Abstract

The high and varied disasters occur in Indonesia, requiring the government and the whole community to be ready for disaster. In the Faculty of Nursing Universitas Padjadjaran (Unpad), there is a group of students who do extracurricular activities to provide health aid called PNC. Training needs to be done to enhance the role of PNC as well as increasing the participation of students in the disaster relief program. This training resulted in increased knowledge and attitudes of PNC members on disaster management, and also increased the knowledge and attitude related to the role and function of students in disaster management effort. In addition, other additional results related to the effectiveness of training methods for PNC members successfully obtained. The methods compared were lecture and discussion (LD) compared with LD plus simulation and FGD methods. It is measured by using the quasi experimental method with pretest-posttest control group design. Data analysis used Mann Whitney test. The result showed that there was no difference in knowledge and attitude (p = 1,000) in both treatment groups (p = 0,424). Which means that there was no difference between LD method and LD plus simulation and FGD method.Keywords: Training, Disaster Management, Padjadjaran Nursing Corps (PNC)

Abstract

We often found emergency condition on children in elementary school which can lead death. Teachers should have good knowledge and favorable attitude toward first aid so they can give right intervention on emergency condition. The objective of this activity was to improve attitude and knowledge of elementary school teachers toward it. This activity be held on training program includes lecture, discussion, and practice. Result showed there was any improvement of elementary school teachers’ attitude and knowledge score. Reevaluation and retraining should be held to improve significant score of teachers’ attitude and knowledge so do their skill toward first aid on children emergency condition in elementary school.Keywords: first aid, emergency condition, elementary school teachers, children

Abstract

Hypertension is a health problem that needs attention in Indonesia. To decrease the incidence of hypertension, we could give health education to the community. Provision of information and education through learning media such as booklet was one effort to empower the community in preventing and performing hypertension treatment at home. Increasing health promotion through the provision of information and education using learning media was expected to improve the knowledge and self-efficacy of the community that will affect the behavior of the community. Pangandaran Regency is one of the target areas of Padjadjaran University where hypertension is one of the highest prevalence diseases. The purpose of this activity was to improve knowledge of community self-efficacy about prevention and treatment of hypertension. This activity was carried out from community surveys, booklet preparation, booklet socialization, and evaluation. At the implementation stage, the activity was followed by 22 people and 13 of them had hypertension. People who had hypertension were given hypertension management booklet then their knowledge and self-efficacy evaluate after one to two weeks. The result showed that there was an increase in the average knowledge and self-efficacy of them after the program. Besides that, there were differences in an average score of public knowledge before and after the giving of booklets, but there was no difference in the average self-efficacy score. This booklet can be used to provide health education to the community related to prevention and treatment of hypertension, so hopefully, there will be changes in behavior to manage hypertension.Keywords: Booklet, learning media, hypertension, knowledge, self-efficacy

