Abstract

Background: Food security at this national level has received attention because it has not yet reached the word "resilience", which is indicated by Indonesia still importing staple food from Vietnam. One of the provinces in Indonesia with the best level of resilience is Central Java province with 93%, but it does not guarantee the condition of overall food security at the household level because it has different access to food. Utilization of social capital can realize food security related to food access, making it easier for people to choose food (food choice).Objectives: This study aims to examine the relationship between food security and insecurity with social capital and household food choice in the Kutowinangun Lor Village, Tingkir District, Salatiga City.Methods: The method used is a descriptive quantitative method. The statistical test used is the Chi Square test.Results: As many as 48.4% of respondents fall into the category of severe food insecurity. 54.8% of respondents do not actively participate in village organizations with moderate levels of social capital. 71% of respondents have a fairly good food choice. Bivariate analysis shows that there is no relationship between food security and social capital with a chi square test result of 0.709, while for food choice with food security, it shows that there is a relationship between the two with an iujiichi square result of 0.015.Conclusions: Most of the underprivileged households in Kutowinangun Lor fall into the category of severe food insecurity, with the result of a Chi Square test for social capital of i0.709. Meanwhile, the results of the Chi Square test on food choice are 0.015. Thus, food choice is the variable that most influences food security and insecurity in underprivileged households in Kutowinangun Lor.

Abstract

Background: Diabetes mellitus increased from 6.9% in 2013 to 8.5% in 2018. This figure shows that only about 25% of people with diabetes know that they have diabetes. According to the International Working Group on the Diabetic Foot (IWGDF), 1 in 4 people with DM will develop an ulcer on their foot during their lifetime with a proportion of 19-34% of the total DM patients. Wound healing of DM ulcers often requires longer time and considerable resources and has a 40% risk of re-formation at 1 year and increases to 60% after 3 years.Methods: Articles selected based on the inclusion criteria were journals that examined the relationship between diabetic foot and amputation, either using a longitudinal design, cohort study (retrospective or prospective) or case control. The exclusion criteria were journals that were systematic literature reviews, screening, diagnosis.Results: Obesity in 2 articles mentioned that OR=1.20-3.20 had a significant association with amputation while blood glucose factor in 2 articles mentioned that OR=2.92-9.43 had a statistically significant association with amputation. It was found in these 3 articles with OR range=6.9-13.97, poorly controlled HbA1c had a statistically significant association with amputation.Conclusion: This literature review study concluded that amputation risk factors are influenced by BMI and blood sugar levels.  Patients should be educated on the importance of maintaining good blood sugar control, in addition to wearing proper footwear, avoiding repetitive mechanical trauma to the foot, and performing regular foot examinations.

Abstract

Background: Diarrhea is the main cause of malnutrition in underfive children. One of the efforts to prevent diarrhea in infants is exclusive breastfeeding. Exclusive breastfeeding is infant that receives only breast milk without any additional consumption from 0 to 6 months. Under six months infant who are not exclusively breastfed have a higher risk of developing diarrhea than those who are exclusively breastfed.Methods: This study aims to determine the relationship between exclusive breastfeeding and the incidence of diarrhea in under six months infants in Indonesia. This study analyze secondary data from the 2017 Indonesian Demographic and Health Survey. The sample in this study consist of all live infants aged 0-6 months born to women aged 15-49 years in Indonesia who were successfully interviewed and had a complete answer (no answer missing). After weighting, 1898 samples were obtained.Result: The result found that the proportion of diarrhea incidence in Indonesia is 9.1%. Meanwhile of all infants, 36% were not exclusively breastfed. The results showed that there was a significant relationship between infants who were not exclusively breastfed and diarrhea incidence in Indonesia after adjusted by mother's education level, maternal employment status, mother’s economic level, area of residence, and latrine ownership (PR = 2.12, 95% CI = 1.46-3.07).Conclusion : Infants who were not exclusively breastfed increase the risk for diarrhea. Therefore it is necessary to have a program to increase the coverage of exclusive breastfeeding. The program can take the form of counseling by health workers in health services.

