Abstract

Background: Self-Awareness Behavior is related to the knowledge, attitudes, skills, motivation, and behaviors carried out by adolescents in maintaining and improving early detection as a preventative measure against breast cancer. This includes developing a woman's awareness of her own breast condition. Purpose: To determine the effect of the demonstration method on knowledge, attitudes, skills, motivation, and self-awareness behavior in adolescent girls. Method: Quantitative research design: Pre-experimental, pre-posttest, using a one-group design. The population in this study was all female students of SMKN 1 Bojonegoro, with a total population of 1,040 female students from grades 10 and 11 selected using stratified random sampling. Results: Analysis using the Wilcoxon test with a significance level of <0.05 demonstrated an increase in self-awareness behavior after the demonstration method, with an Asymp. Sig. (2-tailed) value of 0.000 <0.05. Conclusion: The demonstration method influences self-awareness behavior in adolescents.   Keywords: Demonstration Method; Self-Awareness Behavior; Adolescents; IMB theory.   Pendahuluan: Perilaku Sadari merupakan hal–hal yang berkaitan dengan pengetahuan, sikap, ketrampilan, motivasi, dan perilaku yang dilakukan oleh remaja dalam memelihara dan meningkatkan deteksi dini sebagai upaya pencegahan terhadap penyakit kanker payudara. Hal ini termasuk pengembangan kepedulian seorang perempuan terhadap kondisi payudaranya sendiri. Tujuan: Untuk mengetahui pengaruh metode demonstrasi dengan pengetahuan sikap keterampilan motivasi dan perilaku sadari pada remaja putri. Metode: Desain penelitian kuantitatif Pre – eksperimen pre – post test dengan pendekatan one group design. Populasi dalam penelitian ini seluruh siswi SMKN 1 Bojonegoro dengan jumlah populasi 1040 siswi dari kelas X dan XI dipilih dengan teknik Stratified Random Sampling didapatkan 57 Siswi. Hasil: Analisis menggunakan uji Wilcoxon dengan tingkat kemaknaan <0.05 terbukti ada peningkatan perilaku sadari setelah diberikan metode demonstrasi dengan nilai Asymp. Sig. (2-tailed) yaitu 0.000 < 0.05 Simpulan: metode demonstrasi berpengaruh terhadap perilaku sadari pada remaja   Kata Kunci: Metode Demonstrasi; Perilaku Sadari; Remaja; Teori IMB.

Abstract

ABSTRAK CVA (Cerebrovascular Accident) adalah gangguan fungsi otak yang berlangsung secara tiba-tiba dengan gejala klinis yang bisa bersifat lokal atau menyeluruh, dan terjadi diatas 24 jam (Mutiarasar, 2019). CVA merupakan salah satu penyakit yang memberikan dampak berupa kecacatan, diperkirakan sepertiga pasien dari jumlah penderita CVA di seluruh dunia mengalami cacat permanen (Motta, et al., 2021). Untuk fenomena yang peneliti dapat di RSUD Dr. R. Sosodoro Djatikoesoemo Bojonegoro, kasus pasien CVA menduduki 5 terbesar dengan jumlah 672 pasien dengan presentase 8,04% pada tahun 2024 (dashboard RSUD Sosodoro Djatikosoemo Bojonegoro). Penelitian ini menggunakan metode studi kasus dengan pendekatan asuhan keperawatan dengan mengambil salah satu kasus untuk dianalisis adalah pasien CVA Iskemik dengan masalah keperawatan penurunan kapasitas adaptif intrkranial di ruang saraf RSUD Dr. R. Sosodoro Djatikoesoemo Bojonegoro. Metode pengambilan data ini dilakukan dengan wawancara, pemeriksaan fisik, pemeriksaan penunjang. Instrumen pengumpulan data menggunakan format asuhan keperawatan dengan tahap pengkajian, diagnose, intervensi, implementasi dan evaluasi. Analisa data secara deskriptif. Hasil studi kasus ini didapatkan diagnosa yang sama yaitu penurunan kapasitas adaptif intrakranial b/d edema serebral (CVA iskemik) terjadi karena adanya keluhan nyeri kepala, tekanan darah meningkat,frekuensi nadi menurun dan pasien tampak lemas. Setelah dilakukan tindakan keperawatan selama 3x24 jam gangguan penurunan kapasitas adaptif intrakranial dapat meningkat dan menjadi normal. SOSODORO DJATIKOESOEMO BOJONEGORO ABSTRACT CVA (Cerebrovascular Accident) is a brain function disorder that occurs suddenly with clinical symptoms that can be local or generalized, and occur over 24 hours (Mutiarasar, 2019). CVA is a disease that has an impact in the form of disability, it is estimated that among the total number of CVA sufferers worldwide suffer permanent disability (Motta, et al., 2021). For the phenomenon that researchers found at RSUD Dr. R. Sosodoro Djatikoesoemo Bojonegoro, CVA patient cases occupy the 5th largest with a total of 672 patients with a percentage of 8.04% in 2024 (dashboard of RSUD Sosodoro Djatikosoemo Bojonegoro). This research uses a case study method with a nursing care approach by taking one of the cases for analysis, namely an ischemic CVA patient with a nursing problem of decreased intracranial adaptive capacity in the nerve room at Dr. RSUD. R. Sosodoro Djatikoesoemo Bojonegoro. This data collection method is carried out by interviews, physical examination, supporting examinations. The data collection instrument uses a nursing care format with stages of assessment, diagnosis, intervention, implementation and evaluation. Descriptive data analysis. The results of this case study showed the same diagnosis, namely decreased intracranial adaptive capacity due to cerebral edema (ischemic CVA) occurring due to complaints of headache, increased blood pressure, decreased pulse frequency and the patient appeared weak. After carrying out nursing actions for 3x24 hours, the decrease in intracranial adaptive capacity can improve and become normal. Abstract