Abstract

ABSTRAK Stroke fase rehabilitasi dapat mengakibatkan perubahan fisik dan psikologis sehingga mempengaruhi kualitas hidup pasien yang kemudian menyebabkan masalah psikosial berupa cemas dan depresi. Cemas dan depresi ini memiliki korelasi terhadap kesejahteraan spiritual. Tujuan dari penelitian ini adalah untuk mengidentifikasi tingkat kesejahteraan spiritual  pada pasien pasca stroke di poliklinik RSUD Al Ihsan Provinsi Jawa Barat. Penelitian ini merupakan penelitian deskriptif kuantitatif dengan menggunakan instrumen SIWB. Penarikan sampel menggunakan metode purposive sampling dengan kriteria insklusi pasien stroke fase rehabilitasi yang mempunyai nilai Mini Mental State Examination (MMSE) normal 24-30, dan didapatkan 105 responden. Data disajikan dalam bentuk distribusi frekuensi. Hasil penelitian ini menunjukkan tingkat kesejahteraan spiritual tinggi sebanyak 57 responden (54,3%) dan tingkat kesejahteraan spiritual rendah sebanyak 48 responden (45,7%). Karakteristik responden paling banyak pada rentang usia lansia akhir yaitu berjumlah 41 responden (39,0%), dengan lama stroke 12 bulan 58 responden (55,2%), memiliki penyakit penyerta 82 responden (78,1%), dan mengalami serangan stroke 1x sebanyak 54 responden (51,4%). Pada penelitian ini, antara tingkat kesejahteraan spiritual tinggi dan rendah tidak jauh berbeda persentasenya. Sehingga  masih diperlukan upaya untuk meningkatakan kesejahteraan spiritual melalui perbaikan sarana dan prasaran ibadah ,mengoptimalkan spiritual care, mengadakan seminar-seminar dan pelatihan spiritual care, melakukan berdo’a bersama sebelum pemeriksaan dimulai, menciptakan lingkungan yang nyaman, aman, dan damai, serta mengadakan peer group khusus pasien stroke. ABSRTACTStroke rehabilitation phase can lead to physical and psychological changes. It has an impact on the quality of life of patients that later caused psychosocial problems such as anxiety and depression. These anxiety and depression have a correlation to the spiritual well-being. The purpose of this study was to identify the level of spiritual well-being in post-stroke patients in polyclinic RSUD Al Ihsan West Java Province. This research was a quantitative descriptive research using SIWB instruments. It used a purposive sampling method with the inclusion criteria of rehabilitation phase stroke patients who had 24-30 as the normal score of Million Minimum State Examination (MMSE) and obtained 105 respondents. The data presented in the form of the frequency distribution. The results of this study indicated that the respondent’s amount who reached the high level of spiritual well-being was 57 respondents (54.3%) and low level of spiritual well-being was 48 respondents (45.7%). The most respondents’ characteristic were respondents with age range of elderly as of 41 respondents (39.0%), with stroke length more than 12 months as of 58 respondents (55.2%), had comorbid disease as of 82 respondents (78.1%), and suffered once stroke attack as of 54 respondents (51.4%). The research revealed that the percentage level of spiritual well-being between high and low was nearly similar. As the consequences, it is necessary to increase the spiritual well-being through the improvement of religious facilities and infrastructure, optimizing spiritual care, conducting seminars and spiritual care training, praying together before the medical check-up or treatment, creating a comfortable, safe and peaceful environment, and establish the peer group for stroke patients.

Abstract

Cardiac arrest termasuk kejadian kegawatdaruratan yang membutuhkan bantuan hidup dasar dengan resusitasi jantung paru. Upaya pertolongan bantuan hidup dasar yang terlambat diberikan dapat berdampak kematian pada pasien. Tim reaksi cepat sebagai tenaga kesehatan yang dibentuk khusus untuk menangani pasien kegawatdaruratan dituntut mampu memberikan pertolongan pada pasien gawat darurat dengan cepat dan tepat. Tim reaksi cepat membutuhkan pengetahuan dalam melakukan tindakan bantuan hidup dasar. Tujuan penelitian ini untuk mengidentifikasi pengetahuan Tim Reaksi Cepat tentang Bantuan Hidup Dasar. Penelitian ini menggunakan metode deskriptif kuantitatif pada 37 anggota tim reaksi cepat yang dipilih menggunakan total sampling. Instrumen penelitian menggunakan kuesioner pengetahuan yang disusun dan dikembangkan peneliti. Hasil penelitian menunjukkan bahwa pengetahuan sebagian besar dalam kategori kurang yaitu sebanyak 36 responden (97,3%) dan satu responden (2,7%) memiliki pengetahuan baik. Responden dengan katergori pengetahuan yang kurang memiliki kategori kurang di semua indikator seperti, pada pengetahuan konsep dasar, pengkajian respon dalam BHD, resusitasi paru dalam BHD, resusitasi jantung dalam BHD, dan AED dalam BHD. Perlu adanya peningkatan pengetahuan tentang bantuan hidup dasar dengan mengadakan pelatihan, evaluasi serta pembaharuan standar prosedur operasional BHD.

