Abstract

Menurunnya angka kunjungan pasien lama di poliklinik kebidanan dan kandungan untuk 2 – 3 tahun ber turut – turut, salah satu penyebabnya adalah ketidak puasan pasien terhadap pelayanan yang di berikan,Tujuan penelitian ini adalah menganalisis pengaruh dimensi mutu pelayanan terhadap kepuasan pasien rawat jalan di Poliklinik Kebidanan dan kandunganRumahsakit Umum Daerah DR.R.M.Djoelham Kota Binjai pada tahun 2019. Penelitian ini obsevarsional dengan pendekatan waktu cross sectional  pengumpulan data dengan survey. Populasi penelitian adalah pasien rawat jalan di poliklinik kebidanan dan kandungan ang mendapatkan pelayan dari dokter SpOG dan diberikan tindakan serta pengobatan yang datang kunjungan untuk pertama kali, Prosedur sampel Accidental Sampling sebanyak n=60). Hasil sebagian besar responden berumur dewasa awal 18 – 35 th sebanyak 47 orang (78,3%), berjenis kelamin perempuan 60 orang (100%), tamatan SMA yaitu sebanyak 40 (66,7%) responden. Ibu rumah tangga sebanyak 49 orang (81,7%). terdapat hubungan pada dimensi Reliability mempunyai nilai p = 0,0001, Tangible p = 0,005 , emphaty p=0,001, Assurance p = 0,009, Responsivenes p  = 0,004. Berdasarkan hasil analisis multivariate variabel yang paling besar pengaruhnya terhadap kepuasan pasien adalah variabel Assurance dengan nilai p =0,012 dan Exp (B) 6,246. Berdasarkan hasil tersebut disimpulkan bahwa terdapat hubungan dimensi mutu pelayanan terhadap kepuasan pasien. Saran penelitian adala, Di harapkan kepada pegawai khususnya di bagian pendaftaran pasien untuk meningkatkan pelayanan yang cepat dan tepat dalam penerimaan pasien rawat jalan dan Di harapkan untuk petugas yang ada di rawat jalan untuk menggunakan tanda pengenal pada saat bekerja.

Abstract

The Youth Care Health Program is a model of health services aimed at and accessible to adolescents, This study aims to analyze the implementation of the Youth Care Health Program in the Gunung Tua Community Health Center. This research design used analytical survey with cross sectional study design. The sample in this study were some of the teenagers in the work area of the Gunung Tua health center as many as 108 teenagers using purposive sampling technique. Data collection was carried out using a questionnaire. Data were analyzed using chi-square statistics and logistic regression. The results showed that there was an effect of providing adolescent information (p = 0,000), there was an effect of supporting examinations (p = 0,000). there was an effect of of counseling (p = 0,000), there was an effect of skills education (p = 0,000), there was an effect of referral on the implementation of the Youth Care Health Program in the work area of the Gunung Tua Community Health Center (p = 0,000). The conclusion in this study is that the variables of providing adolescent information, supporting examinations, counseling, education, skills, peer counselor training have an effect on the implementation of the Adolescent Care Health Program in the work area of the Gunung Tua Community Health Center, while the most influential variable is the counseling variable (p = 0.022). . Suggestions in this study are that there is a need for broader socialization of the implementation of the PKPR program to adolescents through socialization to schools, cadets, giving pamphlets or leaflets containing information about the existence of PKPR which provides special services for adolescents.

