Abstract

Indonesia Health Ministrial Decree No. 1027/2004 has been published as a standard of quality assurance in pharmaceutical care provided by pharmacist. As the time goes by, the newest regulation has been provided by government through PP No. 51/2009 to legitimate pharmacists in their responsibility as role player in pharmaceutical care. The implementation of this policy, however, is still unknown yet and needs to be further evaluated. The study aimed to evaluate the pharmaceutical care standard implementation by pharmacist according to this regulation.  This research involved pharmacists in 32 pharmacies in Bantul as respondents. These pharmacies was selected by proportional random from the total of 108 pharmacies in 16 district of Bantul Residence. The data was collected through questionnaire and analized descriptively according to indicators; administration; drug and prescription service; communication, information, and education, inventory control management, human resources, supporting facilities, and evaluation of service quality. The result was  categorized as good if total score persentation 81-100%, adequate if total score persentation 61-80%, and less if total score persentation ≤60%. The study result showed that generally the pharmaceutical care standard has not been well implemented in Bantul Residence. There were 18 pharmacies (56.25%) in the good category, 13 pharmacies (40.63%) in the adequate category, and 1 pharmacy (3.13%) in the less category. The indicators that have been well implemented were supporting facilities indicator (88.95%), human resources indicator (887.67%), inventory control management (82.29%). Other indicators including Communiation, Information, and Education (75.63%), administration (68.23%), and drug and prescription service (64.95%) were in the less category.Keywords: Pharmaceutical Care Standard, Kepmenkes No. 1027/2004, PP No. 51/2009, Bantul Residence

Abstract

Good brand will provide quality assurance for the company, because brand is not just a name but a brand is an asset of the company. Prenagen Junior, a brand extension of Prenagen products for pregnancy and lactation, is a fictitious experiment in this study. The aim of the research was to analyze consumer attitudes towards brand extension. The method used pre-experiment_one shot case study. The population was taken from respondents who had used  Prenagen products either Prenagen Esensis, Prenagen  Emesis,  Prenagen  Mommy,  or  Prenagen  Lactamom  within  last  6 months. A sample of 154 respondents was drawn from that population. The results obtained were analyzed using multiple regression analysis method. The results simultaneously showed that the similarity, brand reputation of origin, perceived risk, and innovativeness delivered a significant influence on consumers’ attitude towards brand extension (R2 : 0.556; p < 0.05).   The similarity of brand extension product with the parent brand, the parent brand’s reputation, perceived  risk  had  a  significant  influence  by  20.8  %,  10.0  %,  and  23.3  % respectively  on  the  consumers’  attitude  towards  the  brand  extension  product. Abstract

Forecasting of pharmaceuticals and medical devices are an effort to estimate the needs of pharmaceuticals and medical devices in hospital. Pharmaceuticals and medical devices of class A based on ABC analysis represents a small amount in stock but has the largest inventory value. This study aimed to determine the exact forecasting that used to estimate the needs of pharmaceuticals and medical devices in class A in the Pharmacy Department Immanuel Hospital Bandung. This research was a descriptive study with retrospective data collection. Forecasting analysis of pharmaceuticals and medical devices needed used the average of last period, seasonal variation, and seasonal index with trend adjustment in 2011. Forecasting analysis was calculated based on consumtion of pharmaceuticals and medical devices in 2008-2010. Accuracy of forecasting methods were determined by the smallest value of mean absolute deviation (MAD) and mean squared error (MSE).  The results from 24 samples of pharmaceuticals and medical devices in class A showed that there were 12 samples (50%) with the smallest MAD for seasonal index with trend adjustment method, 8 samples (66.67%) with the smallest MAD values for average of last period method, and 4 samples (16.67%) with the smallest MAD for seasonal variation method. According to MSE, there were 14 samples (58.33%) with the smallest MSE value for the seasonal index with trend adjustment method, 8 samples (66.67%) with the smallest MSE value for the average of last period method, and 2 samples (8, 33%) with the smallest MSE for seasonal variation method. It can be concluded that seasonal index with trend adjustment analysis was better compared the method of seasonal variation and the average of last period to forecasting needs of pharmaceuticals and medical devices in A category.Keywords:seasonal index with trend adjustment, ABC Analysis, Immanuel Hospital Bandung

