Abstract

Coronary microvascular dysfunction (DMK) is a condition of patients who are accompanied by complaints of chest pain where the results of coronary angiography examination are normal and this is almost 49% with 60% of patients diagnosed with DMK. Another study said that about 40% of patients with DMK showed coronary flow reserve (CFR <2) of about 40% and the WISE study (Women's Ischaemia Syndrome Evaluation) showed that about 47% of patients with chest pain had normal coronary arteries. DMK can be divided into 4 groups; DMK with no coronary arterial disease (CAD) obstruction and myocardial disease, DMK with myocardial disease where this occurs due to remodeling of intramural coronary arteries, DMK with CAD (coronary arterial disease) or acute myocardial infarction with or without ST segment, iatrogenic typhoid DMK occurs after coronary recanalization caused by vasoconstriction and distal embolization. The mechanism of action of DMK can be caused by several factors, namely endothelial dysfunction, smooth muscle dysfunction, decreased diastolic perfusion time, damage to blood vessels and damage to the vascular and microvascular remodeling. And to enforce this DMK, there are several tests carried out in diagnosing the disease, some of which are invasive and non-invasive so that by enforcing the diagnosis of this disease, treatment for DMK can be done immediately and optimally.

Abstract

Hypoxia inducible factor-1 (HIF-1) is a transcription factor that plays an important role in maintaining oxygen balance at both the cellular and systemic levels, and is associated with various controls in the body. HIF-1 is a heterodimer of alpha and beta subunits. Alpha subunits are mostly dependent on oxygen levels in the body. In many cancers, excessive HIF-1α is thought to be involved in the promotion of tumor growth and metastasis. In addition, in the induction of systemic hypoxia, there is an increase of HIF-1α in the heart, brain, and even the kidneys as an adaptation response to hypoxia. Several studies regarding HIF-1a expression in traumatic brain injury, found that HIF-1a increased immediately after TBI, and decreased significantly after 24 hours. This can be used as a basis for further research on HIF-1a control as an effort to stop tissue damage or even help tissue repair.

Abstract

The tendency for autism spectrum disorders or also known as Autism Spectrum Disorder (ASD) is increasing globally, even becoming 1 per 54 in 2016. Until now, the cause of autism is not known for certain, ASD is considered to be a combination of genetic and environmental factors. One of the environmental factors in this case is related to nutrition, one of which is vitamin D. Vitamin D deficiency is often found among children with ASD. Several studies have shown that vitamin D is involved in various brain bioprocesses including neuromodulation and nerve transmission and brain function while also influencing inflammatory processes, autoimmune disorders, oxidative stress and also neurotransmitters that are widely associated with the possibility of ASD. This review aims to describe vitamin D deficiency may contribute to ASD disorders. Based on this, in the future, it is necessary to consider when treating patients with ASD to consider the need to check the patient's vitamin D levels and if there is a deficiency it can be advised to sunbathe and or be given additional vitamin D intake.

Abstract

Abnormal uterine bleeding (AUB) is defined as the abnormal bleeding from the uterine corpus in term of duration, volume, frequency and/or regularity. This condition occurs in 37% of adolescents and may affect the quality of life and increased hospitalization. Etiology is divided into structural and non-structural causes, known as PALM-COEIN. The most common etiology in adolescents is anovulatory menstruation due to immature hypothalamus-hypophysis axis. Diagnostic evaluation should include investigation in the etiology of AUB, anemia signs, and hemodynamic status. Treatment of AUB consists of hormonal and non-hormonal therapy. Therapy in adolescent is given based on the severity of bleeding, grading of anemia, and hemodynamic stability. Follow-up is required after therapy. Understanding AUB in adolescents can help clinicians deliver appropriate and comprehensive treatment. This review was aimed to explain about definition, epidemiology, etiology, pathophysiology, diagnosis, and treatment of abnormal uterine bleeding in adolescent.

Abstract

Hand foot syndrome (HFS) is a reaction that occurs on the skin of the palms and soles. feet, caused by the toxicity of chemotherapy drugs with symptoms such as tingling, pain, erythema, dry skin, swelling, increased pigmentation and itching. This condition is still a problem in the treatment of patients given capecitabine. Although HFS does not cause death, in severe cases it can affect the quality of treatment and interfere with daily activities. The cause of HFS is still unclear but from research it is known that capecitabine and its metabolites will increase the expression of the COX-2 enzyme, inhibit the arachidonic pathway, increase PGE2 activity, causing inflammatory reactions in the form of erythema, edema and pain. The phenolic components contained in EVOO, namely oleocanthal and hydroxytyrosol compounds in several studies have the same properties as celecoxib and ibuprofen as anti-COX-2, will inhibit the arachidonic synthesis pathway, reduce prostaglandin activity and reduce inflammatory reactions.