Abstract

The eradication of pulmonary TB in Indonesia is one of the national priorities for the tuberculosis disease. The purpose of the study was to analyze the factors that influence the activity of health cadres in finding suspected cases of pulmonary TB patients in the working area of ​​Siatas Barita Health Center, North Tapanuli Regency. The design of this research was an analytic survey with a cross-sectional design. This research was conducted in the Working Area of the Puskesmas Siatas Barita, North Tapanuli Regency from Mei to Juni 2021 with a sample of 80 people. Data analysis was carried out by univariate, bivariate, and multivariate analysis. The statistical test used is the chi-square test and logistic regression. The results of the chi-square test showed that the variables of knowledge, attitude, motivation, and monitoring p-value = 0.000 < 0.05, meaning that there was an influence between knowledge, attitude, motivation, and supervision of cadre activity. influential in this study is the motivation variable. The conclusion was that there is an intermediate influence. there was an influence between knowledge, attitude, motivation, and supervision on cadre activity, while multivariate analysis shows that the most dominant factor was the motivation variable on cadre activity. It was recommended to the District Health Office to increase the frequency of supervision and monitoring of the implementation of the pulmonary TB control program in the puskesmas area, specifically the active and sustainable health cadres.

Abstract

Adolescent reproductive health becomes a serious problem. Many adolescents experience unwanted pregnancy to HIV / AIDS. It is hoped that UKS can become a forum to improve students' abilities and skills in shaping healthy behavior. The purpose of the study was to analyze the implementation of UKS in the delivery of information on adolescent reproductive health in Medan AN NIZAM Middle School in 2019. The research method uses mixed methods with a sequential explanatory approach, a population of 164 people, a quantitative sample using a total population while qualitative informants 4 people. Data analysis was performed univariately, bivariate using chi-square, multivariate using multiple logistic regression at a confidence level of 95% (α = 0.05) and depth interview of informants. Quantitative research results show that variables that influence the analysis of the implementation of UKS in the delivery of reproductive health information are knowledge (Pvalue = 0.008) and attitude (Pvalue = 0.001). Qualitative research results show that students have received good reproductive health information but the UKS activities are not going well so that it affects students' attitudes in implementing UKS. Abstract

ABSTRACTOvercoming a variety of health problems for the elderly, the government has made various health care policies for elderly people, one of them is the posyandu for the elderly. Facts on the ground, few elderly people use the elderly posyandu and there are still many elderly people who do not visit for various reasons. The purpose of this study was to analyze qualitatively the influence of elderly visits in posyandu activities. This type of research is qualitative research. The research was conducted in the Work Area of Pintu Langit Health Center in Padangsidimpuan Sub-District, Angkola Julu, Padangsidimpuan City. Research informants were 10 people. Qualitative data analysis with stages of data reduction, data display, and conclusion or verification. The results showed that the elderly were members of the elderly posyandu in the last 2-3 years, but all were not routine or were not active. The reason is not routine because he is lazy, while working in a garden / field, does not remember the schedule of the elderly posyandu. Many elderly people do not know about the elderly posyandu related to the benefits and objectives. The attitude of ordinary elderly people has an impact on enthusiasm or interest in the posyandu is low. The distance between the house and the elderly posyandu is not too far away but there are also those who need transportation equipment to reach it. Some elderly people get support from their families (children / grandchildren) by telling the elderly posyandu schedules, delivering them to the elderly posyandu, but there are also those who do not get support. Elderly considers that the visit to the elderly posyandu is not a necessity if it is not sick. It is recommended that health workers improve the activities of the elderly program by providing counseling to increase knowledge, change negative attitudes, change perceptions about needs. Abstract

