Abstract

Spinal anesthesia commonly induces hemodynamic alterations, particularly post–spinal anesthesia hypotension resulting from sympathetic blockade of efferent preganglionic vasomotor fibers. This sympathetic inhibition reduces vascular tone and venous return, leading to a decline in systolic blood pressure. Leg elevation is a simple, non‑pharmacological maneuver that may enhance venous return through gravitational redistribution of blood toward the central circulation. This study aimed to determine the effectiveness of 30° and 45° leg elevation in increasing blood pressure among patients experiencing hypotension after spinal anesthesia. A quasi‑experimental design employing a Three‑Group Pretest–Posttest Control Group Design was used. A total of 30 respondents were selected through purposive sampling and allocated into three groups. Respondents met the inclusion criteria of post–spinal anesthesia hypotension (80 mmHg < systolic ≤ 100 mmHg). The independent variables were 30° leg elevation and 45° leg elevation, while the dependent variable was systolic blood pressure. Statistical analyses included normality testing, homogeneity testing, paired t‑test, independent t‑test, and one‑way ANOVA. The results demonstrated a significant difference in systolic blood pressure following the application of 30° and 45° leg elevation, with a p‑value of 0.000 (p < 0.005) and a mean difference of 12.3 mmHg. The 45° leg elevation produced a greater increase in systolic blood pressure compared to the 30° elevation. In conclusion, leg elevation at 45° is more effective in increasing systolic blood pressure among post–spinal anesthesia hypotensive patients. A higher elevation angle facilitates greater gravitational blood flow toward the heart, thereby enhancing venous return and improving systolic blood pressure.Keywords: 30° leg elevation; 45° leg elevation; blood pressure; hypotension; spinal anesthesia

Abstract

Ventilator-associated pneumonia (VAP) is a nosocomial infection of the lung tissue that most often occurs in patients undergoing mechanical ventilation in intensive care units, with a high morbidity and mortality rate of 1% per day. The incidence of VAP can be prevented through a specific procedure, known as the VAP Bundle. The method in the VAP Bundle includes head-up positioning at 30°-45°, closed suctioning, and oral hygiene using 0.2% chlorhexidine aseptic solution. This study aims to evaluate the effectiveness of the VAP bundle in preventing VAP risk among mechanically ventilated patients at dr. Soebandi Regional Hospital, Jember. This study was a case report involving one patient who met the inclusion and exclusion criteria. The Modified Clinical Pulmonary Infection Score (MCPIS) was measured daily over a six-day intervention period. The results showed that during the six-day intervention, the patient's MCPIS score remained <5 (indicating no VAP). Clinical findings showed no infiltrates, non-purulent secretions, and normalized temperature by intervention end. It can be concluded that the implementation of independent VAP Bundle activities by nurses was successful in preventing VAP among mechanically ventilated ICU patients.Keywords: ventilator-associated pneumonia; head up; suction; oral hygiene; chlorhexidine

Abstract

ABSTRACTThe transition of newly graduated nurses into professional practice is a complex phase and is highly vulnerable to transition shock, particularly in type C hospitals with high work demands. A preceptorship program was developed to support newly graduated nurses in adapting to the work environment; however, their experiences during and after participation in the program still need to be explored in depth. This study aimed to explore the experiences of newly graduated nurses during and after participating in the preceptorship program at the Training Center of Hospital X. This study employed a qualitative approach with a descriptive phenomenological design. Participants were selected purposively using the principle of maximum variation sampling and involved 18 participants, consisting of 10 newly graduated nurses as the main participants, 3 preceptors, and 5 head nurses as triangulation informants. Data were collected through semi-structured in-depth interviews conducted both offline and online, and were analyzed using Colaizzi’s seven-step method with the support of NVivo 15. Data credibility was strengthened through source triangulation and member checking. The findings generated five major themes: newly graduated nurses’ feelings during the preceptorship program, the support they received during the transition process, the barriers they encountered, the adaptive strategies or responses they employed, and the nature of transition after completing the preceptorship program. Overall, the transition occurred gradually, beginning with initial enthusiasm, followed by anxiety and pressure, and eventually developing into courage, self-confidence, and readiness to perform the professional role. The preceptorship program served as an important bridge in supporting clinical readiness, emotional adaptation, and the formation of professional identity among newly graduated nurses.Keywords: newly graduated nurses; preceptorship; professional transition; descriptive phenomenology; type C hospital

