Abstract

One factor contributing to the low rate of exclusive breastfeeding is the limited knowledge pregnant women have regarding lactation preparation. Pregnancy class is designed to serve as a learning medium to enhance their understanding of this topic. This research was conducted to analyzing the effectiveness of pregnancy class in inreaces knowledge of lactation preparation among pregnant women. The study employed an observational analytic method with a prospective approach and involved 30 pregnant women, who were selected using a multistage cluster random sampling technique from a total population of 78. Data were collected using a questionnaire on lactation preparation that had been adapted from prior studies. Descriptive analysis revealed that all participants who regularly attended the classes demonstrated good knowledge levels, while among those who were less active, two respondents (13%) still had only moderate knowledge. Data were analyzed using the Mann-Whitney test produced a p-value of 0.021 (<0.05), indicating a significant relationship between participation in pregnancy class and pregnant women’s knowledge of lactation preparation in Jabung District, Malang Regency. In conclusion, pregnancy class is effective in increases knowledge of lactation preparation among pregnant women.Keywords: pregnancy class; knowledge level; lactation preparation

Abstract

Exclusive breastfeeding for the first six months is recommended by the World Health Organization and UNICEF because it provides optimal nutrition and immune protection for infants. However, the coverage of exclusive breastfeeding in Indonesia remains below the global target. Support from health workers is considered an important factor influencing breastfeeding practices, yet previous studies have reported inconsistent results. This study aimed to estimate the pooled effect size of the association between support from health workers and exclusive breastfeeding among infants aged 0–6 months in Indonesia, thereby providing robust empirical guidance for national maternal health policy. This systematic review and meta-analysis was conducted in March 2026 following PRISMA guidelines. A literature search was conducted in Google Scholar, PubMed, ScienceDirect, and Garuda databases for articles published between 2021 and 2026. Data analysis was performed using Review Manager 5.4 with the Odds Ratio (OR) effect size and 95% confidence interval using the Mantel–Haenszel method with a random effects model. Sensitivity analysis was performed using Review Manager 5.4. Publication bias was analyzed using funnel plots and Egger's test. A total of 11 cross-sectional studies were included in this study. Meta-analysis showed that support from health workers significantly increased the likelihood of exclusive breastfeeding [5.77 (95% CI: 3.88–8.57)]. Sensitivity analysis using the leave-one-out method demonstrated robust pooled estimates, with ORs ranging consistently from 5.39 (95% CI: 3.60–8.08) to 6.47 (95% CI: 4.13–10.14) across all iterations. Risk of bias was mostly moderate. Egger's test indicated publication bias (p = 0.0126). In conclusion, health worker support is a high-impact intervention that significantly increases exclusive breastfeeding in Indonesia. Abstract

Respiratory Distress Syndrome (RDS) is a respiratory disorder in newborns caused by surfactant deficiency. This disease is also known as Hyaline Membrane Disease (HMD). RDS cases at Mitra Medika Bondowoso Hospital ranked first among the top 10 inpatient diagnoses from 2022 to 2024. This study aims to analyze the relationship between gestational age, birth weight, sex, gestational diabetes, maternal hypertension, method of delivery, premature rupture of membranes, multiple pregnancy, and maternal age with the incidence of RDS in neonates. This is a quantitative analytical study with a case-control design, using medical records from 95 cases and 95 controls selected by simple random sampling. Univariate analysis showed that 14.7% of babies were born preterm, 52.6% were male, and 10.5% had low birth weight. Maternal factors included hypertension (17.9%), cesarean section and assisted delivery (61.1%), premature rupture of membranes (27.4%), multiple pregnancy (3.2%), and age < 20 or > 35 years (15.8%). No mothers with gestational diabetes were found. Bivariate analysis showed that RDS in neonates was associated with gestational age (p=0.001), birth weight (p=0.009), and premature rupture of membranes (p=0.034). Meanwhile, sex (p=0.309), maternal hypertension (p=0.570), method of delivery (p=0.882), multiple pregnancy (p=0.211), and maternal age (p=0.320) were not associated with RDS in neonates. This research concludes that RDS in neonates is related to preterm gestational age, low birth weight, and premature rupture of membranes. Providing education through pamphlets and social media can help prevent premature birth, low birth weight, and premature rupture of membranes.

