Abstract

Objective: We report a rare case of salivary duct carcinoma possibly arising from the malignant component of carcinoma ex pleomorphic adenoma. Methods: A 56-year-old Japanese male complained of swelling in the left side of jaw for 2 years which was painless until 2 days ago. On extraoral examination, the face was bilaterally asymmetrical. An ultrasonography revealed hypoechoic mass of size 1.5x2.5x2.5 cm on left parotid gland. Computed tomography (CT) revealed a slight irregular mass on left parotid gland with no infiltration to adjacent tissues. Based on these findings, a clinical diagnosis of left parotid benign tumor was made. Results: A surgical excision was performed under general anesthesia. On histopathological examination, the connective tissue was rich in fibrous components and was accompanied by hyaline. Epithelial cells with high atypia proliferating invasively as cords into connective tissue were observed. The intraductal component had Roman-bridge-like architecture with central portion undergoing comedonecrosis. Based on these findings, a final diagnosis of salivary duct carcinoma possibly arising from pleomorphic adenoma was made. p16 hypermethylation is a known phenomenon in oral malignancy. We also checked the methylation status of p16 gene and found high level of methylation in this tumor. Conclusion: We reported a rare case of salivary duct carcinoma possibly arising from pleomorphic adenoma and also demonstrated high methylation level of p16 gene in the tumor.

Abstract

Objective: To evaluate the microhardness and porosity of the dentin of Wistar rats after pulp-out application. Materials and Methods: This was laboratory experimental research with posttest design with control group. Twelve teeth of wistar rats were allocated into 2 groups, the test group and the control group. In the test group, teeth were prepared on the labial surface until half thickness of the dentin, then pulpout was applied to the base of cavity, covered with GIC. On the 7th day, euthanasia and extraction were performed to test the dentin microhardness with Vickers Hardness Tester and dentin porosity with Laser Scanning Microscope (LSM). Results: The mean value of tooth hardness after pulp-out application was 22.23 ± 3.51 HV while the mean value of the control group was 27.70 ± 1.70 HV, with p value = 0.31 at 95% significance level. There was a decrease in tooth hardness of treated sample which was not significantly different compared to the control group. CLSM photo shows white lines on the sample surface as the exposed dentinal tubules with a higher intensity compared to the control group. Conclusion: Pulp-out decreased tooth microhardness and cause porosity of the teeth.

Abstract

Objective: To determine prevalence of Idiopathic osteosclerosis (IO) by radiographic evaluation and to investigate the relationship between the finding in relation with age, gender and location in Indonesia Sub-Population. Material and Methods: this was a cross-sectional study that panoramic radiograph of patients (1030 male and 1914 female) whose age range 10-82 years old and who visited Department of Oral and Maxillofacial Radiology Hasanuddin University Dental Hospital from 2017 and 2020. We evaluated group of age, location, appearance and occurrence of IO and analyzed using chisquare test. Results: in total, 2944 panoramic radiographs were examined, IO found in 144 (4.8%) patients and 170 IO’s were detected. IO detected mostly in mandible than maxilla (p<0.005) where premolar and molar area was the most common location of IO (42.3% and 41.7%; respectively). Most of cases was solitary lesion (<0.005) and unilateral (<0.005). In addition, most of IO separate from teeth (45.8%) and also related with periapical site (35.3%). Conclusion: Idiopathic Osteosclerosis can defined as developmental variation that can occur in any age range, no predilection of sex. The frequency of IO in Indonesia Sub-population is slightly lower than other country