Abstract

Ovarian cancer is a leading cause of cancer related mortality among women and is frequently diagnosed at an advanced stage. Management of stage III disease typically requires multimodal treatment, including surgery and chemotherapy, both of which contribute to a substantial symptom burden and increased supportive care needs. As a result, comprehensive nursing care becomes essential to address the complex physical and psychosocial challenges experienced during active treatment. This case report aimed to describe the implementation of the nursing process in two women with stage III ovarian cancer undergoing active treatment in a referral hospital in Central Sulawesi, Indonesia. Data were collected through semi structured interviews, direct clinical observation, and documentation review using the Indonesian Nursing Diagnosis Standards framework. Both patients presented with nutritional deficits and nausea as primary nursing diagnoses. Additional problems identified in the first patient included acute pain, sleep disturbance, activity intolerance, and ineffective peripheral perfusion, whereas the second patient experienced a risk of infection related to colostomy and disturbed body image. Nursing interventions focused on symptom management, nutritional support, pain control, infection prevention, and psychosocial care. Evaluation demonstrated improvements in symptom control, increased tolerance to nutritional intake, and the absence of infection throughout hospitalization. In conclusion, individualized and holistic nursing care plays a critical role in enhancing clinical outcomes and patient comfort during active treatment for advanced ovarian cancer.Keywords: chemotherapy; nutritional deficit; nursing process; ovarian cancer; supportive care; symptom management

Abstract

Hypertension remains a major public health concern among older adults, particularly in low‑ and middle‑income settings where lifestyle factors such as dietary patterns contribute substantially to disease burden. This study aimed to determine the relationship between dietary patterns and the occurrence of hypertension among older adults in the working area of BP Nauli Health Center, Pematangsiantar, in 2024. This study employed a quantitative analytic observational design using an ex post facto approach. The research was conducted from May to August 2024. A total of 2,813 older adults constituted the study population, from which 107 respondents were selected using purposive sampling. Dietary patterns were assessed based on frequency, type, and quantity of food consumed, while hypertension status was classified according to WHO criteria. Data were analyzed using descriptive statistics and Spearman’s rho correlation test. More than half of the respondents (52.3%) exhibited poor dietary patterns. Hypertension Grade 2 was more prevalent (63.6%) than Grade 1 (36.4%). Cross‑tabulation showed that respondents with poor dietary patterns had a higher proportion of Grade 2 hypertension (60.3%). Bivariate analysis using Spearman’s rho revealed a significant relationship between dietary patterns and hypertension (p = 0.03; α = 0.05), indicating that poorer dietary patterns were associated with higher hypertension severity. In conclusion, dietary patterns are significantly associated with hypertension among older adults. These findings highlight the importance of improving nutritional behavior as part of community‑based hypertension prevention and control strategies.Keywords: dietary pattern; hypertension; older adults

