Abstract

Low self-esteem is a common negative symptom observed among patients with mental disorders and can significantly affect their self-confidence and social interaction abilities. This condition often leads patients to feel worthless, withdraw from their environment, and experience difficulties in performing daily activities. The purpose of this case report is to describe the application of nursing interventions using the Implementation Strategies (SP1–SP4) to enhance self-esteem in patients experiencing low self-esteem. The method employed is a case report based on the nursing care process, including assessment, diagnosis, intervention, implementation, and evaluation. The subjects of this report were four patients diagnosed with low self-esteem. The interventions provided included establishing a therapeutic relationship, identifying patients’ positive abilities, and training them in simple activities such as making their beds, washing dishes, and performing light exercise. The interventions were carried out over six days with daily monitoring of patient responses. The results demonstrated a gradual improvement in patients’ self-confidence, ability to maintain eye contact, social interaction, and independence in performing daily activities. In conclusion, the implementation of the SP1–SP4 strategies was effective in improving self-esteem and enhancing independence among patients with low self-esteem.Keywords: low self-esteem; nursing interventions; psychiatric nursing; self-confidence

Abstract

Diabetes mellitus remains a major global health concern, with a substantial proportion of cases remaining undiagnosed. Delayed detection is particularly problematic in countries with high disease burden, where individuals often present only after complications have developed. This study aimed to identify patient-recognizable “warning” indicators observed prior to formal diagnosis to inform health promotion and opportunistic screening strategies. This study used a qualitative case study approach, symptom experiences among previously undiagnosed individuals in Sleman, Yogyakarta, were explored and analyzed. The findings indicated that hypertension, neuropathic complaints (numbness and tingling), excessive daytime sleepiness, and persistent fatigue commonly preceded diagnosis. These manifestations function as pragmatic screening prompts when interpreted alongside established metabolic risk factors. In conclusion, diagnostic delay is effectively reduced by integrating symptom awareness into risk-based screening and structured referral pathways. Formalizing everyday symptom experiences as systematic entry points into healthcare services is essential to improve early detection for undiagnosed diabetes mellitus.Keywords: diabetes mellitus prevention; health promotion; undiagnosed; screening

Abstract

The assumption that financial investment is directly proportional to the quality of construction safety planning is commonly used as a basis for project budgeting. However, the persistently high rate of occupational accidents indicates a gap between cost allocation and the effectiveness of risk mitigation in practice. This study aimed to evaluate the relationship between the allocation of Construction Safety Management System costs and the quality of Construction Safety Plan documents, as well as to identify cost efficiency across construction work packages. This study employed a descriptive-analytical approach using document-based data obtained from Construction Safety Plan documents across several construction work packages. Document quality was assessed using a scoring method and further analyzed using a cost-effectiveness analysis approach to evaluate efficiency. The findings indicate that low-cost work packages are capable of producing high-quality Construction Safety Plan documents, while high-cost packages tend to be inefficient and fail to meet substantive performance evaluation criteria. These results suggest that planning quality is more influenced by personnel competence than by budget size. In conclusion, budget size does not necessarily guarantee compliance with construction safety standards. Therefore, safety planning evaluation should prioritize technical substance and regulatory compliance to enhance the effectiveness of worker protection.Keywords: cost effectiveness; construction safety plan; regulatory compliance; occupational safety

Abstract

Premarital sexual behaviour refers to intimate actions between two individuals who are emotionally attached but not legally married. Such behaviour is influenced by internal factors, including self‑esteem and depression, as well as external factors such as the roles of parents and teachers. Understanding these determinants is essential for developing effective adolescent reproductive‑health interventions. This study aimed to determine the relationship between parental roles, teacher roles, and self‑esteem with premarital sexual behaviour among adolescents at SMP Negeri 26 Samarinda. This research employed an observational analytic design with a cross‑sectional approach. The study involved 183 male and female students selected through stratified random sampling. Data were collected using structured questionnaires, and statistical analysis was performed using the chi‑square test to assess associations between variables. The findings showed no significant relationship between parental roles and premarital sexual behaviour among adolescents at SMP Negeri 26 Samarinda (p = 0.461). Similarly, no significant association was found between teacher roles and premarital sexual behaviour (p = 0.341). Self‑esteem was also not significantly associated with premarital sexual behaviour (p = 0.913). The study concludes that parental roles, teacher roles, and self‑esteem are not significantly associated with premarital sexual behaviour among adolescents at SMP Negeri 26 Samarinda. These results suggest that other factors may play a more substantial role in influencing adolescent premarital sexual behaviour and should be explored in future research.Keywords: parents; teachers; self‑esteem; premarital sexual behaviour