Background: Anemia in pregnant women is a health problem that affects the mother and fetus. Compliance with antenatal care (ANC) visits and good diet play a role in preventing anemia. However, culture and community beliefs influence the diet of pregnant women, Transcultural Nursing emphasizes the importance of considering cultural factors in health services so that education is more effective. Purpose: To analyze the relationship between compliance with ANC visits Health Center. Method: The design of this study was correlational analytic with a cross-sectional approach. The sample consisted of 40 pregnant women in the third trimester selected by purposive sampling. Data was collected through medical record observations, diet questionnaires, and hemoglobin level examinations, then analyzed using the Chi-Square test. Results: ANC visits in pregnant women showed that 87.5% were non-compliant and were not significantly related to anemia (p = 0.496). Diet was mostly good (65%) and sufficient (35%), but also not significant for anemia (p = 0.232). Thus, diet remains a risk factor (OR = 4.167). A cultural approach is needed in education because beliefs about food taboos affect the nutritional intake of pregnant women. Conclusion: ANC visits and diet are not significantly related to the incidence of anemia, but diet remains a risk factor (OR = 4.167). Suggestion: Health workers need to implement Transcultural Nursing in nutrition education, adapting to local culture so that pregnant women understand the importance of nutrition and compliance with ANC visit.

Abstract

Hypertension is a major non-communicable disease in Luwihaji Village due to limited access to health care and low knowledge of non-pharmacological therapy. This study aims to examine the effect of slow deep breathing therapy on lowering blood pressure based on Nola J. Pender's Health Promotion Model theory. Using a quasi-experimental pretest-posttest design with a control group, a population of 40 hypertension sufferers was sampled with 30 respondents (15 intervention, 15 control) via simple random sampling. The digital sphygmomanometer instrument was analyzed univariately (frequency, mean) and bivariately (Paired T-Test, α=0.05) using SPSS. The results showed a significant decrease in systolic blood pressure (161.40 to 151.73 mmHg) and diastolic blood pressure (99.07 to 94.20 mmHg) in the intervention group (p=0.000), 100% of respondents decreased versus stable controls (p=0.082). It was concluded that slow deep breathing therapy was effective in lowering blood pressure significantly.

