Abstract

Introduction: Paroxysmal nocturnal haemoglobinuria is a rare disease, and predominantly affects young adults,although pediatric cases have been reported. It is secondary to mutation in the X-linked gene of PIG-A, leading toloss of GPI-anchored membrane proteins, making the cell vulnerable to hemolysis by complement system.Case report: We report an 11-year-old female patient in follow-up since October 2017, with an initial diagnosis ofautoimmune hemolytic anemia despite DAT (direct antiglobulin test) persistently negative and maintaining haemolyticactivity despite corticosteroid therapy, with fatigue and mild to moderate thrombocytopenia. She was transferred fromhematologic service; a diagnostic review was performed and confirmed classic PNH by immunophenotyping. Dueto the underlying thrombocytopenia, an inventory was performed for bone marrow failures. The other differentialdiagnoses of DAT negative hemolytic anemias (membrane diseases, enzymatic deficiencies and hemoglobinopathies)were investigated and excluded, findings compatible with myelodysplasia were supported by medullary evaluation.Discussion: PNH is a heterogeneous disease and should be considered in all cases of hemolytic anemia with negativeDAT, thrombosis in non-usual sites and in bone marrow failure syndromes. As it is a rare disease and still unknownby most physicians, it is underdiagnosed and several patients are inadvertently given ineffective treatments due toerroneous etiologic diagnoses. The present case is even more relevant given the rarity of the disease in a pediatriccontext, with some clinical presentation peculiarities in this age group.

Abstract

The present study was designed to assess the comparative bio-equivalence of Flumisol®and Maquin® in healthybroiler chickens after oral administration of both products in a dose of 12 mg flumequine /kg.b.wt. Twenty four broilerchickens were divided equally into two groups (12 chickens for each group). The first group was designed to study thepharmacokinetics of Flumisol®, while the 2nd group was designed to study the pharmacokinetics of Maquin®. Eachbroiler chicken in both groups was orally administered with 12 mg flumequine /kg b.wt. Blood samples were obtainedfrom the wing vein and collected immediately before and at 0.08, 0.16, 0.25, 0.5, 1, 2, 4, 6, 8, 12 and 24 hours aftera single oral administration. The disposition kinetics of Flumisol® and Maquin® following oral administration of 12 mgflumequine/kg b.wt, revealed that the maximum blood concentration [Cmax] were 1.88 and 1.76 g/ml, attained at [tmax]of 1.42 and 1.41 hours, respectively.In conclusion: Maquin® is bioequivalent to Flumisol® since the ratios of Cmax, AUC0􀀀24 and AUC0􀀀1 (T/R) were0.93, 0.89 and 0.88, respectively. These are within the Bio-equivalence acceptance range. Flumisol® and Maquin®are therefore bioequivalent and interchangeable.

Abstract

The uterine leiomyosarcoma (LMS) is a rare cancer arising from the smooth myometrial cells. The LMS is clinicallyaggressive malignancy, accounting for 2% to 6% of uterine malignancies and a very low annual incidence.Object of the study: To evaluate management and outcome of the treatment of uterine leiomyosarcoma.Patients and methods: Retrospective study of a series of 13 patients treated for uterine leiomyosarcoma in thedepartment of radiotherapy of CHU Hassan II, Fes between 2012 and 2016Results: The condition is rarely suspected preoperatively, diagnosis is usually made on histological examination ofthe operative specimen. The outcome is related to the mitotic activity of the tumour and to the infiltration of nearlystructures. Surgery is the only effective treatment, chemotherapy is ineffective, adjuvant radiotherapy improves localcontrol of the tumour but has no incidence on survival.Conclusion: In the absence of an effective adjuvant treatment, uterine leiomyosarcoma bears a poor prognosis withthe only exception of small non infiltring tumours with a low mitotic activity.

Abstract

Hodgkin lymphoma is a type of malignant disorder in which Reed-Sterberg cells are present in a characteristicbackground of reactive inflammatory cells of various types, accompanied by fibrosis of variable degree.More than sixty years ago Hodgkin’s Lymphoma was considered a “systemic” malignancy and universally fatal. Morerecently less extensive radiation therapy in combination with chemotherapy has resulted in an excellent overall survivaland prolonged disease free survival in children.There is age specific difference in the representation of the various subtypes of Hodgkin lymphoma (lymphocyticpredominance 2–7% in adults and only 20% in children).

