Abstract

Stroke is cardiovascular disease that causes the world's highest disability and is the most prevalence disease after heart disease and cancer. Stroke is caused by circulatory disorders with 80% of the sufferers are diagnosed with ischemic stroke and 20% of them are diagnosed with hemorrhagic stroke. Patients who survive from the first stroke have high risk to have recurrent stroke. American Heart Association/American Stroke Association and Perhimpunan Dokter Spesialis Syaraf Indonesia recommend secondary stroke prevention therapy including antiplatelet/anticoagulant, antihypertensive agents, and antidislipidemia to minimalize the risk of recurrent stroke. Secondary stroke prevention therapy is only the first step. Patients need to be adhere to those therapies. The non-adherence will increase the risk of recurrent stroke. The study aimed to determine factors which causing the non-adherence to secondary prevention therapy in patients with ischemic stroke. This was a case control study with concecutive sampling method by interviewing patients who met the inclusion criterias i.e., had been diagnosed and were inpatients due to ischemic stroke, but in the time of interview patients were outpatients, patients were able to communicate and agree to participate in the study.Data was analized by bivariate / chi square test and multivariate logistic regression test. During the study period, 184 respondents met the inclusion criterias. Factors affecting non-adherence in the use of secondary prevention therapy were No one reminded to take medicine p = 0.03; OR 4.51, denial of the disease p = 0,036 OR 214, and tired of taking medicine p = 0,045 OR 1,97.

Abstract

Prevalensi penyakit diabetes mellitus (DM) di Kalimantan Selatan tergolong tinggi, mencapai 1,4% pada tahun 2013.Ketidakpatuhan berobat pasien DM tipe 2 dapat mengakibatkan rendahnya kontrol glukosa darah dan meningkatkan resiko komplikasi. Penelitian ini dilakukan untuk mengevaluasi kepatuhan penggunaan obat dan faktor-faktor yang memengaruhi ketidakpatuhan penggunaan obat pada pasien DM tipe 2. Penelitian ini merupakan penelitian dengan metode deskriptif analitik yang dilaksanakan pada Desember 2014 sampai Maret 2015 dengan total 48 responden DM tipe 2 rawat jalan. Kepatuhan pengobatan dievaluasi dengan menggunakan kombinasi8-items Morisky Medication Adherence (MMAS-8) dan Medication Possession Ratio (MPR). Hubungan antara karakteristik pasien dengan kepatuhan dianalisis dengan analisis chi square dan regresi logistik binary. Hasil penelitian menunjukkan bahwa terdapat hubungan signifikan antara MMAS-8 dan MPR (p value 0,004). Hanya 39,6% pasien yang patuh menggunakan obat dan menebus obat. Analisis chi square menunjukkan terdapat hubungan signifikan antara jenis kelamin (p value 0,011) terhadap kepatuhan. Adjusted Odd Ratio (aOR) menunjukkan bahwa pria 5,7 (aOR 5,7; 95% CI: 1,3-25,7) kali lebih patuh daripada pasien wanita dengan mempertimbangkan usia, lama menderita DM, dan jenis obat DM yang diterima. Alasan terbanyak ketidakpatuhan adalah terlambat menebus obat (86,4%) dan lupa minum obat (77,3%). Dari penelitian ini dapat disimpulkan bahwa jenis kelamin adalah faktor yang secara signifikan memengaruhi kepatuhan berobat pasien DM tipe2.

Abstract

There are some factors that influence human resources management, they are the employee’s attribution, working motivation, and organizational commitment. The purpose of this research is to determine the influence of the employee’s attribution toward human resources management policies to employee’s motivation and organizational commitment in PKU Muhammadiyah Yogyakarta Hospital. This research used questionnaire to 51 employees as subjects and had been working at least 1 year in PKU Muhammadiyah Yogyakarta Hospital. The questionnaires that used in this research consists of questionnaire for employee’s attribution toward human resources management policies, working motivation questionnaire, and organizational commitment questionnaire. To test validity and realibility used 25 respondents, then analyzed with regression analysis to determine the influence of the employee’s attribution toward human resources management policies to working motivation and organizational commitment used SPSS 17 for windows program for analyzing data. The results showed that employee’s attribution toward human resources management policies has positive influence 94,9 % to motivation employees with regression equation Y = 6,425 + 0,967X, employee’s attribution toward human resources management policies has positive influence to organizational commitment 92,6% with regression equation Y=10,133 + 0,735X, and working motivation correlates positively with organizational commitment with r 0,980.Keywords : Employee’s attributions, working motivation, organizational commitment