Keselamatan pasien merupakan suatu sistem yang membuat asuhan pasien menjadi lebih aman. petugas kesehatan sering salah mengidentifikasi pasien, identifikasi dan pengelolaan risiko pasien yang kurang tepat, pelaporan dan analisis insiden yang kurang sesuai, serta kurang memberikan solusi untuk meminimalkan timbulnya risiko dan mencegah terjadinya cedera yang disebabkan oleh kesalahan akibat melaksanakan suatu tindakan atau tidak mengambil tindakan yang seharusnya diambil. Tujuan penelitian ini adalah untuk menganalisa faktor-faktor yang berpengaruh terhadap penerapan keselamatan pasien di ruangan rawat inap RSU Permata Madina Panyabungan. Penelitian ini menggunakan metode survei analitik dengan desain cross sectional. Populasi dalam penelitian ini sebanyak 35 orang. Teknik pengambilan sampel dalam penelitian ini dengan menggunakan metode total sampling. Penelitian ini menggunakan data primer yang diperoleh secara langsung dengan membagikan kuesioner, kemudian dilakukan uji Chi Square dengan nilai kemaknaan 95 % ( = 0,05%). Hasil penelitian menunjukkan ada pengaruh pengetahuan dan supervisi terhadap penerapan keselamatan pasien di ruangan rawat inap Rumah Sakit Permata Madina Panyabungan dengan nilai ρ value = 0,000, dan ρ 0,038. Tidak ada pengaruh sikap dan beban kerja terhadap penerapan keselamatan pasien di ruangan rawat inap Rumah Sakit Permata Madina Panyabungan dengan nilai ρ value = 0,081 dan ρ value 0,126. Kesimpulan dari penelitian ini adalah ada pengaruh yang signifikan antara pengetahuan dan supervisi terhadap penerapan keselamatan pasien di ruangan rawat inap Rumah Sakit Permata Madina Panyabungan. Tidak ada pengaruh sikap dan beban kerja terhadap penerapan keselamatan pasien di ruangan rawat inap Rumah Sakit Permata Madina Panyabungan. Disarankan kepada pihak manajemen RSU Permata Madina Panyabungan dapat memberikan pelatihan kepada semua Perawat pelaksana tentang penerapan keselamatan pasien.

Abstract

Stres kerja adalah suatu kondisi ketegangan yang menciptakan adanya ketidakseimbangan fisik dan psikis, yang memengaruhi emosi, proses berpikir, dan kondisi seorang pegawai. Jumlah tenaga dokter spesialis di Rumah Sakit Umum Daerah Djoelham Binjai masih kurang sehingga menyebabkan dokter spesialis memiliki beban kerja yang terlalu banyak serta dokter spesialis tersebut seluruhnya memiliki jam kerja di instansi lain. Sumber stres dapat berasal dari tuntutan tugas, tenaga, dan performansi. Tujuan penelitian ini adalah untuk menganalisa pengaruh beban kerja terhadap stres kerja pada dokter spesialis di ruangan rawat jalan RSUD Djoelham Binjai. Penelitian ini menggunakan metode survei analitik dengan desain cross sectional. Populasi dalam penelitian ini sebanyak 25 orang. Teknik pengambilan sampel dalam penelitian ini dengan menggunakan metode total sampling. Penelitian ini menggunakan data primer yang diperoleh secara langsung dengan membagikan kuesioner, kemudian dilakukan uji Chi Square dengan nilai kemaknaan 95 % ( = 0,05%). Hasil penelitian menunjukkan ada pengaruh yang signifikan antara tuntutan tugas terhadap stres kerja pada dokter spesialis yang menyebabkan gejala fisik dengan nilai p value = 0,018, yang menyebabkan gejala emosional dengan nilai p value = 0,039, yang menyebabkan gejala sosial dengan nilai p value = 0,032. Ada pengaruh tenaga terhadap stres kerja pada dokter spesialis yang menyebabkan gejala fisik dengan nilai p value = 0,005, yang menyebabkan gejala emosional dengan nilai p value = 0,009, dan yang menyebabkan gejala sosial dengan nilai p value = 0,029. Kesimpulan dari penelitian ini adalah ada pengaruh tuntutan tugas dan tenaga terhadap stres kerja yang menyebabkan gejala fisik, gejala emosional, dan gejala sosial pada dokter spesialis di ruangan rawat jalan Rumah Sakit Umum Daerah Djoelham Binjai. Disarankan kepada pihak manajemen Rumah Sakit Umum Daerah Djoelham Binjai untuk menambah jumlah tenaga dokter spesialis agar beban kerja dokter spesialis menurun serta menata kembali aspek-aspek yang berkaitan dengan beban kerja

