Abstract

Rata-rata biaya insulin paling murah adalah jenis insulin manusia+ADO yaitu sebesar Rp 588.541,-Kata kunci : DMT2, Insulin Analog, Insulin Manusia,  BPJS, Pasien Rawat Jalan,  Outcome Klinis ABSTRACTDiabetes Mellitus (DM) is a group of metabolic diseases characterized by an increase in blood sugar levels (hyperglicemia), which occurs due to abnormal insulin secretion and insulin activity or both. This study aims to determine the clinical outcome and treatment profile of Diabetes Mellitus patients in Pasar Rebo Hospital and to know the cost comparison of  DMT2 treatment between patients using human insulin and analog insulin. This analysis uses independent variables such as single human insulin, human insulin + oral DM, human insulin + analog + oral DM, single analog insulin, and insulin analog + oral DM. The dependent variables are clinical outcomes such as GDP, GDPP and HBA1C. The sample of this study were outpatient patients diagnosed with DMT2 with insulin therapy, both insulin analog and human insulin in the period of January 2016 to December 2017 with BPJS guarantee and a minimum of three visits over two years in Pasar Rebo Hospital. The minimum sample calculation is based on the Krejcie-Morgan formula. Data collection is done by using a form that shows the results of therapy with GDP, GDPP and HBA1C, a form that shows the treatment profile of DM, Non DM and Insulin, and the cost profile form. The statistical method used in this study was the Kruskal Wallis non-parametric statistical test. The results showed that the treatment profile based on its type of analog insulin use in 2016 and 2017 amounted to (44,6%) when compared to human insulin which was only (9,8%). The use of short-acting insulin in 2016 was (39,5%) while in 2017 it was more than (55,8%). Lantus insulin the most used brand in 2016 was (42,36%) and Humulin R was most used in 2017 with (36,68%). In clinical outcome of the use of the five types of insulin gives a significant difference to the value of HBA1C but only in the type of human insulin + ADO can make a difference in the value of GDPP. The lowest average cost of insulin is a type of human insulin + ADO that is Rp. 588,541.- Keywords : Diabetes Melitus, Analog Insulin, Human Insulin, BPJS, Outpatients, Clinical Outcomes

Abstract

Temperature, rainfall and relative humidity are considered important climatic factors that contribute to the growth and spread of mosquito vectors and potential dengue outbreaks. The purpose of this study was to determine the relationship of air temperature, humidity, and rainfall with the incidence of DHF in the city of Palu in 2013-2017. This type of research is quantitative with correlation design. The sampling technique uses total sampling. The data source uses secondary data, namely the number of DHF events from the Palu City Health Office and climate data from the Mutiara Palu Meteorological Station. Data analysis using Pearson correlation test, the results of the study showed the strength of the temperature relationship with the incidence of DHF was very weak (r = -0.087), negative patterned and there was no significant relationship (ρ = 0.253). The strength of the humidity relationship with the incidence of DHF was very weak (r = 0.029), positive patterned and there was no significant relationship (ρ = 0.412). The relationship strength of rainfall with the incidence of DHF was very weak (r = 0.001), positive patterned and there was no significant relationship (ρ = 0.496). The conclusion was not found a meaningful relationship between the variables of temperature, humidity, and rainfall with the incidence of DHF. To overcome the DHF incident, it is expected that the Palu City Health Office can increase efforts to control DHF by increasing environmental improvement by running programs such as 3M Plus, Mosquito Nest Eradication (PSN), Abatization, and fogging.
