Indonesian Journal of Cardiology (IJC) is a peer-reviewed and open-access journal established by Indonesian Heart Association (IHA)/Perhimpunan Dokter Spesialis Kardiovaskular Indonesia (PERKI) [www.inaheart.org] on the year 1979. This journal is published to meet the needs of physicians and other health professionals for scientific articles in the cardiovascular field. All articles (research, case report, review article, and others) should be original and has never been published in any magazine/journal. Prior to publication, every manuscript will be subjected to double-blind review by peer-reviewers. We consider articles on all aspects of the cardiovascular system including clinical, translational, epidemiological, and basic studies. Abstract

Background: Atherosclerosis is widely viewed as an inflammatory disease with hypercholesterolemia being a dominant underlying risk factor. This study aimed to determine the effect of mangosteen pericarp in inhibition of NF-?B activation and ICAM-1 expression in rat fed with high cholesterol.Methods and Results: Various doses of crude extract mangosteen pericarp were administered to the high fat diet wistar rats and the activity of NF-?B measured by immunohistochemistry to assess nuclear NF-?B expression and the ICAM-1 expression. The high fat diet resulted significant increased serum LDL levels. Increased nuclear NF- ?B activation and ICAM-1 expression were also observed in high fat diet rats in concurrence with increased serum LDL. The inhibitory effect on NF- ?B activity and ICAM-1 expression was observed when 400 mg of mangosteen pericarp crude extract was administered and even showed a higher inhibitory effect in 800 mg of mangosteen pericap treated rats. The 800 mg extract treatment resulted in decreased ICAM-1 expression similar to those of non high fat rats.Conclusion: The administration of 800 mg mangosteen pericarp crude extract significantly inhibited NF-?B activation and reversed the expression of ICAM -1 to the normal level in high cholesterol diet rats.

Abstract

Oxidation of LDL by oxygen free radicals results in unregulated uptake of modified LDL by macrophages in arterial wall. This is considered a key initiating factor for development and progression of atherosclerotic process. It is then logical to hypothesize that anti-oxidants, which can directly scavenge free radicals, can be useful in preventing atherosclerotic process. Initial support for this hypothesis accumulated rapidly from numerous in-vitro studies.

Abstract

Hypertrophic Obstructive Cardiomyopathy (HOCM) prevalence is about 1 among 500 population. It is caused by genetic malformation. Dyspnoe, angina pectoris and syncope are clinical manisfestation. Pharmacological and non-pharmacological treatments can be performed to relieve the obstructive symptoms. Recently alcohol septal ablation has been proven improve haemodynamic and functional status.We report a case of HOCM patient with refractory lung oedema and intubated due to respiratory failure that was immediatelly improved afterseptal ablation.

Abstract

Coronary angioplasty is a definitive, percutaneous intervention to improve myocardial oxygen supply. The benefit of coronary angioplasty for stable angina pectoris is still a controversy. The COURAGE trial had shown that angioplasty for stable angina pectoris gave no additional benefit compared with optimal medical therapy, while the recently done FAME II trial showed the opposite. FAME II trial proved that angioplasty could reduce MACEs in stable angina pectoris patients. The conflicting results from COURAGE and FAME II trials is due to the difference in decision making methodology used for angioplasty. COURAGE used plain angiogram while FAME II utilized Fractional Flow Reserve (FFR) as a tool to decide whether a lesion should undergo angioplasty. The result of FAME II demonstrate FFR is far more reliable to determine coronary lesion which cause ischemia, hence a better decision making tool for angioplasty. The specificity of FFR is proven high, while angiography has an excessive false positives. The use of FFR before angioplasty is still low although there is a considerable evidence that FFR is a better decion making tool for angioplasty compared to angiography. Health economic analysis displayed the use of FFR for stable angina pectoris is cost effective in the long term.

Abstract

Balloon Mitral Valvulotomy (BMV), sometimes also referred to as Percutaneous Transcatheter Mitral Commissurotomy (PTMC) is becoming a common treatment option for patients with mitral stenosis. Compared to surgical valvotomy, it has less procedural risk, shorter hospital stay and comparable outcome. However, that good result can only be achieved in selected good candidates. This article will give a highlight of some consideration to choose a good candidate for BMV mostly evaluated by echocardiography.

