Abstract

Background: Burning Mouth Syndrome (BMS) is a chronic orofacial pain disorder characterized by a persistent burning sensation of the oral mucosa in the absence of clinically observable lesions. BMS is classified into primary (idiopathic), often associated with neuropathic or psychogenic factors, and secondary forms linked to identifiable local or systemic conditions. Objective: This report describes a case of BMS with overlapping etiologies and evaluates the role of chlorine dioxide as a supportive agent in symptom management. Case: A patient with type 2 diabetes mellitus presented with a persistent burning sensation in the oral cavity without any clinical abnormalities. The condition was diagnosed as a combination of primary BMS with a psychogenic component and secondary BMS associated with diabetic neuropathy. Case Management: Management focused on symptomatic relief and improving oral mucosal comfort. Chlorine dioxide was administered as supportive therapy to enhance oral hydration and provide a soothing effect on the mucosa. The patient reported improvement in burning symptoms following its use. Discussion: BMS is a multifactorial condition requiring comprehensive evaluation of neuropathic, systemic, and psychological factors. In this case, the coexistence of psychogenic components and diabetic neuropathy highlights the complexity of diagnosis and management. Supportive therapy using chlorine dioxide may contribute to symptom alleviation through its hydrating and soothing properties, although its mechanism and efficacy remain insufficiently established. Therefore, a multidisciplinary approach is essential for the effective management of BMS, and further studies are needed to validate the clinical benefits of chlorine dioxide in BMS management.