Abstract

Congenital hypothyroidism is a congenital disorder caused by impaired thyroid gland function in newborns and may lead to growth retardation and permanent intellectual disability if not detected early. Congenital hypothyroidism screening is recommended by the World Health Organization and has been implemented nationally by the Ministry of Health of the Republic of Indonesia as part of newborn screening programs. However, screening coverage in several regions, including Nias Regency, has not yet reached the expected target. Maternal knowledge is considered one of the important factors influencing participation in this screening program. This study aimed to describe pregnant women’s knowledge regarding congenital hypothyroidism screening in newborns. This study employed a quantitative descriptive approach involving 21 pregnant women selected through accidental sampling. Data were collected using a structured questionnaire and analyzed using frequency distribution and percentage. The results showed that 28% of respondents had inadequate knowledge regarding screening indications, 24% had inadequate knowledge regarding screening procedures, and 33% had inadequate knowledge regarding screening benefits. Overall, 38% of respondents had inadequate knowledge regarding congenital hypothyroidism screening. These findings indicate that many pregnant women still have inadequate overall knowledge regarding congenital hypothyroidism screening in newborns.Keywords: congential hypothyroidism screening; knowledge; pregnant women

Abstract

Pulmonary tuberculosis remains a public health burden in endemic urban areas, exacerbated by heterogeneous climate variability and limited health service capacity. This study aimed to describe the temporal patterns of climate variability and the spatial patterns of physician distribution and their relationship to tuberculosis cases in Palembang. Using a spatial-temporal ecological design, we analyzed monthly time series of tuberculosis cases and summarized monthly climate data from the Meteorology, Climatology, and Geophysics Agency of Indonesia, then mapped the cases and availability of general practitioners and pulmonologits in 18 subdistricts using geographic information system to explore distribution patterns and potential detection bias. The results showed that at the aggregate level, there was no consistent linear relationship between climate fluctuations and variations in pulmonary tuberculosis cases; instead, spatial mapping revealed heterogeneity in cases correlated with medical personnel availability, areas with higher health worker density reported higher cases (indicating increased detection), while resource-limited areas risked underreporting and delayed diagnosis. These findings underscore the need for equitable primary care services, strengthened active case finding, and integration of service indicators into spatial surveillance to improve programmatic detection and response. Abstract

The quality of inpatient services in hospitals is strongly influenced by the efficiency of bed utilization, which is measured using key indicators such as Bed Occupancy Rate (BOR), Average Length of Stay (AvLOS), Turnover Interval (TOI), and Bed Turnover (BTO). Preliminary findings at Majalengka District Hospital showed that BOR remained below the optimal standard, AvLOS showed a gradual increase, TOI continued to decline, and BTO was excessively high. These patterns indicate the need to project inpatient bed requirements for the years 2025–2029. This study aimed to predict inpatient bed requirements based on inpatient service efficiency indicators. A descriptive quantitative study with a prospective approach was conducted. Secondary data on inpatient activities from 2022–2024 were collected using purposive sampling. Linear regression analysis was applied to predict inpatient days and patient discharges, while BOR and TOI calculations were used to estimate the number of beds required. The findings indicate that inpatient bed utilization at Majalengka District Hospital during 2022–2024 was still inefficient. Although the number of beds decreased, inpatient days and patient discharges continued to rise, reflecting increasing demand for inpatient services. Based on the standard BOR of 75–85% and TOI of 1–3 days, the projected bed requirement ranges from 167–292 beds in 2025 and increases to 212–368 beds by 2029. In conclusion, inpatient bed requirements at Majalengka District Hospital are expected to continue increasing, highlighting the need for regular evaluation and strategic planning of bed capacity.Keywords: prediction; bed requirement; efficiency; hospital