Abstract

Dalam sistem pelayanan kesehatan di EraJaminan Kesehatan Nasional (JKN), pemerintah menerapkan proses pengadaan obat berdasarkan elektronik katalog (e-katalog) melalui mekanisme e-purchasing dan manual-purchasing. Penerapan sistem akan berpengaruh pada efisiensi pengadaan dan ketersediaan obat yang dapat dinilai dari persepsi farmasi, pelaksana pengadaan dan perawat. Penelitian bertujuan untuk melihat hubungan penerapan e-katalog terhadap efisiensi pengadaan dan ketersediaan obat di RSUD Kelas B Yogyakarta. Penelitian merupakan penelitian deskriptif analitik dengan desain survei cross sectional. Data diambil secara kuantitatif dengan menggunakan kuesioner dengan responden farmasi, pelaksana pengadaan dan perawat. Metode sampling untuk responden perawat sebanyak 120 responden sedangkan responden farmasi dan pelaksana pengadaan dengan populasi sebanyak 21 responden. Data dianalisis menggunakan uji regresi linear untuk melihat hubungan penerapan e-katalog dengan efisiensi pengadaan dan ketersediaan obat.Pengaruh e-purchasing terhadap efisiensi pengadaan pada responden farmasi dan pelaksana pengadaan nilai signifikansi (p) untuk indikator persiapan, pelaksanaan dan manfaat kendala 0,001, 0,024, 0,007 sedangkan manual-purchasing nilai signifikan untuk indikator pelaksanaan 0,014, dengan koefisien korelasi (r) indikator persiapan e-purchasing sebesar 0,690, menunjukkan hubungan antara persiapan e-purchasing dengan efisiensi sangat kuat, sedangkan untuk responden perawat pengaruh penerapan e-katalog baik secara e-purchasing maupun manual-purchasing memiliki hubungan terhadap ketersediaan obat nilai signifikansi (p) 0,000, dan nilai koefisien korelasi (r) 0,765 yang menunjukkan hubungan yang kuat antara penerapan e-purchasing dan manual-purchasing dengan ketersediaan obat. Penerapan e-katalog baik secara e-purchasing dan manual-purchasing meliputi indikator persiapan, pelaksanaan, dan manfaat kendala memiliki hubungan yang signifikan dengan efisiensi pengadaan dan ketersediaan obat di RSUD Kelas B di Yogyakarta.Kata kunci: e-katalog, e-purchasing,manual-purchasing, efisiensi pengadaan, ketersediaan obat

Abstract

Pharmacoeconomic studies, especially the cost-utility analysis method, require a cost-effectiveness threshold for interpretation of the cost-effectiveness ratios of the study. The cost-effectiveness threshold is determined by each country. Therefore, a study to determine the Willingness-To-Pay per Quality Adjusted Life Year (WTP per QALY) as cost effectiveness threshold for pharmacoeconomic studies is indispensable in Indonesia. The purpose of this study is to review the literature and to develop guidelines for estimating WTP per QALY as cost effectiveness threshold in pharmacoeconomic studies. Literature review was conducted using some electronic databases, such as MEDLINE, Pubmed, Science Direct, and Google Scholar. The study was conducted by holding survey on healthy population or patients. Estimated WTP per QALY are differentiated based on therapy for preventive, curative, and life saving or interventions that can prolong life expectancy. The study was conducted in three stages: measuring utility, measuring WTP, and combining WTP and utility values in WTP per QALY. Utility measurements are performed either with single or multi-attribute instruments, whereas WTP measurements use contingent valuation or discrete methods. Survey technique can be done with face to face interview, telephone, and internet. This research can be used as a guide in the development of methods to estimate WTP per QALY as cost effectiveness threshold in pharmacoeconomic studies.

