Abstract

Arthritis gout is a disease caused by a purine metabolism disorder characterized by hyperuricemia and recurrent attacks of acute synovitis causing severe pain in the joint area. One of the treatments for gout arthritis with non-pharmacological therapy is acupressure and warm red ginger hydrotherapy which is able to relax muscles, increase blood circulation, and can stimulate the effector system causing peripheral vasodilation so that blood flow to tissues is smooth, especially tissues that are inflamed so that it can reduce inflammation joint pain. This study aims to determine the effectiveness of the combination of acupressure on the hegu and taixi with red ginger hydrotherapy as a complementary nursing intervention in reducing pain in gouty arthritis sufferers. Quantitative research with true experiment approach pre and post-test control group design involving 30 respondents divided into intervention group (n=10) given acupressure and red ginger hydrotherapy, while control group 1 (n=10) was given acupressure and control group 2 (n =10) was given red ginger hydrotherapy. The sampling technique used probability sampling with simple random sampling based on inclusion and exclusion criteria. Then the research results were analyzed using parametric tests, namely Paired t test, ANOVA test and Post Hoc Bonferroni test. The ANOVA test showed a significant value of p = 0.000 (<0.05), while the Post Hoc Bonferroni test showed that between the intervention group and control 1 there was a difference with a significant value of p = 0.001, between the intervention group and control 2 there was a difference in the value significant p=0.005 and between control group 1 and control 2 there was no difference with a significant value of p=1,000. There was a difference between the intervention group, control group 1 and control group 2, which means that the administration of acupressure at the hegu and taixi with red ginger hydrotherapy was more effective in reducing pain in gouty arthritis patients than acupressure and red ginger hydrotherapy alone.

Abstract

Chronic kidney disease (CKD) is a long-term disease where the kidneys have decreased function. In women who experience prolonged CKD, other problems will arise, namely Menstrual abnormalities. CKD patients who have just undergone hemodialysis therapy are prone to stress, if not treated immediately it will disrupt the menstrual cycle. This research design uses correlation analytic with cross sectional approach. The population in this study were 49 women who suffered from CKD in the hemodialysis unit of RSUD Prof. Dr. Soekandar. Sampling using non-probability sampling technique Consecutive sampling type within a period of 3 weeks. The sample in this study amounted to 43 people. The instruments used in this research are DASS questionnaire and Menstrual abnormalities checklist sheet. Data analysis using logistic regression test. The results showed that 20 respondents (46.51%) had CKD >4 years. For stress levels, it was found that 17 respondents (39.53%) were at mild stress levels. And 39 respondents (90.70%) experienced Menstrual abnormalities. The results of the logistic regression test showed that the p-value = 0.048. It can be concluded that there is a long-standing relationship between chronic kidney disease and stress with menstrual abnormalities in the hemodialysis unit of RSUD Prof. Dr. Soekandar. Women who suffer from CKD in their bodies will experience hyperprolactinemia which will disrupt the menstrual cycle. A person who has just been diagnosed with CKD is prone to stress, which stress will stimulate the release of Corticotropic Releasing Hormone (CRH). Hyperprolactinemia and CRH can affect the performance of Gonadotropin-releasing hormone (GnRH) which lead to menstrual abnormalities.

Abstract

Background: 2022 will see an increase in the number of HIV/AIDS cases in Jombang City, ranking it second in East Java with 1388 PLWHA, 20 cases of mother-to-child HIV transmission during childbirth, 12 HIV (+) cases, 8 AIDS cases, and 100 fatalities. The Independent Practice Midwife (BPM) is the linchpin of a healthcare organization that has the potential to grow into a network of expectant mothers. Midwives are qualified to serve as health educators for expectant women who exhibit habits that increase their risk of contracting HIV/AIDS. In a qualitative study in Jombang City, the aim of this study was to determine the program to develop midwives' ability as health educators for expectant mothers in an effort to prevent HIV/AIDS transmission. Method: This study is qualitative and employs an in-depth interviewing methodology. Up to 8 stakeholders served as primary informants, while 2 NGOs and 10 BPM employees served as secondary informants. analysis of data using the content analysis approach. Results: Because national guidelines for preventing HIV transmission from mother to newborn have been popularized, stakeholders have implemented a policy that uses standard technical programs for midwifery practice. Midwives have received training in APN, PI, and STIs, but due to funding restrictions, they have never received training in HIV/AIDS counseling. There is no such thing as BPM; only the central IBI does monitoring and assessment, which is done only through monthly reports. The coordination of NGOs, KPAD Regional Child Protection Commission, Public Health Office, The Indonesian Midwives Association, and BPM in the execution of monitoring and evaluation is insufficient, and only IBI is carried out, unplanned, carried out at any moment if it is going to prolong the practice permit. BPM has not yet received special supervision. Coordination amongst stakeholders during the implementation of supervision is insufficient. In an effort to stop the spread of HIV/AIDS, health education facilities for pregnant women in BPM have been provided with posters, flipcharts, and MCH handbooks. There are four VCT clinics and three PMTCT clinics. Conclusion: Program to improve midwives' abilities as health educators for expectant mothers in an effort to stop the spread of HIV/AIDS at BPM (independent practice midwives) Jombang City. In order to improve coordination between stakeholders (KPAD Regional Child Protection Commission, Public Health Office, and IBI The Indonesian Midwives Association with BPM in monitoring and supervision, train counselors for BPM (independent practice midwives), and have midwives become more motivated to provide IEC and continue to improve professionalism in providing midwifery services, advocacy is required to the Regional/ Legislative Council