Abstract

Background: Gross motor skills are very important for functional independence and others developmental milestone. Delays in this domain are more pronounced than in other domains in early age, for healthy developmental trajectories, so very early detection is imperative for every parent with a newborn. This study identify risk factors that affect gross motor development in children aged 1 to 3 years.Methods: This retrospective cohort study involved 139 subjects, rehabilitation outpatient clinic. Gross motor delay was assessed using CDC (Milestone Developmental) and Gross Motor Function Classification System (GMFCS) for patient with Cerebral Palsy. Potential risk factors included: prematurity status, asphyxia, epilepsy,  and parent's education.Results: 63 (45.32%) children had delays in gross motor skills, with a mean age of 27±7 months. Cox regression showed asphyxia RR=1.72; [95% CI, 1.49-1.99]; p=0.018. Epilepsy RR=1.50; [95% CI, 1.18-1.92]; p=0.004 was a factor associated with a higher risk ratio in children with gross motor delay. Prematurity was not statistically significant, which may be due to the very small number of subjects with prematurity (less than 5%). Younger children were more at risk of developing symptoms of delayed motor development. Parental education was not associated with the risk of delay.Conclusion: This study demonstrated children with asphyxia or epilepsy have higher risk of being gross motor delay. Therefore, early screening is very important in this population at risk, in order to identify potential delays so that these patients receive appropriate intervention immediately.

Abstract

Background: Tingkat mortalitas COVID-19 di Indonesia sebesar 2,7%, angka ini merupakan yang tertinggi di Asia Tenggara. Pasien COVID-19 yang mengalami perburukan di rumah sakit seperti masuk ICU dan kematian sebanyak 15-20%. Perubahan klinis pasien sebelum terjadinya perburukan dan kematian tidak disadari oleh 27,06% tenaga kesehatan. Menurut WHO, EWS dapat digunakan untuk mendeteksi perburukan pasien lebih awal, namun penelitian yang dilakukan untuk melihat hubungan EWS dengan kematian COVID-19 masih terbatas. Tujuan penelitian ini adalah untuk mengetahui besar risiko highrisk EWS dengan kejadian kematian serta probabilitas kesintasan pada pasien COVID-19 yang dirawat di RS UI.Methods: Penelitian ini menggunakan desain studi kohort retrospektif. Populasi pada penelitian ini adalah seluruh pasien COVID-19 yang dirawat inap di RS UI pada periode Maret – Desember 2020. Penelitian ini menggunakan total sampling. Jumlah sampel pada penelitian ini adalah 740 orang yang terdiri dari 137 orang pada kelompok terpapar dan 603 kelompok tidak terpapar. Analisis kesintasan dilakukan dengan menggunakan Kaplan Meier sedangkan analisis multivariat dilakukan dengan cox regresi.Result: Probabilitas kesintasan pasien COVID-19 yang di rawat di RS UI sampai dengan 72 hari pengamatan sebesar 47,4%. Besar risiko pasien COVID-19 yang memiliki highrisk EWS untuk terjadi kematian sebesar 5,32 (95% CI= 3,26-8,69).Conclusion : Tenaga kesehatan diharapkan dapat mendeteksi perburukan pasien sedini mungkin agar bisa dilakukan intervensi lebih awal untuk mencegah kematian pada pasien COVID-19.

Abstract

Background: COVID-19 cases in Depok City continued to increase until November 2020, so case tracking and close contact (tracing) are needed to suppress COVID-19 cases. However, tracing officers still face obstacles in conducting case tracking and close contact (tracing). The purpose of this study is to improve the knowledge of tracing officers and see the performance of the Depok City COVID-19 program in terms of contact tracing.Methods: The design of this study was One Group Pretest-Posttest, consisting of 121 participants using virtual lecture methods and online assignments through Zoom Meeting and Google Form. The collected data were analyzed with STATA.12 (paired t test) to see the difference in the mean scores of participants between pretest and posttest. In addition, SPSS.24 (paired t test and Wilcoxon) was also used to see the achievement of the COVID-19 search program.Results: The statistical test results showed that this training increased the knowledge of COVID-19 trackers in Depok City with a p-value <0.05. The average score before the training was 69.89 and after the training increased to 84.89.Conclusion: The successful implementation of case and contact finding is strongly influenced by the community and the number of officers needed to trace COVID-19 cases, and is supported by the availability of COVID-19 test kits at the examining laboratory.