Abstract

Human Immunodeficiency Virus (HIV) remains a major public health concern in Indonesia, particularly among men who have sex with men (MSM), who continue to experience increasing HIV prevalence. Behavioral and structural factors, including condom use consistency, knowledge, risk perception, and access to prevention services, contribute to HIV vulnerability within this population. This study aimed to analyze the association between condom use consistency and HIV status among MSM in Indonesia, while examining the influence of demographic and behavioral confounding variables. This cross sectional study utilized secondary data from the 2019 Integrated Biological and Behavioral Surveillance (STBP), covering 2,936 MSM respondents across 13 provinces and 24 districts/cities. Data were analyzed using univariate, chi square bivariate tests, interaction analysis, and binary logistic regression to identify independent and adjusted associations between variables. A total of 20.10% of respondents were HIV reactive, and 51.75% reported inconsistent condom use. Bivariate analysis showed significant associations between HIV status and all independent and confounding variables (p < 0.05). Multivariate analysis revealed that inconsistent condom use was associated with a 29% lower likelihood of HIV infection (aOR = 0.71). Higher odds of HIV infection were observed among respondents aged ≥25 years (aOR = 1.82), unmarried or previously married (aOR = 2.79), those with a history of STI symptoms (aOR = 1.63), and those reporting difficulty accessing condoms (aOR = 1.24). Conversely, respondents who did not perceive themselves at risk (aOR = 0.51) and those who had never undergone HIV testing (aOR = 0.40) showed lower odds of HIV infection. A significant interaction was found between STI symptoms and HIV testing history (aOR = 2.18). In conclusion, condom use consistency, age, marital status, risk perception, STI symptoms, access to condoms, and HIV testing history were significantly associated with HIV status among MSM in Indonesia. Abstract

One of the most common and debilitating long‑term complications of diabetes mellitus is diabetic neuropathy, whose prevalence varies widely across countries. This condition significantly increases the risk of lower‑extremity amputation and diabetic foot ulceration, underscoring the importance of early identification of its predictors. This review aimed to examine recent evidence regarding the association between HbA1c levels and the occurrence of diabetic neuropathy among individuals with diabetes mellitus. A literature search was conducted for articles published between 2015 and 2025 across PubMed, ScienceDirect, and Google Scholar, yielding 12 eligible studies. Of these, eight studies reported a significant association between HbA1c and diabetic neuropathy. Elevated HbA1c levels and wide HbA1c variability were consistently linked to an increased risk and greater severity of neuropathic complications. Longitudinal analyses demonstrated that HbA1c serves as a strong predictor of neuropathy when monitored over time rather than assessed as a single measurement. Moreover, the combination of HbA1c with age and duration of diabetes produced a sensitivity of 78.6% and a specificity of 52.3% (p < 0.0001), indicating enhanced predictive accuracy when these variables are evaluated together. These findings highlight the need for clinicians to assess not only absolute HbA1c values but also HbA1c variability over time to prevent progressive nerve damage. Age and duration of diabetes should likewise be considered essential predictors in evaluating neuropathy risk. In conclusion, this literature review demonstrates that HbA1c is a principal predictor of diabetic neuropathy, reinforcing its critical role in comprehensive glycemic monitoring strategies.Keywords: HbA1c variability; diabetic neuropathy; diabetes mellitus

Abstract

Compassion fatigue is a form of emotional exhaustion experienced by nurses as a result of repeated exposure to patients’ suffering, and this condition can negatively affect job satisfaction. This study aimed to analyze the effect of a motivational message program delivered via WhatsApp on reducing compassion fatigue and its impact on improving nurses’ job satisfaction. A quasi-experimental design with a pretest–posttest and control group was employed. The intervention consisted of motivational messages sent regularly for 21 days to the intervention group. Compassion fatigue and job satisfaction were measured before and after the intervention using questionnaire, and the data were analyzed using parametric statistical tests. The paired samples t-test showed no significant difference between pretest and posttest scores in the intervention group (p = 0.903), suggesting that motivational messages via WhatsApp alone are insufficient to decrease compassion fatigue. Furthermore, the reduction in compassion fatigue did not have a significant impact on improving job satisfaction. Although there was an average increase of 1.39 points, this change was not statistically significant (p = 0.388). Between-group analysis also revealed no significant differences between the intervention and control groups in either compassion fatigue or job satisfaction (p > 0.05). In conclusion, the WhatsApp-based motivational messaging program has not been proven effective in reducing compassion fatigue or improving nurses’ job satisfaction. Future research should develop more comprehensive interventions, such as peer support group programs, that enable nurses to share experiences and provide mutual support.Keywords: compassion fatigue; motivational message; WhatsApp; job satisfaction; nurses