Abstract

Diabetic foot is common foot problems in persons with diabetes mellitus. It is suggested that the prevalence of diabetic food cases increase along with the growing number of patients with diabetes mellitus. Diabetic foot commonly appears with manifestation of ulcers, infection, gangrene and Charcoat atrophy. Infection occurs in fifty percent of diabetic foot with ulcers that could lead to amputation if it is not immediately and appropriately treated. Proper antibiotics treatment prevents diabetic foot patient from amputation. This study aimed to understand the pattern of antibiotics usage amongst diabetic foot patients. The appropriateness of antibiotics therapy against current guideline was studied and compared with culture and sensitivity test results. Effectiveness of therapy and patient’s safety was then evaluated. Prospective cohort approach was used to this study. Data of diabetic foot patients was collected during November 2011 – January 2012 at Endocrinology Clinic of Sardjito Hospital. Collected variables included subject characteristics, antibiotics and other medication treatments, visual sign of diabetic foot infection and laboratory test results. Descriptive analysis was then conducted toward all variables. Majority of subjects obtained single therapy of antibiotic when diagnosed with diabetic foot infection. These antibiotics were ciprofloxacin and cefadroxil. Combination of 2 antibiotics were about 31,1%, ciprofloxacin with clindamycin was the most combination in this therapy (37%). About 28.7% of antibiotics therapy was not appropriate with existing guideline. Culture and sensitivity tests strengthen this inappropriateness. It was found that 84.4% of antibiotics therapy was effective. However, there were 5.7% of antibiotic orders that potentially interact negatively with other medication during therapy.Keywords: diabetic food infection, antibiotic, appropriateness, effectiveness, drug  safety

Abstract

Community Health Center is one of the government’s efforts in providing health services in the community. Information system is very crucial in organizing community health centers in order to manage operational activities effectively. Monitoring the results of management information systems, especially in electronic prescription needs to be studied, viewed from the prevention of medication errors, duplication of drug therapy, and drug interactions. The study was conducted by collecting data from the electronic prescription of 10 disease with highest prevalence in 2011 with a sample of 1200 prescription. This research was non-experimental descriptive using retrospective method. Data were taken by random sampling.  The use of information systems, especially in electronic prescribing in general, can prevent medication error. Duplication of therapy in prescribing occurred in 16 prescription. Drug interactions occured in 48 prescription. Evaluation of the results of the overall prescribing showed that there was a trend in the use of corticosteroids drugs.Keywords: management information systems, electronic prescribing, public health of Gunung Kidul.

Abstract

Diarrhea is the second leading cause of death among children in the world and medical cost to deal with diarrhea is significantly high. WHO and UNICEF recommend the use of zinc to treat diarrhea on children. This research was aimed to assess of diarrhea the influence of giving zinc supplement on the duration, length of stay and frequency, and to identify the amount of direct medical cost borne by the patient who used utilizing zinc in order to treat diarrhea. This research was conducted with non-experimental descriptive design. Data were taken retrospectively from medical record and financial office of PKU Muhammadiyah Yogyakarta Hospital. The research subjects were child patients aged 1 - < 15 years old suffering from acute diarrhea, used antibiotic, had no other diseases, and treated as inpatient during the period of January 1 until December 31st 2011. The number of samples that met the inclusion criteria were 67 patients for each zinc and control group. The data were analyzed based on the characteristics of patients, diarrhea duration in the hospital, length of stay, diarrhea frequency and amount of medical cost. Data were analyzed descriptive quantitatively and statistically to compare the means of variables between both groups. The research result indicated that the group receiving zinc had shorter diarrhea duration of ±0.45 day (p<0.05), shorter length of stay of ±0.09 day (p>0.05), and lower diarrhea frequency from the second to the fourth day of the treatment (p<0.05). In regard to cost analysis, the total medical cost of zinc group was lower than that of control group (p<0.05) accounts for IDR. 1,155,407. However, it could not be concluded that the use of Zinc was effective. The use of zinc was effective in treating acute diarrhea on inpatients children.Keywords: zinc, acute diarrhea, effectiveness, direct medical cost

Abstract

Diabetes Mellitus (DM) is a chronic metabolic disorder characterized by elevation of blood glucose concentration. Non-adherence to diabetes treatment in type 2 DM patients leads to poor glucose control and increases the risk of disease complications. Education is one of the ways to increase medication adherence. Limitation of pharmacists in public primary health care in Indonesia led to education could not be done effectively so that it was necessary to find other alternatives education such as medication booklet. This study was undertaken to evaluate the effect of a medication booklet on adherence rate parameters, in type 2 diabetic patients. This was pre-experimental and prospective study conducted at Bakti Jaya primary care, Depok from January to June 2012. A convenience sample of 30 type 2 diabetic patients was studied to receive medication booklet. The value of The 8-item Morisky Medication Adherence Scale (MMAS-8) and percentage of glycosylated haemoglobin (HbA1C) were measured before and after the 8-week intervention. The results were analyzed by paired t-test for HbA1C and Wilcoxon test for MMAS-8. The research result  showed that there were significant differences (p < 0.05) of the value of HbA1C and MMAS-8 between before and after the 8-week intervention. It indicated that a diabetes education by medication booklet, was effective enhancing their medication adherence. This study also found that patients with a lower score on the Morisky scale had a lower HbA1C measurement.Keywords : MMAS-8, HbA1C, adherence, blood glucose, type 2 diabetes mellitus, booklet, pharmacist

