Abstract

The transformational leadership style of the ward head plays a strategic role in shaping a nursing organizational culture that supports high‑quality healthcare services. This study aims to examine empirical evidence on the influence of the ward head’s transformational leadership style on nursing organizational culture, with a particular focus on three key outcomes: professionalism, workplace spirituality, and nurses’ work effectiveness. The review was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) approach. From a total of 233 identified records, 16 articles met the inclusion criteria. The literature search was performed using the PICOT framework across the PubMed, Gale OneFile, and ProQuest databases, employing the keywords “transformational leadership” AND “head nurse”. The findings demonstrate that the ward head’s transformational leadership exerts a significant influence on nursing organizational culture. This leadership style enhances nurses’ professionalism by strengthening clinical autonomy, ethical competence, clinical leadership, and creativity. Furthermore, transformational leadership fosters workplace spirituality by increasing the meaning of work, psychological well‑being, caring values, and perceptions of organizational justice. These improvements subsequently contribute to greater work effectiveness, reflected in enhanced nurse performance, higher quality of nursing care, improved patient safety, stronger nurse retention, and greater organizational sustainability. Nursing organizational culture is shown to function as a key mediator that links transformational leadership to these diverse outcomes. This scoping review confirms that the transformational leadership style of the ward head constitutes a crucial managerial strategy for cultivating a professional, meaningful, and effective nursing organizational culture, with direct implications for improving the overall quality of healthcare services.Keywords: transformational leadership; ward head; nursing organizational culture; professionalism; workplace spirituality; work effectiveness

Abstract

In Indonesia, the burden of cervical cancer continues to rise, with low screening coverage contributing to delayed diagnosis and preventable deaths. Early detection through screening methods such as Visual Inspection with Acetic Acid (VIA) and Pap smear is essential; however, national coverage remains far below the recommended target. Knowledge deficits among women have been identified as a key barrier to participation in early detection programs. This study aimed to identify the level of knowledge among women regarding early detection of cervical cancer in Sungai Rangas Village, Martapura Barat District. A descriptive quantitative design was employed, and the study was conducted in June 2024. The population consisted of 153 women of reproductive age who had engaged in sexual intercourse and had never undergone cervical cancer screening. Using the Slovin formula and consecutive sampling, 115 respondents were recruited through a door‑to‑door approach across three neighborhood units. Data were collected using a validated and reliable questionnaire comprising 14 multiple‑choice items on cervical cancer and early detection. Validity coefficients ranged from r = 0.353 to r = 0.9707, exceeding the critical value of r = 0.3246, and the reliability coefficient was 0.986. Data were analyzed descriptively to summarize respondent characteristics and knowledge levels, with results presented in tabular form. The findings showed that most respondents were aged 20–30 years, had completed elementary education, were not employed, and primarily received information from healthcare workers. Knowledge levels indicated that 57.4% of women demonstrated good knowledge regarding early detection of cervical cancer, while 42.6% exhibited poor knowledge. These results highlight persistent gaps in awareness that may contribute to low screening uptake in the community. In conclusion, although more than half of the respondents demonstrated adequate knowledge, a substantial proportion still lacked essential understanding of cervical cancer and its early detection methods.Keywords: cervical cancer, early detection, knowledge, women of reproductive age, screening, Indonesia

Abstract

Bullying is a form of chronic social stress that has significant adverse effects on adolescent mental health, including an increased risk of depression, anxiety, psychological distress, post-traumatic stress disorder, and the development of maladaptive self-beliefs. The literature indicates that the impact of bullying victimization is not solely determined by the frequency and intensity of bullying experiences, but also by cognitive processes such as threat perception, self-meaning, resilience, and self-efficacy. This review aims to formulate a conceptual framework for individual counseling based on the Health Belief Model (HBM) for adolescents who have experienced bullying through a synthesis of recent empirical findings. A narrative literature review design was employed, with articles retrieved from Scopus, PubMed, ScienceDirect, and Google Scholar published between 2019 and 2025. Thematic analysis was conducted to identify patterns in the psychological impacts of bullying, the role of protective factors, and the relevance of cognitive-based interventions. The findings indicate that bullying victimization contributes to the internalization of negative cognitive schemas that influence coping behaviors and help-seeking processes. Resilience and self-efficacy were identified as important mediators and moderators in the relationship between bullying and mental health outcomes. The Health Belief Model provides a relevant conceptual basis for individual counseling by emphasizing perceived susceptibility, perceived severity, perceived benefits, perceived barriers, and self-efficacy.Keywords: bullying; adolescents; mental health; individual counseling; health belief model