Abstract

Background: Located in Southeast Asia, Indonesia is one of the countries with the highest incidence of fatal snakebites in the world. Difficulty in identifying snake species and limited availability of antivenom often pose challenges in treating snakebites. Purpose: To illustrate a case of consumption coagulation caused by a hematotoxin-containing snake venom. Method: A case study study describes a snakebite incident managed in the Emergency Department (ED) of a secondary general hospital in Tulungagung, East Java. A 53-year-old male patient presented with pain, swelling, and bleeding in his left thumb, after being bitten by a yellow-brown Bandotan snake, approximately two hours prior to arrival. Results: The patient was given antivenom therapy according to the guidelines. Patients were also given antitetanus, antibiotics, platelet transfusions and fresh frozen plasma (FFP), supportive therapy, and strict monitoring. in the surgical high care unit (HCU). The patient showed improvement on the fifth day and was discharged on the tenth day. Conclusion: The use of local polyvalent antivenom combined with antibiotics, antitetanus therapy, platelets, and FFP transfusion has shown promising results in the management of toxin-induced coagulopathy. Suggestion: Further studies are needed to evaluate the effectiveness of therapy in different age groups, including children, adults, and the elderly. In addition, this study only focused on therapeutic management and did not assess the impact of treatment on organ dysfunction caused by post-treatment lesions.

Abstract

Background: Type 2 diabetes mellitus is a chronic metabolic disorder characterized by hyperglycemia due to insulin resistance or decreased insulin secretion. Herbal complementary therapies are increasingly in demand as an alternative to managing blood sugar levels. However, studies of its effectiveness as a single therapy in the elderly have not been widely researched. Purpose: To evaluate the effectiveness of moringa leaf tea as an antihyperglycemic alternative in elderly patients with type 2 diabetes mellitus. Method: A case report study conducted on one participant to evaluate the effect of moringa leaf tea on fasting blood sugar levels in elderly people with type 2 Diabetes Mellitus (DM). This approach was chosen to document individual responses to herbal interventions in actual clinical conditions and provide an in-depth picture of changes in blood sugar levels after intervention. The patient in this study was a 61-year-old woman who had been diagnosed with type 2 DM for one year and was not taking antihyperglycemic drugs. The participant had a history of non-compliance with pharmacological therapy due to previously felt side effects. Results: Before the intervention, the patient reported experiencing typical symptoms of hyperglycemia, such as frequent thirst, increased urination frequency especially at night, and fatigue that interfered with daily activities. Blood sugar levels were measured before the intervention, and were found to be 256 mg/dL, indicating hyperglycemia. After consuming moringa leaf tea 3 times a day for three consecutive days, blood sugar levels decreased and on the 3rd day it was 220 mg/dL. In addition, the patient also reported improvements in clinical symptoms, such as reduced thirst, increased urination frequency especially at night, and increased energy. Conclusion: Consumption of Moringa leaf tea can lower blood sugar levels in elderly people with type 2 diabetes mellitus without antidiabetic drugs. In addition, improvements in clinical symptoms such as reduced thirst, urination frequency, and increased energy were also observed. Suggestion: Further studies are needed with larger samples and longer intervention periods to evaluate its long-term efficacy and safety. In addition, this study can explore more deeply the molecular mechanisms associated with the antihyperglycemic effects of Moringa leaves and its potential in natural therapy-based diabetes management strategies.

Abstract

Background: Emergency Department (ED) overcrowding is a persistent global challenge that affects the quality and timeliness of patient care, particularly for sepsis management. Despite established guidelines such as the hour-1 sepsis bundle and national sepsis guideline, adherence remains inconsistent. Purpose: To analyze the relationship between sepsis protocol compliance and emergency unit crowding. Method: A retrospective observational cohort study was conducted, analyzing adult sepsis and septic shock patients triaged as Priority 1. Data on patient demographics, sepsis severity scores, ED occupancy rates, and adherence to sepsis protocols using the Hour-1 sepsis bundle and the National Sepsis Guideline were collected. Statistical analysis was performed using Fisher’s Exact Test to examine associations between ED overcrowding and compliance with sepsis protocols, with a significance threshold of p<0.05. Results: This study included 38 cases with a mean age of 56.6 years and the majority (52.6%) of patients were female. Most cases were diagnosed with septic shock (55.3%) and the average occupancy rate of the emergency department was 80.8%. Serum lactate measurements were performed in 68.4% of cases, blood culture sampling in 21.1%, and broad-spectrum antibiotics were given in 94.7%. Full compliance with both sepsis protocols was only 5.3%. Compliance with fluid resuscitation was high at 97.4%, while compliance with blood culture sampling was low at 5.3% and 7.9%. No significant association was found between protocol compliance and ED crowding (p > 0.05). Conclusion: There was no significant correlation between ED crowding and adherence to sepsis protocols. Strategic and clinical policy interventions are needed to improve adherence to sepsis management guidelines. Suggestion: Future research should investigate additional barriers to sepsis care, such as financial constraints, physician decision-making processes, and variations in adherence across healthcare settings to develop more comprehensive strategies to improve sepsis outcomes worldwide.