Abstract

AbstractObjective : To provide early therapy in teenager pemphigus vulgaris patients so that healing of lesions in the oral cavity is faster.Methods : A 14-year-old boy came to the oral medicine installation RSGM Unpad with complaints of canker sores since 5 days before, initially only at buccal mucosa and treated but multiplied and getting worse. Ulsers like this is the first time experienced by patient. The patient is on antibiotic therapy and is undergoing a national examination. Extra oral examination found dry upper and lower lip, exfoliative and bleeding tendency. Intra-oral examination found ulcer lesions covered with sloughing on the upper and lower labial mucosa, right and left buccal mucosa, and tongue. The dorsum of the tongue has a yellowish white plaque that can be scrapped and does not leave the erythema area. The results of laboratory tests showed an increase in leukocytes, platelets and LEDs while total Ig E was normal. Based on the anamnesis and clinical examination, we diagnosed patient with pemphigus vulgaris. Patient get non-pharmacological therapy, namely oral hygiene instructions (OHI), communication, information, and education (CIE) while pharmacological therapy in the form of dexamethasone, prednisone, vitamin B12 and hyaluronic acid.Results : One month after treatment, the lesions have healed.Conclusion : Appropriate early therapy, good communication, information and education about the lesions in patient with oral pemphigus vulgaris will  relieve stress  and accelerate the process of healing lesions, especially in teenager patient.Keywords : Oral pemphigus vulgaris, stress, teenager

Abstract

Objective: To determine the effectiveness of gotu kola leaf extract (Centella asiatica (L.) Urban) with a concentration of 1%, 2.5% and 5% as a mouthwash against the growth of Streptococcus mutans bacteria.Materials and Methods: This was a laboratory experimental study with a post-test only control group design. The research sample was Streptococcus mutans bacteria which was bred and cultured in the Microbiology Laboratory of the Universitas Sumatera Utara Hospital. The gotu kola leaves used in the study were extracted and made in the form of mouthwash at the Traditional Medicine Laboratory, Faculty of Pharmacy, Universitas Sumatera Utara. This study used five treatment groups, namely gotu kola leaf extract mouthwash with a concentration of 1%, 2.5%, 5%, Chlorhexidine Gluconate 0.2% (positive control), and DMSO (negative control). The test for five treatment groups was repeated 5 times with Kirby-bauer diffusion method.Results: The results showed that the mouthwash of gotu kola leaf extract (Centella asiatica (L.) Urban) can inhibit the growth of Streptococcus mutans bacteria by forming a clear zone on disc paper. The results were statistically analyzed using Kruskal-wallis and Mann-Whitney Test showed a significant difference in the diameter of the inhibition zone formed in each treatment group (p <0.005).Conclusion: There is effectiveness of gotu kola leaf extract with a concentration of 1%, 2.5% and 5% as a mouthwash in inhibiting the growth of Streptococcus mutans and the most effective concentration was 5%.

Abstract

Objective: To determine the effectiveness of gotu kola leaf extract gel (Centella asiatica (L.) Urban) as an antimicrobial against the growth of Staphylococcus aureus bacteria in vitro and the most effective concentration in inhibiting the growth of Staphylococcus aureus bacteria.Materials and Methods: This study is a laboratory experimental study with a post-test only control group design. The research sample used Staphylococcus aureus bacteria which was bred and cultured in the Microbiology Laboratory of the Universitas Sumatera Utara Hospital. The research sample was divided into 5 treatment groups, namely gotu kola leaf extract gel 1%, 2.5%, 5%, Chlorhexidine digluconate 0.2% (positive control) and DMSO (negative control). The test for the five groups was repeated 5 times with the Kirby-bauer diffusion method.Results: The results showed the effectiveness of gotu kola leaf extract gel at a concentration of 2.5% and 5% with an inhibition zone diameter of 6.8 mm and 9.4 mm, respectively, while at a concentration of 1% there was no inhibition zone found. The data were statistically analyzed using the Kruskal-wallis and Mann-Whitney tests showing a significant difference in each treatment group (p <0.05), but there was no significant difference between the 1% and negative control groups (p> 0.05).Conclusion: Gotu kola leaf extract gel was effective in inhibiting the growth of Staphylococcus aureus bacteria in vitro at concentrations of 2.5% and 5%. The 5% concentration is more effective than other concentrations.

