Abstract

Pada era JKN apotek dapat berperan sebagai apotek program rujuk balik (PRB) atau apotek jejaring dan apotek klinik pratama (KP). Biaya obat apotek PRB diklaim langsung ke BPJS, sedangkan biaya obat apotek lainnya diklaim ke fasilitas kesehatan (faskes) primer berdasarkan perjanjian kerjasama. Perbedaan pola kerjasama apotek di era JKN berdampak pada biaya obat pasien, sehingga penelitian ini bertujuan untuk mengetahui rata-rata biaya obat, persentase biaya obat terhadap kapitasi, dan perbedaan biaya obat di apotek era JKN. Penelitian ini adalah penelitian observasi yang bersifat analitik cross sectional. Penelitian ini dilakukan pada 6 apotek PRB, 6 apotek jejaring, dan 7 apotek KP di DIY pada bulan Oktober 2015. Subjek penelitian adalah resep pasien JKN bulan Maret 2015 dengan 8.430 lembar resep. Data dianalisis secara statistik deskriptif dan uji komparatif (uji Kruskal-Wallis). Hasil penelitian menunjukkan, rata-rata biaya obat pasien satu kali terapi di apotek jejaring; apotek KP; dan apotek PRB bernilai Rp12.589±Rp8.874,49; Rp14.173 ±Rp6.424,09; Rp143.807 ± Rp162.251,30. Rata-rata persentase biaya obat terhadap biaya kapitasi faskes primer bernilai 13,58% untuk apotek jejaring dan bernilai 15,91% untuk apotek KP. Terdapat perbedaan biaya obat yang signifikan di ketiga jenis apotek tersebut (p=0,000) yang dipengaruhi oleh lama waktu pemberian obat, jumlah lembar resep, margin keuntungan, dan jasa kefarmasian. Biaya obat di era JKN tergantung pada pola kerjasama apotek dengan faskes dan BPJS. Rata-rata persentase biaya obat terhadap kapitasi faskes di apotek jejaring lebih rendah dibandingkan dengan apotek klinik pratama.Kata kunci: analisis biaya obat, jaminan kesehatan nasional (JKN), apotek

Abstract

The marketing concept nowadays is more focused on creating long-term relationships with consumers who could give a positive impact on increasing market share and profitability. One of them can be achieved by establishing strong branding that influence customer loyalty. This study aimed to determine the effect of brand personality and trust on Kiranti’s brand loyalty. This research was an observational analytic study using cross sectional survey method. Data were obtained by questionnaire tools using 1-4 Likert scale. Sampling was conducted using non-probability purposive sampling. The samples were 100 people. The data were analyzed by simple and multiple linear regression analysis with a 95% of confidence level. The result showed that brand excitement and brand emotionality had positive and significant effect on brand trust, whereas the brand sophistication and brand activity had positive effect but notsignificant on brand trust. Brand excitement, sophistication brand, brand activity, and brand emotionality (or called brand personality)simultaneously had a positive and significant effect on brand trust with strong relationship. Brand trust had a positive and significant effect on Kiranti’s brand loyalty.Keywords: brand personality, brand trust, brand loyalty, Kiranti

Abstract

Drug management in Hospital which consist of stages are selection, procurement, distribution and use. Result of preliminary observation done in Pharmacy departement of Tarakan Hospital found some inefficiency problem in distribution steps such as expired and damage control, conformity of record to physical checks on drug management, etc. Therefore, it necessary to evaluation the efficiency rate of inpatient ward drug management distribution. Research design was descriptive, retrospective and concurrent with the evaluation of documents relating to inpatient drug distribution process. The study used a sample of drugs including Class A medicines criteria. Data collected in the form of qualitative and quantitative data from observations of documents and interviews with relevant officers. Data was presented in tabular and textual descriptions. Data evaluation was descriptive using SPSS. Measurement of the efficiency of drug distribution was done by using indicators of the efficiency of Depkes and WHO, then compared with that indicator to determine the existence of inefficiency. The results showed, that according to several indicators inpatient ward drug distribution was not been efficient, inefficiency drug management indicators at this stage of the distribution occurs on a match between the physical card stock while (93.27%).Damaged and / or expired drugs in 2008 was 0.23% in 2009 was 0.48%. Prescriptions are not served in 2008 was 1.52%, 2.28% in 2009. TOR in 2008 was 6 times and in 2008 was 6.9 times. Abstract

