Abstract

Background: Anxiety prior to contraceptive insertion is commonly experienced by prospective family planning acceptors. Psychological discomfort during the procedure may reduce client comfort, influence decision-making, and potentially affect the continuity and effectiveness of contraceptive use. In many clinical settings, anxiety management is often limited to verbal reassurance, while simple non-pharmacological approaches are rarely implemented. One technique that has received increasing attention is hand massage, which may promote relaxation through stimulation of peripheral nerves and improved blood circulation. This intervention is simple, inexpensive, and easy to apply in routine healthcare services. Objectives: This study aimed to examine the effect of hand massage on reducing anxiety levels among prospective intrauterine device (IUD) and implant acceptors at the Independent Midwife Practice of Istri Yuliani, Sleman. Methods: This research employed a quasi-experimental design with a one-group pre-test and post-test approach to examine changes before and after the intervention. This research employed a quasi-experimental design with a one-group pre-test and post-test approach to examine changes before and after the intervention A total of 56 prospective contraceptive acceptors participated in the study. Anxiety levels were measured using the Hamilton Rating Scale for Anxiety (HRS-A) before and after the hand massage intervention. The massage was administered prior to contraceptive insertion as a relaxation technique. Data analysis was performed using descriptive statistics to describe the distribution of anxiety levels and paired t-test to examine the significance of anxiety reduction following the intervention. Results: Before the intervention, 75% of respondents experienced mild anxiety, while only 3.6% reported no anxiety symptoms. After receiving hand massage, the proportion of respondents without anxiety symptoms increased substantially to 80.4%, whereas mild anxiety decreased to 19.6%. The paired t-test indicated a statistically significant reduction in anxiety scores (p = 0.001). Conclusions: Hand massage is associated with a reduction in anxiety prior to contraceptive insertion. This technique may serve as a supportive non-pharmacological intervention to enhance client comfort and improve the overall experience of family planning services.

Abstract

Background: Excessive gestational weight gain (GWG) is associated with adverse maternal and neonatal outcomes, including gestational diabetes mellitus, hypertensive disorders, increased birth weight, and postpartum weight retention. Physical activity and structured exercise are modifiable lifestyle factors that may help prevent excessive GWG, yet evidence varies by intervention type, intensity, duration, and adherence. Objectives: This systematic review aimed to synthesize evidence from randomized controlled trials on the effects of physical activity and exercise interventions during pregnancy on gestational weight gain. Methods: This systematic review followed PRISMA 2020 guidelines and used a Synthesis Without Meta-analysis approach. Searches were conducted in PubMed, ScienceDirect, Scopus, and Emerald for studies published between 2013 and 2023. Eligible studies were randomized controlled trials involving pregnant women receiving physical activity or exercise interventions. GWG was the primary outcome, while gestational diabetes mellitus, hypertensive disorders, neonatal outcomes, and adherence were secondary outcomes. Screening was conducted using Rayyan, and study quality was assessed using the CASP checklist. Results: A total of 2,052 records were identified. After duplicate removal and eligibility screening, 27 full-text articles were assessed, and 4 randomized controlled trials were included. The included studies generally suggest that structured exercise during pregnancy may help reduce excessive GWG, although findings varied across populations and intervention characteristics. Conclusions: Physical activity and structured exercise during pregnancy may help prevent excessive gestational weight gain, although their effectiveness depends on adherence, baseline BMI, and intervention characteristics. Integrating exercise counseling into antenatal care may support appropriate gestational weight gain

