Abstract

Erythema multiforme is an acute vesiculo-bullous disease on the skin and membrane mucous, self limiting, and it presents with clinical variation (multiforme). Patients who suffer EM will feel painful of the oral lesion, wich could interfere in intaking of food and nutrition, and further more can bring down the body resistance. Approximatelly 50% case of EM caused by medicamen hiperensitivity reaction, most often because the class of sulfonamide drugs, NSAIDs, and anticonvulsants. A male patient aged 41 years, suffering of painful blisters with spontaneous bleeding on the mouth, difficulty in eating and talking. Patient was taking carbamazepine and amitriptyline given by neurologist. Patient also finished taking amoxicillin provided by dentist two days ago. Extra oral shows erosion and ulcers, painful and easily bleeding on the red upper and lower lips. Most ulcers were covered by erythematous-brown crust. Intra oral showed erosion/ ulcers, painful, easily bleeding, and covered by yellow-white pseudomembrane. There were no abnormalities on the skin. In conclusion to stop using all drugs that suspected as the cause, systemic corticosteroid administration, topical anesthesia, supportive therapy, and education for patient responded positively to patient’s recovery.

Abstract

Background: Systemic Lupus Erythematosus (SLE) is an autoimmune disease which damages tissues and causes chronic inflammation with an idiopathic etiology. It has been suggested that oral lesions in patients with SLE can be grouped clinically as erythema, discoid lesions and oral ulcerations. The latter have been said to indicate the onset of a severe systemic disease flare and that oral ulcers represent cases of mucosal vasculitis. Palatal lesions generally present in the form of ulcers or, in more severe forms, as perforation. Acquired palatal perforations can be caused by several etiologies including: developmental disorders, malignancy and infections. Purpose: To report the management of palatal perforation in an SLE patient. Case: A 14-year-old female patient attended the Dr. Ramelan Naval Hospital, with both a perforated palate that often caused her to choke when eating or drinking and maxillary anterior tooth mobility. Case Management: The treatment for the patient in this case consisted of debridement and DHE, pharmacological therapy including aloclair gel and minosep mouthwash to maintain oral hygiene and prevent re-infection. At the end of the first consultation, the patient was prescribed an obturator in order to avoid oro-anthral infection. During the second consultation, the patient’s orthodontic bracket was removed to facilitate scaling and splinting of the anterior maxillary teeth carried out to prevent their movement. During the third consultation, a swab was taken by an oral surgeon who also administered antifungal therapy. During the fourth and final consultation, the patient was examined a prosthodontic specialist due to an obturator which was causing discomfort. Conclusion: The management of palatal perforation lesions in an SLE patient requires a multidisiplinary approach.

Abstract

Background: In 2019, a viral disease spread from Wuhan, Hubei Province in China. The disease was caused by severe acute respiratory syndrome related to coronavirus 2 (SARS-CoV-2 virus), which was named by the Coronavirus Study Group of the International Committee on Taxonomy of Viruses. Purpose: This article reports the multi-discipline treatment of Covid tongue and exfoliative cheilitis with a main diagnosis of bilateral pneumonia caused by SARS-CoV-2 infection. Case: A female patient, 70 years old was referred with a diagnosis of bilateral pneumonia by a pulmonologist. She complained of painful wounds all over her mouth. An intraoral clinical examination revealed white-thick lesion and multiple ulcerations, whilst an extraoral exam for exfoliative dermatitis has not been described. The working diagnosis was Covid tongue mixed with exfoliative cheilitis. Case management: The procedure was performed by teledentistry and direct visits to implement oral health care by asepsis, debridement and the application of Oxygene gel. Conclusion: The lesion was treated successfully due to the multidisciplinary approaches by an internist and pulmonologist by implementing integrated knowledge and was supported hugely by patient cooperation.

Abstract

Pendahuluan: Oral lichen planus (OLP) merupakan penyakit inflamasi kronik pada membran mukosa mulut dengan karakteristik tanda klinis adanya retikuler papula berwarna putih yang dimediasi oleh sistem imun seluler ditandai oleh respon sel-T sitotoksik terhadap keratinosit basal dengan stres bisa sebagai faktor pemicu. Diagnosis OLP berdasarkan gambaran klinis yang khas ditunjang dengan pemeriksaan histopatologi. Tujuan: Melaporkan tata laksana kasus oral lichen planus dipicu stres pada pasien diabetes melitus. Kasus: Seorang wanita usia 68 tahun dengan keluhan nyeri, pasien sulit makan, sariawan yang persisten pada pipi kanan dan kiri sejak 4 tahun yang lalu, hilang kambuh. Pasien memiliki riwayat diabetes melitus. Kondisi pasien terlihat cemas pada saat anamnesis. Pemeriksaan klinis intra oral pada mukosa bukal bilateral terdapat papula putih berbentuk jala-jala (wickham’s striae) dan ulserasi. Tata laksana: Pasien diterapi dengan obat kumur anastetikum, kortikosteroid sistemik, deksametason elixir, obat kumur antiseptik dan multivitamin, menghindari makanan yang pedas dan berbumbu tajam serta pemeriksaan DASS 42. Pasien dirujuk untuk pemeriksaan darah lengkap dan glukosa sewaktu, glukosa 2 JPP, HbA1c, dan ke psikiater. Simpulan: Depresi sedang dengan gejala somatik dapat sebagai faktor predisposisi menyebabkan gangguan sistem imun yang memicu penyakit autoimun. Perawatan dengan mengelola stres serta terapi simtomatis dengan kortikoseroid sistemik dan topikal serta mempertahankan oral higiene.