Bupivacaine is used as a local anesthetic with Epidural Lumbal Anasthesia (ELA) for Caesarean Section (CS) in Indonesia. During pregnancy, there are changes in pharmacokinetic parameter values, as a result of changes in the physiology of the body. This study was designed to determine the pharmacokinetic parameters of isobaric bupivacaine 0.5% with non-compartment model and to monitor its effectiveness. The study was conducted on 7 normotensive pregnant patients who treated with 0.5% bupivacaine doses 75 mg at Integrated Central Surgery Building in Dr. Sardjito Hospital. Blood samples (3 mL) from each patients were drawn at the following intervals: 0, 15, 20, 25, 30, 45, 60, 75 and 90 minutes, then the plasma bupivacaine consentration were measured using HPLC and pharmacokinetic parameters were calculated with non-compartment models, i.e. AUC(0-~), AUMC(0-~) and the MRT. Monitoring the effectiveness was conducted by Pinprick test. Relationship of AUC(0-~) with maximum sensory block was analyzed by Pearson correlation. The results of pharmacokinetic parameters of isobaric bupivacaine 0.5% for 7 normotensive pregnant patients with non-compartment model were AUC(0-~) 101,25 ± 15,07 μg.mL-1.min; AUMC(0-~) 6.431,92 ± 2.212,96 μg.mL-1.min2 and MRT 63,74 ± 20,52 minutes. Bupivacaine 0,5% doses 75 mg as an anesthetic for CS with ELA in 7 normotensive pregnant patients was effective for pain blockade, with maximum sensory block was T6. Abstract

In the last five years, the pressure to implement pharmaceutical care focuses on the patient getting stronger. The current standard of pharmacy care at a pharmacy established by regulation of health minister number 73/2016, as a guideline for pharmacists and pharmacy technicians in organizing pharmaceutical care. This research was conducted in the city of Jambi in order to determine implementation of pharmaceutical care standard at pharmacy, to identify the supporting factors and inhibiting factors in implementing pharmaceutical care in the Jambi city’s pharmacy. This study is an observational descriptive study, conducted with a survey phase, followed by a phase of observation and in-depth interviews phase. Total respondents who conducted a survey of 105 pharmacist from 143 pharmacies. Then performed an observation using a checklist sheet at 20 pharmacies and in-depth interviews on 17 pharmacists. The questionnaire survey used convenience sampling technique and sampling techniques for observation and in-depth interviews with maximum variation sampling. Methods of data analysis was done descriptively to see the implementation of pharmaceutical care standard. Qualitative data were presented descriptively to identify supporting and barriers factors. Results of the research are based on a survey, in managing the pharmaceutical, medical supplies and consumable medical materials carried out by pharmacist assisted pharmacy technicians, but its implementation in the field is mostly done by the pharmacy technicians under the responsibility of the pharmacist. Clinical pharmacy services running on prescription services, drug information services and partly on the counseling. Home pharmacy care, monitoring drug therapy and monitoring of drug side effects and clinical documentation has not been done. Based on interviews unknown, factors supporting the implementation of the standard is support pharmacy technicians personnel, support of owner pharmacy, the presence of a pharmacist with a regular schedule of practices, the use of information systems technology and motivation of pharmacist. Whereas the barriers factor are factors in patients where there is doubt the patient to the pharmacist and staff, the limited presence of the pharmacist, the shortage of skills,and there is no space of counseling services and the limited number of pharmaceutical human resources.