Abstract

Open Defecation (OD) remains a persistent environmental health issue in many regions, particularly in low-income and developing countries. This behavior contributes to increased risks of environmentally related diseases, parasitic infections, and reduced quality of life. Numerous studies indicate that OD practices are influenced not only by environmental factors but also reflect demographic, social, economic, cultural, and sanitation-service conditions. This study aims to examine the determinants of OD behavior through a comprehensive literature review and to identify consistent patterns of significant associations reported in recent research. Methods: A literature review was conducted on 30 research articles examining OD determinants, retrieved from Google Scholar, PubMed, ScienceDirect, and Garuda, with publication years ranging from 2021 to 2025. The analysis was guided by a behavioral framework encompassing three groups of factors: predisposing, enabling, and reinforcing factors. Data were synthesized based on the significance of associations, frequency of variable occurrence, and consistency of findings across studies. Results: Predisposing factors emerged as the dominant determinants of OD behavior, particularly education level (63%), knowledge (53%), and attitude (50%). Most variables within this category showed significant associations, especially knowledge, attitude, and education level (>70% of the studies). Enabling factors, including latrine availability (33%) and water availability (27%), also played key roles, with most studies reporting significant relationships concerning basic sanitation infrastructure. Reinforcing factors, such as social support and the role of health workers, were identified as important determinants, contributing significantly in more than 70% of the reviewed studies. Overall, OD behavior was found to result from a complex interaction of individual, environmental, and social dimensions. Conclusion: Efforts to reduce open defecation must integrate multidimensional approaches that include improving sanitation literacy through education, providing adequate and sustainable sanitation facilities, and strengthening the role of community leaders and health workers in promoting hygienic behavior. Further research is recommended to evaluate the effectiveness of community-based interventions and to explore behavioral changes in the context of socio-economic dynamics.

Abstract

Stunting is a chronic nutritional condition that requires rapid and accurate early detection. Conventional assessment methods that rely on the Indonesian Maternal and Child Health (MCH) handbook; commonly referred to as the MCH handbook or manual anthropometric tables often face challenges related to efficiency and susceptibility to human error. The digitization of anthropometric assessment through an Android‑based application offers a promising solution for automating the evaluation of toddlers’ nutritional status. This study aimed to examine the perceived benefits and user acceptance of a machine learning–based Android application designed for stunting detection and management. A descriptive quantitative design was employed. Consecutive sampling was used to recruit individuals who met the inclusion criteria, including the ability to operate the designated Android system, resulting in a sample of 37 respondents. Data were collected using a structured questionnaire with a 1–4 Likert scale. Descriptive statistics were applied to generate frequency distributions, percentages, and mean scores for the indicators of accuracy, usability, utility, and reliability. Findings indicate a high level of user acceptance. Usability received the highest mean score (3.28), followed by utility (3.25) and reliability (3.16). A total of 91.7% of respondents rated the prediction system as accurate, with an average accuracy score of 3.05. Respondents also reported that the application was substantially more efficient and faster than manual assessment methods. In conclusion, the machine learning–based stunting detection application effectively supports users in identifying stunting risk quickly and accurately. Its ease of use and strong perceived validity make it a practical tool for both healthcare workers and the general public, contributing to efforts to accelerate stunting reduction.Keywords: anthropometry; Android-based application; machine learning; stunting

Abstract

Nursing care is a series of professional activities aimed at delivering comprehensive services to patients. Nursing care documentation is a critical component of professional nursing practice because it reflects the nurse’s legal and ethical accountability for the interventions provided. In many healthcare settings, documentation completeness remains suboptimal, requiring structured supervisory approaches to improve quality. This study aims to analyze the effectiveness of a reflective supervision model in improving the completeness of nursing care documentation at Hospital X, Kupang. A quantitative approach was employed using a quasi‑experimental pretest–posttest control group design. A total of 60 nurses participated across both groups. Data were collected through direct observation using a validated documentation completeness checklist based on hospital accreditation standards. Statistical analyses included proportion tests, mean difference tests, and logistic regression. Most respondents were female nurses aged 25–35 years, holding a Bachelor’s degree in Nursing and PK II certification. The intervention group showed an improvement in documentation completeness scores from a mean of 23.07 before the intervention to 35.26 after the intervention. There was a significant difference in the proportion of complete documentation between the intervention and control groups (p = 0.000), and a significant increase in the mean documentation score before and after the intervention (p = 0.000). Multivariate analysis indicated that reflective supervision was the main predictor of improved documentation completeness (p = 0.002). In conclusion, reflective supervision is effective in improving the completeness of nursing care documentation. Hospitals should institutionalize reflective supervision as part of routine nursing development programs through director-level decrees or internal nursing policies. Training for ward heads and supervisors is recommended, including 1–2 working days of facilitator preparation covering reflective communication techniques, constructive feedback, and documentation evaluation based on accreditation standards.Keywords: reflective supervision model; nursing care documentation; nurses

