Abstract

Medication safety is a main focus to maintain patient safety. Recent studies show that almost 50% of adverse incidents in the global health system are rooted in inaccuracy in drug administration. This study aimed to determine the implementation of collaborative interventions for drug administration in the Emergency Room of X Hospital in Banda Aceh. This quantitative descriptive study, using a cross-sectional approach, used a total sampling technique to select 58 respondents. Data was collected through direct observation of drug administration practices and analyzed using descriptive statistical tests. The results showed that the implementation of the principle of the seven rights of drug administration in the Emergency Room of Hospital X in Banda Aceh was in the good category (72.4%); with the implementation of drug administration to the right patient (91.4%), right drug (96.6%), right dose (100%), right route (100%), right time (94.8%), right documentation (93.1%), and right information (96.6%). In conclusion, nurses in the Emergency Room of Hospital X have applied the seven rights of drug administration principle. However, more effort is required to increase nurse adherence to this principle, especially in verifying patient identity and providing drug information. It is expected that hospitals can conduct continuous planning, monitoring, evaluation, and supervision to strengthen the implementation of the seven rights principles of drug administration and to ensure patient safety.

Abstract

Medical record retention is a crucial aspect of medical record management that directly impacts the quality of healthcare services. As a primary healthcare facility, Banyumulek Community Health Center (Puskesmas), facing challenges in managing active and inactive medical records due to limited storage space and increasing patient visits, faces challenges in managing active and inactive medical records due to limited storage space and the increasing number of patient visits. This study aims to analyze the medical record retention process at Banyumulek Community Health Center and efforts to improve the quality of medical record services. A descriptive approach was used, collecting data through observation, interviews, and documentation studies of medical record staff. The results indicate that the implementation of medical record retention does not fully comply with standard operating procedures, particularly regarding the scheduling of retention and destruction of inactive files. This situation impacts service efficiency, speed of file retrieval, and the tidiness of the storage system. Therefore, consistent implementation of retention policies, the development of clear standard operating procedures (SOPs), and increased competency of medical record staff are needed to support the improvement of the quality of medical record services at Banyumulek Community Health Center.

Abstract

Type 2 diabetes is a growing global health problem, especially in low- and middle-income countries. Poor self-management remains a major barrier to good blood sugar control. In Bali, traditional practices known as Usada Bali may support diabetes self-management. This study aimed to review Usada Bali–based self-management approaches for patients with type 2 diabetes. A literature review was conducted using PubMed and Google Scholar. Articles published between 2016 and 2024 were selected based on relevance to diabetes self-management, Balinese culture, herbal medicine, yoga, spiritual healing, and family-based support. Data were analyzed using a narrative synthesis approach. The findings showed that Usada Bali practices, including herbal drinks (loloh), traditional diet, yoga, foot massage, spiritual healing, and family involvement, were associated with improved blood glucose control, reduced anxiety, better insulin sensitivity, and enhanced self-management behavior. Integrating cultural and spiritual elements into diabetes care may increase patient acceptance, adherence, and long-term lifestyle changes. However, more clinical studies are needed to strengthen scientific evidence and standardize interventions.