Abstract

EKG merupakan pemeriksaan penunjang yang sangat penting untuk evaluasi pasien pemakai alat pacu jantung menetap. Melalui EKG fungsi pemacuan dan fungsi sensing alat pacu jantung menetap dapat ditentukan dengan cukup akurat. Dengan mengantongi informasi jenis alat pacu jantung menetap yang digunakan, EKG bagi seorang kardiolog akan memberikan informasi adanya fenomena undersensing, oversensing, loss of capture, loss of output, dan sebagainya.

Abstract

Walaupun sering terabaikan, fungsi jantung kanan dan sirkulasi pulmoner mempunyai peranan penting dalam keseluruhan sistem sirkulasi. Tidak jarang kasus-kasus kritikal disebabkan karena kegagalan sirkulasi jantung kanan. Pada seri III forum ekokardiografi ini akan dibahas penilaian hemodinamik jantung kanan dengan ekokardiografi untuk kasus-kasus kritikal sehingga akan diutamakan cara pemeriksaan yang praktis, sederhana, dan cepat.

Abstract

Prevalence of Left Main Coroner Artery (LMCA) stenosis inpatients undergoing coronary angiography was 2.5 to 10 %, almosrt all patients suffer from concomitantatherosclerotic disease of other coronary branches. Incontrast, an isolated atherosclerotic lesion of LMCA is very rare. with iscidences 0.07 to 0.15 %. Coroner artery by pass graft surgery (CABG) has been recommended as the standard treatment in LMCA disease, however, percutaneous coronary interventions (PCI) on the LMCA remained in scope, as some patients with high risk or contra indications of CABG and very limited life expectancy, still had no other option than PCI.A 58th years old man with factor; smoker, dyslipidemia and hypertention, complain of chest discomfort, he was referred with diagnosis of APS CCS III and MSCT coroner revealed mild plaque burden with critical subtotal occlusion i n in LMCA, calcified plaque in LAD and other vessels were normal. He refuse CABG and went for PCI, angiography revealed significant isolated unprotected LMCA disease. Successful PCI using anchor wire technique and implantation of BMS in the lesion was done. Patient discharged on day 6 of hospitalization with no complication.

Abstract

We are reporting a case of lead extraction from a woman who has been implanted permanent peacemaker (PPM), and later has a complication of a local site infection and infective endocarditis. She had multiple local debridement procedures, generator removal, and multiple cable amputation. Her PPM wire was free-floating inside the right ventricle (RV). Lead extraction was done successfully using a snare catheter and a long sheath.

Abstract

Early repolarization described as J-point elevation with concaved ST segment, notch or slurred at terminal QRS complex on surface electrokardiography. This electrokardiography pattern that usually exists in young male population, for about 60 years has been concluded as benign normal variant electrokardiography. Klatsky in 2002, has strengthed this issues with his study with 1000 elecrokardiographies with early repolarization which were followed for 12 years. There was no significant case for hospitality or malignant ventricular arrhythmias among population with early repolarization comparared with normal elektrokardiography without early repolarization.But, recent studies with larger samples, show different thing, which is there was a correlation between early repolarization and malignat ventricular arrhythmias. So the issue is, whether we can still consider this early repolarization as a normal variant or not. And how about the management of asymptomatic population with early repolarization such as syncope or evidence of malignant ventricular arrhythmias.T-wave Alternans is a difference at amplitude or morphology of ST segment and T wave in every other heart beat. T-wave Alternans measurement is a non-invasive method to value cardiac cells hererogeneity. This examination has been proven to be useful as a risk stratification for malignant ventricular arrhythmias events in large studies. Higher T-wave Alternans value accompanied with higher risk for developing these arrythymias. T-wave Alternans measurement has been approved in the guideline for management of wide QRS complex tachyarrythmias for detection ventricular arrhythmias with I and IIa class of recommedantion. The more practical Modified Moving Average technicque which can be performed in healthy subjects with early repolarization is useful for risk stratification. Large studies have approved the use of Modified Moving Average for measure T-wave Alternans in these arrhythmias risk stratification.
