Abstract

Background: The influx of advanced practice nurses into the healthcare industry has established the need for a postgraduate program for APRNs. Numerous studies have shown the efficacy of an onboarding process; however, there were limitations to the reviews.Purpose: This study aimed to determine formal orientation and orientation programs, compared to current APRN orientation and orientation can improve the competence and quality of APRN care for eight weeks. Methods: The participants were six currently practicing APRNs with eleven months to five years practicing as independent providers. The response rate in the pre-assessment phase was 100% and 66.7% in the post-assessment phase. Two APRNs came onboard after the start of the project, and one APRN opted out. There were no preceptors available for the completion of the CoEPCE competency tool. The APRN onboarding and orientation program utilized a competency tool with pre- and post-assessments. Emails with attached competency tool sent to APRNs for completion of pre- and post-assessments. The APRNs completed the pre- and post-competency assessments on the first day of implementation and the last day of week eight. The collection of data from the pre- and post-competency tool commenced in week nine and imported into Microsoft Excel for analysis. The data collected from the CoEPCE tool, as highlighted in the implementation phase, has significant changesResults: The outcomes revealed that the changes were not significantly statistical at the p-level of 0.05 in the seven domains because none of the p-values were less than 0.05. However, the results revealed that deficits in statistical significance should not deem that the onboarding process was not successful. Conclusion: The results demonstrated evidence of the importance of an APRN postgraduate program in enhancing competencies. However, larger sample sizes are substantial to show statistically changes for reliability and validity

Abstract

Background: Post-operative care is management of patients after any surgery. The main goal of post-operative care is to prevent complications i.e. atelectasis and infection. The other objective is early healing of the surgical incision and return of patient to a state of health. About 17% to 88% of people in postoperative duration will have decreased lung volumes. This decreased lung volume problem can be solved with use of incentive spirometry in postoperative period on Day 1. Incentive spirometer is perioperative respiratory therapy given to postoperative patients to improve lung volume of patients and hasten recovery of patients also.Purpose: This study aimed to identify effect of Incentive spirometry on postoperative patient’s recovery. Methods: It was pre-experimental study with pretest- posttest design only, which was done on post-operative patients at All India Institute of Medical Sciences (AIIMS), Rishikesh. Results: Majority (52 %) of participants were 41 to 50 years age group. Male and female ratio for participants were 46:54. Paired ‘t’ test p value i.e. 0.00* with CI [0.72, 1.03] showed that on Pretest and Day 3 Performance level on incentive spirometry of participants showed a significant value, which indicate that spirometer enhance recovery of post-operative patients by increasing their lung volumes. Conclusions: This study revealed that incentive spirometry is effective in improving of pulmonary function among post-operative patients, which further improves blood circulation and hasten early recovery of surgical wound. This spirometry should be integral part of Post-operative care. All nurses who are involved in Postoperative units should encourage patients to do it on regular basis and document it as a vital sign. Good compliance to incentive spirometry can improve better outcome of patient’s

Abstract

Background: Fever is a clinical manifestation that is often experienced by sick children. Fever can be dangerous if there is a high fever. Seizures can occur as a result of high fever which is not treated early, causing hypoxia of brain tissue and ultimately damage. According to WHO estimates that around 17 million cases of fever worldwide, there are 600,000 deaths each year.Purpose: This study aimed to find out effective interventions to reduce body temperature in children. Methods: This research used quantitative descriptive design with systematic review approach. The research instrument uses Duffy’s Research Appraisal Checklist Approach. Results: After searching the article, there were 9 articles related to giving warm compresses, 6 articles with the location of administration in the axilla and tepid songe areas (wiping the whole body), 7 articles by giving compresses for 30 minutes and 10 articles with compressed water temperature 37oC.Conclusions: Of the 15 articles that have been traced successfully, the most interventions given were warm compresses in the axilla and tepid sponge areas by giving compresses for 30 minutes and compressed water temperature of 37oC

Abstract

Background: Pulmonary TB is an infectious disease caused by mycobacterium tuborkulosis and is contagious. The family of pulmonary TB patients plays a very important role in the transmission of pulmonary TB disease. Preliminary data collection shows that four out of five (80%) families of pulmonary TB sufferers so far have gained knowledge about the transmission of pulmonary TB, but in their daily use personal protective equipment is rarely used. The family said they rarely wear masks because they feel fine without the masks, the family also feels wearing masks will hurt the feelings of people with pulmonary TB.Purpose: This study aimed to determine the effect of Rational Emotive Behavior Therapy on preventive measures for transmission of patients with pulmonary TB.Methods: This study used a quasy experiment research design with a non-randomized control group design pretest-posttest approach. The sample in this study was the family of pulmonary TB patients at UPTD Puskesmas Puhjarak Kediri who met the inclusion and exclusion criteria, with a sampling technique using purposive sampling. The independent variable of this study was Rational Emotive Behavioral Therapy and the dependent variable was family preventive measures. The instrument used to measure preventive measures for pulmonary TB transmission is a questionnaire which contains 12 questions, and for the independent variable of memory therapy is SAK research. The research data were processed using the Wilcoxon signed ranks test and Mann-Whitney. Alternative hypotheses are accepted if p <0.05.Results: The results showed that the Mann Whitney test results in the treatment and control groups at the time of the pretest showed a value of p = 0.690 and at the posttest was p = 0.000. Wilcoxon signed rank test results show in the treatment group after treatment p = 0.005, while in the control group when posttest was p = 0.317.Conclusion: The results of the assessment of preventive measures for pulmonary TB transmission between the treatment group and the control group after the implementation of Rational Emotive Behavioral Therapy showed that there were differences in the value of preventive measures for pulmonary TB transmission between the treatment group and the control group after receiving Rational Emotive Behavioral Therapy
