Conclusion: TPM placement is required in patients with PPM if no pacing rhythm is found. The provision of collaborative medication with appropriate doses and indications is effective in reducing the risk of postoperative complications of MVR and stabilizing the patient's hemodynamics.

Abstract

Cardiac arrest is a sudden cessation of heart activity that requires immediate treatment in the form of Basic Life Support (BLS). Students who are members of the Student Activity Unit (UKM) in the humanitarian field at Unpad are potential first aiders to provide basic life support in the event of cardiac arrest. This study aims to determine the level of knowledge of basic life support in students who are members of UKM in the humanitarian field at Universitas Padjadjaran. The method used in this research is quantitative descriptive with a cross sectional approach. The population in this study amounted to 158 active members of the humanitarian student activity unit and the sample was taken using convenience sampling type (n = 120). Data were collected using a questionnaire containing 30 questions that have been declared valid and reliable with a Cronbach's alpha value of 0.797 and data were analysed using computer software with univariate analysis techniques. The results showed that the level of knowledge of humanitarian UKM members at Unpad regarding basic life support was classified into the good category with a total of 108 respondents (90%). The conclusion in this study is that the level of knowledge of basic life support in members of humanitarian UKM at Universitas Padjadjaran is in the good category, but it is necessary to increase knowledge, especially on procedural topics related to basic life support.
