Abstract

Background: Worldwide healthcare workforce represents 12%, in Uganda, out of the 39000 healthcare workers (HCW), 500 have been deployed at Mbale regional referral hospital (MRRH). The deficient quality service delivery is attributed to adverse occupational health risk that are not addressed and acknowledged posing a great risk to patient safety. Study aims to investigate the unsafe acts and unsafe working conditions of health care workers at (MRRH).Methods: A quantitative observational study with a cross-section design was used using and a semi structured observation checklist for data collection among 120 health care workers of MRRH.Result: 45(37.5%) of the HCW never wore the necessary PPEs versus 75(62.5%) who wore, almost half 55(45.3%) worked over time and 65(54.6%) did not, as well the same figures represent those who over worked and those who did not over work respectively. 70% of the working places had defective working conditions for example no lighting, slippery floors and no danger signs. 75% of the wards had improper waste segregation measures. Conclusion : HCW and institutions should intervene to mitigate the hazards, strengthening occupational health, patient safety, hazard prevention and control

Abstract

Background: Several regions in Indonesia are classified as leptospirosis endemic including Klaten Regency, which ranked 2nd highest case of leptospirosis in Central Java in 2018. Leptospirosis in human was transmitted by animals infected by the Leptospira bacteria, which rat was the main reservoir. This study aimed to identify rat reservoir to the existence of Leptospira bacteria in endemic areas. Methods: This study was an observational survey  with cross sectional approach. The sample was rats caught by traps installed for 6 days in Dukuh Kalitengah, Kalitengah Village, Wedi Subdistrict, Klaten Regency in July 2019. The rats were identified and dissected to take their kidneys for laboratory testing using the Polymerase Chain Reaction (PCR) test to identity the existence of Leptospira bacteria.Result: The result showed that there were 17 rats and 6 Suncus murinus caught in 6 days and their species were Mus musculus (23.5%), Rattus tanezumi (41.2%), Rattus norvegicus (35.3%). The results of PCR test found 2 positive samples of Leptospira bacteria in Rattus tanezumi. Rattus tanezumi's habitat was close with humans in settlement areas where it can be a source of Leptospira infection and can spread to humans and other environments. Conclusion : The people in Dukuh Kalitengah are recommended to maintain the hygiene of their environment and improve their house’s sanitation so it not become nest and breeding grounds and also reduce the existence of rats.

Abstract

Background: Ultrasonography (USG) radiological examination can be an early detection of breast tumors before they develop into malignancy. The aim of our study was to determine the accuracy of USG examination in diagnosing breast cancer at Prima Medika Hospital. Methods: We conducted a cross-sectional study from January to December 2021 at Prima Medika Hospital Denpasar in patients who had symptoms of breast cancer had breast USG examinations and biopsy examinations of 51 samples. We collected data by tracing medical records and in descriptive analysis and cross-tabulation analysis to determine the sensitivity, specificity, positive predictive values and negative predictive values. Results: 72.5% of 51 patients were over 36 years old, 47.1% symptoms of a lump in their left breast. USG examination in detecting breast cancer has a sensitivity of 90.9%, a specificity of 93.1% then a positive predictive value of 92.9% and a negative predictive value of 90.1%.Conclusion: USG examination has a high accuracy in diagnosing breast cancer. Radiology specialists are expected to use USG examination as an early detection tool before conducting a histopathological gold standard examination.

Abstract

Background: Slaughterhouse workers have a risk of contracting leptospirosis because of daily contact with animal blood and urine. The purpose of the study was to describe the incidence of leptospirosis and measure the level of knowledge of workers and examine the relationship between demographic characteristics, the knowledge and the prevention behavior of leptospirosis. Methods: All technical staff of Slaughterhouse at Salatiga (30 people) were selected as participants. Data was collected in July 2016 through structured interviews. Microscopic Agglutination Test (MAT) was used to test the workers serum for leptospirosis. Data were analyzed using the Fisher-exact test.  Result: According to the findings, most respondents were male, between the ages of 26 and 50, and had low education background. Most respondents are unaware that leptospirosis is a serious disease and 73.3 percent of respondents had never heard of leptospirosis. The majority of participants reported negative MAT values. The majority of Slaughterhouse workers have never heard of leptospirosis and have little awareness of leptospirosis preventive methods, so they may be at risk of having the infection. Conclusion: There is no leptospirosis incidence in Slaughterhouse Salatiga, however, one worker had a history of leptospirosis exposure. There was no significant relationship between age, gender, and education level with knowledge and behavior, but the knowledge was significantly correlated with leptospirosis prevention behavior. Slaughterhouse officers are advised to receive health education about leptospirosis prevention to increase awareness of the need for self-protection in the workplace.

Abstract

Background: Tuberculosis (TB) is the leading cause of death among people living with HIV/AIDS worldwide. Despite the availability of highly effective treatment for decades, Tuberculosis (TB) remains a major health problem in Nigeria due to the increasing association between HIV and TB observed over the past three decades when HIV was discovered. However, the proportion of TB and or TB/HIV co-infected patients who have successful TB treatment outcome is not well known. This study determined the treatment outcome of TB/HIV co-infected patients with HIV negative patients at Federal Medical Centre Makurdi, Benue State, Nigeria. Methods: A retrospective study that used secondary data from Directly Observed Treatment Short (DOTS) course center (APIN Unit). 268 cases were considered and the period under review covers from January, 2019 to December, 2021. Results: Out of the total 268 TB cases reviewed, the HIV prevalence rate was 28.7 %. More than two-third (78.4%) of HIV-negative patients had successful treatment compared to 68.9% of HIV positive patients that were treated successful.  Out of the 77 HIV co-infected patients, 10.4% defaulted, 14.2% had died, and 6.5% failed treatment compared to HIV-negative patients amongst whom 15.2% defaulted, 4.8% died and 1.6% failed treatment. TB/HIV co-infection was significantly associated with poor treatment outcome (AOR: 1.158; 95 % CI 0.617-2.173; p = 0.648).Conclusion: The favorable treatment outcome of HIV-negative patients is more than that of HIV-positive patients and the most probable predictable factor responsible is the CD4 count of patient; indicating that TB/HIV co-infection has remained a major public health problem in Benue state. Therefore, there is the need for sustained strengthening and expansion of the national TB/HIV programmes.

Data collection was obtained by measuring height and then the data was adjusted to PB / U or TB / U - 3 SD sd <- 2 SD (stunted) dan <-3 SD (severely stunted). The research data were then analyzed descriptively using Microsoft Excel and displayed in the form of tables and narratives. Data collection in this study was carried out in March-June 2021 in four sub-districts located in Salatiga City.Result: The results showed that the prevalence of stunting in Salatiga City in 2020 in children aged 0-60 months was 6,85%, the incidence of stunting was more common in males by 3,65%.Conclusion: The conclusion from this study, it is known that the prevalence of stunting in Salatiga City in 2020 reached 6.85%.