Abstract

Background: The coverage of exclusive breastfeeding in Bengkulu Province remains low, reaching 67.08% and even declining to 52.9% in 2022, which is below the national average of 71.58%. Mothers who undergo cesarean section (CS) deliveries are more likely to experience early breastfeeding failure. Approximately 68% of post-CS mothers face difficulties in caring for their newborns and moving in and out of bed due to post-surgical pain. Objectives: This study aims to evaluate the effect of an educational video on Biologic Nurturing Baby-Led Feeding on the knowledge, attitudes, and breastfeeding success of post-cesarean section mothers in Bengkulu City. Methods: This study employed a quasi-experimental design using a pre-post test group approach. The research was conducted in several three.Type C hospitals in Bengkulu City from January to November 2024. A total of 60 participants were divided into two groups: the intervention group, which received education through video media, and the control group, which used a pocket book as the educational tool. The data analysis included univariate analysis, bivariate analysis using the Independent Sample T-Test, and multivariate analysis using MANCOVA. Results: Statistical analysis revealed that the use of Biologic Nurturing Baby-Led Feeding educational video had a significant impact on mothers’ knowledge, attitudes, and breastfeeding success (p-value = 0.05).  Effect Size : knowledge : 0.555, attitudes: 0,662 and breastfeeding success : 0.735 Conclusions: Health education can improve mothers' knowledge, attitudes, and breastfeeding success. However, the use of the Biologic Nurturing Baby-Led Feeding educational video proved to be more effective and was preferred by the respondents.

Abstract

Background: Acute Coronary Syndrome (ACS) remains a leading cause of morbidity and mortality, with many patients experiencing high levels of anxiety during recovery. This anxiety is often exacerbated by uncertainty in illness (one of aspects that play a key role in shaping anxiety responses among patients, particularly in life-threatening conditions such as ACS) and lifestyle changes, while limited social support further hinders optimal rehabilitation. Despite its clinical importance, research exploring the combined effects of social support and uncertainty in illness on anxiety in ACS patients is still scarce, highlighting the need for this study. Objectives: This research aimed to analyse the correlation between social support and uncertainty in ilness with anxiety in ACS patient. Methods: This study used a cross-sectional design on 73 respondents at National Heart Center Harapan Kita in May - July 2025. The study was conducted using four instruments consisting of characteristics, MSPSS, SF-MUIS, and HADS. The collected data were analyzed using the Pearson test because the data were normally distributed after the Kolmogorov-Smirnov (KS) test with the results 0.723 for social support, 0.788 for uncertainty in illness, and 0.737 for anxiety (p-value > 0.05). Results: The results of the study showed a significant relationship between social support and anxiety in ACS patients (p-value: 0.012, rpearson: -0.291). Other results showed no promising relationship between uncertainty in illness and anxiety in ACS patients (p-value: 0.481, rpearson: 0.084). Conclusions: The study found that social support significantly correlated with anxiety in ACS patients while uncertainty in illness showed no significant correlation. This research is highlighting the need for further research with larger patients, integration of Mishel's Uncertainty Theory in education, and the application of Family-Centered Care in Clinical Practice

Abstract

Background: Exclusive breastfeeding coverage remains suboptimal in many regions in Indonesia, including Bengkulu City. This is related to several factors, such as pre-lacteal feeding practices, limited maternal knowledge, inadequate family support, and cultural influences. Strengthening breastfeeding education during pregnancy is essential to improve maternal preparedness and support successful exclusive breastfeeding. Objectives: This study aimed to examine the effect of an Android-based KEKASIH Class educational program on breastfeeding knowledge, attitudes, and breastfeeding self-efficacy among pregnant women. Methods: A quasi-experimental study with a non-equivalent control group design was conducted among 70 third-trimester pregnant women in Bengkulu City. Participants were selected using purposive sampling and divided into an intervention group and a control group. The intervention group received breastfeeding education through the Android-based KEKASIH Class application, while the control group received conventional education through antenatal classes. Data were analyzed using univariate analysis and non-parametric tests, including the Wilcoxon signed-rank test and the Mann–Whitney U test. Results: The results showed significant improvements in breastfeeding knowledge, attitudes, and breastfeeding self-efficacy in both groups after the intervention (p < 0.05).There was no statistically significant difference between the intervention and control groups (p > 0.05), indicating that both educational approaches were effective in improving maternal readiness for breastfeeding. Conclusions: The findings demonstrate that both the Android-based KEKASIH Class and conventional antenatal education can effectively improve breastfeeding knowledge, attitudes, and breastfeeding self-efficacy among pregnant women. Digital health education can therefore be considered an alternative strategy to support breastfeeding promotion programs.

