Abstract

Typhoid fever is a problem in the developing country. Phyllanthus niruri L (meniran) is a traditional plant that can be used as an immunomodulator on cellular or humeral immunity. By giving Phyllanthus niruri L extract to patient who suffers typhoid fever can increase cellular immunity responses in intracellular bacteria infection (S.typhimurium).This research was an experimental study using the post test-only control group design to proof of the effects of Phyllanthus niruri L on cellular immunity balb/c infected by salmonella typhimurium. The samples were taken from thirty male Balb/c mice which were divided into five groups including P1 (infected by S.typhimurium and PnL extract 125 g), P2 (infected by S.typhimurium and PnL extract 250 g), P3 (infected by S.typhimurium and PnL extract 500 g), K1 (infected by S.typhimurium) and K2 (healthy mice). In this study was applied parametric test of one way ANOVA and post hoc test Bonfferoni. It was known that phyllanthus niruri L increased the number of activated lymphocytes and phagocyte ability of macrophage (p=0.003 and p=0.001, respectively). By Bonfferoni post hoc test resulted that no significantly difference between the treated groups (increased of dose). There were no significant difference between the number of leukocyte, the difference count of lymphocyte and the nitrite oxide produced among the treatment groups and controls. Phyllanthus niruri L dose 250 μg increased the activated lymphocyte and index phagocytes. To find out the effects of Phyllanthus niruri L compare to antibiotic and Phyllanthus niruri L combination, continuing/advancing studies are needed

Abstract

Fluoroquinolone is used as first line drug for Salmonella sp infection, but there were reports of increasing treatment failure with fluoroquinolone in infection caused by Salmonella sp, which in vitro is still succeptible to fluoroquinolone. The identification of nalidixic acid resistance, a first generation quinolone provides a high sensitivity and specifity for the detection of such fluoroquinolone resistance. The researchers aim is to study the prevalence and the minimum ciprofloxacin inhibitory concentration of nalidixic acid resistant but fluoroquinolone sensitive Salmonella sp at Premier Jatinegara Hospital. Blood cultures sent to Premier Jatinegara Hospital Laboratory during 2010 were evaluated according to Clinical and Laboratory Standards Institute guidelines. Identification and MIC succeptibility testing were determined by VITEK® 2 Compact (Biomerieux®) and nalidixic acid succeptibility testing was performed by disc diffusion method according to Kirby Bauer. Thirty eight Salmonella sp isolates were identified, all were succeptible to ciprofloxacin (MIC  0,25 mg/L), but 5 (13,2%) isolates were resistant to nalidixic acid and reported as resistant. This study found that 13,2% of Salmonella sp were resistant to fluoroquinolone but not detected by the recommended CLSI breakpoint values. The researchers recommend that nalidixic acid testing be included in Salmonella sp succeptibility testing in Indonesia and consider 3rd generation cephalosporin as the first line drug before a succeptibility test result is available.

Abstract

The determination of thyroxin (T4) is known as a good indicator to know the condition of thyroid function. In the hyperthyroid state, are shown clearly the increased levels of T4, while in the hypothyroid state their levels always decrease. The T4 levels change due to the physiological and pathological conditions on the ability of thyroxin binding globulin (TBG). The T4 measurement can be performed using an enzyme-linked fluorescent immunoassay (ELFA) or enzyme-linked immunosorbant assay (ELISA). Both ELFA and ELISA can detect T4 antigen. In these research fifty one randomized samples sera from the Clinical Pathology laboratory, at Dr. Soetomo Hospital, consisting of 11 males and 40 females, were studied. They were comparing using ELFA and ELISA to know the levels of T4. In this study the method used is a cross sectional observational, and the statistical analysis with nonparametric Spearman Correlation Rank’s test and Wilcoxon Signed Rank’s Test. In the results by using ELFA were found the mean and SD values 8.78 μg/dL/5.39 μg/dL, whereas by ELISA 11.06 μg/dL/5.38 μg/dL. These results showed a correlation between ELFA and ELISA with rs 0.576. The T4 levels results showed as follows, hypothyroid ELFA 8 (15.7%), ELISA 6 (11.8%); normal levels ELFA 32 (62.7%), ELISA 29 (56.9%); hyperthyroid ELFA 11 (21.6%), ELISA 16 (31.4%) with p 0.090 which was not considered significant. It can be concluded from the showen results that in the determination of T4 concentration using ELFA gave a lower yield compared with ELISA. Further examination is required by special treatment of the sample or other way to find out which one is the best way for determination of T4 levels.

