Abstract

Medication safety is a main focus to maintain patient safety. Recent studies show that almost 50% of adverse incidents in the global health system are rooted in inaccuracy in drug administration. This study aimed to determine the implementation of collaborative interventions for drug administration in the Emergency Room of X Hospital in Banda Aceh. This quantitative descriptive study, using a cross-sectional approach, used a total sampling technique to select 58 respondents. Data was collected through direct observation of drug administration practices and analyzed using descriptive statistical tests. The results showed that the implementation of the principle of the seven rights of drug administration in the Emergency Room of Hospital X in Banda Aceh was in the good category (72.4%); with the implementation of drug administration to the right patient (91.4%), right drug (96.6%), right dose (100%), right route (100%), right time (94.8%), right documentation (93.1%), and right information (96.6%). In conclusion, nurses in the Emergency Room of Hospital X have applied the seven rights of drug administration principle. However, more effort is required to increase nurse adherence to this principle, especially in verifying patient identity and providing drug information. It is expected that hospitals can conduct continuous planning, monitoring, evaluation, and supervision to strengthen the implementation of the seven rights principles of drug administration and to ensure patient safety.

Abstract

Medical record retention is a crucial aspect of medical record management that directly impacts the quality of healthcare services. As a primary healthcare facility, Banyumulek Community Health Center (Puskesmas), facing challenges in managing active and inactive medical records due to limited storage space and increasing patient visits, faces challenges in managing active and inactive medical records due to limited storage space and the increasing number of patient visits. This study aims to analyze the medical record retention process at Banyumulek Community Health Center and efforts to improve the quality of medical record services. A descriptive approach was used, collecting data through observation, interviews, and documentation studies of medical record staff. The results indicate that the implementation of medical record retention does not fully comply with standard operating procedures, particularly regarding the scheduling of retention and destruction of inactive files. This situation impacts service efficiency, speed of file retrieval, and the tidiness of the storage system. Therefore, consistent implementation of retention policies, the development of clear standard operating procedures (SOPs), and increased competency of medical record staff are needed to support the improvement of the quality of medical record services at Banyumulek Community Health Center.

Abstract

Type 2 diabetes is a growing global health problem, especially in low- and middle-income countries. Poor self-management remains a major barrier to good blood sugar control. In Bali, traditional practices known as Usada Bali may support diabetes self-management. This study aimed to review Usada Bali–based self-management approaches for patients with type 2 diabetes. A literature review was conducted using PubMed and Google Scholar. Articles published between 2016 and 2024 were selected based on relevance to diabetes self-management, Balinese culture, herbal medicine, yoga, spiritual healing, and family-based support. Data were analyzed using a narrative synthesis approach. The findings showed that Usada Bali practices, including herbal drinks (loloh), traditional diet, yoga, foot massage, spiritual healing, and family involvement, were associated with improved blood glucose control, reduced anxiety, better insulin sensitivity, and enhanced self-management behavior. Integrating cultural and spiritual elements into diabetes care may increase patient acceptance, adherence, and long-term lifestyle changes. However, more clinical studies are needed to strengthen scientific evidence and standardize interventions.

Abstract

Chronic Suppurative Otitis Media (CSOM) is a middle ear infection characterized by perforation of the tympanic membrane and persistent otorrhea lasting for more than two months. Local epidemiological data on CSOM in Kota Palopo, particularly from hospital-based settings, remain limited. Therefore, this study aims to describe the epidemiological and clinical characteristics of CSOM patients in a regional hospital setting where local data are still limited, focusing on factors such as gender, age, main complaints, CSOM type, and perforation location. A descriptive quantitative method with a cross-sectional design was used, collecting data from medical records of patients diagnosed with CSOM from January to June 2023 (n = 40). The results showed that the majority of patients were aged 19–60 years, with otorrhea as the most common complaint and benign CSOM type. Most patients had perforation at the pars tensa, while perforations at the pars flaccida and marginal locations were less common but posed a higher risk of severe complications. This study provides preliminary descriptive data on the characteristics of CSOM patients at RSU St. Madyang Kota Palopo. These findings may serve as a baseline for future research and local prevention efforts.

Abstract

Environmental hygiene behavior among adolescents is an important indicator in public health promotion and prevention efforts. However, the low level of awareness among adolescents regarding maintaining school environmental hygiene remains a common problem. Health literacy plays a role in enhancing individuals’ ability to understand, evaluate, and apply health information, thereby influencing environmental hygiene behavior. This study aims to analyze the influence of health literacy on environmental hygiene behavior among adolescents in secondary schools. This study uses a quantitative design with a cross-sectional approach. The respondents were Bhakti Mulia Vocational High School, Kediri selected using simple random sampling (n=113). Data were collected using a structured questionnaire that measured health literacy and environmental hygiene behavior. Data analysis was performed using the Pearson product-moment correlation test. The results showed that most respondents had moderate to high levels of health literacy, as well as fairly good environmental hygiene behavior. There is a significant association between health literacy and environmental hygiene behavior (r=0.512; p < 0.05), such that higher health literacy is associated with better environmental hygiene behavior. Health literacy is significantly associated with influences environmental hygiene behavior among adolescents. Abstract

Introduction: The manual Safe Work Permit (SIKA) system at the Onshore Receiving Facility (ORF) of PT Pertamina Hulu Energi West Madura Offshore (PHE WMO) faces challenges like lack of real-time status visibility, document misplacement, and disjointed information, potent ially increasing occupational safety and health (OSH) risks. Methods: This applied research employed a Research and Development (R&D) approach using the ADDIE (Analyze, Design, Develop, Implement, Evaluate) model because of its systematic and iterative framework for designing, testing, and evaluating digital management systems. The study was conducted at the ORF PHE WMO from July to September 2025. Primary data came from field observations and interviews with HSSE personnel, while secondary data included company procedures and permit registers. Results: The study resulted in a web-based Digital SIKA Dashboard. Key features include digital permit submission, multi-level approval workflows (PTWC, AA, SC), an interactive site plot plan for real-time permit location visualization, and integration of supporting documents. The system centralizes data, replacing physical boards and manual registers. Conclusion: The Digital SIKA Dashboard effectively addresses the limitations of the manual system by providing real-time monitoring, enhancing transparency, reducing administrative errors, and supporting integrated OSH management. Its implementation requires user training and potential future integration with IoT for optimal effectiveness.
