Abstract

Heart failure is a complex clinical syndrome and the risk of high morbidity and mortality in the population. HeartFailure is a disease requiring long therapy, so cost for heart failure treatment is very high.This study design was descriptive non experimental. Data was collected retrospectively. Data was analyzedby descriptive quantitative, and statistical method using correlation analysis. Data was classified based on costingsystem and number of comorbid.The mean and proportion of overall total costs of heart failure on non poor patients were greater than poorpatients and its value increases along with increasing number of comorbid. Laboratory cost dominate total cost for poor group without or with comorbid. While for non poor group was laboratory cost on patient without comorbid (29,80%) and with 1 comorbid (25,93%), on patient with 2 comorbid was medicine cost (21,99%) and onpatient with more than 2 comorbid was hospitalization cost (23,21%). Correlation test among patient characteristicindicating that six variable had significant correlation. Correlation test using a multivariate analysis showed thatvariable which significantly correlated with total cost on poor group was number of comorbid, and on non poorgroup was number of comorbid and LOS. Keyword : Heart failure, cost, type of costing system, number of comorbid

Abstract

Measuring performance comprehensively on Technical Implementation Unit of the Regional Health InsuranceProvider (UPT PJKD) Yogyakarta as Administration Provider of Yogyakarta Regional Health Insurance Program(Jamkesda) with the Balanced Scorecard approach through four perspectives of customers and stakeholders, financial,internal business processes, and employees and organization capacity so that the achievement of performance havebeen made known. In this study designed non-experimental case study was descriptive. Data collected in the form of quantitativedata (questionnaire with closed questions Likert scale system of one to four, and financial statements PJKD YogyakartaUPT) and qualitative. The sampling technique used was purposive sampling. Criteria used for the respondentcustomers are not permanent teachers (GTT) I non-permanent employees (PTT) and auxiliary power (Naban) 100people, and the respondent employees consisting of officers UPTD PJKD Yogyakarta.The results showed on Customers and Stakeholders Perspective showing that the lowest value gap 5 is onthe dimensions of Responsiveness, -0,57. And the lowest perception level is on the dimensions of Empathy 3,17,this suggests that the Office of the UPT PJKD has provided satisfactory service quality for participants Jamkesda.Financial perspective suggests that the realization of UPT PJKD Yogyakarta Reimbursement in 2010 was Rp. 132 734 000, Internal Business Processes perspective suggests that: average administration service waiting time is between30-60 minutes, and the average time required to obtain reimbursement of funds is between 1 week to 1 month.Employees and Organization Capacity perspective suggests that average value Human Capital, Information Capitaldan Organization Capital officer UPTD PJKD Yogyakarta is between 60% 80% from the hope, it means a lot of officershave high performance. Key words: Analysis of the performance, Administration Provider, the Regional Health Insurance, BalancedScorecard.

Abstract

The Pharmacy Departement of Semarang Health Agency is where a management of drugs taking place inSemarang. In order to provide quality health care, we need to evaluate the phases in the planning and procurementof drugs as well as the match between the planning and procurement. Data was collected retrospectively with descriptive observations and the calculation of indicators of the planningand procurement where the obtained data are described and compared with the results of interviews.The results indicated that drug planning using APBD II source of funding was done with consumption method.The planning was done twice, first planning for budget submission and second planning for budget adjustments.Inappropriateness in drug planning and procurement, was not influence by limitations of funding, but caused bydrug procurement that held once each year. This method of drugs procurement could not avoid drgug unavaibility, and delay of drugs delivery and unmet need for drugs by the winner of tender even if sufficient funds was available. Keywords: drug planning, drug procurement, The Pharmacy Departement of Semarang Health Agency.

Abstract

Hypertension is a chronic disease, so patients with hypertension during his life always takes medication to control blood pressure. If blood pressure is not controlled it will result in a blood vessel complications, coronary heart disease, cardiac infarction, stroke and renal failure. The research has been conducted to determine the effect of a pharmacist counseling target blood pressure < 140/90 mmHg in hypertension patients non-diabetes mellitus and < 130/80 mmHg in hypertension patients with diabetes mellitus who was treated at the outpatients of Saras Husada Purworejo General Public Hospital. Retrieved 94 subjects of research, grouped randomly into intervention group (received counseling pharmacist) are 48 subjects and the control group (no counseling pharmacist) as many as 46 subjects. Data analysis using the Statistics Program for Social Science (SPSS). The results showed  that a pharmacist counseling had a significant effect toward the achievement target of < 140/90 mmHg blood pressure on the non-diabetes mellitus hypertension patients (p = 0.000) and < 130/80 mm Hg on the hypertension patients with diabetes mellitus (p = 0.001). Key words : pharmacist counseling, non-diabetes mellitus hypertension, hypertension with diabetes mellitus, decreasing of blood pressure, and the achievement blood pressure target.