Abstract

Background: Diabetes not only causes premature deaths worldwide. This disease is also a major cause of blindness, heart disease and kidney failure. Prevention and control of diabetes mellitus in Indonesia is carried out so that healthy individuals remain healthy and people who already have risk factors can control them so that they do not fall ill with diabetes. Individuals with a history of impaired glucose tolerance (IGT) or the pre-diabetic group need to adopt a healthy lifestyle by paying attention to food and drink intake, especially sweet foods and doing physical activity, so that it does not progress to diabetes mellitus. Objectives: To determine the effect of coconut sugar on blood sugar levels in prediabet sufferers in the Sleman Regency, Special Region of Yogyakarta. This research is focused on promotive and preventive efforts in efforts to prevent non-communicable diseases such as diabetes mellitus so as to achieve optimal levels of public health. Methods: This research was Quasy experimental research design, carried out by giving pre-diabetic sufferers 30 grams of coconut sugar a day for 2 weeks on selected respondents with blood sugar levels of 100 – 126 mg/dl. Respondents who met the criteria were then divided into 2 groups. The intervention group given 30 grams of coconut sugar per day for 2 weeks and the control group consume food and drinks as usual. Results: Based on comparative research of blood sugar levels before and after giving coconut sugar among prediabetes, the results showed that there was a significant decrease in blood sugar in the intervention group after giving with the level blood sugar average is 17.39 mg /dl and in the control group blood sugar level average is 6.83 mg/dl Conclusions: There is significant effect of coconut sugar to decrease level of blood sugar among prediabetes patients.

Abstract

Background: Electronic Nursing Documentation (EDN) practices in Class C hospitals are crucial for implementing the current care system. However, EDN practices in developing cities like Pekan Baru have not been fully implemented, and the factors influencing them remain poorly understood. Objectives: This study aims to analyze the factors influencing electronic nursing documentation among nurses in inpatient wards at Class C hospitals in Pekan Baru City. Methods: This is an observational, analytical, cross-sectional study. The study was conducted at three private Class C hospitals in Pekanbaru: Ibnu Sina Hospital, Syafira Hospital, and Sansani Hospital. Data were collected through questionnaires from 173 nurses who met the inclusion and exclusion criteria. Factors such as demographics, psychological conditions, job satisfaction, stress, personality, organizational factors, and work compensation systems were measured. Analysis used chi-square analysis followed by multiple logistic regression. Results: In this study, hospital type, age, education, biological stress, supervisory organizational factors, and rewards system were significantly associated (p-value < 0.05), while other factors were not. Multivariate analysis revealed that five variables significantly associated nurse documentation: age, education level, biological stress, supervision, and reward systems (p < 0.05). Supervisory organizational factors (OR = 15.429) and reward systems (OR = 8.657) were the most dominant. Conclusions: The finding indicate that supervision and institutional incentive system are significantly associated with electronic nursing documentation practice among nurse. Nurse with better supervision and more supportive reward systems tended to demonstrate improve documentation practices. These findings suggest that organizational factor may contribute to supporting electronic documentation, although further longitudinal or experimental studies are needed to confirm causality.

