Abstract

Approximately 90% of individuals diagnosed with HIV/AIDS have encountered Oral Candidiasis during the progression of their disease, especially in patients with a reduced CD4+ 200 mm3. Oral Candidiasis is a prevalent opportunistic infection of the oral cavity. Reported a case of a patient with oral candidiasis in an HIV/AIDS with CD4+ below 50 and discussed the mechanism of molecular relationship decreasing count CD4+ with virulence of Candida Albicans. In this report, we present a case of a 35-year-old female who presented with oral candidiasis. The patient's CD4+ count test revealed a level below 50 and HIV test I-II-III reactive. Human immunodeficiency Virus causes a decrease in CD4+ count and leads to opportunistic oral candidiasis infection.

Abstract

Background: One of the allergic reactions in the oral mucosa is ulceration caused by IgE-mediated reactions to food substances or non-IgE-mediated reactions and non-immunological mechanisms. Ulceration is a damage to the epithelium of the oral cavity that affects the nerve endings in the lamina propria layer so that it causes pain or heat that requires treatment and therapy. Objective: To report the occurrence of ulcers due to food allergies and therapy with tetracycline mouthwash 0.25% solution. Case: A 22-year-old female patient came with complaints of a small ulcer on the inside of the gums and the entire upper and lower lips and a numb and dry tongue. The patient always experiences an ulcer every time he eats seafood and certain foods that the patient cannot mention because it happens so often and every time an ulcer appears it will continue to get bigger. Management: Patients are advised to avoid allergens and refer for a complete blood allergy test and Ig E. Tetracycline mouthwash 0.25% solution was given to be rinsed three times a day. And added supportive therapy, giving multivitamins. Conclusion: Tetracycline mouthwash has antimicrobial properties and properties as inhibitors of tissue metalloproteinases (TIMPs) can accelerate healing time and ulceration does not continue to get bigger, reduce taste and pain, according to the theory it can reduce secondary speed limits and duration. inhibits collagenase. Keyword: Allergy, Mouthwash, Tetracycline, Ulcer

Abstract

Penyintas COVID-19 rentan mengalami post-COVID syndrome meskipun telah dinyatakan sembuh atau negatif dari COVID-19. Organ yang terserang post-COVID syndrome antaralain paru, telinga, hidung dan tenggorokan serta rongga mulut. Studi kasus ini melaporkan tatalaksana terintegrasi kasus necrotizing periodontal disease yang merupakan manifestasi post-COVID syndrome. Kasus terjadi pada wanita usia 25 tahun dengan keluhan gusi regio 23 mengalami kerusakan yang menyebabkan tulang penyangga gigi terekspose yang makin lama makin meluas disertai gigi goyah. Pasien dinyatakan terpapar COVID-19 pada 11 bulan sebelumnya. Kulit wajah serta seluruh tangan dan kaki tampak bercak merah. Keadaan umum pasien baik, namun terlihat kurus. Radiografi panoramik digunakan untuk memeriksa kondisi jaringan penyangga gigi. Gigi 23 direncanakan untuk diekstraksi sehingga pasien menjalani pemeriksaan darah lengkap dan tes autoimun. Pasien dicurigai mengalami gangguan sistemik, sehingga selanjutnya dilakukan tes human immunodeficiency virus (HIV), pemeriksaan CD4, serta screening hepatitis dan PCR, karena pasien diduga terpapar COVID-19 yang berulang. Tatalaksana selanjutnya pasien di-swab di sekitar regio 23 untuk memastikan peran mikroorganisme dengan dugaan acute necrotizing ulcerative periodontitis (ANUP). Seluruh hasil pemeriksaan menunjukkan kondisi dalam batas normal, dengan hasil swab menunjukkan temuan Klebsiella oxytoca. Tindakan bedah pada kasus post-COVID syndrome memerlukan beberapa pemeriksaan penunjang secara cermat dan perlu kolaborasi yang melibatkan berbagai multidisiplin ilmu. Diagnosis akhir pada kasus ini adalah necrotizing periodontal disease sebagai manifestasi post-COVID syndrome yang dipicu oleh badai sitokin dalam rongga mulut. Diagnosis iniditegakkan berdasarkan elaborasi penelusuran kemungkinan beberapa diagnosis. Pemeriksaan komprehensif yang teliti sangat diperlukan pada penanganan kasus yang langka dan sulit untuk mencegah terjadi komplikasi.