Abstract

Health service facilities which are considered to have an excellency will also have a greater opportunity tobe selected by the customer. The formation of positive perceptions from customer that is expected to bringsatisfaction will provide a positive impact on efforts to increase profit and service in Hospital Pharmacy Unit. Thisstudy was aimed to measure the services quality in Outpatients Pharmacy Unit at Regional General Hospital Slemanwhich were reviewed from input, process, and outcome variables.The input variables were assessed through observation of the pharmacy services supporting resources,interviews with the insurer Pharmacists unit responsible for outpatient and job satisfaction questionnaires ofOutpatient Pharmacy Unit employees. The variable process was assessed through the results of drug use indicators.The variable outcome was assessed through value gap customer satisfaction questionnaires based on SERVQUAL methods. Subjects in the study covered the pharmacy services supporting resources (9 persons of employee), prescription drugs, and customers of Outpatient Pharmacy Unit at General Hospital Sleman (257 prescription drugs and customers). Input variables data were analyzed by descriptive analysis. Process variables data were checkedfor completeness from its data fields, tabulated and analyzed using a triangulation method. Outcome variables datawere analyzed by gap analysis.Results showed that the outcome of service did not match the quality services target of Pharmacy Unit at General Regional Hospital Sleman. The gap values of all dimensions service quality were negative, outpatient customers satisfaction index score was 74.41%. Some of the resources input variables need improvement and have not fulfilledthe Law on Hospitals, Government Regulation on Pharmacy Practice, Standard for Pharmacy Services in Hospital, aswell as there were still some obstacle on process variables that affect the pharmacyservices. Key words: pharmacy services quality, the variable input, process, outcome,Outpatient Pharmacy Unit GeneralHospital Sleman.

Abstract

To attract consumer a company should make creative advertisement. One of them is conducted through the useof serial-drama-based advertisement as implemented in the Ponds’ cosmetics product advertisement. This researchaimed to identify whether consumer perception on the serial advertisements of Ponds’ cosmetic products providedsignificant influences on consumer attitude and purchase intention, and to compare consumer perception on theconsumer attitude towards serial advertising and consumer perception on the consumer attitude toward non serial advertising. In this research, the effects of consumer perception on the consumer attitude and consumer purchaseintention towards Ponds’ cosmetics products were measured.This was an experimental research with participants divided into 2 groups, namely, participant group treated with serial advertisement, and participant group treated with non serial-advertisement. Research instruments involved advertisement video samples of Ponds’ cosmetics and research questionnaires. Total samples were 60 persons in eachgroup of serial advertisement and non serial-advertisement. Data obtained were analyzed using simple regressionand significance test through the use of SPSS 17 for Windows. Based on the regression analysis, it was showed that the significant effects of consumer perception on the consumer attitude toward serial advertisement on t-test 8,205, the significant effects of consumer perception on serialadvertisement towards consumer purchasing intention on t-test 8,797, and the significant effects of consumer attitudeon the serial advertisement toward consumer purchasing intention on t-test 6,274. After comparison between theeffects of consumer perception on consumer attitude towards serial advertisement, and the effects of the effects ofconsumer perception on the consumer attitude on non-serial advertisement using t-test it was indicated that theeffects of consumer perception on the consumer attitude toward serial advertisement was higher than the consumer perception towards the consumer attitude on non-serial advertisement. Keywords: advertisement, serial-advertisement, consumer perception, consumer attitude, consumer purchaseintention, t-test.

Abstract

Drug related problems commonly happen to children. The treatment of these problems should be prioritizedbecause their physiologic factor have not perfect yet, so the drugs metabolism and absorption are different. Lowerrespiratory tract infection and astma were causes of children’s pain, and its also still become major problem in medicalfield. This research was generally aimed for investigating drug related problems happened to children with lower respiratory infection in Panti Rapih Hospital Yogyakarta on January until June 2006.This was a non-experiment research conducted in descriptive concept through retrospective data collecting from the medical record of children with lower respiratory tract infection and astma treated in Panti Rapih Hospital Yogyakarta on January until June 2006. These problems were analyzed from the medical record of the patients andthen analized percentage of drug related problems.There were 77 patients who fit the inclusion criteria in this research, number of children in patients with lowerrespiratory tract infection was 55 patients and 22 patients for asthma. The result of research to children in patients with lower respiratory tract infection showed that unnecessary drug therapy was 20%, Wrong drug was 12.72%,too low dosage was 7.27%, too high dosage was 21.81%, and drug interactions was 12.72%. The result of research tochildren in patients with asthma showed that unnecessary drug therapy was 18.18%, wrong drug was 4.54%, dosagetoo high was 13.63%, Drug interactions was 50% and uncompliance was 4.54%. Key words: Drug Related Problems, Lower Respiratory Tract Infection, Pediatric