Abstract

The need for qualified hospital service has increased along with the improving economy and health ststus of the people. In recent years, the growth of private hospitals in Indonesia has showed significant development with the various regulations and legislation by the government. Therefore, in order to survive in the midst of competition, the Fatimah Islamic Hospital Cilacap should be able to provide a qualified service that’s oriented to achieve patient satisfaction that will lead to patient loyalty. This study was aimed to determine the effect of service quality on satisfaction and to know the effect of satisfaction with the loyalty of inpatients at the Fatimah Islamic Hospital Cilacap. The research was a crosssectional analytic with prospective data collection. The quantitative data were obtained by using a closed questionnaire. The questionnaire were given to 123 inpatients. Then the data were analyzed using multiple linear regression statistical method to investigate the effect of service quality dimension with satisfaction and simple linear regression methods to investigate the effect of satisfaction with the loyalty of inpatients at the hospital. The results showed that there was a positive effect between service quality and satisfaction of inpatients at the Islamic Fatimah Cilacap Hospital. The quality of service that give the most impact on patient satisfaction was tangible dimension with regression coefficient 0.244, while the smallest effect was the dimension of reliability with regression coefficient 0.185. In addition, this study also shows that there was a positive effect between patient satisfaction and loyalty of inpatients at the Fatimah Islamic Hospital Cilacap.Keywords: Service Quality, Patient Satisfaction, Patient Loyalty, Hospital

Abstract

Incidence of ischemic stroke 10 times more often than haemorrhagic stroke in the the west countries, but haemorrhagic stroke has a higher risk of mortality than ischemic stroke. One focus of acute stroke management is the management of hypertension. Blood pressure reduction in ischemic stroke patients could potentially reduce the risk of brain edema, the risk of hemorrhage, and prevent further vascular damage. However, an aggressive blood pressure reduction can cause a decrease in perfusion pressure to the ischemic area. Most hypertensive patients require two or more antihypertensive medications to achieve blood pressure targets. This study used observational study design with retrospective data collection on medical records of acute ischemic stroke patients and performed in RSUP Dr. Sardjito Yogyakarta period January 2010-December 2010. Data were analyzed with descriptive analysis and chi-square evaluative analysis on the relationship between the type of therapy with a reduction in blood pressure of acute ischemic stroke patients. Initial antihypertensive medication use in patients with acute ischemic stroke in RSUP Dr. Sardjito were 13 types of five classes of antihypertensive drugs. After getting a single antihypertensive medication, patients who experienced a decrease in systolic blood pressure on day-3 was 60%, while 17% fixed and 23% up and after getting a combination of antihypertensive medications, patients who experienced a decrease in systolic blood pressure on day-3 was 75%, while 10% fixed and 15% rise. Antihypertensive drug therapy alone or in combination had the same ability to lower systolic blood pressure (p = 0.260) and diastolic (p = 0.567) in patients with acute ischemic stroke in RSUP Dr. Sardjito Yogyakarta. Keywords: Hypertension, acute ischemic stroke, single antihypertensive drug, antihypertensive drug combinations, RSUP Dr.Sardjito Yogyakarta

Abstract

A competitive company is a company which has and implements business strategies that can act in response to external conditions and improve their internal capabilities. The objectives of this study were to identify and evaluate whether the company business strategy is suitable with its external and internal conditions and to arrange alternative strategies based on external and internal conditions.  The research was a descriptive analysis. The data were collected through deep interviews and internal observation to the company, also supporting data from secondary source. The data were analyzed by: External and internal analysis with External Factors Evaluation Matrix (EFE) and Internal Factors Evaluation Matrix (IFE); Alternative strategies formulation was done through Strength-Weakness-Opportunity-Threat Matrix (SWOT), Internal-External Matrix (IE), Boston Consulting Group Matrix (BCG), and Grand Strategy Matrix; Decision stage was done by Quantitative Strategic Planning Matrix (QSPM). The results showed that there was a change on competitive conditions so that company needed to modify the strategy implemented. EFE matrix result was 3.02 which showed that the company’s strategies were responsive to the external conditions, while IFE matrix result was 2.78 which showed that company had quite strong internal capabilities. Alternative strategies formulation through SWOT, IE, BCG and Grand Strategy matrix analysis and the decision through QSPM analysis result showed that “Improving product variation with new product development or cooperation with other companies” was the most suitable with QSPM score 6.183 followed by “Market development and market penetration through existing products with QSPM score 5.726.Keywords: Business Strategy Analysis, PT Soho Industri Pharmasi, SWOT, QSPM