Abstract

Ibu primigravida post sectio caesarea dapat mengalami kecemasan karena merupakan pengalaman pertama dalam persalinan, nyeri luka operasi, keterbatasan mobilisasi, serta kekhawatiran terhadap kondisi bayi. Pengalaman persalinan dan kondisi ibu serta anak yang berbeda dapat menimbulkan respons kecemasan yang berbeda. Penelitian ini bertujuan membandingkan tingkat kecemasan pada dua ibu primigravida post sectio caesarea dengan pengalaman berbeda. Metode penelitian menggunakan desain deskriptif dengan pendekatan studi kasus pada dua klien, yaitu Ny. K dan Ny. N di ruang Obgyn RSUD Dr. R. Sosodoro Djatikoesoemo Bojonegoro. Pengumpulan data dilakukan melalui wawancara, observasi, pemeriksaan fisik, dan pemeriksaan laboratorium. Tingkat kecemasan diperoleh menggunakan Hamilton Anxiety Rating Scale (HARS). Hasil penelitian menunjukkan Ny. K mengalami kecemasan sedang dengan skor HARS 25, disertai kekhawatiran terhadap kondisi bayi dan persepsi bahwa panggulnya sempit. Ny. N mengalami kecemasan berat dengan skor HARS 32 karena kondisi bayinya dengan letak sungsang dan lilitan tali pusat pada leher janin. Hasil tersebut menunjukkan adanya perbedaan tingkat kecemasan pada kedua klien berdasarkan pengalaman serta kondisi ibu dan anak. Dengan tingkat kecemasan yang berbeda, pemberian dukungan psikologis, edukasi kesehatan, relaksasi napas dalam, dan komunikasi terapeutik dapat membantu meredakan kecemasan pada Ny. K dan Ny. N sesuai kebutuhan masing-masing. Perawat diharapkan memberikan dukungan psikologis berdasarkan pengalaman serta kondisi ibu dan anak untuk membantu proses adaptasi pascapersalinan. Kata kunci : ansietas, asuhan keperawatan, primigravida, ibu post sectio caesarea. Primigravida mothers after cesarean section may experience anxiety because childbirth is a first-time experience, as well as due to surgical wound pain, limited mobility, and concerns about the baby’s condition. Different childbirth experiences and maternal and infant conditions may result in different anxiety responses. This study aimed to compare anxiety levels between two primigravida mothers after cesarean section with different experiences. This study used a descriptive design with a case study approach involving two clients, Mrs. K and Mrs. N, in the Obgyn Ward of Dr. R. Sosodoro Djatikoesoemo Hospital, Bojonegoro. Data were collected through interviews, observations, physical examinations, and laboratory examinations. Anxiety levels were assessed using the Hamilton Anxiety Rating Scale (HARS). The results showed that Mrs. K experienced moderate anxiety with a HARS score of 25, accompanied by concerns about her baby’s condition and a perception that her pelvis was narrow. Mrs. N experienced severe anxiety with a HARS score of 32 due to her baby being in a breech presentation and having an umbilical cord wrapped around the fetal neck. These findings indicate differences in anxiety levels between the two clients based on their experiences and maternal and infant conditions. With different levels of anxiety, psychological support, health education, deep breathing relaxation, and therapeutic communication may help alleviate anxiety in Mrs. K and Mrs. N according to their individual needs. Nurses are expected to provide psychological support based on the experiences and maternal and infant conditions to help facilitate the postpartum adaptation process.
