Abstract

Background: Neuropathy and peripheral vascular disease are two major factors that lead to diabetic wounds. Additionally, diabetic wound healing process takes a long time. Collagen is one indicator to assess wound healing. Aim: This study aimed to compare the wound healing process DM and non-DM In Wistar with acute wounds modeling that uses the density collagen as indicators. Methods: This study used a randomized design Post Test Control Group who uses Wistar as research objects. Wistar rats were divided into 2 groups: Wistar DM and Non-DM groups. The total sample were 30 individuals that consisted of 15 rats each group. Wistar DM was induced by injection of Streptozotocin (STZ) intraperitoneal single dose of 40-50 mg / kg. The wound were created by using punch biopsy of each group. The wound were followed up for 14 days. Data analysis was done using the Kolmogorov - Smirnov test with a confidence level of 95%. All data were analyzed by SPSS version 21.0 software (SPSS, Inc. Chicago, IL). Results: The density of collagen in DM group (20%) was lower than in non DM group (40%). However there were no significant difference in collagen density in both groups (p=0,375). Nevertheless, the percentage of collagen density is higher in non DM group compared with DM group (20.0%). This shows the tendency of the wound healing process in the group of Non - DM is better than DM group. This shows the tendency of the wound healing process in the group of Non - DM is better than DM group. Conclusion : There is a tendency that the wound healing in non ?DM group is better than in DM groups. More studies are needed in the future to confirm this findings. Abstract

Background: Colorectal cancer is highly prevalent cancer. Colostomy may affect the changing role of self-esteem, body image, and sexual and social relationships. Post-colostomy problem is the maceration of the skin surrounding the stoma. Aim: This study aimed to determine the factors associated with the occurrence of maceration of the stoma in patients who underwenta colostomy in Dr Wahidin Sudirohusodo Hospital of Makassar. Methods: This study used observational analysisby cross-sectional study. The sampling technique was a purposive sampling with 32 ??respondents. Data analysis was done using the Chi Square test with a confidence level of 95%. All data were analyzed by SPSS software,version 21.0 (SPSS, Inc. Chicago, IL). Results: There are relationships between stoma site (p = 0.000), frequency of stoma care (p = 0.018), and nutritional status (p=0.030) with the maceration of the stoma. There was no association betwen age and maceration of the stoma (p = 0.212). Conclusion: This study shows that the factors that affect the skin maceration around the stoma can be modified (frequency of stoma care and nutritional status), while the location factor can be minimized through the stoma sitting. Thus, the nurses have an important role in the prevention of maceration both at pre and post operative Keywords: stoma site, frequency of stoma care, nutritional status, maceration of stoma.

Abstract

Latar Belakang: Neuropati perifer merupakan salah satu komplikasi mikrovaskuler dari Diabetes Mellitus (DM) yang terjadi pada bagian perifer dan menimbulkan kerusakan fungsi saraf. Kerusakan fungsi saraf tersebut dapat mengenai saraf sensorik, motorik, dan otonom. Tujuan: Tujuan penelitian ini adalah untuk mengetahui gambaran neuropati perifer di wilayah kerja Puskesmas Kedungmundu Semarang. Metode: Penelitian ini merupakan penelitian deskriptif kuantitatif dengan pendekatan cross sectional. Teknik sampling yang digunakan yaitu total population sampling. Pengambilan data dilakukan dengan lembar pemeriksaan neuropati perifer yang merupakan modifikasi dari Michigan Neuropathy Screening Instrument (MNSI) dan Michigan Diabetic Neuropathy Score (MDNS) terdiri atas 38 item. Analisis data yang digunakan yaitu analisis univariat. Hasil: Sebanyak 113 responden berpartisipasi dalam penelitian ini. Mayoritas responden berusia dewasa tengah (73.5%), sebagian besar perempuan (61.9%), telah menderita DM >5 tahun (50.4%), dan memiliki kadar GDS ?200 mg/dL (52.2%). Sebagian kecil responden memiliki riwayat amputasi dan riwayat Diabetic Foot Ulcer (DFU) (3.5% dan 5.3%). Mayoritas kerusakan fungsi saraf adalah kerusakan otonom baik kaki kanan maupun kaki kiri (89.9%;85%). Lebih banyak responden yang memiliki neuropati ringan (55.8%) daripada neuropati sedang ataupun berat (28.3%;9.7%). Kesimpulan: Hasil penelitian ini menunjukkan bahwa sebagian besar responden mengalami neuropati perifer. Oleh karena itu, diperlukan pemeriksaan dini untuk mencegah neuropati yang lebih berat Kata kunci: Diabetes Melitus, Neuropati Perifer