Abstract

Congestive Heart Failure (CHF) is a syndrome associated with disturbances of heart structure and function, as a result of various cardiovascular diseases. These disturbances decrease ventricle ability to pump or fill blood in physiological pressure, which causes limitation on the ability to excercise or daily activities without dyspnea and fatigue.Early diagnosis is important to initiate prompt treatment that can prevent further disease development. To measure heart hormone in this case, NT pro Natriuretic Peptide (NT pro BNP) can be use as a more ideal examination for early detection of CHF. The aim of this study is to investigate the clinical performance of NT pro BNP and assess the cut-off point 125 pg/mL compared to clinic (Framingham criteria) and or echocardiogram in the diagnosis CHF.The diagnostic test was carried out using clinical (Framingham criteria) and or echocardiography as reference methods. The subject of this research was patients with cardiac and blood vessel disorders complaints at risk of developing congestive heart failure, who came to the Emergency Department at Dr. Sardjito Hospital Yogyakarta, supported by clinical data, radiology, other laboratory test, electrocardiography and echocardiography. The diagnosis of CHF was obtained by conducting investigation toward echocardiography examination reading in patient’s medical record. Receiver Operating Characteristics (ROC) curve analysis was conducted using SPSS 13.0 program. Sensitivity, specificity, positive and negative predictive value, accuracy and likelihood ratios were measured with 2x2 table by CAT marker program. Forty-six of fifty-seven specimens were from congestive heart failure (CHF). At 125 pg/mL as a cut-off point, regardless the interval between the onset of the symptoms and taking of blood samples, the sensitivity, specificity, positive and negative predictive value and diagnostic accuracy are 96%, 36%, 86%, 67%, 84% respectively. Likelihood ratios for positive and negative results are 1.50 and 0.12. NT pro BNP measuring is expected to give more benefit than the conventional signal, so that an early diagnosis can be achieved accurately in a timely manner.

Abstract

Recently several serology tests have been used to detect IgM and IgG antidengue, and NS1 antigen. On the last study was launched a new diagnose test using immunochromatography method to detect the specific antidengue IgA antibody. The aim of this study was to evaluate the IgA antidengue performance using immunochromatography method in Dengue Hemorrhagic Fever patients. The study was carried out during March up to July 2010 by a cross sectional method. The samples taken were 1–14 year‘s old patients from the Infectious Tropical Sub Division of the Department of Paediatric, Dr. Wahidin Sudirohusodo Hospital, Makassar. The samples were diagnosed Dengue Hemorrhagic Fever based on the criteria of WHO 1997 and they also subjected to IgM and IgG serology tests as well. The test for IgA is using immunochromatography method. The data was analyzed by Spearman Correlation Test. From the total 40 samples, which consist of 16 (40%) male and 24 women (60%), generally were taken on the fourth day until the sixth day of onset. There were more IgA anti Dengue detected rather than IgM on the secondary infection. The researchers found moderate correlation among the IgA test with IgM (+), IgG (+), or both positive. Based on this study so far it can be concluded that the Dengue IgA test can be used to detect the primary or secondary Dengue virus infection.

Abstract

Cirrhosis hepatic is a diffuse chronic liver disease characterized by fibrosis and nodules formation. The reporter presented a case of an 8 years girl who was referred from RSU Bone with Nephritic Syndrome. Through anamnesis, physical examination, laboratory and USG exam, the girl finally was diagnosed as Cirrhosis Hepatic. Based on the clinical symptoms prolonged icteric, ascites, encephalopathy and neurological disorders and the laboratory tests: increased billirubin level (>1.5 mg%), decreased albumin level (<2.5 gr%), the prognosis was poor

Abstract

Anaemia is an important complication of malaria, and its pathogenesis is not well understood. High level of the Th2 cytokine (such as IL-10), which counteract the Th1 cytokine, might prevent the development of severe malarial anaemia. The purpose of this study was to know the comparation between the plasma level of IL-10 in malaria patients with anaemia and without anaemia. The plasma level of IL-10 was examined in 16 malaria patients with anaemia and 16 malaria caused by P. falciparum patients without anaemia samplestaken from patients at the primary health centres in West Lombok and Centre Lombok during March until July 2008. The samples were measured using ELISA. The concentration of haemoglobin (Hb) was measured using hematological analyzer. The anaemia concentration of Hb is <11 g/dL. The results were analyzed using two (2) sample t test with SPSS ver.13.The plasma level of IL-10 in malaria patients caused by P. falciparum with anaemia was 8.81(3.04) [mean(SD)] pg/mL where as the plasma level of IL-10 in malaria patients without anaemia was 47.99(25.26) pg/mL. The mean of IL-10 level in malaria falciparum patients with anaemia was significantly lower than that of malaria patients caused by P. falciparum without anaemia (p=0.000).