Abstract

Hand foot and mouth disease (HFMD) adalah suatu penyakit menular yang disebabkan oleh sejumlah virus coxsackie A dan B. Penyakit ini biasanya mengenai anak, tapi dapat pula dijumpai pada usia dewasa muda. Tulisan ini bertujuan melaporkan tentang penatalaksanaan HFMD yang menyerang wanita muda 37 tahun dan anak 4 tahun. Penanganan pada kunjungan pertama berupa medikasi antivirus, antipiretik, multivitamin dan obat kumur. Penderita diedukasi tentang pentingnya isolasi, kemudian dikonsul ke ahli penyakit dalam supaya mendapat penanganan lebih lanjut, lalu dirujuk masuk rumah sakit/opname untuk bed rest dan mendapat perawatan lebih intensif. Hasil akhirnya adalah dengan diagnosis dan penatalaksaan yang tepat, maka HFMD mempunyai prognosis yang baik.

Abstract

Background: Oral candidiasis is an infection caused by Candida albicans. Candida infection can occur in patients with Diabetes Mellitus (DM) due to the high sugar content in the whole saliva and immunosuppresive condition of the patient. Purpose: to discuss case and treatment of acute erythematous candidiasis in patients with diabetes mellitus. Case: A 68 year old male came to RSAL dr. Ramelan Surabaya complaining of tendernes, burning and painful when eating and drinking in the left side of the tongue and. The tendernes was felt since about 8 months ago after the patient had a stroke. He had uncontrolled diabetic and hypertension.Three months later the patient felt the pain was getting worse with heartburn and pain. It was treated by given medication Nystatin drop and cefadroxil regularly until the drug ran out. During the first 2 months of treatment the tongue looked better but the pain and burning persisted. Shortly after,the tongue becomes dirty again. Case Management: Screening for diagnosis in this case include pathology anatomy examination by scrabbing the lesion, complete blood count(CBC) and blood sugar test. The result was positive hypae, normal CBC, and blood sugar level 324mg/dl. Patient was diagnosed acute erythematous candidiasis and treated with anti fungal systemic, mouthwash and topical antiseptic for oral case and reffered to interne specialist for diabetic condition. Patient was cured in 36 days. Conclusion: Controlling or eliminate predisposing factor in this case (DM) is very important in the management of acute therapy erythematous candidiasis. Comprehensive treatment for oral candidiasis in diabetic patient must include controlling predisposition factor and the right choose of anti fungal agent.

Abstract

Pemberian edukasi tentang kesehatan gigi dan mulut bertujuan untuuk meningkatkan pengetahuan dan cara pemeliharaan kesehatan gigi dan mulut khuusnya pada anak-anak Panti Asuhan Mahbubiyah Surabaya. Beberapa dari mereka kurang pengetahuan tentang bagaimana cara menjaga kesehatan gigi dan mulut yang baik sehingga diperlukan kegiatan pengabdian masyarakat dalam peningkatan pengetahuan pemeliharaan kesehatan gigi dan mulut agar terhindar dari kerusakaan gigi dan mulut yaitu berupa pemberian edukasi dan pemeriksaan rongga mulut anak anak Panti Mahbubiyah Surabaya. Pemeriksaan gigi dan mulut (oral screening) pada anak anak Panti Asuhan Mahbubiyah dilakukan untuk mendeteksi sejak dini kerusakan gigi dan mulut yang terjadi pada anak-anak. Pemberian edukasi kesehatan gigi dan mulut dilakukan dengan menggunakan bantuan alat peraga berupa phantom gigi dan poster. Setelah pemberian edukasi, anak anak diberikan beberapa pertanyaan langsung untuk mengetahui tingkat pengetahuan anak anak tentang pemeliharaan kesehatan gigi dan mulut. Kemudian dilanjutkan dengan pemeriksaan kesehatan gigi dan mulut pada anak anak. semua anak di panti asuhan sangat kooperatif dan mau diperiksa kondisi rongga mulutnya. Dengan adanya kegiatan ini anak-anak panti asuhan tersebut dapat mengerti mengenai permasalahan gigi dan mulut yang ada pada dirinya, sehingga dapat memotivasi keinginan untuk melakukan pemeriksaan dan tindakan lebih lanjut ke dokter gigi.

Abstract

Approximately 90% of individuals diagnosed with HIV/AIDS have encountered Oral Candidiasis during the progression of their disease, especially in patients with a reduced CD4+ 200 mm3. Oral Candidiasis is a prevalent opportunistic infection of the oral cavity. Reported a case of a patient with oral candidiasis in an HIV/AIDS with CD4+ below 50 and discussed the mechanism of molecular relationship decreasing count CD4+ with virulence of Candida Albicans. In this report, we present a case of a 35-year-old female who presented with oral candidiasis. The patient's CD4+ count test revealed a level below 50 and HIV test I-II-III reactive. Human immunodeficiency Virus causes a decrease in CD4+ count and leads to opportunistic oral candidiasis infection.

Abstract

Oral and dental health is an essential aspect of general health that must be maintained from an early age. This study aims to evaluate the impact of the Training of Trainer (ToT) program on dental and oral health for teachers and parents on changing tooth brushing behavior among students at SDIT Al-Uswah Surabaya. The methods used include surveys, training, evaluations, and observations of a tooth-brushing calendar filled out over 21 days. The results showed an increase in knowledge among teachers and parents, as well as positive changes in students' habits of brushing their teeth twice daily. These results highlight the importance of the role of teachers and parents in supporting dental and oral health promotion for children, which is expected to reduce the risk of dental caries in the future.