Abstract

Background As one of top ten Diabetes Mellitus (DM) country, prevalence of diabetic foot ulcers (DFU) in Indonesia is high. However there is increasing number of diabetic ulcer non foot (DUNF) in clinical setting. Thus, the aim of this study is to describe the wound care process, healing process, and cost analysis of DUNF. Method A case report study based on retrospective data including; demography data, history of DM and history of DFU recorded by minimum data sheet (MDS). Ulcer pictures were taken by digital camera healing process evaluated by using Barbara Bates Jensen (BBJ) score. Results Patient 44 years, female, have been diagnosed as diabetic since 2013. DUNF located at sub scapularis sinistra with duration of treatment was 95 days and frequency of dressing changes was 15 times. Initial BBJ score was 27 and decreased to be 13 at the end of treatment (healing rate by BBJ score: 0.14/day). Cost of dressing (range Rp. 25.000 ? Rp. 160.000) with mean cost Rp. 100.437. Proportion of cost was 53.3 % for dressing. Correlation between severity of ulcer and cost (R2 = 0.570 p = 0.024). Conclusion The occurrence and recurrence of diabetic ulcer can be any part of body including sub scapular region. Current study confirmed that wound care process takes three months until epitealization with mean cost relative expensive.

Abstract

ABSTRACT Pendahuluan : Sitokinin berperan penting pada penyembuhan luka, salah satu sitokain yang berperan pada proses penyembuhan luka adalah interleukin-6 (IL-6). Penggunaan buah naga merah sebagai modalitas penyembuhan luka menjadi salah satu alternatif perawatan. Sayangnya masih sangat terbatas penelitian yang melaporkan penggunaan buah naga merah pada penyembuhan luka. Tujuan penelitian ini untuk mengindentifikasi efek dari pemberian Ekstrak Buah Naga Merah (EBNM) secara topikal terhadap kadar IL-6 pada penyembuhan luka akut pada wistar. Metode: Penelitian ini merupakan penelitian ekperimen dengan desain Randomized Post Test Control Group, dimana wistar jantan albino (n=41) berat badan 250-300 gr dibagi ke dalam 3 kelompok (control negatif (n=12), control positif (n=12), EBNM 7,5% (n=12)). Wistar dilukai pada punggung kiri dan kanan menggunakan Punch Biopsi 5 mm, selanjutnya kadar IL-6 diperiksa melalui serum dengan teknik ELISA. Data dianalisis menggunakan uji One Way Anova menggunakan SPSS 22 dengan nilai p < 0,05. Hasil: Pada hari ke-7, kadar IL-6 pada kelompok EBNM (101.9±32,2) lebih rendah dibandingkan kelompok control negatif (142,4±85,0) dan tidak lebih inferior dari kontrol positif (97.9±41.9). Pada hari ke-17, diameter luka lebih kecil pada kelompok EBNM (2.66±1.505) dibandingkan kelompok positif (3.16±0.516) dan kontrol negatif (4.33±0.707). Kesimpulan: Krim topikal EBNM 7,5% mempercepat penyembuhan luka yang ditandai dengan pengecilan diameter luka, yang disertai dengan penurunan kadar IL-6 yang sama baiknya dengan kontrol positif pada hari ke-7.

Abstract

The wound healing process is complex and is influenced by multifactors, one of which is MMP-9. Dragon fruit is a fruit that became popular because of its healing effect on wounds due to its flavonoid content. However, related research is still limited. This study aims to prove the effect of topical Red Dragon Fruit Extract (RDFE) on changes in MMP-9 levels on acute wistar healing. Researchers used experimental research design with post test with control group design. Animals tried in the form of male wistar (n = 58) with acute injection using 5 mm punch biopsy, divided into 3 groups (control, establish and topical RDFE 7.5%). Measurement of MMP-9 levels with ELISA techniques using wound tissue. Data were analyzed using SPSS 22 with One Way ANOVA Test. The MMP-9 levels in the EBNM group had the lowest levels on day 3 (controls: 1.64 ± 0.82, establish: 1.05 ± 1.09 and RDFE: 0.99 ± 0.95). After 7 days the treatment has approximately the same level (Establish: 1.28 ± 0.61 and RDFE: 1.57 ± 0.72) After 14 days, RDFE has the lowest MMP-9 levels Topical RDFE can decrease MMP-9 levels in all wound healing phases and there is a reduction in wound diameter and no more inferior than establish and control.