Abstract

This study addresses the absence of research analyzing patient satisfaction in relation to the utilization of dental and oral health services at Dental Clinics of Health Center in Jember, particularly based on key determinants such as provider expertise, interpersonal relationships, service time, access, and comfort. The objectives of this study were to assess patient satisfaction with the utilization of dental and oral health services and to identify the dominant determinant influencing service utilization. A cross-sectional design with purposive sampling was employed. Data were collected using a structured questionnaire and analyzed using Pearson correlation and ordinal logistic regression. The average level of patient satisfaction was categorized as very satisfied (89%), while the average level of service utilization was classified as high (85.6%). All determinants of patient satisfaction demonstrated significant relationships with service utilization (α < 0.05). Time emerged as the dominant factor influencing the utilization of dental and oral health services, with a significance value of 0.035. These findings indicate a significant relationship between patient satisfaction and the utilization of dental and oral health services at the Dental Polyclinic of the Jember Health Center, with service time identified as the most influential determinant.Keywords: patient satisfaction; utilization of dental and oral health services; health center dental clinic

Abstract

Dengue infection is a major global health threat, often manifesting as thrombocytopenia. While Carica papaya is traditionally used to enhance platelet counts, synthesized clinical evidence regarding its efficacy remains limited. This study evaluated the effectiveness of Carica papaya leaf extract (CPLE) on platelet count recovery in adult dengue patients. Following PRISMA 2020 guidelines, a systematic literature search was conducted across two databases (PubMed and Google Scholar) to identify randomized controlled trials (RCTs) published between 2012 and 2024. Risk of bias was assessed using Cochrane RoB 2, and data were analyzed in R Studio using a random-effects model. Four RCTs involving 279 adult participants were included. The meta-analysis demonstrated that CPLE significantly increased platelet counts compared to controls, with a pooled mean difference of 14.34 × 10³/µL (95% CI: 5.90 to 22.77; p = 0.0124 for overall effect). Statistical heterogeneity was low-to-moderate (I2 = 27.0%), indicating a generally consistent therapeutic direction. In conclusion, CPLE serves as a viable adjunctive therapy for accelerating platelet recovery in adult dengue patients. However, given the sensitivity of the pooled estimates to specific highly weighted trials, future large-scale, rigorously blinded RCTs are needed to solidify these findings and confidently support their integration into clinical management protocols.Keywords: Carica papaya; dengue fever; thrombocytopenia; platelet recovery; meta-analysis; adjucntive therapy

Abstract

Stature is a fundamental anthropometric indicator used in nutritional assessment and pharmacokinetic calculations; however, direct measurement is often challenging in intensive care settings or among individuals with skeletal deformities. Estimation based on ulna length offers a practical alternative. Although numerous formulas have been proposed, none have been specifically developed or tested for the Madurese ethnic group. This study aimed to estimate stature from ulna length among early‑adult Madurese individuals. An analytical observational design with a cross‑sectional approach was employed. A total of 34 students aged 18–29 years from the Faculties of Medicine, Nursing, and Pharmacy at Universitas Muhammadiyah Malang were selected using purposive sampling. Stature was measured using a microtoise, whereas ulna length was assessed using a sliding caliper. Data were analyzed using univariate statistics, independent t‑tests, and linear regression. The findings demonstrated significant differences between actual stature and estimated values generated by existing formulas, with the Pureepatpong equation yielding estimates closest to actual measurements compared with the Thummar formula. Ulna length showed a significant association with stature. In conclusion, ulna‑based measurement is a valid method for estimating stature in early‑adult Madurese individuals. The Pureepatpong formula exhibited superior accuracy and is therefore recommended for application in this population.Keywords: stature estimation; ulna length; early adulthood; Madurese ethnicity; anthropometry

