Abstract

In a recent article, Thompson et al. reported about seven patients carrying the heterozygous mutations c.239G>A (4 patients) and c.703C>G (3 patients) in the SLC25A4 gene, clinically manifesting with severe mitochondrial DNA depletion syndrome1. All patients were born with respiratory insufficiency requiring artificial ventilation since birth1. We have the following comments and concerns.

Abstract

Delirium is a widespread neurological confusion detected in intensive care units (ICU). Patients in ICU are at veryhigh risk for the development of delirium due to numerous elements. Routine examining of all patients in the ICU forthe presence of delirium is extremely important to its successful management. Nurses are at the forefront to identify,control, and even prevent ICU delirium. The main aim of the study was to assess the delirium in ICU patients. Thestudy involved 52 patients were recruited from 80 consecutive admissions to the General ICU, Mansoura InternationalHospital during a period of 6 months (February to July 2017). Patient assessment sheet was used for data collectionincluding Demographic characteristic and health relevant data, The Richmond Agitation-Sedation Scale (RASS), fordelirium recognition in acute care setting, the confusion estimation method can be utilized as it is a legitimized toolwith excessive level of sensitivity and specificity. The results of the study revealed that the delirium detected in largestpercentage (65.3%) of patients, and there were numerous threat factors for delirium were recognized together with, longduration of ICU stay, respiratory diseases, heart diseases, renal diseases, infection, and hypertension. The findings ofthe study highlighted the need for more interest ought to be paid to the implementation of a tested delirium screeninginstrument which includes the Confusion Assessment Method for the ICU ( CAM-ICU ) and should be a part of habitualevery day vital care.

Abstract

In a recent article, Li et al. reported about the application of3D pseudocontinuous arterial spin labelling (3D pCASL) to9 patients with mitochondrial encephalopathy, lactacidosis,and stroke-like episodes (MELAS) syndrome within onemonth after onset of a stroke-like episode (SLE)1 . 3DpCASL revealed hyperperfusion during the acute andsubacute stage of a SLE in all 9 MELAS patients1 . We havethe following comments and concerns.

Abstract

The information technology may provide alternative approaches to Osteoporosis disease diagnosis. This systematicreview was performed to compare the diagnostic accuracy of vertebral fracture assessment. In this study, we examinethe potential use of classification techniques on a massive volume of healthcare data, particularly in prediction ofpatients that may have Osteoporosis through its risk factors. For this purpose, we propose to develop a new solutionapproach based on Random Forest decision tree to identify the osteoporosis cases. There has been no researchin using the afore-mentioned algorithm for Osteoporosis patients‘ prediction. The reduction of the attributes consiststo enumerate dynamically the optimal subsets of the reduced attributes of high interest by reducing the degree ofcomplexity. A computer-aided system is developed for this purpose. The performance of the proposed model in thisstudy is analyzed and evaluated based on set of benchmark techniques applied in this classification problem.

Abstract

Objective: To assess the problems faced by Lady Health Visitors during polio vaccination campaignin district Quetta.Method: This study is a qualitative study based on interviews with LHVs in Quetta. The saturation point was 8interviews however, a total of 10 interviews were conducted for accuracy. The subjects were selected based on snowballtechnique. Data was transcribed, and frequencies were drawn on SPSS version 20, after three themes were generatedusing the standard procedure namely; Pre- campaign issues, Issues during campaigning and Post- campaign issues.Result: The most prevalent pre-campaign issue recorded was conveyance issue i.e. travel of LHVs from their houseto Polio center and vice versa. The most common issue faced during the door to door campaign was rude behavior ofpeople and taunting. Character calling was the second most common issue recorded. Furthermore, not opening doorsfor LHVs was the thirst commonest issue. victimization and harassment by pedestrians and public. The most commonpost-campaign issues recorded was delay in provision of pay to the LHVs along with non-cooperation of departmentsin solving issues of LHVs.Conclusion: This study is an admonition for concerned departments and authorities. With such issues being faced,appropriate incentives should be given to the on duty LHVs. Departments need to solve the issues on priority basis.Most incidences of victimization by public are based on their religious views hence it is strongly recommended thatreligious authorities be involved in the campaigns to play their effective role in changing the perceptions of people aboutPolio immunization campaigns. Nonetheless, mass media campaigns are recommended to aware the community aboutPolio and the role of LHVs in eradication of polio from Pakistan.

Abstract

Introduction: Kaposi’s sarcoma is an angio proliferative disorder related to a malignant neoplasm predominantly of mucocutaneous tissues. There are four clinical presentations of Kaposi’s sarcoma: Classic, Endemic, Epidemic or related to HIV and Iatrogenic, all of which are mainly caused by human herpes virus type 8. 
Objective: We present a case of a 73-year-old male, type II diabetic, with a family history of cutaneous carcinoma, diagnosed with non-HIV related Kaposi’s sarcoma.Discussion: Kaposi’s sarcoma is mainly associated with the herpes virus 8, this seems to be necessary for the development of the disease. We present a case of classical Kaposi’s sarcoma, commonly affecting people older than 60 years. It is characterized by violaceous red maculae in lower limbs progressing to nodules. The health care provider must be able to make the diagnosis based on skin lesions and perform a biopsy only to confirm histological diagnosis. The 5-year survival for low eruptivity 97%, moderate 92.7% and high 75%.Conclusion: We found a classic Kaposi sarcoma with no relationship to HIV infection, a case not well documented, with palliative management and with a good response to it, since there are not established management protocols available.

Abstract

Otomycosis, is called “swimmers ear”, it is a common ear disease in moist environment.The aim of this survey was to detect otomycosis among the fishing community of Bénisaf harbour, Ain-Témouchent (Algeria). The study involved 220 artisanal fishermen, 18 specimens of ear swabs, and ear scrapings from individuals with clinical otitis were examined from June to August 2015. These specimens were submitted for mycological investigation for reasons such as pruritus, ear blocking or tinnitus. Prevalence of otomycoses among fishermen with otitis was 38,9% (7 positive cases). The maximum of positive patients was seen in the age group above 41 years (57,1%), and 57,1% of positive cases had over 21 years’ experience of working as a fisherman. Candida genus was the most encountered fungi (57,14%), followed by moulds (42,58%). Fishermen work in aquatic environment and are exposed to heat and moisture.In summary, this study demonstrates that the diagnosis of otomycoses among fishermen requires vigilance from clinician given its non-specific symptoms.