Abstract

Background: Tuberculosis is a contagious infectious disease where its control efforts are one of the world's targets, namely the Sustainable Development Goals (SDGs). Compliance with taking medication in pulmonary TB patients is an obstacle often faced by patients because they have to take medication continuously until the specified time, however in Indonesia there are still many cases of drop out for various reasons such as drug side effects, level of knowledge, habits, lack of family support, environment, socio-economic and health education strategies. The education provided is still one-way and has not touched on psychological, social, and motivational aspects, even though patients have the potential to act as facilitators of behavior change. Objectives: The purpose of this study is to determine the development of health coaching nursing education based on the precede-proceed model theory on medication adherence for TB patients in the Pleret Bantul DIY Community Health Center Working Area. Methods: This research design is quasi- experimental with Non Equivalent Control Group approach. The number of samples in this study refers to Non Probability Sampling type of total sampling amounting to 32, which are divided into 2 intervention and control group of 16 respondents each. The d ata analysis of this study used the Wilcoxon Signed Rank Test and Mann-Withney. Instrument for medication adherence was measured using the Medication Morisky Adherence Scale (MMAS-8). Results: The results showed a significant difference between the control group and the intervention group after health coaching nursing education based on the precede-proceed model theory with a value of p= 0.049 < 0,05. Conclusions: It was concluded that health coaching nursing education based on the precede-proceed model theory influenced medication adherence of TB patients in the Pleret Bantul DIY Community Health Center Work Area

Abstract

Background: Diet plays a crucial role in maintaining health status and boosting immune system of young children against different illnesses. The context based on ongoing pediatric illness due to suboptimal feeding patterns. An inadequate diet can depress the child's resistance and his ability to fight off illness. Objectives: This research aimed to analyse the effect of diet on child morbidity in Yogyakarta Special Region. Methods: This research was a quantitative study with a cross-sectional approach. The study population was all children aged 0–7 years in the Special Region of Yogyakarta, with a sample of 366 children selected using multistage cluster random sampling. Data analysis was performed using logistic regression analysis to determine the extent of the influence of diet on the incidence of illness. Results: The results showed that the majority of children had sufficient dietary patterns (51.9%), followed by good dietary patterns (44.3%) and poor dietary patterns (3.6%), while illness incidence was found in 48.6% of respondents. Logistic regression analysis indicated that dietary patterns significantly affected illness incidence among early childhood children (p = 0.012). Children with poor dietary patterns had a 7.23 times higher risk of experiencing illness compared to children with good dietary patterns (OR = 7.23). The Nagelkerke R Square value of 0.039 indicated that dietary patterns contributed 3.9% to illness incidence, while the remaining proportion was influenced by other factors not examined in this study. Conclusions: The conclusion of this study is that dietary patterns have a significant effect on the incidence of illness in early childhood, although the contribution is relatively small; therefore, strengthening other supporting factors for comprehensive child health is necessary.

Abstract

Background: Childbirth is a critical physiological process experienced by women that requires optimal physical and psychological preparation. Inadequate preparation may contribute to prolonged labor and increased pain intensity. Prenatal exercise is recognized as a non-pharmacological approach to improve maternal readiness. A combination of pelvic floor muscle (PFM) exercise and abdominal exercise is considered beneficial for strengthening muscles involved in the labor process, potentially influencing both labor duration and pain perception. Therefore, this study aimed to analyze the effect of combined pelvic floor muscle and abdominal exercises on labor pain intensity and duration of labor among pregnant women. Methods: This study employed a quasi-experimental design with a posttest-only nonequivalent control group approach. The total sample consisted of 60 pregnant women, divided equally into an intervention group and a control group (1:1 ratio). Participants were selected using convenience non-probability sampling. The intervention group received prenatal exercise combining PFM and abdominal exercises starting from 38 weeks of gestation until delivery, while the control group received standard care. Data were analyzed using independent t-tests to compare outcomes between groups. Results: The findings demonstrated that the intervention group had significantly lower mean pain scores compared to the control group at 8 cm cervical dilation (t = -3.828, p = 0.001) and at complete dilation (t = -3.865, p = 0.001). Additionally, the duration of the first stage of labor (t = -3.118, p = 0.003) and the second stage of labor (t = -2.122, p = 0.038) were significantly shorter in the intervention group. Conclusions: Prenatal exercise combining pelvic floor muscle and abdominal exercises has a positive effect on the childbirth process by reducing labor pain and shortening the duration of labor stages. This intervention can be recommended as part of routine antenatal care to improve maternal outcomes.