Abstract

Increasing prevalence of hypertension in almost countries cause by the changes of life style such as smoking,obesity,limitness of physical activity and psychosocial stress. Comprehensive and intensive treatment could bedone to control blood pressure. Patient understanding, knowledge and adherence expected to increase with correctdruginformation given by pharmacist. This study aimed to determine the effect of pharmacist counseling ontherapeutic outcomes and patient adherence. The correlation between therapeutic outcomes and patient adherencealso was studied. of outpatients hypertensive in the internal disease clinic at RSUD Kraton Pekalongan District. This research was experimental intervention with pre-post intervention with randomized without matching. Data was collected by prospective method. The control group given a leaflet at the first month, and the intervention group given a leaflet at the first month and counseling every early month, during 3 months. There was 75 patients as respondent of this research. The intervention group (N=39) experienced a significantdecrease in blood pressure at the end of the study, on systolic amounted 11.28±8.33 mmHg [P=0.000(P<0.05)] anddiastolic amounted 7.18±6.86 mmHg [P=0.000(P<0.05)], whereas in the control group (N=36) did not experiencea significant decrease in blood pressure is systolic amounted 2.22±10.45 mmHg [P=0.203(P>0.05)] and diastolicamounted 0.28±6.09 mmHg [P=0.782(P>0.05)]. Adherence of patients as measured by MMAS scale. The interventiongroup had better adherence than the control group (33.33% : 2.78%). There was have correlation a means betweenMMAS score with change of systolic (P=0.019; r=0.270) and diastolic (P=0.001; r=0.372) which positive direction. It was concluded that counseling had an effect on adherence positively in the intervention group better than the controlgroup and decreasing systolic and diastolic blood pressure until reach targets. There was a close correlation betweendecrease in blood pressure with MMAS score in hypertensive patients. Key Words : Hypertension, counseling, outpatient, adherence

Abstract

Nurses are dominant health staffs in Jasa Kartini Hospital. The activities of nurses illustrate hospital’s image.Nurse performance influence by leadership of head of ward. Progressively according to leadership style appliedby head of ward will have an effect with the growing of nurse’s job satisfaction. There was problems that found atJasa Kartini Hospital related to nursing care service, there was complaint from patients and their family.This studyaimed to identify head of ward’s situational leadership style, to know nurse’s job satisfaction because of head of ward leadership style, and to know the correlation between head of ward leadership style with nurse’s job satisfaction.This research used the observation technique. The study was included into non experimental research. Research’sinstruments used leadership style questionnaire and job satisfaction questionnaire. The subject of this research was145 nurses of Jasa Kartini Hospital Tasikmalaya. The data were analyzed using multiple regression analysis .The result of the study showed that head of ward’s leadership style in Jasa Kartini Hospital was directiveleadership. The directive leadership,consultative leadership and participative leadership head of ward has positive effect toward the nurse’s job satisfaction.There was significant influence between head of ward’s leadership style withnurse’s job satisfaction that was 19%. Keywords: leadership style, job satisfaction

Abstract

Antihypertensive selection accuracy determines the achievement of therapy target toward type 2 diabetes mellituswith chronic kidney disease patients who received regular hemodialysis treatment. The use of drugs in a longtimepotentially causes side effects and drug interactions as well.This research employed prospective observation by evaluating the appropriateness of therapy and monitoringdirectly the side effects of antihypertensive agent used toward type 2 diabetes mellitus with chronic kidney diseasepatients who received regular haemodialysis treatment at RSU PKU Muhammadiyah Yogyakarta during February– April 2011.For 42 patients having type 2 diabetes mellitus with chronic kidney disease who received regular haemodialysis treatment, it was found that patients who received appropriate therapy and achieved blood pressure therapy target was none, 4 patients (9.53%) received appropriate therapy yet the blood pressure remained uncontrolled, 2 patients(4.76%) received inappropriate therapy yet achieved controlled blood pressure and 36 patients (85.71%) receivedinappropriate therapy with remained uncontrolled blood pressure. From 42 patients, it was found that there were 11patients (26,20%) were possible and 5 patients (11,90%) were probable suffered from dry cough, 26 patients (61.91%) were possible for suffered from dizziness, 39 patients (92.86%) were possible suffered from headache, 2 patients (4.76%) were potentially suffered from fatigue and 13 patients (30,95%) were possible and 2 patients (4,76%) wereprobable suffered from drowsiness. Keywords: antihypertensive, type 2 diabetes mellitus, haemodialysis