Abstract

The Department of Health and Human Services owns US Patent 6630507 which states that cannabinoids are powerfulantioxidants and have the potential to treat myriad ischemic, age-related, inflammatory, and auto-immune disorders. Thepatent claims cannabinoids are also a natural neuroprotectant which could help treat neurological damage caused bystroke and trauma, as well as other neurological disorders like Alzheimer’s, Parkinson’s Disease, and Multiple Sclerosis.This perspective paper assesses the properties and usefulness of the patented cannabinoid.

Abstract

AbstractObjective: To compare the periodontal condition and the microbiological status between the vestibular group (VG), lingual group (LG) and control group (CG).Mataerial and Methods: 18 patients(mean age: experimental group: 32 years old; and control group: 25 years old): 6 treated with vestibular fixed appliance, 6 with lingual appliance and 6 subjects in the control groupwho did not receive orthodontic treatment. A periodontal exam was performed before placing braces (T0) and after 4 weeks (T1). The gingival index (GI), plaque index (PI), Bleeding on probing (BOP) and periodontal pocket depth (PPD) was evaluated. Through PCR was detected the presence or absence ofEikenellacorrodens (Ec), Fusobacterium nucleatum (Fn), Agregati bacteractinomycetemcomitans (Aa), Porphyromonas gingivalis (Pg), Tannerella forsythensis (Tf), Terponema denticolla (Td), and Prevotella intermedia (Pi).Results: It was found a significant increased in the plaque index and gingival index. No significant changes were found according to BOP and PPD. The microbiological results show significant differences between the control group and the experimental groups in Aa, Tf, Pi, Fn and a tendency to an increase of Pg, Td in both experimental groups. Conclusions: There’s a direct relation between the placement of fixed appliance (vestibular and lingual) and the increase of plaque index and gingival index. Also, the growth of periodonto pathogens in subgingival plaque is promoted. Clinical relevance : This study stresses the importance of monitoring orthodontic patients. Vestibular orthodontic appliance seems to be better than the lingual in terms of less worsening condition during time.

Abstract

Buccal drug delivery has become a relevant route of administration. It has got various advantages, masking the firstpass metabolism being the primary one, thus ensuring increased drug bioavailability at the site of action. Tacrolimus,an immunosuppressant, calcineurin inhibitor is the drug of choice. Oral tablet suffers from high first pass metabolismand topical formulations such as ointments readily washout from the site of application. Tacrolimus formulated asmucoadhesive buccal film provided prolonged effect than topical formulations. Films (F1 & F2) were prepared bysolvent casting technique. Chitosan being the best mucoadhesive polymer with wound healing properties increased theretention time of the film at the wound site. HPMC ensured good film property. Propylene glycol was used as plasticizerand citric acid as salivary stimulator and cross linker. The compatibility of drug in the formulation was confirmed byFTIR. Formulated films were subjected to various evaluation parameters, and all the physical parameters were withinthe acceptable limits. In vitro release was found to be better for F1. Ex vivo studies of F1 using goat mucous showed apermeation of 76.5%.

Abstract

Introduction: Osteoarticular infections (OAI) are an infectious pediatric pathology, generally of bacterial etiology, relatively frequent and potentially severe. Objective: To determine the clinical, etiological and evolutionary characteristics of osteoarticular infections in hospitalized children in a referral center. Materials and methods: Observational, retrospective cross-sectional study. We reviewed the medical records of all children from 1 month to 15 years of age hospitalized in the Pediatric Service of the Institute of Tropical Medicine (IMT) from 01/01/2010 to 12/31/2015 with diagnosis of osteomyelitis, Osteoarthritis or Septic Arthritis Results: A total of 59 children with an OAI diagnosis were hospitalized. 25 (42.4%) diagnosed with septic arthritis, 6 (10.2 %) polyatritis, 21 (35.6 %) osteomyelitis and 7 (11.9 %) osteoarthritis. The mean age was 8.9 + 3.8 years, 40 (67.8 %) were males. Pain and functional impotence were the most frequent symptoms. 47 (79.6%) were located in lower limbs. Staphylococcus aureus methicillin resistant (MRSA) 17 (41.5 %), methicillin sensitive Staphylococcus aureus (SAMS) 18 (43.9 %), Pseudomonas sp. 2 (4,9 %), Klebsiella pneumoniae BLEE 3 (7.3 %), another 5 (12,1 %). In this series 2 patients diagnosed with septic shock at the joint point (Hip and knee respectively) without germ isolation died. Conclusion: OAI remains to be pathology of high morbidity in our environment. They affect more often the lower limbs as well as school-age male children. The UAA was the most frequent isolated germ. The mortality rate is low and the sequelae continue to appear.