Abstract

HIV & AIDS have become a global problem throughout the world including Indonesia. The incidence is rapidly increasing. Various treatment and efforts have been carried out but until now have not yet been succeeded, the mortality remains high. Examination of CD4+ lymphocyte-T which is carried out to determine the immune status and monitoring the treatment has some limitation. Besides the degradation of CD4+ lymphocyte-T, the increase of the disease progression is also followed by an increase of interleukin-10 as well. The determination of interleukin-10 is expected to be use as an alternative examination if CD4+ lymphocyte-T can not be performed. To analyze the correlation between the levels of plasma interleukin-10 and amount of CD4+ lymphocyte-T in the asymptomatic HIV infected patients. A cross sectional, observational analytical study of 41 patients with HIV infection stage I, has been conducted from March–April 2009, at the Intermediate Infectious Disease Care Unit Dr. Soetomo General Hospital, Surabaya. The diagnosis of HIV patients was based on positive result of HIV test using three (3) different methods. Eight mL venous blood were taken from each patient, 6 mL is put into a heparin tube for the examination of IL-10 plasma with ELISA method and 2 mL is put into a K3EDTA tube for examining the CD4+ lymphocyte-T using flowcytometry. The results showed of plasma IL-10 level and the amount of CD4+ lymphocyte-T, which were analyzed by Pearson correlation test to determine the correlation between the two (2) variabels. The IL-10 levels in HIV patients were 3.80–44.50 pg/mL (mean 18.09 pg/mL, SD 8.84 pg/mL). The amount of absolute CD4+ lymphocyte-T was 5–846 cells/µL (mean 322.07 cells/µL and SD 221.89 cells/µL), while the amount of percentage CD4+ lymphocyte-T was 0.41–29.48% (mean 13.99%, SD 7.62%). Statistical analyzes show a significant negative correlation either between plasma IL-10 level with absolute CD4+lymphocyte-T and plasma IL-10 level with percentage of CD4+ lymphocyte-T, e ach level of r was –0.652 and –0,683. A significant negative correlation was also formed between plasma IL-10 level and the amount of CD4+ lymphocyte-T in the HIV infected patients. In the HIV infected patients, the increase of plasma IL-10 level was followed by a decrease of CD4+ lymphocyte-T.

Abstract

Diabetic foot ulcer is a common complication of diabetic disease which causes morbidity and foot amputation. One major pathogenesis is infection. Microbes which infect are varied. The use of inappropriate antimicrobials can or may cause drugs resistance. Data of microbes pattern and sensitivity test is important and may guide the clinician in giving the initial therapy. The aim of this retrospective study was to know the characteristics, microbial pattern, and the sensitivity to antimicrobial drugs of 31 diabetic foot ulcer patients who were hospitalized at Wahidin Sudirohusodo Hospital, Makassar in the period of January 2009 until June 2010. The study results showed the age mean was 54.06±12.4 years old, and the onset of diabetes mellitus was 7.62±5.77 years. The common microbes were gram negative bacteria (73.52%) including Enterobacter agglomerans, Proteus mirabilis and Klebsiella pneumonia, while the most common gram positive microbe was Streptococcus sp. Gram positive microbes were sensitive to meropenem, ceforoxim and amoxilin, while Gram negative microbes were sensitive to meropenem. Based on this study, the researchers concluded that the most common microbes which infect the feet were gram negative microbes. Gram positive microbes were still sensitive to meropenem, ceforoxim and amoxilin. Gram negative microbes were sensitive to meropenem.

Clinical laboratories should have higher competitiveness and healthiness to be survived in the globalization era. The very basic assets are vision, mission, and objectives which are agreed upon them. To establish these objectives are required the availability of the human resources which can perform professionally. They can serve efficiently and effectively due to their increasing professionalism. This happened by a continuing enhanced development of the human resources. The only way to achieve this objective is by effecting the Total Quality Management which emphasizes the personal development through an objective appraisal system