Abstract

Dokter penanggung jawab pasien (DPJP) mengalami kendala pada banyaknya pasien yang harus ditangani, pemahaman tentang INA CBG’s dan penulisan kode diagnosis sesuai ICD-10. Verifikator melakukan verifikasi kode diagnosis sesuai dengan sistem INA CBG’s. Penelitian ini bertujuan menjelaskan bagaimana terjadinya perbedaan koding diagnosis antara DPJP dan verifikator BPJS. Metode pengumpulan data kualitatif menggunakan informan utama sebanyak 3 orang yaitu 1 orang DPJP dan 2 orang verifikator. Untuk informan triangulasi sebanyak 3 orang yaitu 1 orang Kabid. Pelayanan Medis dan 2 orang koder rumah sakit. Data yang diambil meliputi data dokter tentang pengetahuan, sikap dan praktek yang dilakukan dengan metode wawancara mendalam. Analisis data digunakan dengan analisis isi. Hasil penelitian ini adalah manajemen rumah sakit tidak menyiapkan fasilitas dalam penulisan kode diagnosa untuk Dokter Penanggung Jawab Pasien (DPJP) sehingga menyebabkan kurangnya pemahaman DPJP tentang sistem INA CBG’s, tidak adanya intervensi dari manajemen rumah sakit menimbulkan ketidakpedulian DPJP dalam penulisan kode diagnosa sehingga mengakibatkan rumah sakit mengalami kerugian dalam pengklaiman BPJS namun rumah sakit terus aktif karena mendapatkan suntikan dana dari pemerintah, keterbatasan waktu yang dimiliki dokter menimbulkan hambatan dalam pengodean diagnosis. Saran peneliti meliputi melengkapi fasilitas dalam mendukung penulisan kode diagnosa, adanya intervensi manajemen kepada DPJP untuk melengkapi penulisan kode diagnosis, adanya standar operasional prosedur dan clinical pathway dalam pengodean.

Abstract

The problem that is often faced by hospitals as a place for health services is that they have not been able to provide something that is really expected by service users. So that complaints of dissatisfaction or patient complaints are often found in the process of providing health services. The purpose of this study was to analyze the quality of service on the satisfaction of BPJS inpatients at Bethesda General Hospital Gunungsitoli Nias. This research uses quantitative research with cross sectional design. The study population was BPJS inpatients at Bethesda Gunungsitoli Hospital, the sample using the Accidental Sampling technique was n = 165 people. This study used a questionnaire. The analysis of this study used univariate, bivariate and multivariate. The statistical test used was the chi square test and logistic regression. The results of the chi-square test were obtained on the variables of tangiable health service quality (p=0.000), reliability (p=0.000), responsiveness (p=0.000), assurance (p=0.000), attention (p=0.000) indicating a quality relationship service on satisfaction of BPJS inpatients at Bethesda Gunungsitoli General Hospital. The most influential variable with satisfaction is reliability p = 0.031 Exp (B 6.623) with an OR value of 25.234 (10.213-62.314). Based on these results, it was concluded that there was a relationship between service quality and satisfaction of BPJS inpatients at Bethesda Gunungsitoli General Hospital.

Abstract

Breast cancer can be detected early by conducting breast self-examination (REALIZING) which aims to prevent the advanced risk and can reduce the mortality rate in patients, because cancer can be found early so it will give Longer life expectancy. The aim of this researcher is to know the factors of late treatment in women with breast cancer in the district of Simeulue HOSPITAL in 2018. This research is a qualitative method of research that is done with the main goal to make a description or a descriptions of the delay treatment in women who suffer from breast cancer in the district HOSPITAL, Simeulue. Data analysis includes data processing with data reduction, display data, and conclusion or verificasion stages. The results of the study obtained the delay of treatment of breast cancer one of them by the level of education of the informant, knowledge and treatment facilities that are less complete, resulting in a delay in Breast cancer treatment. As a family support factor, friends and health workers only convince the informant to immediately treat the illness completely. The delay in treatment is low education and knowledge, late treatment at the previous treatment is incomplete, the delay of treatment due to the three components of this factor either family, friends or officers Health only strengthens the informant to immediately treat the disease.