Abstract

Musculoskeletal complaints are a common occupational health problem experienced by nurses in hospital settings, primarily resulting from patient‑handling activities such as lifting, transferring, and maintaining static or awkward postures for prolonged periods. These conditions have been reported with a very high prevalence and may lead to chronic pain, disability, and increased work absenteeism. The aim of this study was to analyze and identify the risk factors contributing to musculoskeletal complaints among hospital nurses. This study employed a systematic review design by searching the electronic databases ScienceDirect and PubMed using the keywords “risk factors”, “musculoskeletal complaints”, and “hospital nurses”. The inclusion criteria consisted of English‑language articles published between 2020 and 2025, free‑access, and full‑text availability. The study selection process followed the PRISMA protocol, resulting in 15 eligible articles. The review identified 18 risk factors associated with musculoskeletal complaints among hospital nurses: sex, BMI, age, marital status, education level, smoking habits, exercise habits, length of employment, lack of assistance and non‑use of assistive devices, working posture, work unit, work shift, employment status, workload, lack of training, job stress, low social support, and fatigue. The most frequently reported musculoskeletal complaints were located in the lower back, neck, and shoulders. These findings highlight the need for preventive measures such as ergonomics‑related training, stretching exercises, workload regulation, adequate rest periods, and the promotion of healthy lifestyle behaviors.Keywords: risk factors; musculoskeletal complaints; hospital nurses

Abstract

Hearing impairment is known to interfere with communication ability and may lead to social isolation, emotional distress, depression, and anxiety. Cochlear implant technology represents an effective rehabilitative solution for individuals with severe to profound hearing loss. This study aimed to evaluate the benefits of cochlear implant use through measurements of hearing thresholds and Categorical Auditory Performance II (CAP II) scores before implantation and after six months and one year of device use. A mixed‑methods design with a pre–post group approach was employed, involving a sample of 60 cochlear implant users with more than six months of device experience who met the predetermined eligibility criteria. Data were obtained from the CMS database, questionnaire administration, and interviews with otolaryngologists, audiologists, therapists, and parents of cochlear implant users. Qualitative analysis demonstrated improvements in hearing thresholds and auditory performance following cochlear implantation, which subsequently enhanced verbal communication abilities. These improvements were associated with consistent device use, auditory listening therapy, and adaptation over time. Statistical analyses using Friedman, Wilcoxon, and multiple linear regression tests revealed a significant improvement in hearing thresholds, decreasing from a mean of 104.42 dB before implantation to 48.42 dB after six months and 33.33 dB after one year of use (p < 0.05). CAP II scores also increased substantially, from a mean of 0.57 pre‑implantation to 3.08 at six months and 4.82 at one year. Age, baseline test results, and therapy consistency were identified as influential factors affecting final outcomes. In conclusion, younger age, consistent cochlear implant use, and adherence to auditory verbal or speech therapy were shown to significantly improve hearing ability and auditory performance in individuals with severe to profound hearing impairment.Keywords: cochlear implant; hearing threshold; categorical auditory performance II