Abstract

LATAR BELAKANG Proses penyembuhan luka di pengaruhi oleh beberapa faktor. salah satu item pada pengkajian di identifikasi lokasi luka akan tetapi jarang dianggap sebagai parameter dalam proses penyembuhan luka. Bisa jadi ini disebabkan karena masih jarang penelitian tentang bagaimana lokasi berpengaruh terhadap kecepatan proses penyembuhan luka. TUJUAN Mengindentifikasi kecepatan proses penyembuhan LKD antara dorsal dan plantar. METODE Desain penelitian retrospective 2014 ? 2015, di ETN Centre Indonesia. Sampel dengan purposive sampling. Inklusi pasien yang mengalami LKD pada Dorsal dan Plantar pada waktu yang bersamaan, sementara menjalani perawatan di rumah perawatan ETN CENTRE. Data demografi melalui wawancara. Kecepatan proses penyembuhan luka menggunakan Barbara Bates Janse (BBJ) Modifikasi skor. Gambar luka diambil menggunakan kamera digital (sony DSC-W510) , analisa data menggunakan Microsof Exel 2007. HASIL Dari data base kami menemukan 6 orang sampel yang memilki LKD pada dorsal dan plantar. 4 laki- laki,usia 56,6 (40-74 ) tahun. Pendidikan : 3 SMA, 2 SD, 1 S1. Pekerjaan : 2 URT, 2 Petani,1 PNS,1 Pensiunan. Medikamentosa : 4 insulin dan 2 obat oral. Riwayat luka 25,5 (21-30) hari. Rawat Sebelumnya : 3 RS dengan surgikal debridement,2 rawat sendiri dengan obat herbal,1 dokter praktek dengan mekanikal debridement. Penyebab luka trauma Tajam 6 orang. Skor BBJ dorsal 0,278 ( 0,095 ? 0,392 ) dan Plantar 0,613 (0,194 ? 1,333). KESIMPULAN Ada perbedaan kecepatan proses penyembuhan luka kaki diabetik antara dorsal dan plantar KATA KUNCI LKD ,kecepatan penyembuhan,plantar,dorsal.

Abstract

Background: Recently, Diabetic Foot Ulcer Assessment Scale (DFUAS) has been developed based on the characteristic of Diabetic Foot Ulcers (DFU) in Indonesia. DFUAS consist of 11 sub scales and its validity reported adequately. However, the reliability of DFUAS has not investigated. Therefore, application of DFUAS in clinical setting remains unclear. Objective: The aim of the current study was to evaluate interobserver reliability of DFUAS. Method: This was a cross sectional study, conducted in RUMAT (Rumah Perawatan) with the eligibility criteria of the observer as Wound Care Nurses. DFU?s status was scored using DFUAS based on 10 various pictures (Wagner I, II, III, IV and V) from Griya Afiat database. Interobserver reliability was calculated using Cronbach Alpha for each sub scales (SPSS 16.0). Results: Current study involved 44 wound care nurses (11 males, 32 females), nursing education background (vocational: 10 nurses, professional: 34 nurses) with experience as a wound care nurse (< 2 year, 31 nurses) and (> 2 years 11 nurses). Interobserver agreement confirmed by Cronbach Alpha for each sub scale, including depth (0.509), inflammation/infection (0.690), maceration (0.628), type of necrotic tissue (0.296), the proportion of necrotic tissue (0.530), the proportion of slough (0.467), the proportion of granulation (0.560), size (0.627), size score (0.177), tunneling (0.759) and type of wound edge (0.400). Overall Cronbach Alpha was 0.654. Conclusions: This study confirmed moderate reliability of DFUAS among different wound care nurse in Indonesia. Training how to use DFUAS is essential to increase its reliability.

Abstract

Background: There is increasing number wound care nurses in Indonesia, following increasing number of private wound care clinic with different standard. Currently, national wound care clinic standard has been developed Indonesia which consisted 5 main standards and 27 sub standards. However, the validity remain unclear. Objective: The objective of this study was to evaluated content validity regarding clarity and relevancy. Methods: This was a cross sectional study, involved wound care nurses using purposive sampling technique. Eligibility criteria of partisipants were wound care nurse. Results: 36 nurses participate in this study (20, 55.6% female), mayority graduated form university (23, 63.9%) with experience as wound care nurse (mean, ± SD; 9.00, 6.84) and daily working hours 4-8 hours (14, 38.9%).