Abstract

ABSTRAKInsidensi Penyakit Jantung Koroner (PJK) di Indonesia semakin meningkat. Hal ini berkaitan dengan tingkat kesadaran masyarakat tentang pencegahan PJK masih kurang termasuk faktor risiko PJK yang mungkin dimilikinya. Pangandaran merupakan daerah pesisir pantai di Jawa Barat yang belum memiliki pelayanan yang memadai sehingga deteksi dini faktor risiko PJK pada masyarakatnya belum maksimal. Penelitian ini dilakukan untuk mengidentifikasi faktor risiko PJK pada masyarakat pangandaran. Penelitian ini merupakan penelitian deskriptif kuantitatif dengan pendekatan cross sectional study yang dilakukan pada 87 masyarakat Pangandaran dengan mengkaji faktor risiko PJK seperti riwayat penyakit sebelumnya, tingkat aktivitas, usia, pekerjaan, pemeriksaan gula darah, kadar kolesterol, pengukuran lingkar perut dan body Mass Index (BMI). Berdasarkan hasil deteksi dini tersebut dapat dilihat bahwa sebagian besar masyarakat Desa Pangandaran berada pada status nutrisi overweight (47,1%) dan memiliki lingkar perut yang kelebihan ringan (36,8%), tidak berolahraga (64%), memiliki kadar gula darah normal (69%), dan kadar kolesterol tinggi (74,7%). Sedangkan tekanan darah sebagian besar masyarakat desa Pangandaran adalah normal (60,9%). Masyarakat Pangandaran memiliki faktor risiko PJKdiantaranya overweight, kelebihan lingkar perut, kurang beraktivitas, dan hiperkolesterolemia. Oleh karena itu, petugas kesehatan diharapkan dapat memberikan pendidikan kesehatan mengenai pencegahan PJK terutama untuk mengatasi hiperkolesterolemia, overweight dan kelebihan lingkar perut yaitu dengan beraktivitas yang cukup dan menjaga pola diet.Kata kunci: Faktor risiko, Pangandaran, Penyakit Jantung Koroner.  ABSTRACTIncidence of Coronary Heart Disease (CHD) in Indonesia has increased. This relates to the level of public awareness on the prevention of CHD is still less including CHD risk factors that may be held. Pangandaran is a coastal area in West Java that has not had adequate services so that early detection of risk factors for CHD in the community has not been maximized. This study was conducted to identify risk factors of CHD in Pangandaran society. This research is a quantitative descriptive research with cross sectional study approach conducted on 87 Pangandaran community by examining CHD risk factors such as previous disease history, activity level, age, occupation, blood glucose examination, cholesterol level, measurement of abdominal circumference and body mass index (BMI). Based on the results of early detection it can be seen that most of the people of Pangandaran Village are in overweight nutritional status (47.1%) and have abdominal circumferences that are overweight (36.8%), not exercised (64%), have normal blood glucose (69%), and high cholesterol (74.7%). While the blood pressure of most people in Pangandaran village is normal (60.9%). Pangandaran community has risk factors for CHD including overweight, excess abdominal circumference, lack of activity, and hypercholesterolaemia. Therefore, health workers are expected to provide health education on the prevention of CHD, especially to overcome hypercholesterolemia, overweight and excess abdominal circumference that is with enough activity and maintain diet patterns.Keywords: Coronary Heart Disease, Pangandaran, Risk factors

Abstract

There are many cases of Sudden Cardiac Arrest (SCA) in the community that cannot be rescued, because they did not get the proper and fast action due to ignorance and incompetence of the community. The purpose of this training was  increasing the knowledge, attitudes, intentions, and abilities of the community in identifying and handling SCA in Cileles Village, Jatinangor District. The community service activities were integrated with student’s KKN activities. This activity was carried out starting from the initial assessment related to the CPR ability of  the community, socialization to KKN students, preparation of materials and learning media, the implementation of socialization and training activities for the community, and also this evaluation. At the implementation stage, 33 training participants participated. Pre-test and post-test measurements were carried out with the response rate as much as 90.9%. As much as 53.3% of respondents had never received information about cardiac arrest, and 93.3% had never received information about CPR before. After training, the respondents' average knowledge score was 75.7. On the attitude aspect, before training, 90% of respondents supported the implementation of CPR and after training, 96.7% of participants supported the implementation of CPR. Meanwhile, before the training, no participants intended to do CPR if they found victims of SCA, but 100% of participants intended to do CPR after training. There was an increase in knowledge, attitudes, and community intentions related to the handling of cardiac emergencies, especially cardiac arrest. This was an indication that CPR will be carried out by respondents if they found victims of SCA. Similar training needs to be conducted periodically to prevent the decline in knowledge or skills related to CPR actions.Keywords: Pemberdayaan Masyarakat, Kegawatdaruratan, Sudden Cardiac Arrest.