Abstract

Background. Lumbar disc herniation (LDH) is a disorder that involves rupture of the annulus fibrosus so that the nucleus pulposus protrudes and compresses the lumbar canal. LDH is the most common cause of lumbar radiculopathy. LDH causes functional disorders in patients, causing disability to carry out daily activities. The purpose of this study was to determine the risk factors that influence the degree of disability in lumbar nucleus pulposus herniation. Methods. Analytical research with a consecutive cross-sectional study approach in patients with lumbar LDH at the Neurology Outpatient Clinic, Dr. M. Djamil General Hospital Padang from March to November 2021. The risk factors assessed were age, gender, occupation, length of illness, and body mass index (BMI). The measurement of the degree of disability uses the Oswestry disability index (ODI) score. The relationship between risk factors and the degree of disability was analyzed using chi-square test. Differences in the mean age of groups with mild and moderate degrees of disability using unpaired t-test. Results. 40 subjects met the inclusion and exclusion criteria. Mean age 57 ± 10,976 years, male and female ratio 1:2, work with heavy lifting activities was found in 70% of subjects, duration of suffering from LDH more than one year in 60% of subjects. BMI overweight and obesity as many as 77.5% subjects. As many as 60% of the subjects experienced mild to moderate degrees of disability. There is a difference in the average age of the subjects with mild to moderate degree of disability with a severe degree of p-value of 0.044. There was no significant relationship between gender (p=0.054), occupation (p=0.398), length of illness (p=0.503) and body mass index (p=0.757), and the degree of disability. Conclusion. The degree of disability in patients with LDH is influenced by age. Meanwhile, gender, occupation, length of illness and body mass index were not significantly related to the degree of disability in patients.

Abstract

Background. Most of foreign body aspiration cases are found in children under the age of fifteen. Pediatric patients often presents with non-food foreign body aspiration, such as toys. The most common clinical manifestation are history of choking following foreign object insertion into the mouth (85%), paroxysmal cough (59%), wheezing (57%) and airway obstruction (5%). Case presentation. Main principle of airway foreign body extraction is to do it immediately in the most optimal condition with slightest possible trauma. Rigid bronchoscopy is a suitable choice for tracheal foreign body extraction. We reported a case of seven years old male with tracheal foreign body presented with history of whistle ingestion five hours prior to admission. This patient was discharged from hospital after third days of rigid bronchoscopy procedure. Conclusion. History of foreign body aspiration in children should be suspected as a tracheobronchial foreign body. Rigid bronchoscopy is preferred to extract foreign bodies present in the trachea. The prognosis for tracheobronchial foreign body aspiration is good if the foreign body is treated early and without complications.

Abstract

Background: Arteriovenous fistula (AVF) is the preferred vascular access for hemodialysis (HD). Several factors can affect AVF maturity, such as diabetes. This study aims to determine the differences in the maturation of arteriovenous fistulas between diabetes mellitus and non-diabetes mellitus in patients with chronic kidney failure in Padang. Methods: This was a comparative cross-sectional study of patients with chronic kidney failure who underwent arteriovenous fistula surgery in Padang. The sampling technique in this study was consecutive sampling. The analysis was carried out using statistical analysis according to the existing data scale and using SPSS software. Results: This study involved 46 patients with chronic kidney failure who underwent arteriovenous fistula surgery in Padang, of which 23 patients with diabetes mellitus and 23 patients with non-diabetes mellitus. The diameter of the arteriovenous fistula between diabetes mellitus and non-diabetes mellitus in patients with chronic renal failure had a significant difference (p<0.05). There was no significant difference in flow, depth, and maturation of arteriovenous fistulas in patients with diabetes mellitus and non-diabetes mellitus with chronic renal failure. Conclusion: There is no difference in the maturation of arteriovenous fistula between patients with diabetes mellitus and non-diabetes mellitus

Abstract

Background. Chronic kidney disease (CKD) is a terminal disease requiring hemodialysis. Hemodialysis requires vascular access using a double lumen catheter (DLC). However, the use of DLC may increase complications mainly infection, either infection of exit site or bloodstream. This study is aimed to seek risk factors contributing to the incidence of central catheter-related infections in CKD patients with DLC undergoing HD at Dr. M. Djamil Hospital, Padang. Methods. This study is a prospective study on patients undergoing HD at Dr. M. Djamil Hospital, Padang. This study using consecutive sampling technique. Data were analyzed using SPSS version 25.0. Results. This study involved 40 CKD patients undergoing HD. Majority of the samples had bloodstream infections (67.5%). Clinical manifestations of purulent secretion, duration of catheter used, and hypoalbuminemia had significant differences in the incidence of DLC infection based on bloodstream infection and exit site infection (p<0.05). The most common bacterial found was Pseudomonas aeruginosa (22.5%) which was sensitive to ceftazidime, cefepime, meropenem, amikacin, gentamicin, ciprofloxacin. Conclusion. Factors contributing to the incidence of DLC-related infections at Dr. M. Djamil Hospital are duration of catheter use and hypoalbuminemia. Pseudomonas aeruginosa is the most common cause of DLC-related infections.

Abstract

Background: According to the American Cancer Society, Colorectal Cancer (CRC) is the third leading cause of cancer death in men and women in the United States. In Indonesia, CRC ranks as the third most common malignancy in both men and women. Programmed death-ligand 1(PD-L1) is a trans-membrane receptor ligand and negative regulatory signal for T cells that is elevated in several tumors including CRC and binds to programmed death 1 (PD-1) on T cells, B cells, dendritic cells and natural killer T cells. PD-L1 expression was found in tumor cells and tumor cells that infiltrate immune cells in several malignancies, including CRC. Methods: This study is a comparative analytic cross sectional study with consecutive sampling method. This study was conducted at Hasan Sadikin Hospital, Bandung from September to October 2021. Further statistical analysis was done SPSS version 25.0 for Windows (SPSS Inc., Chicago, Ill., USA). Results: The total sample in this study was 50 subjects with colorectal cancer patients at RSHS. The number of CRC patients who expressed PD-L1 were 23 (46%) and 27 subjects (54%). The majority of cancer patient survival 2 Years was 38 subjects (76.0%) and survivors of more than 2 years was 12 subjects (24.0%). Conclusion: There is no significant relationship between PD-L1 and survival rates in CRC patients.