Abstract

ABSTRACTObjective: This case performs minimally invasive crown lengthening to minimize the incidence of gingival recession in the interdental area after the crown lengthening procedure. Methode: A patient has subgingival caries in teeth 12, 11, 22, and a lack of clinical crown length of tooth 22 for retention of the restoration. Clinical examination, radiographic photograph, and analysis of case models indicated the need for a crown lengthening procedure with a bone reduction to support the restoration. Crown lengthening was performed minimally invasive by maintaining the attachment of the papilla interdental of central incisors intact. The flap is only performed in areas that required bone reduction without breaking the attachment of the interdental gingiva. The bone reduction was performed using tungsten carbide bur. Results: One week after the crown lengthening showed fast healing and good repositioning of the gingival margins. After 3 months of crown lengthening showed no recession and no inflammation of the gingiva. Conclusion: This technique was effective to minimize gingival recession and increase the stability of the periodontal tissue after crown lengthening.

Abstract

 Background               : There are some individual characteristics variables including age, level of education and socioeconomic. Age, level of education and socioeconomic affect a person's behavior in health including oral health. Mother's behavior about oral health is a significant factor affecting children's oral health. At elementary school age, children are often unable to perform their self-care behavior independently, so they need the role of parents, especially mothers in maintaining their oral health. Objectives: Explain correlation between mother’s individual characteristics (age, level of education and socioeconomic) and mother’s behavior (knowledge, attitude and practice) about children’s oral health of elementary school in Bintaro. Method: This research is a quantitative analytic study with cross sectional design. The study sample consisted of 290 mother with simple random sampling technique. Results: The result were obtained using pearson correlation test and spearman correlation test showed a significant correlation between mother’s individual characteristics and mother’s behavior about children’s oral health (p<0,05). Conclusion: There is eight significant and positive correlations and there is one significant and negative correlation is correlation between age and mother’s practice about children’s oral health.

Abstract

Objective: Hyperdivergent Class II division 1 malocclusion with severe crowding generally requires premolars extraction to create space for crowding correction, anterior retraction, and facial profile improvement. The purpose of this case report is to demonstrate the anchorage management of a hyperdivergent Class II division 1 malocclusion with severe crowding using orthodontic miniscrew as skeletal anchorage.Method: This case was treated by extraction of the upper and lower first premolars, using a conventional orthodontic appliance MBT system and orthodontic miniscrew as skeletal anchorage. Result: Correction of severe crowding, protrusive incisors, and improvement of the facial profile were obtained in 26 months of treatment.Conclusion: Anchorage management with orthodontic miniscrew was effective in correcting severe crowding, protrusive incisors, and convex facial profile of a hyperdivergent Class II division 1 malocclusion

Abstract

Objective: A carcinoma ex-pleomorphic adenoma is caused by the long-term presence of pleomorphic adenoma. It is a relatively rare occurrence in the palatine glands.Methods: A 40-year-old man was referred to our department with a complaint of an intraoral mass. At the time of the initial examination, a large elastic hard mass of approximately 10 cm in diameter accompanied by ulceration in parts was found extending from the gingiva of the molar region in the right maxilla throughout the entire palate. Based on the imaging and histopathological examination findings, the clinical diagnosis was suspected to be malignant tumor of the palate.Results: The patient underwent palatal tumor enucleation, right-sided supraomohyoid neck dissection, and immediate reconstruction with an osteocutaneous radial forearm flap under general anesthesia. The histopathological diagnosis was myoepithelial carcinoma ex-pleomorphic adenoma. In addition, postoperative radiation therapy was performed. There has been no evidence of recurrence or metastasis for three years postoperatively.Conclusion: We report a case of large myoepithelial carcinoma ex-pleomorphic adenoma of the hard palate that was immediately reconstructed with an osteocutaneous radial forearm flap.