Abstract

Pelaksanaan JKN baru dimulai di awal tahun 2014 dan masih memerlukan perbaikan sistem yang terus menerus agar penerapan standar pelayanannya dapat terlaksana dengan baik. Oleh karena itu, untuk mengetahui keberhasilan sistem tersebut perlu dilakukan survei mengenai ekspektasi pasien dibandingkan dengan pelayanan yang pasien rasakan di rumah sakit. Penelitian bertujuan untuk menganalisis perbedaan antara ekspektasi dan persepsi pasien rawat jalan terhadap mutu pelayanan kefarmasian di RSUP Dr. Sardjito Yogyakarta dan Rumah Sakit Panti Rapih pada era JKN serta mengetahui perbedaan skor gap dimensi mutu pelayanan kefarmasian. Subjek penelitian adalah pasien JKN yang berada di apotek rawat jalan di RSUP Dr. Sardjito Yogyakarta dan di rumah sakit Panti Rapih Yogyakarta pada Bulan Oktober sampai Desember 2014 masing-masing 100 responden. Metode penelitian adalah cross sectional dengan teknik consecutive sampling. Penelitian menggunakan kuesioner yang diadopsi dari konsep penelitian kualitas pelayanan Parasuraman dkk (1985) dengan metode SERVQUAL. Perbedaan ekspektasi dan persepsi pasien dianalisis dengan uji Wilcoxon, sedangkan untuk melihat perbedaan kepuasan pasien di kedua rumah sakit digunakan uji Mann Whitney. Hasil penelitian menunjukkan terdapat perbedaan bermakna antara ekspektasi dan persepsi pasien baik di RSUP Dr. Sardjito maupun di Rumah Sakit Panti Rapih Yogyakarta (p=0,000). Selain itu hasil penelitian menunjukkan bahwa tingkat kepuasan responden di RSUP Dr. Sardjito dan di Rumah Sakit Panti Rapih tidak berbeda secara signifikan (p=0,057), namun terdapat satu dimensi yang dinyatakan berbeda signifikan secara statistik, yaitu pada dimensi empati (p=0,037).

Abstract

Obat merupakan bagian penting dalam pelayanan kesehatan, sehingga pemerintah berkewajiban menjamin ketersediaan, pemerataan dan keterjangkauan obat.Metode yang dilakukan Pemerintah adalah dengan menerapkan e-procurement obat dengan prosedur e-purchasing berdasarkan e-catalogue. Kebijakan ini diharapkan mampu mewujudkan pengadaan obat yang efisien. Tujuan penelitian untuk mengetahui bagaimana variabel manajemen dan kontrol data, kualitas hasil dan produksi, dan hubungan dengan mitra kerja berpengaruh pada kinerja e-procurement obat dengan prosedur e-purchasing berdasarkan e-catalogue, dan kinerja eprocurement obat berpengaruh pada efisiensi pengadaan obat. Penelitian menggunakan metode penelitian deskriptif analitik melalui survei cross sectional. Variabel penelitian terdiri dari manajemen dan kontrol data, kualitas hasil dan produksi, hubungan dengan mitra kerja, kinerja e-procurement obat dengan prosedur e-purchasing, dan efisiensi pengadaan obat. Data diambil dengan metode survei menggunakan kuesioner dengan skala Likert. Sampel penelitian yaitu apoteker yang terlibat dalam implementasi eprocurement obat di dinas kesehatan kabupaten/ kota di propinsi Jawa Tengah. Penelitian dilakukan bulan Januari sampai Februari 2015. Data dianalisis menggunakan analisis regresi linear. Hasil penelitian menunjukan variabel manajemen dan kontrol data, kualitas hasil dan produksi, dan hubungan dengan mitra kerja baik secara parsial maupun simultan berpengaruh terhadap kinerja eprocurement obat dengan prosedur e-purchasing berdasarkan e-catalogue dengan nilai signifikansi 0,000. Kinerja e-procurement obat dengan prosedur e-purchasing berdasarkan e-catalogue berpengaruh terhadap efisiensi pengadaan obat dengan nilai signifikasi 0,001.Kata kunci: e-procurement, e-purchasing, e-catalogue, kinerja, efisiensi pengadaan obat

Abstract

Increase of public interest in using herbal medicine causes increase in the number of herbal medicine producers and the competition will be harder. Marketing concept states that companies have to satisfy their consumers in order to gain profits. The satisfied consumers will be loyal to certain products or companies, and they will buy the products again in the future. The more important things is that they will carry out word of mouth activity about the products to people around them. The aims of this research were to know the influence of consumers’ satisfaction on their loyalty, the influence of their satisfaction on their word of mouth behavior, and the influence of their loyalty on their word of mouth behavior. The subjects of this research were the consumers of An Nuur’s herbal medicine in Yogyakarta. This research was conducted by survey with questionnaire as an instrument. The samples were taken with non probability sampling design with purposive sampling procedure. The sample consisted of 65 respondents and was taken from November 2011 to May 2012. The criteria of the respondents were they who had used An Nuur’s herbal medicine at least three times, bought it from stores in Yogyakarta, aged at least 17, and agreed to be subjects of this research. Then, the data were analyzed using linear regression analysis to know the relation between variables. The hypothesis was tested using t test. The result of this research showed that consumers’ satisfaction had significant positive influence on their loyalty. The highest consumers perception was the easiness in consuming An Nuur’s herbal medicine, while the lowest consumers perception was the easiness in purchasing An Nuur’s herbal medicine. Consumers’ satisfaction had significant positive influence on their word of mouth behavior. Consumers’ loyalty had significant positive influence on their word of mouth behavior. Consumers’ loyalty had greater influence than their satisfaction on their word of mouth behavior.Keywords: Satisfaction, loyalty, word of mouth, An Nuur.