Abstract

The primary component for maintaining the long term sustainability of a company is customer loyalty. Many factorsaffect customer loyalty in the business-to-business relationship. This study aimed to determine the factors that affect the pharmacy loyalty to the pharmaceutical wholesaler.The study was obtained using questionnaire with 1-4 Likert scale. The sampling method was purposive sampling and the sample was 130 respondents. The questionnaire validaty was tested by factor analysis, while the reliability test was conducted usingCronbach’s Alpha. Data were analyzed using SEM-PLS (Structural Equation Modeling-Partial Least Square).The results showed that pharmaceutical wholesaler’s service quality had a positive effect on pharmacy satisfaction (p-value <0.001 and the path coefficient value = 0.566). Pharmacy satisfaction had a positive influence on pharmacy trust (p-value <0.001 andthe path coefficient value = 0.32), pharmacy commitment (p-value = 0.005 and the path coefficient value = 0.191), and pharmacyloyalty (p-value = 0.014 and the path coefficient value = 0.161). Pharmacy trust had a positiveinfluence on commitment pharmacy (p-value is 0.037 and the path coefficient value = 0.13) and a pharmacy loyalty toward the pharmaceutical wholesaler(p-value = 0.001 and the path coefficient value = 0.254). Commitment pharmacies had a positive effect on pharmacy loyalty (p-value= 0.034 and the path coefficient value = 0.134). To sum up, the most powerful factor among the three variables in influencingloyalty was pharmacy trust.Keywords: service quality, satisfaction, trust, commitment, loyalty

Abstract

The implementation of INA-DRGs system for the third-class patients of Jamkesmas (Public Health Insurance Program) in hospital is expectedly able to increase the access and the health service quality for the poor and underprivileged people in effectively and efficiently realizing the optimal health service. This research was aimed to obtain the average cost of medical treatment of cervical cancer and also learn the compatibility between cost of medical treatment of cervical cancer in RSUD Dr. Moewardi and the constancy of INA-DRGs based health financing. This was a descriptive research under the perspective of hospital. Data was collected retrospectively by investigating the medical record document of patients with cervical cancer and the data of the medical treatment cost of patients. The research subject was cervical cancer case with the third-class patients of JAMKESMAS in RSUD Dr. Moewardi in the period of January-July 2010 that was in line with the inclusion criteria. Further, the data analysis was done by cross tab with Chi Square test and by comparing the average cost and ALOS (Average length of stay) through the constancy of INA-DRG. The Chi Square Test shows a correlation between LOS, patient factor (the number of parities), and disease factors (co-morbidity and severity level) with the health care cost for cervical cancer in RSUD. Dr. Moewardi. The health care average cost of cervical cancer based on the tariff of RSUD Dr. Moewardi for the severity level I included: IDR2.893.243 for surgery, IDR3.313.342 for chemotherapy, IDR3.988.027 for radiotherapy and IDR10.039.072 for radical hysterectomy. Meanwhile, in the severity level II, it included IDR5.885.859 for surgery, IDR4.474.084 for chemotherapy. In the severity level III, it includes IDR13.347.209 for radical hysterectomy. The ALOS was longer compared to ALOS of INA-DRG except in the severity level III. This then gives an effect on the health care cost for cervical cancer that in general was higher than the tariff of INA-DRG. In conclusion, implementation of INA-DRG for the treatment of cervical cancer in patients of JAMKESMAS in RSUD Dr. Moewardi was in efficient.Keywords: INA-DRG, cervical cancer, JAMKESMAS

Abstract

Salah satu bentuk pelayanan komprehensif bagi pasien diabetes melitus (DM) tipe 2 pada fasilitas kesehatan tingkat pertama adalah home care. Penelitian bertujuan melihat pengaruh pemberian home care oleh apoteker terhadap tingkat kepatuhan, kadar glukosa darah sewaktu (GDS), dan kualitas hidup pasien DM tipe 2. Penelitian termasuk kuasi eksperimental yang dilakukan di Puskesmas Srandakan, Bantul pada Maret hingga Juni 2015 dengan metode pretest-posttest design with control group. Sampel diperoleh dengan metode purposive sampling. Sebanyak 58 pasien DM tipe 2 dibagi menjadi kelompok kontrol dan perlakuan masing-masing sebanyak 29 pasien. Pemberian home care dilakukan dalam bentuk konseling di rumah pasien. Pengukuran kepatuhan menggunakan Morisky Modified Adherence Scale 8, kadar GDS didapat dari rekam medis pasien, dan kualitas hidup diukur dengan Diabetes Quality of Life Clinical Trial Questionnaire. Data diolah menggunakan uji Wilcoxon dan Mann-Whitney. Hasil menunjukkan bahwa pemberian home careoleh apoteker dapat meningkatkan kepatuhan, kualitas hidup total, dan menurunkan kadar GDS pasien kelompok perlakuan secara signifikan. Namun demikian,kenaikan kepatuhan, kualitas hidup total, dan penurunan kadar GDS pasien tersebut belum berbeda secara bermakna jika dibandingkan dengan kelompok kontrol. Hanya perbaikan domain efek pengobatan dan frekuensi gejala dari kualitas hidup pasien yang berbeda signifikan jika dibandingkan dengan kelompok kontrol.Kata kunci: home care, apoteker, diabetes mellitus, kepatuhan, kualitas hidup

