Abstract

Introduction: Nursing credentials are a formal system that aims to assess and recognize the competence of nursing staff in providing care that meets professional standards. This process is not only legal and administrative, but also reflects the level of expertise and professional responsibility of a nurse. Objective: to analyze and understand the application of Patricia Benner's Theory philosophy of "From Novice to Expert" in the nursing credential process. Research method: Qualitative descriptive with a case study approach, data collection was carried out through in-depth interviews, document searches and field observations, Results/findings: the implementation of nursing credentials at Dumai City Hospital has been implemented since 2015. With a total of 418 nursing staff with career levels PK I: 115 people, PK II: 114 people, PK 3; 80 people. BP 1: 23 people, BP 2: 44 people, BP 3: 36 people, while those who already have clinical authority according to career level PK 3: 38 people, PK 2: 21 people, and still on clinical authority PK1. Meanwhile, the number of midwives is BP 3: 7 people and BP 2: 3 people. Conclusion: The credential system at Dumai City Hospital has been implemented well, not only assessing the suitability of practice, but also fostering professionalism in a hierarchical, ethical manner, and adapting to field conditions

Abstract

Pendahuluan: Penyakit Paru Obstruktif Kronik (PPOK) merupakan masalah kesehatan global yang ditandai dengan hambatan aliran udara, air trapping, serta penurunan oksigenasi yang progresif. Intervensi nonfarmakologis seperti Pursed Lips Breathing (PLB) direkomendasikan untuk meningkatkan ventilasi dan saturasi oksigen. Tujuan: menganalisis dan memahami penerapan filosofi Teori Patricia Benner tentang From Novice to Expert dalam proses kredensial keperawatan. Metode penelitian: Desain penelitian menggunakan pendekatan deskriptif dengan durasi implementasi enam hari berturut-turut, waktu 23-28 oktober 2025 dilaksanakan di RSUD Arifin Achmad Propinsi Riau Ruang Jasmin melibatkan tiga pasien PPOK yang memenuhi kriteria inklusi. Intervensi PLB diberikan selama 15–20 menit setiap sesi, dilakukan tiga kali sehari. Pengukuran SpO₂ dilakukan sebelum dan sesudah intervensi menggunakan pulse oximeter terkalibrasi, kemudian data dicatat, dibandingkan, dan dianalisis secara deskriptif untuk melihat perubahan harian dan keseluruhan, Hasil/temuan: adanya peningkatan konsisten pada nilai SpO₂ setelah intervensi pada seluruh pasien, dengan peningkatan harian berkisar antara 1% hingga 3%. Rata-rata SpO₂ sebelum intervensi adalah 94,39%, meningkat menjadi 96,17% setelah PLB, dengan rata-rata peningkatan sebesar 1,78%. Tidak ditemukan efek samping, dan seluruh pasien dapat mengikuti teknik PLB dengan baik. Temuan ini menegaskan bahwa PLB efektif meningkatkan oksigenasi melalui perpanjangan fase ekshalasi, pengurangan air trapping, dan peningkatan efisiensi pertukaran gas. Simpulan: PLB merupakan terapi tambahan yang sederhana, aman, dan bermanfaat bagi pasien PPOK, serta dapat diintegrasikan ke dalam asuhan keperawatan rutin untuk meningkatkan fungsi pernapasan dan kenyamanan pasien

Abstract

Introduction: Nurses’ workload is an important factor influencing the quality of nursing performance in hospitals. Excessive workload may affect nurses’ physical and psychological conditions, including fatigue, work stress, burnout, and decreased work motivation, which ultimately impact the quality of patient care. This issue is important because nurses are healthcare professionals who maintain direct and continuous contact with patients during inpatient care. Imbalanced workload may also increase the risk of medical errors, reduce patient satisfaction, and hinder the achievement of hospital service standards. Objective: To analyze the relationship between workload and the performance of staff nurses in inpatient wards through a literature review. Methods: This study employed a literature review approach by searching articles from Google Scholar, PubMed, and SINTA databases using keywords related to workload and nurse performance. The inclusion criteria included quantitative studies published between 2019 and 2025 discussing workload and nurse performance. A total of 12 articles were analyzed narratively. Results: Most studies demonstrated a significant relationship between workload and nurse performance. High workload tended to reduce performance through increased fatigue and work-related stress. In addition, organizational factors such as staffing availability, work systems, and task distribution also influenced nurse performance. Conclusion: Workload has a negative effect on nurse performance. Therefore, optimal workload management is necessary to improve the quality of healthcare services and patient safety.