Abstract

The high mortality rate in Emergency Department (ED) is consistent with the high demand for end of life (EOL) care. However, ED work system required to life saving quickly and precisely often makes EOL care is not working optimally.The purpose of this study was to determine nurses’perceptions in emergency departments related to obstacles and supportive behaviors (SB) in providing EOL care to dying patients. Descriptive-quantitative method was used with cross-sectional approach in 50 nurses Emergency Department at one of the tertiary hospitals in Indonesia. The samples were chosen using the total sampling technique. Data collection used the emergency nurse perceptions of end of life care questionnaire. Data were analyzed using univariate analysis in the form of distribution, frequency, and mean. The result of the study showed that the main obstacles in EOL care were:  ED nurses’ work loads (10.626), intra-family disagreements (9,802), and dealing with angry family members (9.472). The three main perceptions in supportive behaviors were teaching families how to act (11.962), allowing family members adequate time with the patient after patient died (11.09), and good collaboration from paramedics personnel (10.886. It can be conclude that nurses facing obstacles in taking care end-of-life patients. Based on this research, further studies are needed to minimize obstacles and improve supportive behaviors related to the nurse's workload, the provision of spiritual teams, special training for nurse and the provision of special rooms for EOL patients and their families.

Abstract

Post-stroke patients experience in various disturbances including physical, psychological and spiritual aspect. Post-stroke patients in the rehabilitation phase focus more on physical needs while the unfulfilled spiritual needs can reduce the patient's health condition. Therefore, it takes action to meet the spiritual needs of patients. This study was conducted to identify the spiritual needs among post-stroke patients.The descriptive quantitative was used among post-stroke patients in Neurological Polyclinic and stroke center in one of the Hospital in Bandung in 2017. The sampling technique used was consecutive sampling with 83 samples included post-stroke patients who have full awareness and didnot experience of aphasia. The instrument of this study used Spiritual Needs Questionnaire (SpNQ) which consists of 4 dimensions including religious, peace, self-existence, and dimension of the giving needs. SpNQ had a validity coefficient of 0.73 and r 0.75. Data were analyzed using frequency distribution. The results showed that the most of respondents feel the spiritual need in all dimensions. In the religious dimension, the majority of respondents desired to pray with others, to move closer to God and participate in various religious activities (98.8%). In the dimension of peace showed the most of respondents desired to be more cherished by others (96.4%). The most perceived item of respondents were forgiving someone from the past in the dimension of self-existence (98.8%) and a solace for others in the dimension of the giving needs (98.8%). The conclusion from this study was most respondents feel the spiritual needs in all dimensions. Based on these results, nurses are expected to provide advice to people around the patients to invite them in various religious activities. Nurses supposed more expressing affection to them and involving the patients in some family activities.

Abstract

Depression has been related to poor quality of life and recurrence in CHD patients. It is important to explore factors associated with the depression to prevent problems and to assist with appropriate intervention. Furthermore, the determinants of depression in CHD patients in Indonesia are still unknown. This study aimed to identify the determinants of depression in CHD patients in Indonesia. This study was a quantitative study with a cross-sectional approach. Samples were recruited from outpatient care in a referral hospital in Bandung using a consecutive sampling technique for a-2 month period (n=101). Data were collected using a questionnaire package consisting of a demographic questionnaire, Beck Depression Index [BDI] (II), Zung Self-rating Anxiety Scale, Seattle Angina Questionnaire, and Spirituality Index of Well-Being. Data were analyzed using mean, frequency distribution, chi-square and logistic regression analysis. In terms of its association with depression in CHD patients, low spiritual well-being had the highest odds ratio [OR] (OR = 9.3, 95% CI = 2.968 - 29.451, p <0.01); non-anxious state and medication history had low ORs (sequentially OR = 0.2, 95% CI = 0.067 - 0.644, p < 0.01; OR=0.2 95% CI = 0.062 - 0.772, p < 0.01);  PCI (percutaneous coronary intervention) with medication had the lowest OR value (OR = 0.02, 95% CI = 0.002 - 0.278, p <0.01). On the other hand,  unmarried status and male gender were identified as confounding variables. Low spiritual well-being was a major predictor of depression in CHD patients, whereas no anxiety, and history of medication, consist of CHD medication and reperfusion therapy contributed to a lower risk of depression. It is recommended to include strategies in improving spiritual well being and managing anxiety to reduce the risk of depression among CHD patients.