Abstract

Balai POM of Jambi is obliged to act professional and improve the organization’s performance in order to serve the society. Learning and growth perspective is set to get outcome advantage for other third perspective Balanced Scorecard. The purpose of this research was to evaluate learning and growth perspective in Balai POM Jambi, while also determining facilitating and obstructing factors in this perspective. The research was conducted in descriptive method with qualitative and quantitative data gathered from secondary data provided by Balai POM Jambi in 2015 to 2016. Data was gathered using questionnaire and interview. Qualitative and quantitative descriptive analysis were performed by comparing target performance with real achieved performance for each indicators then referring to the standard. Interviews was conducted to determine facilitating and obstructing factors in achieving the best performance. The result of this study in performance learning and growth perspective for human capital result is good; organizational capital results is moderate; informational capital results is moderate. Facilitating factors learning ang growth perspective of Balai POM Jambi were budget plan, competency staff, organizational capital, implementation management information system, implementation QMS, guidance, reward and punishment. Obstructing factors were limitation in human resources and facilities. In general term, performance learning and growth perspective need to be improved, by improving leadership and facilities in Balai POM Jambi.

Abstract

BPJS kesehatan sebagai pelaksana sistem Jaminan Kesehatan Nasional (JKN) dalam memenuhi pelayanan kesehatan pada peserta, melakukan kerjasama dengan Fasilitas Kesehatan Tingkat Pertama (FKTP) seperti klinik pratama dan dokter keluarga serta fasilitas penunjuang seperti Apotek Rujuk Balik (PRB) untuk memberikan pelayanan obat. klinik pratama harus memiliki Apoteker atau melakukan kerjasama untuk melakukan pelayanan kefarmasian (apotek klinik). Dokter keluarga harus memiliki jejaring dengan apotek (apotek jejaring). Pada ketiga jenis Apotek ini terdapat perbedaan mekanisme sistem pembayaran. Penelitian ini bertujuan untuk mengetahui pengaruh karakteristik apoteker, dan jenis tempat praktek apoteker terhadap persepsi apoteker, serta untuk mengetahui apakah terdapat perbedaan persepsi apoteker pada apotek PRB, apotek klinik pratama dan Apotek jejaring tentang pelaksanaan sistem pembayaran (kapitasi atau pembayaran langsung) di era JKN. Penelitian ini dilakukan di Daerah Istimewa Yogyakarta dengan desain observasi cross sectional, dengan menggunakan keseluruhan populasi apoteker yang bersedia mengisi kuisioner, yaitu sebanyak 48 responden dari apotek program rujuk balik, apotek klinik pratama dan apotek jejaring. Persepsi tentang sistem pembayaran di bagi menjadi 4 indikator yaitu indikator profit, klaim biaya, perjanjian kerjasama, dan pelayanan. Hasil penelitian menunjukkan bahwa jenis tempat praktek berpengaruh signifikan pada persepsi indikator profit dengan nilai sig 0,025 dan pada indikator pelayanan (sig 0,007). Tingkat pendidikan berpengaruh signifikan terhadap persepsi indikator klaim biaya (nilai sig 0,038). Usia dan lama praktek berpengaruh signifikan terhadap persepsi indikator perjanjian kerjasama (sig 0,042 dan 0,043). Perbedaan persepsi apoteker untuk indikator profit terjadi antara apoteker PRB, klinik pratama dan jejaring (p=0,003). Perbedaan persepsi apoteker untuk indikator pelayanan menunjukkan adanya perbedaan persepsi apoteker PRB, klinik pratama dan jejaring (p=0,001). Untuk Indikator klaim biaya dan perjanjian kerjasama tidak terdapat perbedaan persepsi antara apoteker di Apotek PRB dan Apotek jejaring.