Abstract

Shoulder musculoskeletal disorders are a persistent occupational health problem in the construction sector, arising from the interaction of physical, individual, psychosocial, and environmental demands. These conditions reduce functional capacity, impair work performance, and increase long‑term health burdens among construction workers. This study aims to synthesize evidence on the prevalence and multidimensional risk factors contributing to shoulder musculoskeletal disorders among construction workers, and to identify protective factors relevant for prevention. A narrative literature review was conducted using seven primary study that met predefined inclusion criteria. Evidence was extracted on worker characteristics, ergonomic exposures, psychosocial conditions, and environmental factors associated with shoulder pain. Findings were integrated to develop a comprehensive understanding of determinants influencing shoulder musculoskeletal disorders. The synthesis indicates that shoulder pain among construction workers is shaped by interconnected determinants. Individual factors such as elevated BMI, older age, and prolonged work experience increase vulnerability through heightened biomechanical strain and reduced recovery capacity. Physical and ergonomic exposures, including non‑ergonomic postures, repetitive arm movements, manual handling, high work speed, and heat strain consistently contribute to musculoskeletal overload. Psychosocial pressures, including stress and job dissatisfaction, amplify muscle tension and diminish coping ability, further worsening symptoms. Conversely, regular exercise emerges as a protective factor that enhances musculoskeletal resilience and reduces symptom severity. In conclusion, shoulder musculoskeletal disorders among construction workers are driven by multidimensional and cumulative risk factors. Effective prevention requires integrated interventions combining ergonomic improvements, safe‑work training, physical‑health promotion, and psychosocial support, with particular attention to high‑risk groups such as obese workers, older workers, and those with long occupational tenure.Keywords: shoulder pain; construction workers; ergonomic risk factors; psychosocial factors; occupational health; physical workload

Abstract

Basic sanitation represents fundamental efforts to improve human health by ensuring the availability of a clean and safe environment. Core components of basic sanitation include access to clean water, household latrines, solid‑waste disposal facilities, and domestic wastewater drainage systems. Inadequate sanitation infrastructure is widely recognized as a major contributor to diarrheal diseases, particularly among vulnerable groups such as under‑five children. This study aimed to analyze the relationship between household waste‑bin conditions and latrine conditions with the incidence of diarrhea among under‑five children in the Baqa Primary Health Center area, Samarinda, in 2023. A quantitative approach with a cross‑sectional design was employed. The study involved 96 respondents selected through observational assessment. Data collection utilized structured observation sheets (checklists) to evaluate household waste‑bin conditions, latrine conditions, and diarrheal incidence among under‑five children. Data analysis was conducted using the Chi‑square test and Fisher’s Exact Test as an alternative when assumptions were not met. The findings indicated a significant association between household waste‑bin conditions and diarrheal incidence among under‑five children (p = 0.002). A significant relationship was also found between latrine conditions and diarrheal incidence (p = 0.000). These results suggest that inadequate waste‑bin facilities and substandard latrine conditions contribute to a higher risk of diarrhea among toddlers. In conclusion, improving household sanitation, particularly by providing proper waste‑bin facilities and ensuring latrines meet health standards is essential to reducing diarrheal incidence among under‑five children. Strengthening community awareness and promoting compliance with sanitation requirements are recommended to minimize disease occurrence.Keywords: under‑five children; basic sanitation; waste‑bin condition; latrine condition; diarrhea incidence