Abstract

 Background: Low birth weight (LBW) is a baby born weighing less than 2500 grams which causes infant mortality rate (IMR). The purpose of this study is to describe the implementation of the Puskesmas program in preventing LBW in Salatiga City.Method: The research uses a qualitative method with a case study approach. Also, data collection techniques are in-depth interviews using an interview guide as a research instrument. Moreover, the research participants were 4 health workers. Meanwhile, data analysis techniques were data reduction and conclusion drawing. Also to support this research, the researcher used narrative presentation of data.Results: Therefore, the results showed 5 LBW prevention programs at the Sidorejo Kidul Health Center, in the form of a complete ANC 10T examination service. The giving supplementary Feeding (PMT) in the form of biscuits, green beans, sugar, margarine, eggs, regal, and shredded for 3 months. Neonatal visits (KN) are carried out at health facilities or home visits using the integrated management of young toddlers (MTBM). The maternal care group (KSI) consists of health workers, cross-sectors, and cadres who monitor and collect data on the target group. The class for pregnant women is carried out by providing education to pregnant women and their closest family which includes pregnancy, childbirth and the postpartum period.Conclusion: Thus, the researcher concluded that the Puskesmas program for the prevention of LBW events has been implemented.

Abstract

Background: Prevalence of drugs abuse among workers is still high. Drug use among workers has a negative impact on the health and safety of both workers and the community. This study was aimed to examine the relationship between impaired work productivity and drug behaviour among workers in Indonesia.Methods: This study used secondary data from the national survey on drug abuse among workers in 2017.  The total sample size was 34,397 persons. We used cox regression model to estimates the Prevalence Ratios (PR) and its 95% confidence intervals for associations between drug abuse and impaired work productivityResults: Drug abuse, marijuana and methamphetamine, used by workers were associated with impaired work productivity. Drug abuse in workers increased the likelihood of impaired work productivity by 2.3 times (p-value: <0.001; 95% CI: 1.97-2.73). Meanwhile, the use of marijuana types increased the probability of impaired work productivity by 1.6 times (p-value <0.001; 95% CI: 1.32-2.00). Workers using methamphetamine drugs were 1.29 times more likely to experience impaired work productivity than workers who did not use methamphetamine (p-value: 0.03; 95% CI: 1.03-1.62).Conclusion: impaired work productivity among workers was significantly associated with drug abuse

Abstract

Background: IDHS 2017 reported that there are 20.5% of births occur at home. Among births at home, the presence of a skilled health provider as a birth attendant is critical to ensure safe birth care, especially for mothers who could not access health facilities. However, the presence of skilled birth attendants has not been accessible during home births.Methods: This study used a cross-sectional study design using the 2017 IDHS data. The sample of this study was all women of childbearing age who gave birth at home in 2012-2017. The variables analyzed included maternal education, ownership index, region, place of residence, insurance, pregnancy complications, delivery complications, ANC visits, parity, maternal age at delivery, and delivery assistants. Predictive model analysis was performed using Cox regression.Results: 59.5% of home deliveries were attended by skilled birth attendants, while 40.5% were attended by traditional birth attendants/relatives.  Higher wealth index/socioeconomic status (PR=1.35; 95% CI 1.23-1.48), >4 ANC visits (PR=1.28; 95% 1.18-1.40), and higher education (PR=1.21; 95% CI 1.10-1.32) were the most dominant characteristics affecting the odds of delivery by a skilled birth attendant. Conclusion: Increasing coverage of skilled birth attendants can be pursued by increasing the coverage of recommended ANC visits (>4 times), improving socio-economic status, and increasing maternal education levels.

Abstract

Background: Diabetes Mellitus (DM) is a degenerative disease triggered by unhealthy lifestyles such as obesity. Obesity is a condition of someone who has a body weight above normal or more than the Body Mass Index (BMI) value of 27 kg/m2. Obesity is prone to occur in adolescence. The purpose of this study was to identify risk factors for diabetes mellitus (DM) in obese adolescents.Methods: The method used was descriptive quantitative with a cross sectional approach conducted in October-December 2020 on adolescents at SMA Lab Salatiga and SMK Kristen BM Salatiga. The samples in this study were adolescents with obesity at SMA Lab Salatiga and SMK Kristen BM Salatiga. The data collection technique in this study used google form. The research data were processed using SPSS (Univariate and Chi Square Test).Results: From the results of the study, several factors of DM were found, including age, gender, family history of Diabetes Mellitus. There were 50.8% of respondents who had normal blood pressure and most of the blood sugar levels were normal (57.1%). In this study, diet and physical activity had no relationship with obesity. The results of the analysis using chi square obtained a p-value (p>0,05).Conclusion: Factors of diabetes mellitus (DM) include age, gender and family history of diabetes mellitus while blood pressure, obesity diet, physical activity do not jeopardize the occurrence of diabetes mellitus and diet.