Abstract

Local Drug anf Food Agency in Manado (Balai Besar POM) is a Strategic Public Service Unit of National Drug and Food Agency (Badan POM) which expected capable to work effectively and efficiently to detects, prevents, and controls products, and also to protects consumer’s safety, security and health. Therefore, its necessary to know the current performance of quality control sector staff in Balai Besar POM Manado in order to increasing it. To detect staff performance of quality control sector in Balai Besar POM Manado which suitable to its importance level, and to detect the rank of performance variable which has biggest value on performance forming at Balai Besar POM Manado, its needs an approach using Importance‐Performance analysis of cartesius diagram to find out which organizational elements that most need improvement. This research is carried out on 34 quality control sector staff in Balai Besar POM Manado using descriptive research design and data collecting is obtained from primary and secondary data. Primary data is questionnares which distributed to quality control sector staff in Balai Besar POM Manado. Secondary data is comes from annual report of Balai Besar POM Manado. The results of this research based on Importance‐Performance analysis of cartesius diagram shows that ability to complete job and productivity variables has to get top priority from Balai Besar POM Manado to increase both variables, while ability to make decision and cooperation relationship with other people variables are also needs to increase, but these variables aren’t top priority. The order of performance variable rank based on its importance level is knowledge and understanding job, working attitude, productivity, ability to complete job, cooperation relationship with other people, and ability to make decision variables. Key Words : Balai Besar POM Manado, Performance, Importance‐Performance Analysis

Abstract

Asma merupakan penyakit kronis yang tidak dapat disembuhkan. Penanganan asma terutama ditujukan untuk mengurangi frekuensi kekambuhan. Berkurangnya frekuensi kekambuhan dapat meningkatkan kualitas hidup penderitanya. Penelitian ini bertujuan untuk mengetahui pengaruh karakteristik penyakit dan jenis terapi terhadap kualitas hidup asma. Penelitian bersifat non eksperimental dengan rancangan analitik dan cross- sectional. Data diambil secara retrospektif dan concurrent. Subyek penelitian adalah pasien asma rawat jalan berusia ≥ 18 tahun yang menerima terapi asma berupa sediaan inhalasi minimal 3 bulan di RSUD Panembahan Senopati Bantul Yogyakarta. Data diperoleh dari Asthma Quality of Life Questionnaire yang diikuti dengan wawancara dan penelusuran data pada rekam medis pasien. Pengaruh karakteristik penyakit dan jenis terapi diuji menggunakan analisis regresi berganda dan sederhana.Hasil penelitian menunjukkan dari 54 pasien, 61,11% adalah wanita. Berdasarkan usia, pasien usia 53 – 59 tahun paling banyak menderita asma (18,52%). Faktor terbesar yang berperan sebagai pencetus asma adalah lingkungan (48,15%). Obat yang dipakai adalah fenoterol HBr, salbutamol, teofilin, aminofilin, mebhidrolin napadisilat, metilprednisolon, OBH sirup, dan ambroxol HCl. Karakteristik penyakit yang mempunyai pengaruh terhadap kualitas hidup pasien asma di RSUD Panembahan Senopati Bantul Yogyakarta adalah tingkat keparahan asma (p=0,000) dan pemicu asma (p=0,045). Semakin berat keparahan asma, kualitas hidup pasien semakin menurun. Jenis terapi tidak berpengaruh terhadap kualitas hidup pasien. Kata kunci: asma, kualitas hidup, jenis terapi, RSUD Panembahan Senopati BantulYogyakarta