Abstract

Hypertension remains a major public health concern in Indonesia, with a steadily increasing prevalence across age groups, including young adults. Individuals with a family history of hypertension are at heightened risk due to genetic predisposition and lifestyle factors, making early detection of cardiovascular reactivity essential. The cold pressor test is a well‑established physiological stress test used to assess sympathetic activation and acute blood pressure responses. This study aimed to examine the effect of the cold pressor test on systolic and diastolic blood pressure among young adults with a hereditary history of hypertension in Tasikmalaya, West Java. A quasi‑experimental quantitative design with a single‑group pre‑test and post‑test structure was employed. The study was conducted over three months at Universitas Bakti Tunas Husada and involved 178 respondents selected through consecutive sampling based on predefined inclusion and exclusion criteria. Blood pressure was measured before and after cold pressor test using a calibrated digital sphygmomanometer, and data were analyzed using descriptive statistics and paired t‑tests. The findings demonstrated a significant increase in both systolic and diastolic blood pressure following cold pressor test exposure. Mean systolic pressure rose from 118.80 mmHg to 137.22 mmHg, while mean diastolic pressure increased from 77.70 mmHg to 82.95 mmHg. Both changes were statistically significant (p < 0.05), indicating heightened cardiovascular reactivity among individuals with a family history of hypertension. These results suggest that cold pressor test may serve as a useful early screening tool for identifying young adults at risk of developing hypertension, thereby supporting timely preventive interventions.Keywords: hypertension; cold pressor test; blood pressure reactivity; young adults; family history of hypertension

Abstract

The collection of blood samples for Congenital Hypothyroidism Screening (CHS) is a routine neonatal procedure that induces moderate pain, thereby requiring effective interventions to reduce discomfort. This study aimed to examine the effect of the kangaroo method on neonatal pain scores during CHS blood sampling. A quasi‑experimental design with a posttest‑only control group approach was employed, involving 56 neonates selected through quota sampling at Garuda and Ibrahim Adjie Primary Health Centers in Bandung. Pain intensity was assessed using the Neonatal Infant Pain Scale (NIPS) and analyzed with an independent sample t‑test. The findings demonstrated a significant difference between groups, with the intervention group showing a mean pain score of 2.83 compared to 5.79 in the control group. Statistical analysis confirmed a significant effect of the kangaroo method in reducing neonatal pain scores (p = 0.000; p < 0.05). The analgesic mechanism of the kangaroo method operates through the modulation of dual neural pathways that inhibit pain impulses, thereby decreasing neonatal pain responses. These results indicate that the kangaroo method is an effective, practical, and safe non‑pharmacological intervention for reducing neonatal pain during CHS blood sampling and can be recommended for routine pain management in neonatal care.Keywords: kangaroo method; pain; congenital hypothyroidism screening

Abstract

Knee osteoarthritis is a degenerative joint disease that commonly causes pain, stiffness, and decreased functional capacity, particularly in older adults. This study aims to evaluate the effect of physiotherapy interventions in the form of Hip Abductor Strengthening Exercise and Quadriceps Strengthening Exercise on reducing pain and muscle stiffness in patients with grade 3 right knee osteoarthritis. A case report design was used involving a 57-year-old male patient who underwent a physiotherapy program consisting of three treatment sessions, with an exercise dosage of 10 repetitions in 2 sets and a holding time of 5 seconds for each repetition. Pain intensity was assessed using the Numeric Rating Scale (NRS), while muscle stiffness was evaluated through measurements of joint range of motion (ROM) to assess joint mobility and Manual Muscle Testing (MMT) to assess muscle strength. The results demonstrated a reduction in pain intensity from NRS 5 to 3, accompanied by an increase in knee flexion range of motion from 80° to 100° and an improvement in muscle strength from MMT grade 2+ to grade 4. These findings indicate a reduction in muscle stiffness and an improvement in the patient’s motor function. In conclusion, the combination of Hip Abductor Strengthening Exercise and Quadriceps Strengthening Exercise is effective in reducing pain and muscle stiffness and in improving knee joint function in patients with grade 3 right knee osteoarthritis.Keywords: knee osteoarthritis; hip abductor strengthening exercise; quadriceps strengthening exercise; pain; muscle stiffness; physiotherapy