Abstract

Patient safety culture encompasses shared values, beliefs, and norms regarding the importance of safety in healthcare settings. Ineffective communication during handoffs and information exchange can lead to medical errors, complications, and even an increased risk of mortality. This study aimed to analyze the effectiveness of I-PASS training on the quality of handoffs and information exchange among nurses at Hospital X, West Jakarta. This study employed a quantitative approach with a pre-experimental design and was conducted from February to March 2026. The sample consisted of 56 nurses who met the inclusion criteria: working in inpatient wards, ICU, and emergency departments; willing to participate; levels PK 2 and 3; and actively involved in handoffs and information exchange processes. The data collection tool was a questionnaire. The results showed that the majority were aged 26–35 years (66.1%), female (75%), PK level 2 (85.7%), and D3 Nursing (60.7%). The results of the I-PASS training for nurses after training showed that the majority (80.4%) had good results, and the quality of handoffs and information exchange after training was good (57.1%). There was a relationship between age (p-value = 0.032) on the quality of handoffs and information exchange. There was a significant difference in the quality of handoffs and information exchange before and after the intervention (p-value = 0.000). The results of the multivariate analysis showed that age (p-value = 0.019) and I-PASS training (p-value = 0.029) significantly influenced the quality of handoffs and information exchange (p < 0.05). The coefficient of deter mination (R²) was 0.112, indicating that these two factors contributed 11.2% to explaining the quality of handoffs and information exchange. Hospitals are recommended to establish I-PASS training as a structured and sustaiable nursing development program by integrating I-PASS-based learning mechanisms into standard operating procedures (SPOs), ensuring consistent implementation among all nursesKeywords: I-PASS training; quality of handoffs and information exchange; nurses

Abstract

ABSTRACTBackground: Hospitals operate in an increasingly competitive environment that demands continuous improvement of service quality, patient safety, efficiency, and organizational performance. Strategic management, comprising strategy formulation, implementation, and evaluation, is a key determinant of hospital sustainability. However, the implementation stage remains the most challenging, as many well-formulated strategies fail to be translated into measurable performance. Purpose: To review scientific evidence regarding the implementation of hospital strategic management and its role in improving service quality and organizational performance. Method: A literature review using a systematic approach based on PRISMA guidelines. Literature searches were conducted through PubMed, ScienceDirect, Google Scholar, and Garuda databases for articles published between 2020 and 2026. Eight articles met the inclusion criteria and were included in the analysis. Results: The success of hospital strategic management implementation was determined by the interaction of strategic planning and environmental analysis (SWOT and PESTEL), performance measurement systems (the Balanced Scorecard and key performance indicators), strategic leadership and organizational culture, adequate organizational structure and resource allocation, and the use of hospital management information systems. These factors collectively contributed to improved service quality, patient safety, operational efficiency, and cost control. Conclusion: Hospital strategic management implementation is a multidimensional process influenced by the alignment of leadership, planning, performance measurement, organizational systems, and information technology. Hospitals should strengthen strategic leadership, structured strategic planning with measurable indicators, and supportive organizational systems to improve service quality and organizational performance.Keywords: hospital; lmplementation; organizational performance; service quality; strategic management

Abstract

Diabetes mellitus is a major non‑communicable disease that poses a serious global health threat across socioeconomic and national boundaries. Individuals with diabetes are at high risk of developing chronic complications, including diabetic neuropathy, which may progress to diabetic foot ulcers. Approximately 90% of patients with diabetic foot ulcers face the risk of lower‑extremity amputation. Inadequate foot‑care behavior among people with diabetes further increases the likelihood of severe complications, including recurrent foot ulcers and amputation. This study aimed to analyze the relationship between foot‑care behavior among patients with diabetes mellitus and the occurrence of recurrent diabetic foot ulcers. A quantitative cross‑sectional design was employed using accidental sampling. The sample consisted of 51 patients with recurrent diabetic foot ulcers. Descriptive analysis and Spearman’s rho correlation test were used to examine the association between foot‑care behavior and ulcer recurrence. The findings demonstrated a significant relationship between foot‑care behavior and recurrent diabetic foot ulcers, with a p‑value of (0.022) and a correlation coefficient of (–0.320). Most patients with recurrent ulcers were male (60.8%), nearly half were aged ≥65 years, and 47.1% had lived with diabetes for 1–5 years. A majority experienced two episodes of recurrent ulcers (78.4%) and exhibited poor foot‑care behavior (72.5%). Poor foot‑care behavior is significantly associated with recurrent diabetic foot ulcers. The demographic profile of affected patients indicates predominance among older males with a moderate duration of diabetes. Strengthening foot‑care practices is essential to reduce recurrence and prevent severe complications.Keywords: diabetes mellitus; diabetic foot ulcer; complications; foot care
